OBJECTIVE: To analyze the obesity, overweight, and thinness trends among Brazilian schoolchildren by sex, age group, and type of school according to World Health Organization (WHO) and International Obesity Task Force (IOTF) criteria. METHODS: We conducted four surveys between 2002 and 2018/19 involving schoolchildren aged 7-14 years from a state capital in southern Brazil. Weight status was classified using both WHO and IOTF criteria. RESULTS: In the total sample, obesity prevalence based on the IOTF and the WHO criteria were 72% and 44% higher in 2018/19 compared with 2002, respectively. Over the whole period, the obesity prevalence increased among children (WHO: 10.1% vs. 14.1%; IOTF: 5.0% vs. 8.3%), and those from public schools (WHO: 10.5% vs. 16.4%; IOTF: 5.6% vs. 10.1%). There was no significant reduction in thinness prevalence over the analyzed period. CONCLUSION: Obesity prevalence remains on an upward trend in a state capital in southern Brazil, especially among children from public schools. A higher prevalence of overweight and obesity was observed using the WHO criteria compared with IOTF criteria.

ABSTRACT Objective Evaluate the consumer's food environment in food retailers around public schools in the municipality of Governador Celso Ramos, state of Santa Catarina, in southern Brazil. Methods A survey was carried out in the surroundings of three public schools, from March to May 2022. Food retailers located in a pre-established 800m buffer zone were identified. A location system device recorded latitude and longitude. An audit of the establishments was performed using a specific validated instrument for auditing the consumer food environment (AUDITNOVA). These establishments were grouped into: Group 1 - predominantly selling fresh foods, Group 2 - mixed establishments, and Group 3 - predominantly selling ultra-processed foods. These establishments were assessed using a Consumer Food Environment Healthiness Score evaluation system. The data were analyzed using the statistical software Stata version 16.0. Results The study audited 21 establishments, 47.6% of which were small neighborhood food markets, 23.8% bakeries, 14.3% butchers/fishmongers, 9.5% grocery stores and 4.8% supermarkets. It was found that privately-owned vegetable markets had a higher availability of fresh and minimally processed food categories, while the lowest scores for this category were found in bakeries. Regarding ultra-processed foods, the most commonly found items were sugary beverages and candies, chocolates, and filled cookies. Conclusion More audit studies ought to be carried out in the surroundings of schools to broaden the understanding of the relationship between access, availability, prices and food advertisement in order to support the construction of strategies aimed at preventing obesity in the population.


RESUMO Objetivo Avaliar o ambiente alimentar do consumidor em estabelecimentos de comercialização de alimentos no entorno de escolas públicas no município de Governador Celso Ramos, estado de Santa Catarina, no Sul do Brasil. Métodos Foi realizada, entre março e maio de 2022, no entorno de três escolas públicas, auditoria nos comércios varejistas localizados em uma zona pré-estabelecida de 800m. Um equipamento com sistema de localização realizou o registro da latitude e longitude. Esta auditoria foi realizada utilizando um instrumento validado próprio para auditoria do ambiente alimentar do consumidor (AUDITNOVA). Estes foram agrupados em Grupo 1 - com venda predominante de alimentos in natura; Grupo 2 - estabelecimentos mistos; Grupo 3 - com venda predominante de alimentos ultraprocessados. Os estabelecimentos foram avaliados por um sistema de avaliação de saudabilidade do ambiente alimentar do consumidor. Os dados foram analisados no programa estatístico Stata versão 16.0. Resultados O estudo auditou 21 estabelecimentos, sendo 47,6% mercados de bairro, 23,8% padarias, 14,3% açougues/peixarias, 9,5% sacolões/hortifrutis privados e 4,8% supermercados. Foi identificado que sacolões/hortifrutis privados apresentaram maior oferta de alimentos da categoria in natura e minimamente processados, enquanto os menores scores desta categoria de alimentos foram encontrados em padarias. Em relação aos alimentos ultraprocessados, os alimentos mais presentes foram bebidas açucaradas e balas, chocolates e biscoito recheado. Conclusão Sugere-se a realização de mais estudos de auditoria em comércios varejistas no entorno de escolas para ampliar a compreensão da relação entre o acesso, disponibilidade, preços e propagandas dos alimentos de modo a subsidiar a construção de estratégias que visem a prevenção da obesidade na população.

PURPOSE: To describe the methodological aspects and characteristics of the participants of the EPOCA survey. METHODS: The study was conducted with schoolchildren aged between seven to 14 years old from 30 schools in Florianópolis, Southern Brazil. Body mass, height, girths, and skinfold thicknesses were measured. Food consumption and physical activity from the previous day were self-reported using the validated Web-CAAFE questionnaire. Adolescents completed a specific questionnaire about physical activity, meal consumption, and weight control behaviors. Parents/guardians responded to a sociodemographic and habits questionnaire. RESULTS: A total of 1671 schoolchildren participated in the study (response rate: 27.2%). About 63% of schoolchildren were enrolled in public schools. Most studied in the morning shift (54.2%), were female (53.1%) and aged between seven and 10 years (58.1%). The prevalence of overweight was 33.7% and obesity was 11.3%. CONCLUSIONS: The data obtained will allow us to assess the trend in the prevalence of overweight and obesity and associated factors when compared to other surveys performed. Descriptions of the logistics and protocols can help in the development and improvement of similar studies. It is hoped that the results of EPOCA 2018/2019 may help in the design of obesity prevention policies and programs for this population.

This study aimed to verify the association between the consumption of omega-3-rich (n-3) fish and depressive symptoms in older adults living in Southern Brazil. This is a cross-sectional analysis with data from the second wave of the EpiFloripa Aging cohort study (2013/2014) including 1,130 individuals aged 60 years or older. The presence of depressive symptoms was measured by the 15-items Geriatric Depression Scale (GDS-15), and the consumption of n-3-rich fish by a question of weekly frequency. The minimum set of variables for adjustment was defined using directed acyclic graph (DAG). Poisson regression with robust error variance was applied (adjusted by Model 1: demographic and socioeconomic variables, Model 2: added behavioral variables, Model 3: added health variables). We identified the prevalence of depressive symptoms in 19% of older adults and 51.8% reported eating n-3-rich fish once a week. Models 1 and 3 showed an inverse association between n-3-rich fish and depressive symptoms. However, the association was reduced when behavioral factors (leisure-time physical activity) were included in Model 2. These findings suggest that n-3-rich fish intake tends to be associated with depressive symptoms in older adults. However, other factors, such as physical exercise, are as pivotal as n-3 fatty acids in preventing the development of depressive symptoms.

Este estudo buscou verificar a associação entre o consumo de peixes ricos em ômega-3 (n-3) e sintomas depressivos em idosos residindo no Sul do Brasil. Esta é uma análise transversal com dados da segunda onda do estudo de coorte EpiFloripa Idoso (2013/2014) e incluiu 1.130 indivíduos com 60 anos ou mais. A presença de sintomas depressivos foi medida pela Escala de Depressão Geriátrica de 15 itens (GDS-15) e pela frequência semanal de consumo de peixes ricos em n-3. O conjunto mínimo de variáveis para ajuste foi definido utilizando-se um gráfico acíclico dirigido (GAD). Foi aplicada a regressão de Poisson com variância robusta de erros (ajustada pelo Modelo 1: variáveis demográficas e socioeconômicas, Modelo 2: variáveis comportamentais adicionadas e Modelo 3: variáveis de saúde). Identificamos a prevalência de sintomas depressivos em 19% dos idosos e 51,8% relataram comer peixes ricos em n-3 uma vez por semana. Os Modelos 1 e 3 apresentaram uma associação inversa entre peixes ricos em n-3 e sintomas depressivos. No entanto, a associação foi reduzida quando fatores comportamentais (atividade física de lazer) foram incluídos no Modelo 2. Esses achados sugerem que a ingestão de peixes ricos em n-3 tende a estar associada a sintomas depressivos em idosos. No entanto, outros fatores como o exercício físico são tão cruciais quanto os ácidos graxos n-3 em prevenir o desenvolvimento de sintomas depressivos.


El objetivo de este estudio fue verificar la asociación entre el consumo de pescado rico en omega-3 (n-3) y los síntomas depresivos en adultos mayores que viven en el Sur de Brasil. Análisis transversal con datos de la segunda oleada del estudio de cohortes EpiFloripa Anciano (2013/2014) que incluyó a 1.130 individuos de 60 años o más. La presencia de síntomas depresivos se midió mediante la Escala de Depresión Geriátrica de 15 ítems (GDS-15), y el consumo de pescado rico en n-3 mediante una pregunta sobre la frecuencia semanal. El conjunto mínimo de variables para el ajuste se definió mediante un gráfico acíclico dirigido (GAD). Se aplicó la regresión de Poisson con varianza de error robusta (ajustada por el Modelo 1: variables demográficas y socioeconómicas, Modelo 2: variables de comportamiento añadidas, Modelo 3: variables de salud añadidas). Se identificó la prevalencia de síntomas depresivos en el 19% de los adultos mayores y el 51,8% informó de que comía pescado rico en n-3 una vez a la semana. Los Modelos 1 y 3 mostraron una asociación inversa entre el pescado rico en n-3 y los síntomas depresivos. Sin embargo, la asociación se redujo cuando se incluyeron los factores conductuales (actividad física en tiempo libre) en el Modelo 2. Estos resultados sugieren que la ingesta de pescado rico en n-3 tiende a asociarse con los síntomas depresivos en los adultos mayores. Sin embargo, otros factores como el ejercicio físico son tan fundamentales como los ácidos grasos n-3 para prevenir el desarrollo de síntomas depresivos.

OBJECTIVE: The aim of the current study was to identify and describe the meal and snack patterns (breakfast, mid-morning snack, lunch, mid-afternoon snack, dinner and evening snack) of public schoolchildren. DESIGN: Cross-sectional study. Information on the previous day's food intake was obtained through the Web-CAAFE (Food Intake and Physical Activity of Schoolchildren), an interactive questionnaire, which divides daily food consumption into three meals (breakfast, lunch and dinner) and three snacks (mid-morning, mid-afternoon and evening). Each meal contains thirty-one food items and the schoolchildren clicked on the food items consumed in each meal. Factor analysis was used to identify meal and snack patterns. The descriptions of the dietary patterns (DP) were based on food items with factor loads ≥ 0·30 that were considered representative of each DP. SETTING: Schoolchildren, Florianopolis, Brazil. PARTICIPANTS: Children (n 1074) aged 7-13 years. RESULTS: Lunch was the most consumed meal (96·0 %), followed by dinner (86·4 %), breakfast (85·3 %) and mid-afternoon snack (81·7 %). Four DP were identified for breakfast, mid-morning snack, lunch, dinner and evening snack, and three for mid-afternoon snack. Breakfast, lunch and dinner patterns included traditional Brazilian foods. DP consisting of fast foods and sugary beverages were also observed, mainly for the evening snack. CONCLUSIONS: The results of the current study provide important information regarding the meal and snack patterns of schoolchildren to guide the development of nutrition interventions in public health.

Resumo Introdução A compra institucional para a alimentação escolar é uma oportunidade de incentivo a novas cadeias de abastecimento, sobretudo em nível local. Objetivo Avaliar a gestão municipal do Programa Nacional de Alimentação Escolar (PNAE) relacionada ao fomento e aquisição de alimentos da agricultura familiar. Método Foi realizado levantamento bibliográfico de estudos avaliativos no âmbito do PNAE para seleção dos indicadores e medidas pertinentes ao objeto e desenvolvido um modelo avaliativo. O modelo avaliativo foi pactuado após submissão a oficinas de consenso. Foi elaborado um questionário e enviado por meio eletrônico a 18 municípios de Santa Catarina. A amostra foi definida de forma intencional, e os municípios selecionados por meio de sorteio. Para avaliar as ações da gestão municipal do PNAE destinadas ao fomento e aquisição de alimentos da agricultura familiar, foram utilizados 5 indicadores, 9 medidas e suas respectivas questões. Resultados A avaliação dos municípios foi predominantemente "regular" (88%; n=14). Conclusão A avaliação apontou fragilidades da gestão municipal em relação ao desconhecimento da legislação e não aplicabilidade dos critérios previstos em normativa, especialmente quanto ao Planejamento de Cardápios, realização de Chamada Pública e seleção dos Projetos de Venda.


Abstract Background The institutional purchase for school feeding is an opportunity to encourage new supply chains, especially at the local level. Objective To evaluate the municipal management of the National School Feeding Program (PNAE) related to the promotion and acquisition of food from family agriculture. Method A bibliographic survey of evaluative studies was carried out within the scope of the PNAE to select indicators and measures pertinent to the object and develop an evaluation model. The evaluative model was agreed after submission to consensus workshops. A questionnaire was developed and sent electronically to 18 municipalities of Santa Catarina. The sample was intentionally defined and the municipalities selected by lot. In order to evaluate the actions of municipal management of the PNAE for the promotion and acquisition of food from family agriculture, five indicators, 9 measures and their respective issues were used. Results The evaluation of municipalities was predominantly "regular" (88%, n = 14). Conclusion The evaluation pointed to fragilities of municipal management in relation to the lack of knowledge of the legislation and non-applicability of the criteria set forth in regulations, especially regarding menu planning, public call and selection of sales projects.

Oxidative stress produced by adjuvant treatments is associated with cell injury; however, a healthy diet can help mitigate it. The aim of this study is to investigate the association between diet quality and oxidative stress parameters in women subjected to adjuvant treatment for breast cancer. The sample comprised 70 women. Oxidative stress biomarkers and diet quality parameters based on the Brazilian Healthy Eating Index-Revised (BHEI-R)-were evaluated at baseline (p0) and after adjuvant treatment (p1). Ferric reducing antioxidant potential (FRAP) was associated with diet quality at p0. BHEI-R scores were not different between p0 and p1; however, scores from total vegetables, total fruits, milk and dairy products, and meat, eggs and legumes were lower during treatment. On the other hand, lower sodium and saturated fat intake observed at p1 counterbalanced the BHEI-R score. Oxidative stress parameters have increased at p1, but they were not associated with diet quality; thus, changes in component intake were not enough to promote changes in oxidative stress during treatment. It appears that diet can enhance patients' antioxidant defense before treatment, which could lead to better outcomes in the long term. Further investigations may help to clarify the association between diet and oxidative stress in women with breast cancer.

BACKGROUND & AIMS: Metabolic syndrome is a multi-causal disease. Its treatment includes lifestyle changes with a focus on weight loss. This systematic review assessed the association between Selenium intake and metabolic syndrome. METHODS: Data were collected mainly from four databases: PubMed, CENTRAL (Cochrane), Scopus and Web of Knowledge. Keywords related to metabolic syndrome, selenium, as well as metabolic syndrome features were searched. This review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Statement. A systematic review protocol was registered at PROSPERO (n. 42016046321). Two reviewers independently screened 2957 abstracts. Six studies were included to perform data extraction with standardized spreadsheets. The risk of bias was assessed by using specific tools according to the design of the relevant studies. An assessment was carried out based on the appropriateness of the study reports accordingly to STROBE and the CONSORT-based checklist for each study design. RESULTS: Three studies found no association between Selenium intake and metabolic syndrome; two of them found an inverse association; and one study found a direct association between Selenium intake and metabolic syndrome. One study also showed an inverse association between Selenium intake and the prevalence of high waist circumference, high diastolic blood pressure, and hyperglycaemia in women. CONCLUSIONS: Overall, based on the argumentation and results of this study, it is possible to conclude that Selenium intake and metabolic syndrome are not clearly associated in adults and elderly.

Negative binomial regression was used for data analysis. RESULTS: We included 2,195 schoolchildren in the study. We found that buying foods from snack bars or fast-food outlets was associated with the intake frequency of ultra-processed foods among 11-14 years old in an adjusted model (incidence rate ratio, IRR: 1.11; 95% confidence interval, CI: 1.01;1.23). Use of butchers was associated with the intake frequency of unprocessed/minimally processed foods among children 11-14 years old in the crude model (IRR: 1.11; 95% CI: 1.01;1.22) and in the adjusted model (IRR: 1.11; 95% CI: 1.06;1.17). CONCLUSIONS: Use of butchers was associated with higher intake of unprocessed/minimally processed foods while use of snack bars or fast-food outlets may have a negative impact on schoolchildren's dietary habits.

Aims: To identify the effect of adjuvant antineoplastic treatment on body weight change of women with breast cancer.METHODS: A non-randomized clinical study included women with a recent surgical diagnosis of breast cancer, admitted tothe Maternidade Carmela Dutra hospital in Florianópolis, Santa Catarina, southern Brazil, between October 2006 and July2008. Food intake, weight, body mass index, hip circumference, waist circumference and other variables were measuredbefore (baseline) and after the adjuvant antineoplastic treatment (pos-treatment). A mixed effects linear regression modelwas used to estimate the longitudinal changes occurring in weight.Results: The sample comprised 53 patients. A significant increase (P<0.05) was observed in body weight (2.81 kg), bodymass index (1.08 kg/m2), hip circumference (3.62 cm) and waist circumference (1.93 cm). In relation to diet, there was asignificant increase (P<0.05) in the intake of energy (272.7 kcal), total fat (11.2 g) and polyunsaturated fatty acids (5.4 g).The final regression model for the change in body weight demonstrated that the women who were exposed to chemotherapytreatment, and to chemotherapy associated with radiotherapy, had the largest mean increase in body weight (2.47 kg and 5.21kg, respectively). Socio-economic, demographic and nutritional factors were not associated with the increase in body weight.Conclusions: Weight gain was associated with chemotherapy treatment either alone or in combination with radiotherapy.


Objetivos: Identificar o efeito do tratamento antineoplásico adjuvante sobre a mudança de peso corporal em mulheres com câncer de mama. Métodos: Um estudo clínico não randomizado incluiu mulheres com diagnóstico cirúrgico recente de câncer de mama, admitidas no hospital Maternidade Carmela Dutra em Florianópolis, Santa Catarina, entre outubro de 2006 e julho de 2008. Os dados de consumo alimentar, peso corporal, índice de massa corporal, circunferência do quadril, circunferência da cintura e outras variáveis, foram avaliados antes (basal) e depois do tratamento antineoplásico adjuvante (pós-tratamento). Um modelo de regressão linear de efeitos mistos foi utilizado para estimar as mudanças longitudinais que ocorreram no peso corporal. Resultados: A amostra foi composta por 53 pacientes. Foi observado um aumento significativo (P<0,05) no peso corporal (2,81 kg), índice de massa corporal (1,08 kg/m2), circunferência do quadril (3,62 cm) e circunferência da cintura (1,93 cm). Em relação aos aspectos dietéticos, houve um significativo aumento (P<0,05) na ingestão de energia (272,7 kcal), gorduras totais (11,2 g) e ácidos graxos poliinsaturados (5,4 g). O modelo final para a mudança no peso corporal demonstrou que as mulheres as quais foram expostas ao tratamento quimioterápico, e quimioterápico associado ao radioterápico, tiveram um maior aumento médio no peso corporal (2,47 kg e 5,21 kg, respectivamente). Os fatores socioeconômicos, demográficos e nutricionais não foram associados com o aumento no peso corporal. Conclusões: O aumento de peso esteve associado com o tratamento quimioterápico sozinho ou em combinação com o tratamento radioterápico.

PURPOSE: To investigate changes in the dietary consumption as well as the influence of the general characteristics, of the sociodemographic, clinical and nutritional factors, and of the antineoplastic therapy on the changes in the energy intake of women from southern Brazil, before and after adjuvant therapy for breast cancer. METHODS: A non-randomized clinical study was conducted on 53 patients at a hospital of the public health network. Dietary information was collected with a food frequency questionnaire. A mixed-effects linear regression model was used to evaluate the factors that influenced longitudinal alterations of energy intake. RESULTS: A significant increase was observed in daily energy intake of fats, calcium, iron, copper, polyunsaturated fatty acids, omega 6 and omega 3, and a significant decrease in vitamin B2 intake. The final regression model for the change in energy intake showed an average increase of 19.2 kcal/month. Fruit and legume consumption showed the highest association with energy intake, with each 100 g consumed resulting in an average increase of 68.4 and 370.5 kcal, respectively. Women in the 51 to 60 year age range consumed 403.5 kcal less than those in the 31 to 50 year age range. CONCLUSION: There was an increase in energy intake during treatment and the increase in the ingestion of fruits and legumes was associated with significant increases in energy intake.

RESUMO OBJETIVO: Investigar alterações no consumo dietético, bem como a influência das características gerais, dos fatores sociodemográficos, clínicos e nutricionais, e do tratamento antineoplásico sobre as mudanças do consumo energético, em mulheres do sul do Brasil, antes e após a realização de terapia adjuvante para o câncer de mama. MÉTODOS: Ensaio clínico não randomizado, conduzido em hospital da rede pública de saúde, com 53 pacientes. Informações dietéticas foram coletadas com questionário de frequência alimentar. Para avaliar os fatores que influenciaram alterações longitudinais da ingestão energética, usou-se modelo de regressão linear de efeitos mistos. RESULTADOS: Houve aumento significativo no consumo diário de energia, de gorduras, cálcio, ferro, cobre, ácidos graxos poli-insaturados, ômega 6 e ômega 3, e uma diminuição significativa da vitamina B2. O modelo final de regressão mostrou aumento médio de 19,2 kcal/mês. As maiores associações com ingestão energética foram frutas e leguminosas, sendo que cada 100 g de consumo destas, resultou um acréscimo médio de 68,4 e 370,5 kcal, respectivamente. Mulheres com idade compreendida entre 51 e 60 anos consumiram 403,5 kcal menos do que aquelas com idade de 31 a 50 anos. CONCLUSÃO: Observou-se que houve aumento na ingestão energética durante o tratamento e que o aumento na ingestão de frutas e leguminosas foi associado com aumentos significativos na ingestão de energia.
