RESUMO

In this article, the authors describe the indications, contraindications, surgical technique and its fixation considerations, postoperative care, results, and potential complications of the offset V and the traditional Z osteotomies. Because of its versatility, inherent stability, minimal first metatarsal shortening, good intermetatarsal reduction, and ease of rigid internal fixation, shaft osteotomies are gaining popularity as procedures for the correction of hallux valgus. Strong and stable internal fixation allows for earlier functional recovery and primary bone healing. Shaft osteotomies can provide predictable and rewarding results for the patient and surgeon for the correction of hallux valgus.

RESUMO

Most complications of calcaneal fractures are due to the combination of the dynamic nature and the mechanism of the injury itself. Complications are usually secondary to three integral parts of this complex fracture pattern: 1) fracture and depression of the subtalar joint, 2) loss of the height of the calcaneal body with varus rotation, and 3) expansion of the lateral wall of the calcaneus. Other complications occur with less frequency. This article describes the complications and sequelae of old, malunited intra-articular calcaneal fractures in relation to the nature and the components of these unique fractures.

RESUMO

A satisfactory outcome after isolated subtalar joint arthrodesis is dependent on proper patient selection, understanding the pathology and indications, and choosing the appropriate operative technique. Performing a thorough history and physical examination will assist in this process. Ancillary tests, including radiographs and advanced imaging, should be obtained to reinforce the surgeon's suspicions and to aid in the proper surgical planning for the patient. Also, a proper postoperative protocol should be used to allow for adequate healing, and,thus, optimize functional outcome.

RESUMO

Four different techniques for the fixation of an offset V bunionectomy were tested on solid-foam saw-bone models for the purpose of determining the strongest form of fixation for the osteotomy. Twenty identical models were placed into 4 different groups. Groups varied as to the placement and caliber of fixation. Models were loaded with a servo-hydraulic testing machine until failure of fixation occurred. Video analysis was used to record the pattern of failure of the fixation. Failure occurred either distal to the first screw, through the first screw hole, between the 2 screws, through the second screw hole, or proximal to the second screw. The mean force to failure of the groups was group 1, 58.1 N; group 2, 59.3 N; group 3, 64.0 N; and group 4, 105.66 N. There was a statistical significant difference between group 4 and the other 3 groups (F(1) = 55.45, P < 0.05). There was no statistical difference between groups 1 to 3. In groups 1 to 3, 87% of the failures were through the distal screw hole, whereas the remaining 13% were through the proximal screw hole. In group 4, 60% of the failures were through the proximal screw hole and 40% were through the distal screw hole. It was concluded that, in this model, the strongest form of fixation for an offset V osteotomy was the 2.7-mm cortical screw placed distally with the proximal point of fixation being a threaded 0.062-inch Kirschner wire.

RESUMO

The bacterium Aeromonas hydrophila is an anaerobic gram-negative bacillus commonly found in natural bodies of water and can cause infection in patients who suffer water-associated trauma or in immunocompromised hosts. The authors present 5 cases of penetrating wound trauma that did not involve any aquatic environment and developed rapidly forming infections. All patients presented with severe pain, cellulitis, ascending lymphangitis, fever, and pain on range of motion of the joint near the traumatic site. Presentation of clinical symptoms mimicked that of a septic joint or of severe streptococcal infection. All patients required surgical incision and drainage, intravenous and oral antibiotics using levofloxacin or bactrim, and local wound care. Results from cultures taken intraoperatively showed only A hydrophilia in every case. Resolution of symptoms occurred rapidly after surgery, and clinical resolution was seen within 72 hours. Each patient healed uneventfully and returned to preinjury status.
