RESUMO

Hydroxylated monoterpenes (HMTPs) are differentially emitted by tomato (Solanum lycopersicum) plants resisting bacterial infection. We have studied the defensive role of these volatiles in the tomato response to bacteria, whose main entrance are through stomatal apertures. Treatments with some HMTPs resulted in stomatal closure and pathogenesis-related protein 1 (PR1) induction. Particularly, α-terpineol induced stomatal closure in a salicylic acid (SA) and abscisic acid-independent manner and conferred resistance to bacteria. Interestingly, transgenic tomato plants overexpressing or silencing the monoterpene synthase MTS1, which displayed alterations in the emission of HMTPs, exhibited changes in the stomatal aperture but not in plant resistance. Measures of both 2-C-methyl-D-erythritol-2,4-cyclopyrophosphate (MEcPP) and SA levels revealed competition for MEcPP by the methylerythritol phosphate (MEP) pathway and SA biosynthesis activation, thus explaining the absence of resistance in transgenic plants. These results were confirmed by chemical inhibition of the MEP pathway, which alters MEcPP levels. Treatments with BTH, a SA functional analogue, conferred enhanced resistance to transgenic tomato plants overexpressing MTS1. Additionally, these MTS1 overexpressors induced PR1 gene expression and stomatal closure in neighbouring plants. Our results confirm the role of HMTPs in both intra and inter-plant immune signalling and reveal a metabolic crosstalk between the MEP and SA pathways in tomato plants.

BACKGROUND: Cell-free DNA (cfDNA) analysis has become a promising tool for the diagnosis, prognosis, and monitoring of lymphoma cases. Until now, research in this area has mainly focused on aggressive lymphomas, with scanty information from other lymphoma subtypes. METHODS: We selected 256 patients diagnosed with lymphomas, including a large variety of B-cell and T-cell non-Hodgkin and Hodgkin lymphomas, and quantified cfDNA from plasma at the time of diagnosis. We further selected 49 large B-cell lymphomas (LBCL) and analyzed cfDNA levels at diagnosis (pre-therapy) and after therapy. In addition, we performed NGS on cfDNA and tissue in this cohort of LBCL. RESULTS: Lymphoma patients showed a statistically significant higher cfDNA concentration than healthy controls (mean 53.0 ng/mL vs. 5.6 ng/mL, p < 0.001). The cfDNA concentration was correlated with lymphoma subtype, lactate dehydrogenase, the International Prognostic Index (IPI) score, Ann Arbor (AA), and B-symptoms. In 49 LBCL cases, the cfDNA concentration decreased after therapy in cases who achieved complete response (CR) and increased in non-responders. The median cfDNA at diagnosis of patients who achieved CR and later relapsed was higher (81.5 ng/mL) compared with levels of those who did not (38.6 ng/mL). A concordance of 84% was observed between NGS results in tumor and cfDNA samples. Higher VAF in cfDNA is correlated with advanced stage and bulky disease. CONCLUSIONS: cfDNA analysis can be easily performed in almost all lymphoma cases. The cfDNA concentration correlated with the characteristics of the aggressiveness of the lymphomas and, in LBCL, with the response achieved after therapy. These results support the utility of cfDNA analysis as a complementary tool in the management of lymphoma patients.

Serratia spp. is a well-recognized pathogen in neonates; however, limited data are available in adults. We studied microbiological and clinical characteristics of Serratia spp. causing bloodstream infections (BSI) in our institution (January 2005-July 2020). Overall, 141 BSI episodes affecting 139 patients were identified and medical records reviewed. Antimicrobial susceptibility was recovered from our informatics system and 118 isolates from 116 patients were available for further microbiological studies. Whole genome sequencing (WGS) was completed in 107 isolates. Incidence of Serratia BSI was 0.3/1000 overall admissions (range 0.12-0.60), with maximum prevalence (27 episodes, 19.1%) during 2017-2018. Relevant patients' clinical characteristics were 71.9% ≥60 years (n = 100), with high comorbidity rates (49%, ≥2), 23 (74.2%) of them died within 1 month of the BSI episode. WGS identified all isolates as Serratia marcescens when Kraken bioinformatics taxonomic tool was used despite some which were identified as Serratia nematodiphila (32/118) or Serratia ureilytica (5/118) by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. Nevertheless, when using MASH distance, Serratia nevei (63/107), S. ureilytica (38/107), and S. marcescens (6/107) were assigned. Carbapenemase (blaVIM-1) and extended-spectrum ß-lactases (ESBL) (blaSHV-12) genes were found in seven and three isolates, respectively, one of them expressing both genes. The worldwide-disseminated IncL/M scaffold plasmid was identified in six VIM producers. Four genotypes were established based on their virulence factors and resistome. Serratia spp. emerged as a relevant nosocomial pathogen causing BSI in elderly patients in our hospital, particularly in recent years with a remarkable increase in antibiotic resistance. ESBL and carbapenemases production related to plasmid dissemination are particularly noteworthy.IMPORTANCESerratia spp. is the third most frequent pathogen involved in outbreaks at neonatal facilities and is primarily associated with bacteremia episodes. In this study, we characterized all causing bloodstream infection (BSI) in patients admitted to our hospital during a 16-year period (2005-2020). Despite having no neonatal intensive care unit in our hospital, this study revealed that Serratia spp. is a relevant pathogen causing BSI in elderly patients with high comorbidity rates. A significant increase of antimicrobial resistance was detected over time, particularly in 2020 and coinciding with the coronavirus disease (COVID-19) pandemic and nosocomial spread of multidrug-resistant Serratia spp. isolates. extended-spectrum ß-lactases and carbapenemases genes associated with plasmid dissemination, typically detected in other Enterobacterales species, were also identified, reinforcing the role of Serratia spp. in the antimicrobial resistance landscape. Additionally, this work highlights the need to reclassify the species of Serratia, since discrepancies were observed in the identification when using different tools.

Carotenoids are health-promoting plastidial isoprenoids with essential functions in plants as photoprotectants and photosynthetic pigments in chloroplasts. They also accumulate in specialized plastids named chromoplasts, providing color to non-photosynthetic tissues such as flower petals and ripe fruit. Carotenoid accumulation in chromoplasts requires specialized structures and proteins such as fibrillins (FBNs). The FBN family includes structural components of carotenoid sequestering structures in chromoplasts and members with metabolic roles in chloroplasts and other plastid types. However, the association of FBNs with carotenoids in plastids other than chromoplasts has remained unexplored. Here, we show that Arabidopsis (Arabidopsis thaliana) FBN6 interacts with phytoene synthase (PSY), the first enzyme of the carotenoid pathway. FBN6, but not FBN4 (a FBN that does not interact with PSY), enhances the activity of plant PSY (but not of the bacterial PSY crtB) in Escherichia coli cells. Overexpression of FBN6 in Nicotiana benthamiana leaves results in a higher production of phytoene, the product of PSY activity, whereas loss of FBN6 activity in Arabidopsis mutants dramatically reduces the production of carotenoids during seedling de-etiolation and after exposure to high light. Our work hence demonstrates that FBNs promote not only the accumulation of carotenoids in chromoplasts but also their biosynthesis in chloroplasts.

Isoprenoids are a wide family of metabolites including high-value chemicals, flavors, pigments, and drugs. Isoprenoids are particularly abundant and diverse in plants. The methyl-D-erythritol 4-phosphate (MEP) pathway produces the universal isoprenoid precursors isopentenyl diphosphate and dimethylallyl diphosphate in plant plastids for the downstream production of monoterpenes, diterpenes, and photosynthesis-related isoprenoids such as carotenoids, chlorophylls, tocopherols, phylloquinone, and plastoquinone. The enzyme deoxy-D-xylulose 5-phosphate synthase (DXS) is the first and main rate-determining enzyme of the MEP pathway. In tomato (Solanum lycopersicum), a plant with an active isoprenoid metabolism in several tissues, three genes encode DXS-like proteins (SlDXS1 to 3). Here, we show that the expression patterns of the three genes suggest distinct physiological roles without excluding that they might function together in some tissues. We also confirm that SlDXS1 and 2 are true DXS enzymes, whereas SlDXS3 lacks DXS activity. We further show that SlDXS1 and 2 co-localize in plastidial speckles and that they can be immunoprecipitated together, suggesting that they might form heterodimers in vivo in at least some tissues. These results provide novel insights for the biotechnological use of DXS isoforms in metabolic engineering strategies to up-regulate the MEP pathway flux.

In humans, aggregation of polyglutamine repeat (polyQ) proteins causes disorders such as Huntington's disease. Although plants express hundreds of polyQ-containing proteins, no pathologies arising from polyQ aggregation have been reported. To investigate this phenomenon, we expressed an aggregation-prone fragment of human huntingtin (HTT) with an expanded polyQ stretch (Q69) in Arabidopsis thaliana plants. In contrast to animal models, we find that Arabidopsis sp. suppresses Q69 aggregation through chloroplast proteostasis. Inhibition of chloroplast proteostasis diminishes the capacity of plants to prevent cytosolic Q69 aggregation. Moreover, endogenous polyQ-containing proteins also aggregate on chloroplast dysfunction. We find that Q69 interacts with the chloroplast stromal processing peptidase (SPP). Synthetic Arabidopsis SPP prevents polyQ-expanded HTT aggregation in human cells. Likewise, ectopic SPP expression in Caenorhabditis elegans reduces neuronal Q67 aggregation and subsequent neurotoxicity. Our findings suggest that synthetic plant proteins, such as SPP, hold therapeutic potential for polyQ disorders and other age-related diseases involving protein aggregation.

BACKGROUND: Discrepancies between therapists' and patients' measures regarding therapeutic results indicate the need to analyze which symptoms and processes are being taken into consideration when reporting clinical change. This study analyzes the concordance between patient and therapist, at pre- and post-treatment, when reporting about anxiety, depression, Experiential Avoidance (EA), Cognitive Fusion (CF) and Activation (A). METHOD: Convergence was examined between information obtained by means of standardized measures and visual analogical scales (VAS) in 94 patients with anxiety and/or depression who participated in a controlled clinical study (TRANSACTIVA study). RESULTS: Statistically significant correlation ( p < .05) was found between all the measures of anxiety and depression, regardless of the source, timepoint, and measures procedure at 95% confidence. In the VAS, patient and therapist agreed ( p < .05) in their evaluation of specific symptoms. For EA, CF and A, the therapists' measures demonstrated stronger correlations than those of the patients, although, in each condition, all the patients' measures correlated with each other ( p < .05). CONCLUSIONS: Suitable agreement was found between therapist and patient when reporting clinical change. One-item VAS appeared to b suitable for identifying anxiety, depression and the transdiagnostic patterns of EA, CF and A.

OBJECTIVE: Zinc transporter 8 autoantibodies (ZNt8A) are 1 of the 4 main autoantibodies used for the diagnosis of type 1 diabetes (T1D), with glutamic acid decarboxylase autoantibodies (GADA), islet antigen-2 autoantibodies (IA-2A), and insulin autoantibodies (IAA). The objective of this study is to evaluate the diagnostic efficiency of these autoantibodies for the diagnosis of T1D in pediatric patients. METHODS: A retrospective analysis of patients under 16 years of age with suspected T1D was made between June 2020 and January 2021. A total of 80 patients were included in the study, with 1 sample per patient. Subjects were classified according to diagnosis. RESULTS: Of the subjects included in the study, 50 developed T1D. The diagnostic efficacy was IA-2A (cutoff ≥ 28 U/L) sensitivity 0.26 (95% CI: 0.14-0.38) and specificity 0.97 (95% CI: 0.79-1.0); GADA (cutoff ≥ 17 U/mL) sensitivity 0.40 (95% CI: 0.26-0.54) and specificity 0.87 (95% CI: 0.75-0.99); ZnT8A (cut off ≥ 15 U/L) sensitivity 0.62 (95% CI: 0.49-0.75) and specificity 0.97 (95% CI: 0.90-1.0). ZnT8A obtained the most significantly global diagnostic accuracy (0.75), and GADA with ZnT8A showed the highest correlation. CONCLUSION: The results obtained indicate a higher efficiency of anti-ZnT8 autoantibodies for the diagnosis of T1D in pediatric patients. Clinical efficiency of diabetic autoantibodies is method and assay dependent and influences combined diagnostic strategies.

Waldenström Macroglobulinemia (WM) is a lymphoplasmacytic lymphoma with bone marrow (BM) involvement and IgM monoclonal gammopathy. To date, no studies have focused specifically on peripheral blood (PB) involvement. In this study, 100 patients diagnosed with WM according to the World Health Organization (WHO) criteria were included based on the demonstration of MYD88mut in BM and the availability of PB multiparametric flow cytometry (MFC) analysis. Leukemic involvement by MFC was detected in 50/100 patients. A low percentage of mature small lymphocytes in PB smears was observed in only 15 cases. MYD88mut by AS-qPCR was detected in PB in 65/100 cases. In cases with leukemic expression by MFC, MYD88mut was detected in all cases, and IGH was rearranged in 44/49 cases. In 21/50 patients without PB involvement by MFC, molecular data were consistent with circulating disease (MYD88mut by AS-qPCR 3/50, IGH rearranged 6/50, both 12/50). Therefore, PB involvement by standard techniques was detected in 71/100 patients. MYD88mut was detected in PB by dPCR in 9/29 triple negative cases. Overall, 80% of the patients presented PB involvement by any technique. Our findings support the role of PB MFC in the evaluation of patients with IgM monoclonal gammopathy and provide reliable information on correlation with molecular features. The development of a feasible MFC assay may stand as an objective tool in the classification of mature B cell neoplasms presenting with IgM monoclonal gammopathy.

Variegation is a rare type of mosaicism not fully studied in plants, especially fruits. We examined red and white sections of grape (Vitis vinifera cv. 'Béquignol') variegated berries and found that accumulation of products from branches of the phenylpropanoid and isoprenoid pathways showed an opposite tendency. Light-responsive flavonol and monoterpene levels increased in anthocyanin-depleted areas in correlation with increasing MYB24 expression. Cistrome analysis suggested that MYB24 binds to the promoters of 22 terpene synthase (TPS) genes, as well as 32 photosynthesis/light-related genes, including carotenoid pathway members, the flavonol regulator HY5 HOMOLOGUE (HYH), and other radiation response genes. Indeed, TPS35, TPS09, the carotenoid isomerase gene CRTISO2, and HYH were activated in the presence of MYB24 and MYC2. We suggest that MYB24 modulates ultraviolet and high-intensity visible light stress responses that include terpene and flavonol synthesis and potentially affects carotenoids. The MYB24 regulatory network is developmentally triggered after the onset of berry ripening, while the absence of anthocyanin sunscreens accelerates its activation, likely in a dose-dependent manner due to increased radiation exposure. Anthocyanins and flavonols in variegated berry skins act as effective sunscreens but for different wavelength ranges. The expression patterns of stress marker genes in red and white sections of 'Béquignol' berries strongly suggest that MYB24 promotes light stress amelioration but only partly succeeds during late ripening.

Carotenoids are plastidial isoprenoids required for photoprotection and phytohormone production in all plants. In tomato (Solanum lycopersicum), carotenoids also provide color to flowers and ripe fruit. Phytoene synthase (PSY) catalyzes the first and main flux-controlling step of the carotenoid pathway. Three genes encoding PSY isoforms are present in tomato, PSY1 to PSY3. Mutants have shown that PSY1 is the isoform providing carotenoids for fruit pigmentation, but it is dispensable in photosynthetic tissues. No mutants are available for PSY2 or PSY3, but their expression profiles suggest a main role for PSY2 in leaves and PSY3 in roots. To further investigate isoform specialization with genetic tools, we created gene-edited lines defective in PSY1 and PSY2 in the MicroTom background. The albino phenotype of lines lacking both PSY1 and PSY2 confirmed that PSY3 does not contribute to carotenoid biosynthesis in shoot tissues. Our work further showed that carotenoid production in tomato shoots relies on both PSY1 and PSY2 but with different contributions in different tissues. PSY2 is the main isoform for carotenoid biosynthesis in leaf chloroplasts, but PSY1 is also important in response to high light. PSY2 also contributes to carotenoid production in flower petals and, to a lesser extent, fruit chromoplasts. Most interestingly, our results demonstrate that fruit growth is controlled by abscisic acid (ABA) specifically produced in the pericarp from PSY1-derived carotenoid precursors, whereas PSY2 is the main isoform associated with ABA synthesis in seeds and salt-stressed roots.

BACKGROUND: Isoprenoids are a very large class of metabolites playing a key role in plant physiological processes such as growth, stress resistance, fruit flavor, and color. In chloroplasts and chromoplasts, the diterpene compound geranylgeranyl diphosphate (GGPP) is the metabolic precursor required for the biosynthesis of tocopherols, plastoquinones, phylloquinone, chlorophylls, and carotenoids. Despite its key role for the plant metabolism, reports on GGPP physiological concentrations in planta have been extremely scarce. RESULTS: In this study, we developed a method to quantify GGPP and its hydrolysis product geranylgeranyl monophosphate (GGP) from tomato fruit, using ultra-high performance liquid chromatography coupled with tandem mass spectrometry (UHPLC-MS/MS). Quantification was done by external calibration and the method was validated in terms of specificity, precision, accuracy, and detection and quantitation limits. We further demonstrate the validity of our approach by analysing GGPP contents in the ripe fruits of wild-type tomatoes and mutants defective in GGPP production. Finally, we also show that the sample preparation is key to prevent GGPP hydrolysis and mitigate its conversion to GGP. CONCLUSION: Our study provides an efficient tool to investigate the metabolic fluxes required for GGPP supply and consumption in tomato fruit.

Shade tolerance is an ecological concept used in a wide range of disciplines, from plant physiology to landscaping or gardening. It refers to the strategy of some plants to persist and even thrive in environments with low light levels because of the shade produced by the vegetation proximity (e.g. in the understory). Shade tolerance influences the organization, structure, functioning, and dynamics of plant communities. However, little is known about its molecular and genetic basis. By contrast, there is a good understanding on how plants deal with the proximity of other plants, a divergent strategy used by most crops to respond to vegetation proximity. While generally shade-avoiding species strongly elongate in response to the proximity of other plants, shade-tolerant species do not. Here we review the molecular mechanisms that control the regulation of hypocotyl elongation in shade-avoiding species as a reference framework to understand shade tolerance. Comparative studies indicate that shade tolerance is implemented by components also known to regulate hypocotyl elongation in shade-avoiding species. These components, however, show differential molecular properties that explain how, in response to the same stimulus, shade-avoiding species elongate while shade-tolerant ones do not.

BACKGROUND: the success of strategies with earlier anti-TNF drugs for the treatment of inflammatory bowel disease (IBD) have been shadowed by the development of anti-drug antibodies that reduce their effectiveness. The HLA-DQA1*05 allele has been shown to increase the risk of immunogenicity to anti-TNF drugs by approximately two-fold. The negative impact of this allele has not been fully investigated for newer biotherapies. OBJECTIVE: whether the presence of the HLA-DQA1*05 allele is associated with a reduction of response to ustekinumab and vedolizumab was investigated. MATERIAL AND METHODS: the impact of HLA-DQA1*05 on disease activity in 93 patients with IBD, treated with ustekinumab (n = 39) or vedolizumab (n = 54) was investigated in a retrospective cohort study. Treatment response and remission was assessed at 6 and 12 months for ustekinumab, and up to 18 and 24 months for vedolizumab, using Harvey-Bradshaw index (Crohn's disease) and Mayo score (ulcerative colitis). RESULTS: the HLA-DQA1*05 allele was found in 35.9 % and 38.9 % of patients treated with ustekinumab and vedolizumab, respectively. Clinical response was not affected by the presence of the HLA-DQA1*05 allele for both treatment groups. CONCLUSIONS: in contrast to anti-TNF drugs, HLA-DQA1*05 presence does not correlate with the decreased response to ustekinumab or vedolizumab.

Carotenoids are photoprotectant pigments and precursors of hormones such as strigolactones (SL). Carotenoids are produced in plastids from geranylgeranyl diphosphate (GGPP), which is diverted to the carotenoid pathway by phytoene synthase (PSY). In tomato (Solanum lycopersicum), three genes encode plastid-targeted GGPP synthases (SlG1 to SlG3) and three genes encode PSY isoforms (PSY1 to PSY3). Here, we investigated the function of SlG1 by generating loss-of-function lines and combining their metabolic and physiological phenotyping with gene co-expression and co-immunoprecipitation analyses. Leaves and fruits of slg1 lines showed a wild-type phenotype in terms of carotenoid accumulation, photosynthesis, and development under normal growth conditions. In response to bacterial infection, however, slg1 leaves produced lower levels of defensive GGPP-derived diterpenoids. In roots, SlG1 was co-expressed with PSY3 and other genes involved in SL production, and slg1 lines grown under phosphate starvation exuded less SLs. However, slg1 plants did not display the branched shoot phenotype observed in other SL-defective mutants. At the protein level, SlG1 physically interacted with the root-specific PSY3 isoform but not with PSY1 and PSY2. Our results confirm specific roles for SlG1 in producing GGPP for defensive diterpenoids in leaves and carotenoid-derived SLs (in combination with PSY3) in roots.

Introducción: La enfermedad traumática grave es la sexta causa de muerte a nivel mundial, un elevado porcentaje de fallecidos se atribuye a hemorragias no controladas. En Cuba se carece de estudios sobre variables predictoras de mortalidad precoz en pacientes hospitalizados con trauma hemorrágico. Objetivo: Identificar variables predictoras de mortalidad precoz en pacientes hospitalizados con trauma hemorrágico. Métodos: Se realizó un estudio observacional, analítico, transversal, que incluyó 207 pacientes con trauma hemorrágico, en el Hospital General Provincial "Camilo Cienfuegos" de Sancti Spíritus, En el período comprendido entre los años 2012 y 2017. Las variables se agruparon en sociodemográficas, enfermedades crónicas, mecanismo lesional, tipo de trauma, localización topográfica, complicaciones precoces, tratamiento médico-quirúrgico y mortalidad precoz. Se elaboró un modelo de regresión logística binaria mediante el método hacia delante de Wald. Resultados: El porcentaje global del modelo de regresión logística mostró la relación entre lo pronosticado respecto a lo observado en un 94,6 %. Los predictores explicaron el 83,8 % de la variabilidad de la variable dependiente. Se determinaron cinco variables predictoras de mortalidad precoz, hipertensión arterial, enfermedad pulmonar obstructiva crónica, hepatopatía crónica, coagulopatía aguda e hipotermia. Conclusiones: Se concluye que las enfermedades crónicas como la hipertensión arterial, la enfermedad pulmonar obstructiva crónica y las hepatopatías crónicas, asociadas a la coagulopatía aguda e hipotermia fueron identificadas como variables predictoras de muerte precoz en pacientes hospitalizados por trauma hemorrágico.


Introduction: Severe traumatic disease is the sixth leading cause of death worldwide, high percentage of deaths is ascribed to uncontrolled bleeding. There are no studies in Cuba on predictive variables of early mortality in hospitalized patients with hemorrhagic trauma. Objective: To identify predictive variables of early mortality in hospitalized patients with hemorrhagic trauma. Methods: An observational, analytical, cross-sectional study was carried out, which included 207 patients with hemorrhagic trauma, at Camilo Cienfuegos Provincial General Hospital in Sancti Spíritus, from 2012 to 2017. The variables were grouped into sociodemographic, diseases chronic, lesion mechanism, type of trauma, topographic location, early complications, medical-surgical treatment and early mortality. A binary logistic regression model was developed using the Wald forward method. Results: The global percentage of the logistic regression model showed the relationship between what was predicted with respect to what was observed in 94.6%. The predictors explained 83.8% of the variability of the dependent variable. Five predictors of early mortality, arterial hypertension, chronic obstructive pulmonary disease, chronic liver disease, acute coagulopathy, and hypothermia were determined. Conclusions: It is concluded that chronic diseases such as arterial hypertension, chronic obstructive pulmonary disease and chronic liver disease, associated with acute coagulopathy and hypothermia, were identified as predictors of early death in patients hospitalized for hemorrhagic trauma.

Introducción: El choque hemorrágico traumático es una de las principales causas de muerte en pacientes con trauma grave. Objetivo: Describir las características de los pacientes con choque hemorrágico traumático. Métodos: Estudio observacional, descriptivo, transversal, que incluyó 207 pacientes con choque hemorrágico traumático. Para la descripción de las características de los pacientes se consideraron variables sociodemográficas, clínicas y quirúrgicas. Se realizó un análisis de frecuencias, con un nivel de significación de p< 0,05 y para las variables cuantitativas se estimó la media, desviación típica. Resultados: Hubo predominio del sexo masculino (85 % de los pacientes), con una edad media de 43,7 ± 15,7 años. El 42,5 % (p= 0,000) de los pacientes eran hipertensos. Predominaron los accidentes de tránsito (63,2 %), los traumas contusos (57,5 %) y politraumatizados (42,5 %). La acidosis metabólica estuvo presente en 66,7 % (p= 0,000) de los pacientes. La media del tiempo entre ingreso y tratamiento definitivo fue de 3,52 ± 1,19 horas El tratamiento médico quirúrgico de control de daño se aplicó en el 2,9 % y 5,4 % de los pacientes respectivamente. La muerte precoz fue de 30 %. Conclusiones: Predominaron los pacientes masculinos, menores de 60 años, las complicaciones precoces que tuvieron significación estadística y el tiempo entre ingreso y comienzo del tratamiento definitivo de 3 horas y más. El tratamiento médico quirúrgico de control de daño no se aplicó con frecuencia y la muerte precoz fue elevada.


Introduction: Traumatic hemorrhagic shock is one of the main causes of death in patients with severe trauma. Objective: To describe the characteristics of patients with traumatic hemorrhagic shock. Methods: Observational, descriptive, cross-sectional study, which included 207 patients with traumatic hemorrhagic shock. To describe the characteristics of the patients, sociodemographic, clinical, and surgical variables were considered. A frequency analysis was performed, with a significance level of p<0.05 and for the quantitative variables the mean and standard deviation were estimated. Results: There was a predominance of the male sex (85% of the patients), with a mean age of 43.7 ± 15.7 years. 42.5% (p= 0.000) of the patients were hypertensive. Traffic accidents (63.2%), blunt trauma (57.5%) and polytraumatized (42.5%) predominated. Metabolic acidosis was present in 66.7% (p= 0.000) of the patients. The mean time between admission and definitive treatment was 3.52 ± 1.19 hours. Surgical medical treatment for damage control was applied in 2.9% and 5.4% of the patients, respectively. Early death was 30%. Conclusions: There was a predominance of male patients, under 60 years of age, more, and early complications that had statistical significance and the time between admission and the start of definitive treatment of 3 hours. Damage control surgical medical treatment was not frequently applied and early death was high.

Introducción: La enfermedad pulmonar obstructiva crónica es un problema de salud; constituye la tercera causa de muerte en el mundo, la exacerbación de esta enfermedad es responsable del fallecimiento de un elevado porcentaje de pacientes. Objetivo: Identificar factores de riesgo de mortalidad en pacientes hospitalizados con enfermedad pulmonar obstructiva crónica exacerbada. Métodos: Se realizó una investigación transversal, analítica, en el Hospital General Provincial "Camilo Cienfuegos" de Sancti Spíritus, durante dos años. Se incluyeron 335 pacientes. Se estudiaron variables sociodemográficas y clínicas. Para la identificación de factores de riesgo se elaboró un modelo de regresión logística binaria mediante el método hacia delante de Wald. Resultados: En el modelo de regresión logística el índice de porcentaje global que mostró la relación entre lo pronosticado respecto a lo observado fue de un 94,6 %. Los predictores utilizados explicaron el 86,3 % de la variabilidad de la variable dependiente. El análisis estadístico permitió identificar factores de riesgo de mortalidad: insuficiencia respiratoria aguda, diagnóstico de neumonía, tromboembolismo pulmonar, edad mayor de 60 años y adicción tabáquica. Conclusiones: La identificación de cinco factores de riesgo de mortalidad en pacientes hospitalizados por enfermedad pulmonar obstructiva crónica exacerbada, con el empleo de un modelo predictivo para el cálculo de la probabilidad de fallecimiento; contribuyó al perfeccionamiento de la atención médica individualizada, en función de minimizar la ocurrencia de eventos fatales en estos pacientes.


Introduction: Chronic obstructive pulmonary disease is a health problem; It is the third cause of death in the world, the exacerbation of this disease is responsible for the death of a high percentage of patients. Objective: To identify risk factors for mortality in hospitalized patients with exacerbated chronic obstructive pulmonary disease. Methods: A cross-sectional, analytical investigation was carried out, based on the epidemiological design of correlational studies, at the "Camilo Cienfuegos" Provincial General Hospital of Sancti Spíritus, for two years. 335 patients were included. Sociodemographic and clinical variables were studied. To identify risk factors, a binary logistic regression model was developed using the Wald forward method. Mortality during hospitalization for exacerbated COPD was considered the dependent variable. Results: In the logistic regression model, the global percentage index that showed the relationship between what was predicted with respect to what was observed was 94, 6 %. The predictors used explained 86, 3 % of the variability of the dependent variable. Statistical analysis allowed us to identify the following risk factors for mortality: acute respiratory failure, diagnosis of pneumonia, pulmonary thromboembolism, age over 60 years, and smoking addiction. Conclusions: The identification of five mortality risk factors in patients hospitalized for exacerbated COPD, with the use of a predictive model to calculate the probability of death; contributes to the improvement of individualized medical care, in order to minimize the occurrence of fatal events in these patients.

The enrichment of plant tissues in tocochromanols (tocopherols and tocotrienols) is an important biotechnological goal due to their vitamin E and antioxidant properties. Improvements based on stimulating tocochromanol biosynthesis have repeatedly been achieved, however, enhancing sequestering and storage in plant plastids remains virtually unexplored. We previously showed that leaf chloroplasts can be converted into artificial chromoplasts with a proliferation of plastoglobules by overexpression of the bacterial crtB gene. Here we combined coexpression of crtB with genes involved in tocopherol biosynthesis to investigate the potential of artificial leaf chromoplasts for vitamin E accumulation in Nicotiana benthamiana leaves. We show that this combination improves tocopherol levels compared to controls without crtB and confirm that VTE1, VTE5, VTE6 and tyrA genes are useful to increase the total tocopherol levels, while VTE4 further leads to enrichment in α-tocopherol (the tocochromanol showing highest vitamin E activity). Additionally, we show that treatments that further promote plastoglobule formation (e.g., exposure to intense light or dark-induced senescence) result in even higher improvements in the tocopherol content of the leaves. An added advantage of our strategy is that it also results in increased levels of other related plastidial isoprenoids such as carotenoids (provitamin A) and phylloquinones (vitamin K1).
