Antecedentes La satisfacción laboral, el burnout y la calidad de vida profesional de los fisioterapeutas es clave para mantener la calidad de los servicios prestados en los pacientes. Objetivos Describir la satisfacción laboral, el burnout y la calidad de vida profesional de los fisioterapeutas de Atención Hospitalaria de la Sanidad pública de Castilla y León (SACYL); identificar las diferencias existentes en la satisfacción laboral, el burnout y el nivel de calidad de vida profesional en los mismos; determinar la relación existente entre las tres variables. Materiales y métodos Se realizó un estudio descriptivo transversal, con muestreo de conveniencia, en fisioterapeutas de Castilla y León que trabajan en atención hospitalaria. Se recogieron datos de la satisfacción laboral (Font Roja), el burnout (Maslach Burnout Inventory) y la calidad de vida profesional (CVP-35) en su puesto de trabajo. Se llevó a cabo un análisis descriptivo y relacional de las variables analizadas. Resultados Participaron 72 fisioterapeutas, siendo 75% mujeres, con una media de edad de 42,8 ± 8,37 años. El nivel de burnout resultó medio-alto en las dimensiones de agotamiento emocional y despersonalización, bajo en realización personal y la satisfacción laboral baja (SMG = -0,04). Los valores de calidad de vida profesional alcanzan una puntuación de notable (7,15 ± 1,43). Conclusiones Los fisioterapeutas de atención hospitalaria del SACYL presentan una elevada motivación intrínseca, a pesar del escaso apoyo directivo que reciben. La mejora de ambas dimensiones conlleva un aumento de la satisfacción laboral y de la calidad de vida profesional, y un descenso de los niveles de burnout (especialmente en el agotamiento emocional y la despersonalización). Materials and methods A cross-sectional descriptive study was carried out, with convenience sampling, in physiotherapists from Castilla y León working in hospital care. Data were collected on job satisfaction (Font Roja), burnout (Maslach Burnout Inventory) and professional quality of life (CVP-35) in the workplace. A descriptive and relational analysis of the variables analysed was carried out. Results Seventy-two physiotherapists participated, 75% of whom were women, with an average age of 42.8 ± 8.37 years. The level of burnout was medium-high in the dimensions of emotional exhaustion and depersonalisation, low in personal fulfilment and low in job satisfaction (SMG = −0.04). Professional quality of life values reach a score of remarkable (7.15 ± 1.43). Conclusions Hospital care physiotherapists of the SACYL present high intrinsic motivation, despite the low managerial support they receive. The improvement of both dimensions leads to an increase in job satisfaction and professional quality of life, and a decrease in burnout levels (especially in emotional exhaustion and depersonalisation). Objetivo La relación terapéutica (RT) es determinante para el desarrollo y los resultados terapéuticos en fisioterapia. Se precisa formación del personal sanitario en métodos para llevarla a cabo. No se han encontrado publicaciones sobre la repercusión de este tipo de formación en la RT entre fisioterapeutas y pacientes. El objetivo fue evaluar las competencias de los fisioterapeutas españoles para el establecimiento de la RT y determinar si la realización de formación específica en RT influye en dicha relación. Métodos Estudio descriptivo transversal mediante encuesta electrónica a fisioterapeutas colegiados en España. Se incluyeron datos sociodemográficos, el cuestionario Working Alliance Inventory-Short (WAI-S) compuesto por 3 subescalas: establecimiento del vínculo y acuerdo en objetivos y tareas entre fisioterapeuta y paciente, y una pregunta dicotómica sobre la formación específica en RT de más de 5horas de duración. Resultados Participaron 264 fisioterapeutas. La puntuación más alta se obtuvo en la subescala vínculo y la más baja en metas. Las mujeres obtuvieron mayores puntuaciones en la subescala vínculo. Los fisioterapeutas con formación puntuaron más en todas las subescalas, y significativamente en la subescala metas. Una proporción mayor de mujeres realizó formación específica. Existe asociación directa entre la experiencia y las subescalas vínculo y tareas. No se encontraron diferencias significativas en los resultados del WAI-S en función de ninguna de las variables laborales analizadas. Conclusiones Los fisioterapeutas españoles tienen dificultades para establecer objetivos conjuntos. La formación específica en RT mejora todos los componentes del WAI-S, sobre todo la instauración conjunta de objetivos. Mayor experiencia profesional y ser mujer influye positivamente en una mejor RT (AU)


Objective The therapeutic relationship (RT) is determinant for the development and therapeutic results in physiotherapy. Training of health personnel in methods to carry it out is required. No publications have been found on the impact of this type of training on the RT between physiotherapists and patients. To evaluate the competencies of Spanish physiotherapists in establishing RT and to determine whether specific training in RT influences this relationship. Methods Cross-sectional descriptive study by means of an electronic survey of registered physiotherapists in Spain. We included sociodemographic data, the Working Alliance Inventory-Short (WAI-S) questionnaire composed of three subscales: establishment of the BOND and agreement on OBJETIVES and TASKS between physiotherapist and patient, and a dichotomous question on specific training in RT of more than 5h duration. Results A total of 264 physical therapists participated. The highest score was obtained in the subscale BOND, the lowest in GOAL. Women obtained higher scores on the BOND subscale. Physiotherapists with training scored higher on all subscales, and significantly higher on the GOAL subscale. A higher proportion of women undertook specific training. There was a direct association between experience and the subscales BOND and TASK. No significant differences were found in WAI-S scores as a function of any of the work variables analyzed. Conclusions Spanish physiotherapists have difficulties in establishing joint objectives. Specific training in RT improves all the components of the WAI-S, especially the joint establishment of objectives. More professional experience and being a woman have a positive influence on a better RT (AU)

BACKGROUND: There have been few studies carried out into empathy in physiotherapists. Burnout can debilitate the quality of care and the efficacy of treatment as the empathetic capacity of the professional diminishes. OBJECTIVE: The objective of the study was to examine the association between the construct burnout, empathy and sociodemographic aspects in Spanish physiotherapists. METHODS: A cross-sectional electronic survey including the Maslach Burnout Inventory (for burnout assessment), the Interpersonal Reactivity Index (for empathy assessment) and sociodemographic data was answered by 461 Spanish physical therapists. A descriptive, bivariate and lineal regression analysis was performed. RESULTS: There was an association between burnout and empathy. Specifically, higher levels of burnout are associated with lower levels of empathy, whilst years of work experience is associated with lower levels of burnout. CONCLUSIONS: The results of this study contribute to a greater understanding of the relationship between the level of burnout and the dimensions of empathy in physiotherapists. The influence of burnout, which causes difficulties in the mobilization of the professional towards the establishment of a quality therapeutic relationship, is highlighted.KEY MESSAGESEmotional exhaustion is associated to greater personal discomfort and less empathy.The depersonalization is associated to personal discomfort and less empathy.Depersonalization is negatively associated to the lack of perspective.

Antecedentes El dolor crónico afecta a alrededor de un 20% de la población en países desarrollados. Uno de cada cinco europeos lo sufre, alterando la vida del individuo, deteriorando su salud y siendo fuente de discapacidad. Existe una mayor incidencia del dolor crónico sobre el sexo femenino y la zona lumbar. Actualmente, la fisioterapia propone como tratamientos la educación en neurociencia del dolor (PNE) y el ejercicio terapéutico (TE). Objetivo Valorar la efectividad de la PNE aislada o combinada con TE respecto al dolor y sus variables clínicas más relevantes en pacientes con dolor lumbar crónico (CLBP). Material y métodos Se realizó una revisión sistemática en las bases de datos PubMed, Cochrane Library, ScienceDirect, Web of Science y Scopus entre enero y marzo del 2020. Se incluyeron ensayos clínicos aleatorizados (RCT) con seis o más puntos en la escala PEDro, publicados en 2004 o posterior con participantes mayores de 18 años diagnosticados con CLBP de tres o más meses de duración, sin dolor secundario a cirugías, cáncer o fracturas. Resultados Se seleccionaron ocho artículos. Tres ensayos concluyen que la PNE en solitario es efectiva a corto plazo, reduciendo la intensidad de dolor, uno la discapacidad y dos la kinesiofobia. Dos de ellos muestran que el TE es eficaz disminuyendo la intensidad del dolor y la discapacidad, y uno la kinesiofobia a corto y mediano plazo. Conclusión Parece ser que la PNE en solitario obtiene resultados significativos a corto plazo que no se mantienen en el tiempo. En el caso de PNE junto a TE la mejoría es más lenta, pero tiende a mantenerse a largo plazo. La intervención conjunta parece ser más efectiva. Una correcta diferenciación de las características de cada paciente y la individualización del tratamiento aumentan la efectividad de las intervenciones (AU)


Background Around 20% of the population in developed countries suffer from chronic pain, which can alter their lives and adversely affect their health becoming a source of disability. Chronic pain has a greater incidence in the female sex and the low back region. Physiotherapists currently propose pain neuroscience education (PNE) and therapeutic exercise (TE) as treatment. Objective to assess the effectiveness of PNE alone or combined with TE in pain and its most relevant clinical variables in chronic low back pain (CLBP) patients. Materials and methods a systematic review of articles was performed in different databases (PubMed, Cochrane Library, ScienceDirect, Web of Science and Scopus) from January to March 2020. Only randomized controlled trials with more than five points in the PEDro scale, published between 2004 and 2020 were included. Participants had to be over 18 years old and diagnosed with chronic low back pain of at least three months’ duration, and without a history of cancer, surgeries, or fractures as the source of pain. Results Eight articles were selected. PNE alone appears to be is effective in the short-term reducing pain intensity, disability and kinesiophobia. Two articles show effectiveness of TE in the short- and medium-term reducing pain intensity and disability, one of them also shows decreased kinesiophobia levels. Conclusion PNE alone seems to be more effective in the short term, not so in the long term. Slower improvements in the combined intervention of PNE and TE are maintained in the long term. Combined intervention seems to be the most effective. In addition, a correct differentiation of patients’ characteristics and treatment individualization improve the effectiveness of the intervention (AU)

OBJECTIVES: This study sought to develop a tool for evaluating person-centered therapeutic relationships within physiotherapy services, and to examine the content validity of the same. METHODS: A mixed qualitative and quantitative study was performed in three distinct phases: 1) the items were generated based on a literature review and a content analysis of focus groups of patients and physiotherapists; 2) an e-Delphi survey process was performed based on three rounds to select and refine the proposed questionnaire; 3) two rounds of cognitive interviews were conducted to evaluate the comprehension of items, the clarity of language and the appropriateness and relevance of content. RESULTS: Thirty-one items were generated based on the seven domains identified after the analysis of four focus groups of physiotherapists and four patient focus groups. Nine experts participated in the e-Delphi survey. Fifty-five patients participated in the two rounds of the cognitive pre-tests. Participating patients were from public and private physical therapy services. Based on the participants' suggestions, four items were removed, and four were added, whereas 16 were reworded. CONCLUSIONS: The final tool comprised 31 items divided into seven domains. The response format was based on a 5-point Likert frequency scale. The response options ranged from "strongly agree" to "strongly disagree".
