BACKGROUND: The increasing use of intracardiac echocardiography (ICE) in the ablation of premature ventricular complexes (PVCs) has raised questions about its true efficacy and safety. METHODS: This retrospective study collected the periprocedural complications and PVC burden post ablation. The risk factors of PVC recurrence was further explored. RESULTS: The study included patients treated without ICE (control group, n = 451) and with ICE (ICE group, n = 155) from May 2019 to July 2022. The ICE group demonstrated significantly lower fluoroscopy times and X-ray doses. There were no major complications in the ICE group, and the difference in the occurrence of periprocedural complications between the groups was not statistically significant (p = 0.072). The long-term success rates were similar for the control and ICE groups (89.6% and 87.1%, respectively). The origin of PVCs was identified as the independent factor for ablation success. CONCLUSIONS: The use of ICE did not confer an advantage with regard to long-term success in PVCs ablation. To thoroughly evaluate the safety and effectiveness of ICE in PVCs ablation, a prospective, multicenter, randomized study is warranted.

The re-mobilization risks of potentially toxic elements (PTEs) during stabilization deserve to be considered. In this study, artificial simulation evaluation methods based on the environmental stress of freeze-thaw (F-T), acidification and variable pH were conducted to assess the long-term effectiveness of PTEs stabilized by MgO in Pb/Zn smelter contaminated soils. Among common stabilizing materials, MgO was considered as the best remediation material, since PTEs bioavailability reduced by 55.48% for As, 19.58% for Cd, 10.57% for Cu, and 26.33% for Mn, respectively. The stabilization effects of PTEs by MgO were best at the dosage of 5 wt%, but these studied PTEs would re-mobilize after 30 times F-T cycles. Acid and base buffering capacity results indicated that the basicity of contaminated soils with MgO treatment reduced under F-T action, and the leached PTEs concentrations would exceed the safety limits of surface water quality standard in China (GB3838-2002) after acidification of 2325 years. No significant changes were found in the pH-dependent patterns of PTEs before and after F-T cycles. However, after F-T cycles, the leaching concentrations of PTEs increased due to the destruction of soil microstructure and the functionality of hydration products formed by MgO, as indicated by scanning electron microscopy (SEM) coupled with energydispersive Xray spectroscopy (EDS) results. Hence, these findings would provide beneficial references for soil remediation assessments of contaminated soils under multi-environmental stress.

Urbanization has been considered a driver of PM2.5 pollution and the attributed health burden. This study systematically measured the spatiotemporal and urban-rural heterogeneity of PM2.5-attributed health burden drivers, including income, population, baseline mortality rate, and PM2.5 level. The results reveal the significantly positive contribution of disposable income and the periodical and urban-rural differentiation of population contribution to PM2.5-attributed health burden. The difference in driver performance due to socioeconomic development and urbanization stages might be an important determinant for different or even opposite results of previous studies. Policymaking for mitigating PM2.5-attributed health risk could incorporate the re-assessment and driver determination for PM2.5-attributed health burden into the construction and development plan from the overall urbanization perspective. The urbanization-perspective driver decomposition could be synergized with the flow analysis, equality evaluation, and policy benefit estimation to achieve further direction-determining and quantitative assessment of the urban-rural PM2.5 health risk management strategies.

Chromium (Cr) is a highly toxic element, which is widely present in environment due to industrial activities. One of most applicable technique to clean up Cr pollution is chemical reduction. However, the Cr(VI) concentration in soil increases again after remediation, and meanwhile the yellow soil would appear, which is commonly called as "yellowing" phenomenon. To date, the reason behind the phenomenon has been disputed for decades. This study aimed to introduce the possible "yellowing" mechanism and the influencing factors based on the extensive literature review. In this work, the concept of "yellowing" phenomenon was explained, and the most potential reasons include the reoxidation of manganese (Mn) oxides and mass transfer were summarized. Based on the reported finding and results, the large area of "yellowing" is likely to be caused by the re-migration of Cr(VI), since it could not sufficiently contact with the reductant under the effects of the mass transfer. In addition, other driving factors also control the occurrence of "yellowing" phenomenon. This review provides valuable reference for the academic peers participating in the Cr-contaminated sites remediation.

OBJECTIVE: To analyze the efficacy of Biejiajian Pill (BJJP) on intestinal microbiota in patients with hepatitis B cirrhosis/liver fibrosis, and explore its relationship with liver fibrosis. METHODS: This was a prospective, randomized double-blind controlled trial. Using the stratified block randomization method, 35 patients with hepatitis B liver cirrhosis/liver fibrosis were randomly assigned (1:1) to receive entecavir (0.5 mg/d) combined with BJJP (3 g/time, 3 times a day) or placebo (simulator as control, SC group, simulator 3 g/time, 3 times a day) for 48 weeks. Blood and stool samples were collected from patients at baseline and week 48 of treatment, respectively. Liver and renal functions as well as hematological indices were detected. Fecal samples were analyzed by 16S rDNA V3-V4 high-throughput sequencing, and intestinal microbiota changes in both groups before and after treatment were compared, and their correlations with liver fibrosis were analyzed. RESULTS: Compared with the SC group, there was no significant difference in liver function, renal function and hematology indices in the BJJP group, however, the improvement rate of liver fibrosis was higher in the BJJP group (94.4% vs. 64.7%, P=0.041). Principal coordinate analysis (PCoA) based on weighted Unifrac distance showed significant differences in intestinal microbiota community diversity before and after BJJP treatment (P<0.01 and P=0.003), respectively. After 48 weeks' treatment, the abundance levels of beneficial bacteria (Bifidobacteria, Lactobacillus, Faecalibacterium and Blautia) increased, whereas the abundance levels of potential pathogenic bacteria, including Escherichia coli, Bacteroides, Ruminococcus, Parabacteroides and Prevotella decreased, among which Ruminococcus and Parabacteroides were significantly positively correlated with degree of liver fibrosis (r=0.34, P=0.04; r=0.38, P=0.02), respectively. The microbiota in the SC group did not change significantly throughout the whole process of treatment. CONCLUSION: BJJP had a certain regulatory effect on intestinal microbiota of patients with hepatitis B cirrhosis/liver fibrosis (ChiCTR1800016801).

OBJECTIVES: The catheter ablation of ventricular arrhythmias (VAs) arising from the left ventricular (LV) papillary muscles (PMs) is challenging. This study sought to address whether the combination of intracardiac echocardiography (ICE) and contact force sensing (CFS) can improve the acute and long-term ablation outcomes of left ventricular papillary muscle arrhythmias. METHODS AND RESULTS: From May 2015 to August 2022, a total of thirty-three patients underwent catheter ablation for LV PM arrhythmias: VAs were located in anterolateral PMs in 11 and posteromedial PMs in 22. A combination of intracardiac echocardiography (ICE) and contact force sensing (CFS) was used in 21 of the 33 procedures. A mean of 6.93 ± 4.91 for lesions was used per patient, comparable between the CFS/ICE and no ICE/CFS (4.90 ± 2.23 vs. 10.17 ± 5.89; p = 0.011). The mean CF achieved in the ICE/CFS group was 7.52 ± 3.31 g. Less X-ray time was used in the combination group (CFS/ICE: 165.67 ± 47.80 S vs. no ICE/CFS: 365.00 ± 183.73 S; p < 0.001). An acute success rate of 100% was achieved for the ICE/CFS group (n = 22) and 66.67% for the no ICE/CFS group (n = 8). VA recurrence at the 11.21 ± 7.21-month follow-up was 14.2% for the ICE/CFS group and 50% for the no ICE/CFS group (p = 0.04). No severe complications occurred in all patients. CONCLUSIONS: The combination of intracardiac echocardiography (ICE) and contact force sensing (CFS) could provide precise geometries of cardiac endocavitary structures and accurate contact information for the catheter during ablation, which improved acute and long-term ablation outcomes. The routine adoption of this strategy should be considered to improve the outcomes of LV PM VA ablation.

Background: We aimed to investigate perineal nerve block versus periprostatic block in pain control for men undergoing a transperineal prostate biopsy. Methods: In this prospective, randomised, blinded and parallel-group trial, men in six Chinese hospitals with suspected prostate cancer were randomly assigned (1:1) at the point of local anaesthesia to receive a perineal nerve block or periprostatic block and followed by a transperineal prostate biopsy. Centres used their usual biopsy procedure. Operators who performed anaesthesia were trained in both techniques before the trial and were masked to the randomised allocation until the time of anaesthesia and were not involved in the subsequent biopsy procedure and any assessment or analysis. Other investigators and the patients were masked until trial completion. The primary outcome was the level of the worst pain experienced during the prostate biopsy procedure. Secondary outcomes included pain (post-biopsy at 1, 6 and 24 h), changes in blood pressure, heart rate and breathing rate during the biopsy procedure, external manifestations of pain during biopsy, anaesthesia satisfaction, the detection rate of PCa and clinically significant PCa. This trial is registered on ClinicalTrials.gov, NCT04501055. Findings: Between August 13, 2020, and July 20, 2022, 192 men were randomly assigned to perineal nerve block or periprostatic block, 96 per study group. Perineal nerve block was superior for the relief of pain during the biopsy procedure (mean 2.80 for perineal nerve block and 3.98 for periprostatic block; adjusted difference in means -1.17, P < 0.001). Although the perineal nerve block had a lower mean pain score at 1 h post-biopsy compared with the periprostatic block (0.23 vs 0.43, P = 0.042), they were equivalent at 6 h (0.16 vs 0.25, P = 0.389) and 24 h (0.10 vs 0.26, P = 0.184) respectively. For the change in vital signs during biopsy procedure, perineal nerve block was significantly superior to periprostatic block in terms of maximum value of systolic blood pressure, maximum value of mean arterial pressure and maximum value of heart rate. There are no statistical differences in average value of systolic blood pressure, average value of mean, average value of heart rate, diastolic blood pressure and breathing rate. Perineal nerve block was also superior to periprostatic block in external manifestations of pain (1.88 vs 3.00, P < 0.001) and anaesthesia satisfaction (8.93 vs 11.90, P < 0.001). Equivalence was shown for the detection rate of PCa (31.25% for perineal nerve block and 29.17% for periprostatic block, P = 0.753) or csPCa (23.96% for perineal nerve block and 20.83% for periprostatic block, P = 0.604). 33 (34.8%) of 96 patients in the perineal nerve block group and 40 (41.67%) of 96 patients in the periprostatic block group had at least one complication. Interpretation: Perineal nerve block was superior to periprostatic block in pain control for men undergoing a transperineal prostate biopsy. PURPOSE: To observe psychological conditions such as anxiety, depression and somatic symptoms of temporomandibular disorders(TMD) patients using psychological scales recommended by DC/TMD and evaluate their clinical significance as the psychological axis for TMD diagnosis. METHODS: The experimental group included 100 TMD patients, and the control group comprised 100 normal prosthodontics outpatients without TMD symptoms. General information were collected including age, gender, education level, and personal income. The anxiety disorder scale (Generalized Anxiety Disorder-7, GAD-7), depression symptom scale (Patient Health Questionnaire-9, PHQ-9) and Patient Health Questionnaire-15 (PHQ-15) were used to evaluate patients' psychological conditions. SPSS 20.0 software package was used for data analysis. RESULTS: Patients less than 30 years old and between 30-50 years had similar TMD occurrence rates, both significantly higher than those older than 50 years old(P<0.05). The proportion of highly educated patients in TMD group was significantly higher than that in the control group(P<0.05), while the income level was not a risk factor for TMD (P=0.642). The incidence and average scores of anxiety, not the depression or somatic symptoms, in experimental group were significantly higher than the control group(P<0.05). The level of anxiety and depression in painful TMD patients was significantly higher than patients with joint disease(P>0.05). CONCLUSIONS: Gender(female), age (<50 years old) and high education level (undergraduate and above) are risk factors of TMD, but the income level is irrelevant. The incidence and scores of anxiety in TMD patients are higher than normal prosthodontics outpatients, while there is no significant difference in the incidence of depression and somatic symptoms between two groups.

Objective: To investigate the clinical efficacy of entecavir combined with Biejiajian pills and its influence on TCM syndrome scores during the treatment of chronic hepatitis B with hepatic fibrosis and blood stasis syndrome by prospective, randomized and controlled study. Methods: Patients with chronic hepatitis B with hepatic fibrosis and blood stasis syndrome were selected as the research subjects and randomly divided into a treatment group and a control group. Entecavir plus Biejiajian pills or entecavir plus a simulant of Biejiajian pills were given for 48 weeks. The changes in liver stiffness measurement (LSM) and TCM syndrome scores before and after treatment were compared between the two groups to analyze the correlation. The data between groups were analyzed by t-test/Wilcoxon rank sum test or χ(2) test. Pearson correlation coefficient was used to analyze the correlation between TCM syndrome scores and LSM values. Results: After 48 weeks of treatment, the LSM values of the two groups were significantly lower than those of the baseline (P < 0.001), liver fibrosis was significantly improved, and the LSM values of the treatment group were lower than those of the control group [(8.67 ± 4.60) kPa and (10.13 ± 4.43) kPa, t = -2.011, P = 0.049]. After 48 weeks of treatment, the TCM syndrome scores of the two groups were significantly reduced compared with the baseline (P < 0.001), and the clinical symptoms were significantly relieved, and the total effective rates of the improvement of the TCM syndrome scores in the two groups were 74.19% and 72.97%, respectively, but the differences between the groups were not statistically significant (χ(2) = 0.013, P = 0.910). Correlation analysis showed that there was no obvious trend between TCM syndrome scores and LSM values. There were no serious adverse reactions associated with the drug during the observation period of this study. Conclusion: Based on antiviral treatment with entecavir, regardless of whether it is combined with the Biejiajian pill, it can effectively reduce the LSM value, improve liver fibrosis, reduce TCM syndrome scores, and alleviate symptoms in patients with chronic hepatitis B with liver fibrosis and blood stasis syndrome. Compared with entecavir alone, the combined Biejia pill has greater efficacy in improving liver fibrosis and a favorable safety profile, meriting its implementation and widespread application.

BACKGROUND: Patients' lack of correct understanding of cardiovascular disease and interventional therapy is often accompanied by varying degrees of fear, depression and anxiety. Negative emotion will affect the hemodynamic fluctuation of patients undergoing interventional surgery, which is not conducive to the smooth and safe operation of interventional surgery. Therefore, it is very important to implement effective nursing intervention in the operating room. AIM: To explore the intervention effect of motivational psychological nursing combined with programmed nursing on compliance and bad mood of patients in interventional operating room. METHODS: A total of 98 patients in the interventional operating room of our hospital from October 2019 to March 2021 were randomly divided into study group (n = 49) and control group (n = 49). The control group took routine nursing. However, the study group took motivational psychological nursing combined with procedural nursing on the basis of the control group. Statistics were made on rehabilitation compliance, Positive and Negative Affect Schedule of bad mood, Simplified Coping Styles Questionnaire score of coping style and satisfaction of intervention between the two groups before and after intervention. RESULTS: The rehabilitation compliance of the study group (95.92%) was higher than that of the control group (81.63%) (P < 0.05). After intervention, the scores of upset, fear, irritability, tension and fear in the study group were respectively, which were lower than those in the control group (P < 0.05). After intervention, the score of positive coping in the study group was higher than that in the control group. However, the score of negative coping in the study group was lower than that in the control group (P < 0.05). The intervention satisfaction of the study group (93.88%) was higher than that of the control group (79.59%) (P < 0.05). CONCLUSION: The intervention of motivational psychological nursing combined with procedural nursing can improve the rehabilitation compliance, and alleviate the bad mood. In addition, it can change their coping style to the disease, and the patients are more satisfied with the nursing work.

Since lager quantities of the zinc (Zn) smelting slags were traditionally dumped at the indigenous Zn smelting sites, the release characterization of potentially toxic elements (PTEs) from the Zn smelting slags under various environmental conditions were of great significance for an environmental risk analysis. The acidification of the Zn smelting slags to pH= 4 and 6 would result in the leaching concentrations of Cd and Mn exceeding the fourth-class standard of surface water quality standard in China (GB3838-2002). Notably, most metals exhibited an amphoteric leaching pattern, where the highest leached concentrations of As, Cd, Cu, Mn, Pb, and Zn were 4.15, 4.21, 140.0, 78.1, 156.9 and 477.0 mg/L, respectively. In addition, the highest release of toxic metals within 96 h reached 0.17 % of As, 3.50 % of Cd, 2.77 % of Cu, 6.92 % of Mn, 0.13 % of Pb, and 2.57 % of Zn, respectively. The combined results of various characterization techniques suggested that the PTEs remobilization effected by rhizosphere-like organic acids were mainly controlled by the precipitation of newly formed Fe, Mn and Al (hydr) oxides and the complexation of organic ligands. The present study results could provide valuable insights into the long-term leaching behaviors of PTEs from the Zn smelting slags to reduce ecological hazard.

BACKGROUND: A wealth of evidence suggests that alcohol consumption is a risk factor for atrial fibrillation (AF); however, the mechanisms underlying this association are unclear. This makes it challenging to develop therapeutic strategies in patients with AF attributed to alcohol consumption. PURPOSE: To investigate the echocardiographic and electrophysiologic changes caused by alcohol consumption in patients with AF. METHODS: The study was registered in Chinese Clinical Trial Registry (Registration number ChiCTR2000041575). Data on 134 consecutive non-valvular AF patients who underwent radiofrequency catheter ablation in our center was collected from April 1, 2019 to June 30, 2020. Patients were divided into no-alcohol (72) and alcohol groups [categorized into light (34), moderate (11) and heavy (17) alcohol consumption]. All patients underwent echocardiographic and electrophysiologic examinations for the assessment of left atrial (LA) strain, inter-atrial conduction, intra-atrial conduction, and atrial effective refractory period (ERP). RESULTS: Overall, the mean age was 61 ± 11 years and 87 (65%) were males. Compared with the no-alcohol group, impaired peak LA longitudinal strain, obvious inter-atrial conduction delay and increasing ERP dispersion were observed in the alcohol group. Intra-atrial conduction delay and ERP dispersion increased with increasing amounts of alcohol consumption. CONCLUSION: Alcohol consumption was associated with substantial abnormal echocardiographic and electrophysiologic changes in AF patients. These changes may contribute to the occurrence and progression of AF attributed to alcohol consumption, which may help in the development of new strategies for the prevention and management of AF. However further investigation is required.

RESUMO

Coronary venous spasm has never been reported during premature ventricular extrasystole ablation. We report a 20-year-old female patient who experienced a severe spasm of the great cardiac vein during radiofrequency ablation for premature ventricular contractions, which were relieved eventually by the administration of intracoronary nitroglycerine. The operation was successfully completed, leading to a long-term resolution of her palpitation symptoms.

Antimony (Sb) pollution in soils is an important environmental problem, and it is imperative to investigate the migration and transformation behavior of Sb in soils. The adsorption behaviors and interaction mechanisms of Sb in soils were studied using integrated characterization techniques and the batch equilibrium method. The results indicated that the adsorption kinetics and isotherms of Sb onto soils were well fitted by the first-order kinetic, Langmuir, and Freundlich models, respectively, while the maximum adsorbed amounts of Sb (III) in soil 1 and soil 2 were 1314.46 mg/kg and 1359.25 mg/kg, respectively, and those of Sb (V) in soil 1 and soil 2 were 415.65 mg/kg and 535.97 mg/kg, respectively. In addition, pH ranging from 4 to 10 had little effect on the adsorption behavior of Sb. Moreover, it was found that Sb was mainly present in the residue fractions, indicating that Sb had high geochemical stability in soils. SEM analysis indicated that the distribution positions of Sb were highly coincident with Ca, which was mainly due to the existence of calcium oxides, such as calcium carbonate and calcium hydroxide, that affected Sb adsorption, and further resulted in Sb and Ca bearing co-precipitation. XPS analysis revealed the valence state transformation of Sb (III) and Sb (V), suggesting that Fe/Mn oxides and reactive oxygen species (ROS) served as oxidant or reductant to promote the occurrence of the Sb redox reaction. Sb was mobile and leachable in soils and posed a significant threat to surface soils, organisms, and groundwater. This work provides a fundamental understanding of Sb adsorption onto soils, as well as a theoretical guide for studies on the adsorption and migration behavior of Sb in soils.

Mineralogy was an important driver for the environmental release of heavy metals. Therefore, the present work was conducted by coupling mineral liberation analyzer (MLA) with complementary geochemical tests to evaluate the geochemical behaviors and their potential environmental risks of heavy metals in the smelter contaminated soil. MLA analysis showed that the soil contained 34.0% of quartz, 17.15% of biotite, 1.36% of metal sulfides, 19.48% of metal oxides, and 0.04% of gypsum. Moreover, As, Pb, and Zn were primarily hosted by arsenopyrite (29.29%), galena (88.41%), and limonite (24.15%), respectively. The integrated geochemical results indicated that among the studied metals, Cd, Cu, Mn, Pb, and Zn were found to be more bioavailable, bioaccessible, and mobile. Based on the combined mineralogical and geochemical results, the environmental release of smelter-driven elements such as Cd, Cu, Mn, Pb, and Zn were mainly controlled by the acidic dissolution of minerals with neutralizing potential, the reductive dissolution of Fe/Mn oxides, and the partial oxidation of metal sulfide minerals. The present study results have confirmed the great importance of mineralogy analysis and geochemical approaches to explain the contribution of smelting activities to soil pollution risks.

With the popularization of higher education and the promotion of college enrollment expansion, the number of college graduates increases sharply. At the same time, the continuous transformation and upgrading of the industrial structure put forward higher requirements on the employability of college students, which leads to the imbalance between supply and demand in the labor market. The key to dealing with employment difficulties lie in the improvement of college students' employability. Therefore, we make a regression analysis of 263 valid samples from universities in Anhui Province and extract the factors that influence the improvement of college students' employability in the process of talent cultivation in university. The result shows that there is a positive correlation between course setting, course teaching, club activities, and college students' employability, among which the course teaching and club activities are the most critical factors which may influence college students' employability. In addition, from the viewpoint of individual college students, the overall grades of college students and the time of participating in the internship are also closely related to their employability, i.e., college students with good overall grades and long internship time should also have stronger employability.

To reduce carbon emissions and achieve carbon neutrality in China, it is pivotal to explore low-carbon wastewater treatment processes and carbon-neutral wastewater treatment plants (WWTPs). This study investigated the Beijing Gaobeidian WWTP to explore the current energy consumption and carbon emission status of representative WWTPs in China. Furthermore, it explored a possible low-carbon operating model. Results show that the current total energy consumption of Gaobeidian WWTP is 280,717 MWh/y, while its energy recovery is 268,788 MWh/y. As a result, the energy neutralization ratio is 95.8%, and the plant is close to reaching energy neutrality. The carbon emission of this plant is 446,468 t/y. However, it reduced its carbon emissions by 252,994 t/y and reached only 56.7% of carbon neutrality. Although the plant almost reached energy neutrality, it has a long way to go before reaching carbon neutrality. It was found that a subsequent increase in the recovery of residual heat from secondary effluent can increase the energy and carbon neutralization ratio to 523.1% and 219.0%, respectively, meaning that the WWTP can become a power production unit and a carbon sink. This study can provide a reference for exploring efficient energy use and reaching carbon neutrality for domestic WWTPs.
