RESUMO

56 patients with Bell's palsy were tested with electroneuronography (ENoG and Blink reflex (BR) within the 5th and 15th day of onset of facial paralysis. The recovery at the end of evolution was evaluated with the study of facial function. This retrospective study showed that ENoG is able to predict with a very low risk of error a good prognosis when more than 10 percent (strictly, of fibers (non degenerated and blocked) are active. On the other hand only persistence or early recovery of BR can assure a good prognosis whereas the absence do not permit to conclude.

RESUMO

The authors report one case of tonsillar cyst with area of dysplasia in the epithelium, in situ carcinoma, and invasive carcinoma which are peculiar to branchiogenic carcinoma. A tonsillarectomy made ten month after the operation contradict this diagnosis. The criteria for making the diagnosis of branchiogenic carcinoma are discussed so as the clinical sensibility of this cysts. Present imagery is not able to assert the primarity of such lesion. Removing of the tonsil would be made systematically.

Midline granuloma includes diverse clinicopathological entities, such as Wegener granulomatosis, polymorphic reticulosis, lethal midline granuloma and conventional malignant lymphoma of the nose usually of B-cell origin. The authors describe five patients with LMG clinically and pathologically typical. Using an extensive panel of monoclonal antibodies, they demonstrate an "activate" T-cell phenotype observed on the initial lesion of the face in one patient, similar to that found in two patients with LMG but studied after dissemination in peripheral T-cell lymphoma. Furthermore, many atypical cells were found in LMG, and stained with the Ki-67 monoclonal antibody, a marker of proliferating cells. These findings support the view that LMG is closely related to T-cell malignancies. Two of them were treated with recombinant Interferon alpha 2a followed by a response rapidly objective. Immunohistologic studies are very important for confirming the T-cell origin of such a disease and for selecting patients to be treated with Interferon alpha.

The response to acute oral administration of 50 mg of Captopril was assessed in 17 normal volunteers and 47 patients with hypertension; 17 had renovascular (RVH) abnormalities and 30 patients had essential hypertension (EH). All patients were maintained on ad libitum sodium intake. The effect of Captopril on mean arterial pressure (MAP) was rapid a maximal within 60 minutes. The converting enzyme inhibitor induced a similar decrease in MAP in normal subjects (-5.1 +/- I mm Hg) and patients with EH (-7.2 +/- I mm Hg). Control plasma renin activity (PRA) was similar in both groups; however, the increase in PRA following Captopril was more marked in normals (8.1 +/- 1.7 ng/ml/h) than in EH (1.7 +/- 0.7 ng/ml/h). In patients with RVH a marked fall in MAP occurred (-25.4 +/- 4 mm Hg). A fall in MAP higher than 20 mm Hg was observed in 65% of patients with RVH and none of the EH group. A negative correlation between log PRA and the change in MAP induced by Captopril was obtained (r = 0.65). Assessment of the response to acute administration of Captopril may be useful for screening patients with RVH.
