Reform in Washington state.

The Washington State Healthcare Reform Act of 1993 is in the early stages of implementation. The barriers to implementation and the impacts on HMOs like GHC are an example of the types of issues that will be faced (or are being faced) by other similar organizations throughout the country. The most dramatic observation is that the pace of change is far more rapid than our organization has experienced in recent years; and that pace of change is likely to continue for years to come.

Ganglia (synovial cysts) of bone.

Endocrine disorders which manifest in the lower extremity.

This article has attempted to alert the podiatric medical practitioner to those endocrine disorders which have pedal manifestations. With the clinical information presented here, the podiatrist is in a unique position to identify early signs of endocrine disease. By doing so, the podiatric practitioner may play a vital role as a member of the primary care team.

Risks of coronary arteriography and bypass surgery in patients with left main coronary artery stenosis.

The risk of coronary arteriography is considerably increased in patients who have left main coronary artery stenosis (LMCAS). Among 1,060 patients undergoing coronary arteriography over a three-year period, 83 were found to have LMCAS and three of these patients died (3.6 percent) during or shortly after the arteriographic procedure. Bypass surgery, however, can now be carried out at a very low risk irrespective of the degree of coronary disease present--there was no hospital mortality in 74 patients with LMCAS undergoing bypass surgery at our institution during this period. In addition to careful attention to detail, techniques that can minimize the risk of both of these procedures in patients with LMCAS include invasive hemodynamic monitoring and various pharmacologic manipulations to prevent myocardial ischemia. In a small percentage of patients, rest angina may prove refractory to inhospital medical treatment, and intraaortic balloon pumping may be necessary to stabilize the conditions of these patients before proceeding with arteriography and myocardial revascularization.

Pulsed Doppler echocardiography: principles and applications.

A new recording and display system is described for use with pulsed Doppler blood flow velocity detectors in the diagnosis of valvular and septal defects. The principles of the pulsed Doppler device are described along with the methods used to analyze and display the Doppler shifted signal from flow jets resulting from various valve defects. An M-mode display is combined with blood flow display to provide a convenient record of the clinical procedure. Examples of aortic stenosis, aortic insufficiency, mitral stenosis and regurgitation are presented along with signals from other valves.

Detection of aortic insufficiency by pulse Doppler echocardiography.

Sixty-five patients were studied for the presence of aortic insufficiency by auscultation, conventional M mode echocardiography (echo), range-gated pulse Doppler flowmeter (Doppler), and supravalvular aortic angiography. Aortic insufficiency was demonstrated angiographically in 49 patients, of which eight (16%) were missed by auscultation. Echo missed 20 (41%), and Doppler missed seven (14%). Only two (4%) were missed by auscultation and Doppler combined. All methods were highly specific, but the combination of auscultation and Doppler was the most sensitive noninvasive method for detecting aortic insufficiency.
