El objetivo del estudio fue relacionar valores de VFC post-ejercicio con componentes de carga interna de entrenamiento medida por tres métodos de Estímulo de Entrenamiento (Training Impulse, TRIMP) en triatletas juveniles. Los métodos de TRIMP estuvieron relacionados entre ellos (r = 0,970 a 0,983, R2 = 0.940 a 0.967), sin embargo, mostraron diferencias significativas en sus valores promedio (p < 0.01). El entrenamiento redujo significativamente la VFC (p < 0.01) y existe un efecto significativo del tiempo en la recuperación de la VFC (p < 0.01). La reducción de la VFC en Post5 está negativamente relacionada con la intensidad del ejercicio, volumen en >90% FCres y valores de TRIMP (r = -0.302 a -0.495, p < 0.01) y positivamente relacionada con el volumen en <50% FCres (r = 0.488, p < 0.01). La reducción post-ejercicio de la VFC puede proporcionar información valiosa para evaluar efectos fisiológicos del entrenamiento interválico o continuo en triatletas juveniles


The aim of the study was to stablish relationships between post-exercise heart rate variability (HRV) values and internal training load (TL) components measured by three Training Impulse (TRIMP) methods in youth triathletes. TRIMP methods were related with each other (r = 0,970 to 0,983, R2 = 0,940 to 0,967) but showed significant differences in mean values (p < 0,01). Training significantly reduced HRV (p < 0,01) with a significant time effect on HRV recovery (p < 0,01). Post5 HRV reduction has a negative relationship to exercise intensity, volume at >90% HRres, and TRIMP values (r = -0,302 to -0,495, p < 0,01) and a positive relationship with volume at <50% HRres (r = 0.488, p < 0,01). Immediate post-exercise HRV reduction can provide valuable information for evaluation of physiological effects at intervallic or continuous training in youth triathletes

RESUMO

Hybrid 2D hard-soft composites have been fabricated by combining soft (Co73Si27) and hard (NdCo5) magnetic materials with in-plane and out-of-plane magnetic anisotropies, respectively. They have been microstructured in a square lattice of CoSi anti-dots with NdCo dots within the holes. The magnetic properties of the dots allow us to introduce a magnetostatic stray field that can be controlled in direction and sense by their last saturating magnetic field. The magnetostatic interactions between dot and anti-dot layers induce a completely tunable exchange bias-like shift in the system's hysteresis loops. Two different regimes for this shift are present depending on the lattice parameter of the microstructures. For large parameters, dipolar magnetostatic decay is observed, while for the smaller one, the interaction between the adjacent anti-dot's characteristic closure domain structures enhances the exchange bias-like effect as clarified by micromagnetic simulations.

Cada año ocurren en el mundo actual más de un millón de fracturas de la cadera. Países desarrollados gastan en ellas tres millones de dólares en igual periodo. Según expertos de la OMS los costos de tratamiento para esta patología en décadas venideras harán colapsar los presupuestos de salud de algunos países del tercer mundo, financiamiento cuya tendencia es a decrecer, víctimas de las políticas neoliberales y globalización del mundo. La elevada esperanza de vida de la población así como el creciente por ciento de personas que igualan los 60 años, nos obliga a desarrollar métodos eficaces de tratamiento a la vez eficiente desde el punto de vista económico de su aplicación; que teniendo en cuenta las particularidades de nuestro país y posibilidades de su sistema de salud permitan enfrentar con éxito lo que ha dado en llamarse "Epidemia Silenciosa". Por lo que creamos un modelo de atención integral y dinámico al paciente geriátrico con fracturas de la cadera, que corresponde al manejo del mínimo por un equipo multidisciplinario, conceptos actuales en cuanto a valoración funcional del anciano, profilaxis antibiótica y de la trombosis venosa profunda y reincorporación precoz del enfermo a su medio habitual así como el grado de satisfacción de todo lo cual es regido en este informe preliminar...(AU)

Mutations in exons 4-8 of the p53 gene by the PCR-SSCP analysis in preneoplastic and neoplastic lesions of the colon (n=11) and esophagus (n=18) were screened. p53 overexpression by immunohistochemistry in 11 colonic lesions and 13 microsatellites, in all the patients (n=29), were also studied. A positive result concordancy between the three techniques was found in 1 adenoma and 2 adenocarcinomas of the colon, each with loss of heterozygocity of microsatellites. Metaplastic lesions of esophagus showed biallelic mutations and low frequency of microsatellite alterations. The relationship between genetic alterations in p53, microsatellites and type of colon and esophageal lesions is discussed.

BACKGROUND AND STUDY AIMS: The increase in infectious diseases of the gastrointestinal tract related to immunosuppression is becoming an important topic for the endoscopist. To improve the diagnostic efficacy of tissue acquisition while at the same time restricting costs, we have developed a new device for obtaining material from the upper gastrointestinal tract that can also be used in the diagnosis of neoplastic disease. PATIENTS AND METHODS: A total of 90 patients were examined and assigned to two groups according to indications. Group A consisted of 53 symptomatic patients with positive human immunodeficiency virus (HIV) serology with a suspicion of gastrointestinal infection. Group B included 37 patients in whom there was an endoscopic suspicion of neoplasia in the upper gastrointestinal tract. Cell fragments for cytological study were obtained using a device introduced through the endoscopic instrumentation channel (abrasive cytology). Different staining methods were used to isolate bacteria or diagnose tumors from cell fragments. The findings were compared with those obtained from conventional bioptic histology. RESULTS: Potentially responsible pathogens were isolated in 48 of the 53 patients in Group A, while bioptic histology provided a diagnosis in only 32 patients. In the 37 patients in group B, the cytological diagnosis matched the histological results. The costs of this new technique are similar to those for conventional cytological staining, and the time from sampling to obtaining a final diagnosis is less than one hour. CONCLUSIONS: This new device provides a fast and low-cost method of isolating pathogens and obtaining cell fragments from the gastrointestinal mucosa during routine upper gastrointestinal endoscopy.

In vivo and in vitro studies have demonstrated that beta-adrenoceptor blockers enhance the airway smooth muscle contraction to antigen, but the mechanisms are not fully understood. In the present work, we corroborated that propranolol (3.9 x 10(-6) M, 30 min incubation) induces hyperreactivity to antigenic challenge (cumulative concentration-response curve to ovalbumin, 0.01 to 100 micrograms/ml) in lung parenchyma strips from sensitized guinea-pigs. This hyperreactivity was enhanced by indomethacin (3.2 x 10(-5) M) and was unaffected by the lipoxygenase/cyclooxygenase inhibitor phenidone (1 x 10(-4) M). However, the histamine H1 receptor antagonist pyrilamine (1 x 10(-6) M) reduced the potentiation effect of propranolol. These results suggest that bronchoconstrictor prostaglandins, thromboxane A2 and leukotrienes, are not involved in the propranolol-induced lung parenchyma strips hyperreactivity to antigen in vitro, and that histamine may account, at least in part, for such propranolol effect.

The production of platelet-derived growth factor (PDGF) was studied in small cell lung carcinoma, lung squamous carcinoma and lung adenocarcinoma cell lines, and in seven human lung tissues obtained from each type of lung cancer. By indirect immunofluorescence, PDGF was detected in all the cell lines. Likewise, five out of seven biopsies derived from patients with adenocarcinoma and squamous carcinoma, and four out of seven specimens with small cell carcinoma displayed a positive pattern for PDGF. In addition, lung carcinoma cell lines expressed both PDGF-A and PDGF-B/sis genes, as judged by Northern blot analysis. Biologically active, serum-free conditioned media obtained from all three cell lines stimulated the incorporation of [3H]thymidine into quiescent BALB/c-3T3 cells. This effect was abolished when IgG-PDGF antiserum was used. These findings suggest that an abnormal expression of PDGF occurs in the three more frequent types of lung cancer, which can play a potential role in neoplastic transformation and uncontrolled cell growth.

The activity of adenosine deaminase in the pleural fluid of 218 consecutive patients was studied. According to the etiology of exudative pleural effusions, the patients were divided into the following five groups: (1) tuberculosis; (2) lung cancer; (3) pneumonias; (4) miscellaneous; and (5) idiopathic. Patients with pleural tuberculosis presented significantly higher ADA activity than patients with nontuberculous pleural effusions (p less than 0.0001). The results indicated that in a population with a relatively high prevalence of tuberculosis, the analysis of ADA levels in pleural effusions constitutes a useful marker for the diagnosis which, in addition, can be made quickly and cheaply. Additionally, a comprehensive review of the literature on the role of ADA in the diagnosis of tuberculous pleural effusions is presented.

Frente a todo paciente traumatizado se debe establecer inicialmente el índice de trauma. El diagnóstico de trauma abdominal complicado se realiza por el antecedente, el examen físico y por procedimientos diagnósticos auxiliares. Como norma se puede efectuar sutura primaria o resección y sutura primaria si se encuentra un índice de trauma mayor o igual a 10 y un índice de trauma abdominal menor o igual a 25; si han transcurrido menos de 12 horas del trauma; si la contaminación peritoneal es leve o moderada; si no hay hipotensión y el contenido de deposiciones del colon distal es leve o moderado. Si algunos de estos índices se encuentra alterado se puede efectuar la sutura exteriorizada del colon, sutura primaria y colostomía proximal, exteriorización de la lesión o bien resección y colostomía

RESUMO

Laryngeal tuberculosis continues to exist in spite of extensive tuberculosis eradication campaigns. We present 19 cases of laryngeal tuberculosis seen in our institution between 1982 and 1987. From the clinical findings it is clear that the localization of the lesions has changed since the preantibiotic era. Today the spread appears to be mainly via lymphatic and hematogenic routes, as opposed to the direct spread more common previously. The fibrosis caused by the lesions is incapacitating and frequently requires surgical correction.

Chronic nonspecific bulbitis (CNB), the number of G-cells in the antrum and the level of serum gastrin were unrelated in a group of 24 patients. Gastrin may be produced by cells outside the antrum or by a few but hyperactive antral G-cells.

Se presenta un estudio prospectivo y randomizado sobre 40 pacientes con peritonitis difusa, intervenidos en el SUAO. A 20 de ellos se les dejó tubo de drenaje como parte del tratamiento quirúrgico. Los grupos en estudio son comparativamente similares. El uso del tubo de drenaje significó un retardo en la recuperación del tránsito intestinal, una evolución febril mas prolongada y patología agregada. La morbilidad intraabdominal no fue prevenida con el uso del tubo de drenaje y significó un 20% mayor, derivada de éste. Si la técnica quirúrgica trata el foco y se efectúa un prolijo aseo de la cavidad abdominal, creemos que en las peritonitis difusas es innecesario dejar tubo de drenaje y además produce complicaciones, razones por las que en estas circunstancias no lo usamos
