RESUMO

Background: The first inquisitorial trials were against Muslims and Jews. Later, they focused on women, especially caregivers. Progressively, they were linked to witchcraft and sorcery because of their great care, generational and empirical knowledge. The historiography of health in the 15th-18th centuries still has important bibliographical and interpretative gaps in the care provided by women. Objective: To analyse the care provided by healers as health providers, accused by the Inquisition, justifying the importance of nursing in the diversity of community care in the 15th-18th centuries. Method: A scoping review was conducted following the Dialectical Structural Model of Care (DSMC). A database search was conducted for the period 2013-2022. Bibliographic and legislative resources were used. Cases and convictions from Castilla la Nueva were found in the National Historical Archive and the Diocesan Archive of Cuenca. Results: The concepts of healer, witch and sorceress envolved during the study period. They reflect and reveal the collective imaginary of the social structure. They had healing laboratories, practised psychological and sexual care. They used to accompany their therapeutic action with prayers and amulets. They shared their professional activity with their main denouncers, doctors, apothecaries and priests. They were usually women in socially vulnerable situations, who did not conform to social stereotypes. Conclusions: They were predecessors of today's nursing, they overcame socio-cultural difficulties, although they were condemned for it. Healers did not manage to regulate their profession, but they acted as agents of health in a society that demanded them while participating in the "witch-hunt".

BACKGROUND: Obesity is characterized by adipose tissue dysregulation and predisposes individuals to insulin resistance and type 2 diabetes. At the molecular level, adipocyte dysfunction has been linked to obesity-triggered oxidative stress and protein carbonylation, considering protein carbonylation as a link between oxidative stress and metabolic dysfunction. The identification of specific carbonylated proteins in adipose tissue could provide novel biomarkers of oxidative damage related to metabolic status (i.e prediabetes). Thus, we aimed at characterizing the subcutaneous and omental human adipose tissue carbonylome in obesity-associated insulin resistance. METHODS: 2D-PAGE was used to identify carbonylated proteins, and clinical correlations studies and molecular biology approaches including intracellular trafficking, reactive oxygen species assay, and iron content were performed using in vitro models of insulin resistance. RESULTS: The carbonylome of human adipose tissue included common (serotransferrin, vimentin, actin, and annexin A2) and depot-specific (carbonic anhydrase and α-crystallin B in the subcutaneous depot; and α-1-antitrypsin and tubulin in the omental depot) differences that point out the complexity of oxidative stress at the metabolic level, highlighting changes in carbonylated transferrin expression. Posterior studies using in vitro prediabetic model evidence alteration in transferrin receptor translocation, linked to the prediabetic environment. Finally, ligand-receptor molecular docking studies showed a reduced affinity for carbonylated transferrin binding to its receptor compared to wild-type transferrin, emphasizing the role of transferrin carbonylation in the link between oxidative stress and metabolic dysfunction. CONCLUSIONS: The adipose tissue carbonylome contributes to understanding the molecular mechanism driving adipocyte dysfunction and identifies possible adipose tissue carbonylated targets in obesity-associated insulin resistance.

Our understanding of the interplay between human adipose tissue and the immune system is limited. The mesothelium, an immunologically active structure, emerged as a source of visceral adipose tissue. After investigating the mesothelial properties of human visceral and subcutaneous adipose tissue and their progenitors, we explored whether the dysfunctional obese and Crohn's disease environments influence the mesothelial/mesenchymal properties of their adipocyte precursors, as well as their ability to mount an immune response. Using a tandem transcriptomic/proteomic approach, we evaluated the mesothelial and mesenchymal expression profiles in adipose tissue, both in subjects covering a wide range of body-mass indexes and in Crohn's disease patients. We also isolated adipose tissue precursors (adipose-derived stem cells, ASCs) to assess their mesothelial/mesenchymal properties, as well as their antigen-presenting features. Human visceral tissue presented a mesothelial phenotype not detected in the subcutaneous fat. Only ASCs from mesenteric adipose tissue, named creeping fat, had a significantly higher expression of the hallmark mesothelial genes mesothelin (MSLN) and Wilms' tumor suppressor gene 1 (WT1), supporting a mesothelial nature of these cells. Both lean and Crohn's disease visceral ASCs expressed equivalent surface percentages of the antigen-presenting molecules human leucocyte antigen-DR isotype (HLA-DR) and CD86. However, lean-derived ASCs were predominantly HLA-DR dim, whereas in Crohn's disease, the HLA-DR bright subpopulation was increased 3.2-fold. Importantly, the mesothelial-enriched Crohn's disease precursors activated CD4+ T-lymphocytes. Our study evidences a mesothelial signature in the creeping fat of Crohn's disease patients and its progenitor cells, the latter being able to present antigens and orchestrate an immune response.

Introduction: According to figures released by UNICEF (United Nations Children's Fund), more than 200 million girls and women have suffered female genital mutilation (FGM) in 30 African and Middle East countries. An increasing number of African women who come from ethnic groups where FGM is practised are arriving in Western countries. Healthcare professionals play a fundamental role in its prevention. Goals: To learn about the factors that healthcare professionals consider as facilitators for prevention and action when faced with female genital mutilation. Methods: A cross-sectional descriptive study developed on the basis of the qualitative methodological perspective, where 43 healthcare professionals participated. A series of analysis dimensions were established, based on which, the interview and discussion group scripts were designed. Results: Addressing FGM requires a series of structural adaptations of the healthcare system that facilitate the recording and monitoring of cases, both for treatment and for prevention. In addition, it is necessary to establish coordination between the healthcare, social services and education sectors. Conclusions: The existence of a protocol of action and training in its use is one of the key tools to take into account.

Adipose tissue dysregulation in obesity strongly influences systemic metabolic homeostasis and is often linked to insulin resistance (IR). However, the molecular mechanisms underlying adipose tissue dysfunction in obesity are not fully understood. Herein, a proteomic analysis of subcutaneous (SC) and omental (OM) fat from lean subjects and obese individuals with different degrees of insulin sensitivity was performed to identify adipose tissue biomarkers related to obesity-associated metabolic disease. Our results suggest that dysregulation of both adipose tissue extracellular matrix (ECM) organization and intracellular trafficking processes may be associated with IR in obesity. Thus, abnormal accumulation of the small leucine-rich proteoglycan, lumican, as observed in SC fat of IR obese individuals, modifies collagen I organization, impairs adipogenesis and activates stress processes [endoplasmic reticulum and oxidative stress] in adipocytes. In OM fat, IR is associated with increased levels of the negative regulator of the Rab family of small GTPases, GDI2, which alters lipid storage in adipocytes by inhibiting insulin-stimulated binding of the Rab protein, Rab18, to lipid droplets. Together, these results indicate that lumican and GDI2 might play depot-dependent, pathogenic roles in obesity-associated IR. Our findings provide novel insights into the differential maladaptive responses of SC and OM adipose tissue linking obesity to IR.

Introducción: El conocimiento tácito puede definirse como aquel que se utiliza de forma intuitiva e inconsciente, y que se adquiere mediante la propia experiencia, caracterizándose por ser personal y contextual. Términos como «intuición», «saber cómo» y «conocimiento implícito» se han utilizado para describirlo. Diferentes disciplinas en el ámbito de la gestión y la salud han estudiado el conocimiento tácito y lo han identificado como una potente herramienta para la creación de conocimiento y la toma de decisiones clínica. El objetivo de esta revisión fue analizar la definición y las características que componen el conocimiento tácito, así como determinar el rol que desempeña en la disciplina enfermera. Método: Se realizó una revisión integrativa de la literatura publicada hasta noviembre de 2016 en las bases de datos CUIDEN, SciELO, PubMed, Cochrane y CINAHL. La síntesis y la interpretación de los datos fue realizada por dos investigadores mediante análisis del contenido. Resultados: De los 819 artículos localizados, se seleccionaron 35 sobre el conocimiento tácito y la disciplina enfermera. No existe consenso en la denominación y la interpretación de los resultados en conocimiento tácito. Las principales características del conocimiento tácito son su carácter personal y social, siendo utilizado a partir de una estructura mental organizada (mindline). Esta estructura está relacionada con el uso del conocimiento tácito en la toma de decisiones clínicas. Conclusiones: Los estudios realizados sobre conocimiento tácito y la disciplina enfermera aportan diversas perspectivas sin profundizar en la materia. Se plantea la elaboración de un marco de relaciones que clarifique los conceptos implicados y su papel en la gestión del conocimiento enfermero

AIM: To find out whether the internal target volume (ITV) vaginal procedure ensures dosimetric coverage during intensity-modulated radiation therapy (IMRT) of post-operative gynaecological tumours without instructions on rectal filling. BACKGROUND: The ITV vaginal procedure does not necessarily include all movements of the bladder, and does not include changes in the rectal volume. We should know if the vaginal ITV is a useful tool in maintaining CTV coverage during treatment. MATERIALS AND METHODS: A retrospective analysis of 24 patients treated between July 2012 and July 2014 with adjuvant IMRT for gynaecological cancer. All patients underwent empty and full bladder CT on simulation (CT-planning) and three weeks later (CT-control). ITV displacement was measured and the 3D vector was calculated. ITV coverage was then evaluated by comparing the volume covered by the prescription isodose on both CT's. Patients were asked to have full bladder but they did not follow recommendations for the rectum. RESULTS: The mean 3D vector was 0.64 ± 0.32 cm (0.09-1.30). The mean ITV coverage loss was 5.8 ± 5.7% (0-20.2). We found a significant positive correlation between the 3D vector and the loss of coverage (Pearson correlation, r = 0.493, 95% CI: 0.111-0.748, p = 0.0144). We did not find any significant correlation between the bladder and rectal parameters with the 3D vector and loss of dosimetric coverage. We found a trend between the maximum rectal diameter in CT-planning and 3D vector (r = 0.400, 95% CI: -0.004 to 0.692, p = 0.0529). CONCLUSION: ITV vaginal procedure contributed to ensuring a good dose coverage without instructions on rectal filling.

Current sensor networks need to be improved and updated to satisfy new essential requirements of the Internet of Things, where cutting-edge applications will appear. These requirements are: total coverage, zero fails (high performance), scalability and sustainability (hardware and software). We are going to evaluate Bluetooth Low Energy as wireless transmission technology and as the ideal candidate for these improvements, due to its low power consumption, its low cost radio chips and its ability to communicate with users directly, using their smartphones or smartbands. However, this technology is relatively recent, and standard network topologies are not able to fulfil its new requirements. To address these shortcomings, the implementation of other more flexible topologies (as the mesh topology) will be very interesting. After studying it in depth, we have identified certain weaknesses, for example, specific devices are needed to provide network scalability, and the need to choose between high performance or sustainability. In this paper, after presenting the studies carried out on these new technologies, we propose a new packet format and a new BLE mesh topology, with two different configurations: Individual Mesh and Collaborative Mesh. Our results show how this topology improves the scalability, sustainability, coverage and performance.

La existencia de diarios oficiales permite a los Estados informar de la actividad legislativa y administrativa. Diferentes estudios han utilizado estos boletines para analizar la información sociosanitaria de un momento histórico determinado. Nos planteamos como objetivo valorar la evolución de las referencias respecto a los expósitos y su relación con las nodrizas en la ‘Gazeta de Madrid’, permitiendo la categorización en función de los aspectos tratados. Se plantea la revisión y análisis de contenido de las 130 referencias aparecidas entre 1751 y 1926 que cumplían los criterios establecidos. Tras la lectura, transcripción, cumplimentación y categorización de la información recopilada se realiza la síntesis de la información. El estudio permite valorar la importancia social de la nodriza como un elemento normalizado resaltando una caracterización negativa debido a la falta progresiva de la oferta de nodrizas. Las nodrizas se presentarán como una figura laboral normalizada en el siglo XIX, apareciendo como tal en la prensa como en las normas legislativas en las que se regulan los aspectos del trabajo productivo. La nodriza se convierte junto al niño/a en el escalón más débil, produciéndose una discriminación fruto de la concepción patriarcal de la sociedad y por el hecho de ser mujer (AU)


The existence of official journals allows States to provide information on legislative and administrative activity. Different studies have used these bulletins to analyze the sociosanitary information of a particular historical moment. Our objective was to assess the evolution of the references regarding foundlings and their relationship with wet nurses in the Journal ‘Gazeta de Madrid’, and to stablish their categorization according to the issues that have been covered. We perform the review and content analysis of 130 references published between 1751 and 1926 who met the established inclusion criteria. After reading, transcription, compilation and categorization of gathered information, a process of information’s synthesis is performed. The study allows us to assess the social importance of wet nurses as a standard element in society, also highlighting a negative characterization of this figure, due to the progressive decrement of its offer. The wet nurses will perform as a labour standard figure in the 19th century, appearing as such in the press and also in the legislation which regulates aspects of productive work. Wet nurse becomes, together with children, the society’s weakest link, leading to a discrimination result of the patriarchal conception of society and the fact of being a woman (AU)

Nós plano destinado a avaliar a evolução das referências sobre enjeitados e sua relação com as enfermeiras na ‘Gazeta de Madrid’, permitindo a categorização com base em questões de tratados. Revisão e análise de conteúdo de 130 referências publicadas entre 1751 e 1926, que preencheram os critérios estabelecidos surge. Depois da leitura, da transcrição, e realização, categorização das informações recolhidas síntese de informação é executada. O estudo nos permite avaliar a importância social da mãe como um item padrão, destacando a caracterização negativa devido à crescente falta de fornecimento de enfermeiros. Aleitamento ser apresentado como um padrão figura de trabalho no século XIX, aparecendo como tal na imprensa e na legislação em que são regulados aspectos do trabalho produtivo. Torna-se a mãe com a criança / a no passo mais fraco, produzindo um resultado discriminação da concepção patriarcal da sociedade e do fato de ser uma mulher (AU)

Justificación: Durante la adolescencia y la juventud acontecen cambios psicológicos y fisiológicos que forman parte del desarrollo normal de la persona. La irrupción de la escoliosis en esta edad influye en la vivencia de la autopercepción corporal. Objetivo: Conocer cómo una deformidad corporal influye en la estabilidad emocional de las jóvenes diagnosticadas de escoliosis. Diseño: Metodología cualitativa fenomenológicas situada en la hermenéutica. Método: Se utilizó una entrevista semiestructurada y dirigida a mujeres jóvenes con escoliosis idiopática ingresadas en la unidad de raquis del Hospital Vall d´Hebron en Barcelona. Resultados: Los principales resultados señalan un compromiso emocional que afecta a las actividades de la vida diaria de las jóvenes, manifestándose, en algunas ocasiones, con sentimientos de vergüenza, tristeza, preocupación, miedo, amargura, complejo y rabia. La experiencia se vive como un duelo ante la pérdida de expectativas futuras. Conclusiones: Los sentimientos y las emociones vividas y referidas por estas pacientes comprometen su estabilidad emocional (AU)


Justification: During adolescence and youth psychological and physiological changes occur that are part of the normal development of the individual. The emergence of scoliosis in this age influences the perception of body experience. Background: Knowing how a bodily deformity affects the emotional stability of young people diagnosed with scoliosis. Design: Qualitative research with phenomenological methodology at the hermeneutics. Method: A semi-structured interview was used and aimed at young women with idiopathic scoliosis entered in the spine unit at the Hospital Vall d'Hebron in Barcelona. Results: The main results indicate an emotional commitment that affects the activities of daily life for young people, demonstrating, sometimes with feelings of shame, sadness, worry, fear, bitterness, and rage complex. The experience is lived as a duel to the loss of future expectations. Conclusions: Feelings and emotions experienced and reported by these patients commit their emotional stability (AU)

OBJETIVO: Conocer las experiencias vividas por las mujeres diagnosticadas de fibromialgia aplicando la Teoría de la Incertidumbre propuesta por M. Mishel. DISEÑO: Estudio cualitativo con enfoque fenomenológico. Emplazamiento una Asociación de enfermas de fibromialgia de la provincia de Alicante durante los meses de junio 2012 a noviembre de 2013. PARTICIPANTES: Catorce mujeres diagnosticadas de fibromialgia con edades comprendidas entre 45 y 65 años participaron de forma voluntaria. MÉTODO: Generación de información a través de entrevistas semiestructuradas con grabación y transcripción previo compromiso de confidencialidad y consentimiento informado. Análisis de contenido extrayendo diferentes categorías de acuerdo con la teoría propuesta. RESULTADOS: Las pacientes del estudio perciben un elevado nivel de incertidumbre vinculado a la dificultad para hacer frente a los síntomas, incertidumbre ante el diagnóstico y la complejidad del tratamiento. Por otra parte, la capacidad de afrontamiento de la enfermedad se verá influida por el apoyo social, la relación con los profesionales de la salud y la búsqueda de ayuda e información acudiendo a las asociaciones de pacientes. CONCLUSIONES: El profesional de la salud debe ofrecer una información clara sobre la patología a las enfermas de fibromialgia; cuanto mayor es el nivel de conocimientos de las pacientes sobre su enfermedad y mejor es la calidad de la información que se presta, se informa de menor ansiedad e incertidumbre en la vivencia de la enfermedad. Así mismo, las asociaciones de pacientes deberían disponer de profesionales de la salud para evitar sesgos en la información y el asesoramiento con evidencia científica

Non-T cell activation linker is an adaptor protein that is tyrosine phosphorylated upon cross-linking of immune receptors expressed on B lymphocytes, NK cells, macrophages, basophils, or mast cells, allowing the recruitment of cytosolic mediators for downstream signaling pathways. Fas receptor acts mainly as a death receptor, and when cross-linked with Fas ligand, many proteins are proteolytically cleaved, including several signaling molecules in T and B cells. Fas receptor triggering also interferes with TCR intracellular signals, probably by means of proteolytic cleavage of several adaptor proteins. We have previously found that the adaptor linker for activation of T cells, evolutionarily related to non-T cell activation linker, is cleaved upon proapoptotic stimuli in T lymphocytes and thymocytes, in a tyrosine phosphorylation-dependent fashion. Here, we describe non-T cell activation linker proteolytic cleavage triggered in human B cells and monocytes by Fas cross-linking and staurosporine treatment. Non-T cell activation linker is cleaved, producing an N-terminal fragment of ∼22 kDa, and such cleavage is abrogated in the presence of caspase 8/granzyme B and caspase 3 inhibitors. Moreover, we have identified an aspartic acid residue at which non-T cell activation linker is cleaved, which similar to linker for activation of T cells, this aspartic acid residue is located close to tyrosine and serine residues, suggesting an interdependence of phosphorylation and proteolytic cleavage. Consistently, induction of non-T cell activation linker phosphorylation by pervanadate inhibits its cleavage. Interestingly, the truncated isoform of non-T cell activation linker, generated after cleavage, has a decreased signaling ability when compared with the full-length molecule. Altogether, our results suggest that cleavage of transmembrane adaptors constitutes a general mechanism for signal termination of immune receptors.

OBJECTIVES: Finding out women's experiences diagnosed with fibromyalgia applying the Theory of Uncertainty proposed by M. Mishel. DESIGN: A qualitative study was conducted, using a phenomenological approach. LOCATION: An Association of patients in the province of Alicante during the months of June 2012 to November 2013. PARTICIPANTS: A total of 14 women diagnosed with fibromyalgia participated in the study as volunteers, aged between 45 and 65 years. METHOD: Information generated through structured interviews with recording and transcription, prior confidentiality pledge and informed consent. Analysis content by extracting different categories according to the theory proposed. RESULTS: The study patients perceive a high level of uncertainty related to the difficulty to deal with symptoms, uncertainty about diagnosis and treatment complexity. Moreover, the ability of coping with the disease it is influenced by social support, relationships with health professionals and help and information attending to patient associations. CONCLUSIONS: The health professional must provide clear information on the pathology to the fibromyalgia suffers, the larger lever of knowledge of the patients about their disease and the better the quality of the information provided, it is reported to be the less anxiety and uncertainty in the experience of the disease. Likewise patient associations should have health professionals in order to avoid bias in the information and advice with scientific evidence.

In planning treatment for each new patient, radiation oncologists pay attention to the aspects that they control. Thus their attention is usually focused on volume and dose. The dilemma for the physician is how to protract the treatment in a way that maximizes control of the tumor and minimizes normal tissue injury. The initial radiation-induced damage to DNA may be a biological indicator of the quantity of energy transferred to the DNA. However, until now the biophysical models proposed cannot explain either the early or the late adverse effects of radiation, and a more general theory appears to be required. The bystander component of tumor cell death after radiotherapy measured in many experimental works highlights the importance of confirming these observations in a clinical situation.

RESUMO

The transformation of industrial areas towards greater sustainability results from a strategic objective to address the effects of economic and environmental crisis. Such transformation, however, requires methodologies and tools that support and facilitate the process. This paper proposes an analytical model that favours the redesign of industrial areas based on sustainable strategies for eco-industrial parks. The proposed model is enhanced by the definition of building blocks of an agent-based modelling method. The methodology that was followed favours the detailed description of the objectives of the system, with individual elements and adaptation to the surrounding environment, amongst other features. The proposed model integrates a knowledge database that supports the process of identification of cooperative strategies such as material exchange networks in industrial areas. The underlying theory for the assessment of cooperative interactions is game theory, which supports the resolution of problems with strategic choices. This work covers the stage of analytical model formulation that is essential for advancement towards the inference process based on simulation models.

La formación de enfermeras se caracteriza por ser una práctica compleja y multifuncional vinculada al contexto social y sanitario donde se desarrolla. Dada la complejidad de este proceso, las enfermeras asistenciales deben asumir un rol preponderante en la formación de alumnos, siendo imprescindible dotarles de voz en todo el proceso. El objetivo de este trabajo se centra en clarificar la etimología empleada para denominar a las enfermeras asistenciales responsables del proceso formación y delimitar las funciones que deben de realizar durante el proceso de prácticas de alumnos del grado de enfermería. La nomenclatura más utilizada para las tareas de asesoramiento y evaluación del alumnado, son: mentor, counselling, coaching y tutor, siendo esta última la más utilizada en nuestro entorno. La principal función del tutor se centra en la necesidad de potenciar la capacidad crítica del alumno, así como el desarrollo de la reflexión ante un determinado fenómeno de salud-enfermedad (AU)


The training of nurses is characterized by a complex and multifunctional practice linked to health and social context in which it develops. Given the complexity of this process, practicing nurses must take a leading role in the training of students and it is essential to provide them with a voice in the process. The aim of this work focuses on clarifying the etymology used to refer to practicing nurses responsible for training process and define the functions that must be performed during the process graders practice nursing. The nomenclature used for most tasks the students support and advice are: mentoring, counseling, coaching and tutor latter being the most used in our environment. The main function of the coach focuses on the need to enhance the student's critical thinking and reflection development to a phenomenon of health and disease (AU)

Background and Objectives: Inconsistencies in the relationship between schizotypy dimensions and neurocognitive functions found in correlational studies may be clarified with the use of alternative methodological approaches. The aim of this study was to examine the existence of different profiles of schizotypal traits and their neurocognitIve correlates in non-clinical subjects by means of cluster analysis. Methods: We examined seventy six healthy adults from the general population with a comprehensive neurocognitive battery and a schizotypal personality self-report. Results: Four neurocognitive factors were extracted: visuospatial, semantic evocation, verbal memory, and set-shifting. A three cluster model yielded the following clusters:“low schizotypy”, “positive schizotypy”, and “negative/disorganized schizotypy”. The positive and negative/disorganized schizotypy clusters showed poorer performance on semantic evocation compared with the low schizotypy cluster. Conclusions: We found different patterns of specific schizotypy features in a healthy adult community sample and these clusters presented differential performance in relation with the ability to evoke semantic information (AU)

RESUMO

The high treatment demand which outpatient nurses in the Outpatient Oncology Unit have to deal with requires a protocol which guarantees an integrated and continuous patient treatment process. The purpose of this article is to design a standardized format adapted to the Outpatient Oncology Unit at the General University Hospital in Elche in order to evaluate and follow up on patients undergoing chemotherapy treatment. This register makes it possible to improve the organization in this ward as well as improve the scientific knowledge nurses tasks have; the authors emphasize the need to register cases, not only for ethical and legal considerations, but in order to provide maximum usefulness in treatment extended to patients.
