La técnica quirúrgica para el cierre de la pared abdominal se basa a menudo en la tradición más que en la evidencia. Un conocimiento de la función dinámica de la pared abdominal es esencial para su cierre adecuado. Es necesario que los cirujanos identifiquen y utilicen técnicas y materiales apropiados para minimizar la incidencia de fallos incisionales como la evisceración y la eventración. Los errores técnicos locales como el cierre bajo tensión (mala relajación) que estrangula la fascia y la toma inadecuada de tejido con la sutura son las causa más frecuentes de eventración (AU)

Abdominal wall closure surgical practice continues to rely largely on tradition rather tan evidence. We do need to identify appropriate technique and suture material in order to achieve a lower rate of incisional hernia. Local technical errors as high tension closure which stretches fascia and an inadequate fascia´s margin with suture are the most frequent causes of incisional hernia (AU)

[Three cases of unusual non-germ cell tumors of the testicle]. / Tumores no germinales inusuales del testículo: tres casos clínicos.

By describing 3 clinical cases of unusual testicular non germinal tumors, including an adenoma of the rete testis, an undifferenciated sex cord tumor and a mesothelioma of the tunica vaginalis, we make a literature review of the unusual testicular tumors and testicular apendix, including their incidence and management. Also and as one of our conclusions, we expose the importance of the intraoperatory biopsy in the testicular cancer surgery, because even if it is infrecuent, the presence of this rare testicular tumors, in which if they are proven to be benign, the testicular unit could be preserved and the radical orquiectomy could be avoided.

Esfinter urinario artificial: abordaje transescrotal / Artificial urinary sphicter

Se presenta un caso clínico de paciente masculino con insuficiencia esfinteriana genuina y disfunción eréctil posterior a una prostatectomía radical, revisando en detalle el estudio urodinámico y sus hallazgos. Se realiza un abordaje combinado transescrotal para la colocación simultánea del esfínter AMS 800 y de la prótesis de pene AMS 700, describiendo resultados y seguimiento posterior. Se realiza una revisión bibliográfica de los esfínteres colocados por vía transescrotal, descripción de la técnica quirúrgica e indicaciones y se comparan resultados con los obtenidos en nuestro centro. Demostrando que la vía transescrotal es plausible pero con indicaciones precisas y que aún sus resultados son inferiores a los descritors para la vía perineal

We prresent a case of post prostatectomy incontinence and erectile dysfunction, reviewing its urodynamic findings. Using a combined transcrotal approach, is implanted an AMS 800 penile prosthesis on the same procedure, describing results and follow up. We have done a review of the literature involving the transcrotal approach, describing the surgical technique and its indications, comparing the results with the ones obtained in our centre and demonstrating that the transcrotal approach is possible, but with its specific indications and still with weaker results that the perineal approach

A propósito de tres casos clínicos con tumores de histología infrecuente del testículo que incluyen el adenoma de la rete testis, el tumor indiferenciado de los cordones sexuales y el mesotelioma de la túnica vaginal, se realiza una revisión bibliográfica de los tumores inusuales del testículo y sus anexos, incluyendo su incidencia y manejo. Finalmente y a manera de conclusión, se explica la importancia de la biopsia rápida en el caso de los tumores testiculares dado que, aunque infrecuente, si ésta resultara benigna, podría evitarse la orquectomía radical y realizar una cirugía conservadora
