RESUMO

BACKGROUND: Whether surgical repair of an acute Achilles' tendon rupture by an open-repair or minimally invasive approach is associated with better outcomes than nonsurgical treatment is not clear. METHODS: We performed a multicenter, randomized, controlled trial that compared nonoperative treatment, open repair, and minimally invasive surgery in adults with acute Achilles' tendon rupture who presented to four trial centers. The primary outcome was the change from baseline in the Achilles' tendon Total Rupture Score (scores range from 0 to 100, with higher scores indicating better health status) at 12 months. Secondary outcomes included the incidence of tendon rerupture. RESULTS: A total of 554 patients underwent randomization, and 526 patients were included in the final analysis. The mean changes in the Achilles' tendon Total Rupture Score were -17.0 points in the nonoperative group, -16.0 points in the open-repair group, and -14.7 points in the minimally invasive surgery group (P = 0.57). Pairwise comparisons provided no evidence of differences between the groups. The changes from baseline in physical performance and patient-reported physical function were similar in the three groups. The number of tendon reruptures was higher in the nonoperative group (6.2%) than in the open-repair or minimally invasive surgery group (0.6% in each). There were 9 nerve injuries in the minimally invasive surgery group (in 5.2% of the patients) as compared with 5 in the open-repair group (in 2.8%) and 1 in the nonoperative group (in 0.6%). CONCLUSIONS: In patients with Achilles' tendon rupture, surgery (open repair or minimally invasive surgery) was not associated with better outcomes than nonoperative treatment at 12 months. BACKGROUND: Osteoarthritis is among the most common causes of functional disability and severe pain, and the prevalence of arthritic symptoms among adults is more than 50%. The article discusses epidemiology, pathology and treatment options. MATERIAL AND METHODS: The review is based on a non-systematic search in PubMed and the authors' experience with treating this patient group. RESULTS: Osteoarthritis is a degenerative disease which leads to loss of joint functioning. Symptoms usually present in the hip, hands and knees. Women are affected more often than men and the prevalence increases with increasing age. Some families have an increased prevalence of osteoarthritis, but the genetic etiology is not clear. Mechanic conditions such as overweight and heavy physical work explain some of the pathogenesis, but non-mechanical factors are probably involved as well. Loss of weight is likely to have a preventive effect, and surgical correction of mechanic conditions such as hip dysplasia and varus deformity can prevent development of osteoarthritis. Treatment of symptomatic osteoarthritis includes educating the patient and continues with stretching, physical exercise, weight reduction, technical aids (supporting braces, walking sticks) and analgesics. Subsequent options are treatment with paracetamol, NSAIDs and possibly opiates and finally insertion of an artificial joint. Many patients with disabling osteoarthritis function much better and have markedly less pain with an artificial joint. INTERPRETATION: Current treatment options alleviate but do not cure arthritic symptoms; preventive actions should be instigated when possible. Treatment of osteoarthritis involves many medical specialties and treatment modalities.

STUDY DESIGN: Cross-sectional study. OBJECTIVE: To compare functioning and disability in patients with hip osteoarthritis (OA) not candidates for surgery, to a matched control group, and thereby to examine the relationship between the functioning and disability components used in this study in patients with hip OA. BACKGROUND: It is well known that patients with severe hip OA have deficits in functioning and disability. However, in patients with hip OA not candidates for surgery, the knowledge regarding functioning and disability is sparse. METHODS: Twenty-six patients (12 men, 14 women; mean age, 60 years) with radiographic and symptomatic hip OA were matched to 26 controls without hip pain. The following variables were measured: muscle strength using isokinetic peak force, hip passive range of motion, submaximal aerobic capacity using a cycling test, walking ability using the 6-minute walk test, self-reported pain, stiffness, and physical function using the Western Ontario and McMaster University Osteoarthritis Index, and health-related quality of life using the SF-36. RESULTS: The patients with hip OA had mild to moderate pain, as indicated by the Western Ontario and McMaster University Osteoarthritis Index, and significantly lower knee extension strength (mean difference [95% confidence interval {CI}]: -19.5 [-34.3, -4.7] Nm). Hip range of motion was significantly less in the patients with hip OA, with mean (95% CI) differences of -10° (-14°, -6°) for extension, -18° (-26°, -11°) for flexion, -9° (-14°, -4°) for abduction, -2° (-5°, 0°) for adduction, -16° (-23°, -9°) for internal rotation, and -21° (-28°, -14°) for external rotation. The patients with hip OA walked a significantly shorter distance in 6 minutes (mean difference, -75 m; 95% CI: -131, -20 m). There were no significant differences in hip extension/flexion, knee flexion, ankle dorsiflexion/plantar flexion muscle strength, or aerobic capacity between the 2 groups. There were significant associations between body function and activity components. CONCLUSION: Physical therapists should consider including quadriceps-strengthening and hip range-of-motion exercises when developing rehabilitation programs for patients with hip OA, with mild to moderate pain, aiming to improve functioning and reduce disability.
