BACKGROUND: The purpose of this study is to investigate the effect of wearing neutral density (ND) filters with different transmittance levels over multifocal contact lenses (MTF CLs) on pupil size, visual functions and the dynamic accommodative response under daylight conditions in early presbyopes. METHODS: Seventeen individuals aged between 40 and 48 years (mean age 42.35±2.62 years) were recruited to participate in this study. This study involved a repeated-measures design with two within-subject factors: optical condition (which had five levels), and fixation distance (which had three levels). The five optical conditions were no correction (baseline), MTF CL wear, MTF CL wear + 0.3 ND filter (transmission [Tv]=48.38%), MTF CL wear + 0.6 ND filter (Tv=27.12%) and MTF CL wear + 0.9 ND filter (Tv=14.58%). The three fixation distances were 3 m, 40 cm and 20 cm. Outcome measures were pupil size, the measured accommodative response (optical aid + physiological accommodation), distance visual acuity (VA), near VA and contrast sensitivity (CS). RESULTS: There was no effect of optical condition on the accommodative response; however, there was a significant effect of fixation distance. (Accommodative response was 0.44±0.29, 1.83±0.37 and 4.03±0.46 D for fixation distances of 3 m, 40 cm and 20 cm, respectively; p<0.05.) There was a significant effect of optical condition (p<0.05) and of fixation distance on pupil size variation (5.13±0.90 mm for 3 m, 4.71±0.92 mm for 40 cm and 4.11±1.04 mm for 20 cm; p<0.05). Optical condition had a significant effect on distance VA, near VA and CS (p<0.05). Compared to the reduction in distance VA, near VA was relatively small (two-letter reduction with 0.9 ND) and unlikely to be clinically significant. CONCLUSION: This study demonstrated that in a group of adults with early presbyopia, the use of ND filters enlarged pupil size. However, the combination of enlarged pupils and MTF CL did not improve distance vision, and if transmittance was reduced by 50% it resulted in decrements in vision and CS. MTF CL designs which incorporate tints, or tints that vary in darkness in response to ultraviolet exposure, should consider these findings.

Attention may influence the motor performance of poststroke patients. The attentional focus implies "where" attention is focused while performing a specific movement. Focusing attention on the inside of the body while performing a specific exercise is called an internal focus (IF) and focus on the external environment is called an external focus (EF). This study examined the effect of attention on the gait of patients with chronic stroke. Sixteen subjects voluntarily participated in the study. The subjects were guided to walk while maintaining the required attentional focus, which is control, IF, and EF condition. Dependent variables were as follows: step length, stride length, step width, 10-m walk time, and weight distribution on the paretic limb. The results are as follows. First, the step length and the stride length were observed to be longer in the EF condition. Second, 10-m walk time was faster in EF condition. Third, under the EF condition, more weight was applied to the paretic limb. We suggest that EF instruction useful to improve the poststroke gait.

Purpose: Individuals in the later stages of cerebellar ataxia usually experience serious balance and immobility problems. Currently, there is a lack of adequate rehabilitative programs for individuals with severe cerebellar ataxia that can help improve ataxia-related motor impairment. The purpose of the present study was to explore the potential physiotherapeutic benefits of partnered dance on balance, motor functions, and psychological well-being in an individual demonstrating severe cerebellar ataxia symptoms. Methods: The individual was a 39-year-old male diagnosed with cerebellar atrophy. He had the disease for more than 15 years prior to the study. The individual attended 24 intervention sessions over an 8-week period of dance-based movement training that aimed to improve his balance and postural stability by facilitating the perception and control of static and dynamic balance movements and body alignment. Results: The individual demonstrated improvements in independent standing balance, gait characteristics, and functional mobility. In addition, improvements in self-reported depression and quality of life scores were observed after completion of the intervention. Conclusion: Although interpreting the findings of this study is limited to a single participant, partnered dance could be a suitable alternative physiotherapeutic intervention method for people with severely impaired mobility due to cerebellar dysfunction.

[Purpose] This study examined the effect of the degree of the contact area between the insoles and soles on static balance. [Subjects and Methods] Thirteen healthy male and female adults voluntarily participated. All of the subjects wore three different types of insoles (no orthotic insole, partial contact, full contact) in the present experiment. The subjects were instructed to place both feet parallel to each other and maintain static balance for 30 seconds. Center of pressure parameters (range, total distance, and mean velocity) were analyzed. [Results] The results show that the anteroposterior range and mediolateral (ML) total distance and velocity decreased when orthotic insoles with partial contact or full contact were used in comparison to when a flat insole (no orthotic insole) was used. Also, the ML range and total distance were lower with full contact than in the other two conditions. These results indicate that static balance improves as the degree of contact between the soles and insoles increases. [Conclusion] The results of this study suggests that using insoles with increased sole contact area would improve static balance ability.

[Purpose] The purpose of the current study was to examine age-related differences in control of a perception-action coordination skill. We adapted a visuomotor tracking experiment requiring various coordination patterns between a limb's motion and an external signal. [Subjects and Methods] A total of 12 subjects (6 elderly and 6 young) voluntarily participated in the study. The experimental session consisted of 3 trials for 3 different relative phase patterns: 0°, 90°, and 180°, defined by the relationship between the online visual feedback of the joystick motion and the white dot signal. [Results] The 0° and 180° tracking patterns were stable compared with the 90° tracking pattern for both age groups. The present results also showed that the elderly subjects were less stable than were young subjects for all tracking patterns. [Conclusion] The intrinsic coordination dynamics predicted by the Haken-Kelso-Bunz (HKB) mathematical model did not change with age, whereas utilization of visual feedback information declined overall. Further research is needed regarding methods for increasing utilization of visual feedback information from the perspective of rehabilitation.

[Purpose] This study applied whole body vibration (WBV) at different vibration frequencies to chronic stroke patients and examined its immediate effect on their postural sway. [Subjects and Methods] A total of 14 (5 males, 9 females) stroke patients participated. The subjects were randomly assigned to one of the two vibration frequency groups (10 Hz and 40 Hz). Right before and after the application of WBV, the subjects performed quiet standing for 30 seconds, and COP parameters (range, total distance, and mean velocity) were analyzed. [Results] The 10 Hz WBV did not affect the postural sway of stroke patients. The 40 Hz WBV increased postural sway in the ML direction. [Conclusion] The results suggest that WBV application to stroke patients in the clinical field may have adverse effects and therefore caution is necessary.

BACKGROUND AND PURPOSE: Regular exercise can delay age-related risk factors and can maintain or improve physical health and activity in older adults leading to a decrease in fall risk. The purpose of this study was to compare 2 different interventions for fall prevention, tai chi (TC) and Otago, by examining lower extremity strength, balance, and spatiotemporal gait parameters in community-dwelling older women. METHODS: We performed a randomized trial in which subjects were assigned to 1 of 2 groups: the TC group (n = 21; age, 72.8 ± 4.7 years, range: 65-83 years), which participated in a modified Sun-style TC exercise program; and the Otago group (n = 24; age, 71.5 ± 3.6 years, range: 65-79 years), which participated in the Otago exercise program. The Timed Up and Go (TUG) test, functional reach (FR) test, one-leg standing (OLS) test, 5 times sit-to-stand test (5×STS), 30-second sit-to-stand (30s STS) test, and gait parameters (gait velocity, step length, step width, stride time, and cadence) were measured before and after the intervention. RESULTS: Both groups showed statistically significant improvements in balance (TUG and OLS tests), lower extremity strength (5×STS and 30s STS tests), and spatiotemporal gait parameters, except for step width and step length (P < .05). The Otago group showed a significantly improved FR, whereas the TC group showed a significantly improved step length after the intervention (P < .05). Furthermore, the Otago group exhibited greater improvements in the TUG (P < .001), FR (P < .001), 5×CST (P < .01), and 30-second CST (P < .01) tests: a faster cadence (P < .001) and shorter stride time (P < .001) when compared with the TC group. The TC group showed greater improvements in the OLS test, step length, and step width (P < .01) and faster gait velocity (P < .05) than the Otago group. DISCUSSION AND CONCLUSIONS: The findings from this study support the efficacy of the TC and Otago exercise programs in improving mobility in this sample of subjects. Furthermore, the Otago group showed greater improvement in lower extremity strength, whereas the TC group showed greater improvement in balance (OLS test). Also, the TC group showed a greater improvement in gait velocity after TC training program compared with the Otago exercise program. However, this study does not elucidate which exercise program is a more effective intervention method with older women for fall prevention.

[Purpose] Proprioceptive neuromuscular facilitation (PNF) stretching is known to be effective in increasing joint ROM. The PNF stretching technique first induces an isometric contraction in the muscles to be stretched, but no agreement concerning the optimal contraction intensity has yet been reached. The purpose of the present study was to examine the effect of contraction intensity on ROM while applying PNF stretching. [Subjects and Methods] Sixty male subjects were randomly assigned to one of four groups (three experimental groups and one control group). Each experimental group applied one of three contraction intensities (100%, 60%, and 20%) defined by the MVIC ratio, and the control group did not receive any intervention during the experiment. PNF stretching was applied to left knee extensors to compare changes in the knee joint flexion angle. [Results] The results showed that the changes in ROM were larger for the 60% and 100% groups compared with the 20% group. The changes in ROM were lowest in the control group. [Conclusion] The present results indicate that while applying the PNF stretching, it is not necessary to apply the maximum intensity of muscle contraction. Moderate isometric contraction intensities may be optimal for healthy young males, while a sufficient effect can be obtained even with a low contraction intensity.

[Purpose] This study investigated the effects of craniocervical flexion exercise on upper-limb postural stability by measuring upper-limb postural tremor during a goal-directed pointing task. [Subjects and Methods] Twenty-four subjects were randomly assigned to the exercise or control group. The exercise group performed craniocervical flexion exercise four days per week for five weeks. Upper-limb postural tremor was measured by using a three-dimensional electromagnetic motion tracking system (trakSTAR™, Ascension Technology Corporation, Burlington, VT, USA) during a goal-directed pointing task. [Results] In the exercise group, the range and velocity of the trajectories of the shoulder, wrist, and finger in the lateral direction improved significantly. However, no significant changes were observed in the control group. [Conclusion] Craniocervical flexion exercise reduces the range and velocity of upper-limb postural tremor, thereby increasing postural stability.

[Purpose] The present study reviewed studies that examined the effects of attentional focus on balance. [Methods] Keywords such as "attentional", "focus", and "balance" were used to find relevant research papers in PubMed (www.ncbi.nlm.nih.gov/pubmed). Forty-five papers were found, and 18 of them were used for this study, excluding review papers and papers irrelevant to the topic of this study. [Results] Among the papers used for the review, the number of papers in which external focus produced effective outcomes was 15 (83.3%). The number of papers in which both external and internal focus produced effective outcomes was 2 (11.1%). The number of paper in which no instruction about attentional focus was effective was 1 (5.5%), and the number of papers in which internal focus was effective was zero. [Conclusion] This short review suggests clinical implications about how physical therapists can use attentional focus for balance rehabilitation of patients. Instructions about external focus of attention can generally be useful as a method to improve posture and balance control. Furthermore, the present reviews indicates that external focus of attention would be more useful in a rehabilitation stage in which the difficulty level of balance performance is gradually increased.

[Purpose] Systemic lupus erythematosus (SLE) is a chronic, immune-mediated disease, affecting 0.1% of the general population. To date, few studies have investigated the efficacy of physical therapy for SLE patients with CNS involvement. The aim of this study was to report whether the combined use of corticosteroids and physical therapy, consisting of reflex inhibition and functional training, was beneficial to functional recovery. [Subjects and Methods] A 22-year-old male SLE patient with CNS involvement requested physical therapy due to strong spasticity of the trunk and limbs in a bedridden state. Corticosteroid intervention and physical therapy were undertaken for 16 days. [Result] After 16 days of the interventions, the patient demonstrated stabilized and alleviated neurological symptoms and an improved functional level. [Conclusion] The present case indicates that physical therapy combined with corticosteroids might be a possible treatment and rehabilitation method to effectively recover motor function for SLE patients who have strong spasticity due to CNS involvement.

BACKGROUND: The purpose of this study was to evaluate the possibility of using text-mining to identify clinical distinctions and patient concerns in online memoires posted by patients with fibromyalgia (FM). MATERIAL AND METHODS: A total of 399 memoirs were collected from an FM group website. The unstructured data of memoirs associated with FM were collected through a crawling process and converted into structured data with a concordance, parts of speech tagging, and word frequency. We also conducted a lexical analysis and phrase pattern identification. After examining the data, a set of FM-related keywords were obtained and phrase net relationships were set through a web-based visualization tool. RESULTS: The clinical distinction of FM was verified. Pain is the biggest issue to the FM patients. The pains were affecting body parts including 'muscles,' 'leg,' 'neck,' 'back,' 'joints,' and 'shoulders' with accompanying symptoms such as 'spasms,' 'stiffness,' and 'aching,' and were described as 'sever,' 'chronic,' and 'constant.' This study also demonstrated that it was possible to understand the interests and concerns of FM patients through text-mining. FM patients wanted to escape from the pain and symptoms, so they were interested in medical treatment and help. Also, they seemed to have interest in their work and occupation, and hope to continue to live life through the relationships with the people around them. CONCLUSIONS: This research shows the potential for extracting keywords to confirm the clinical distinction of a certain disease, and text-mining can help objectively understand the concerns of patients by generalizing their large number of subjective illness experiences. However, it is believed that there are limitations to the processes and methods for organizing and classifying large amounts of text, so these limits have to be considered when analyzing the results. The development of research methodology to overcome these limitations is greatly needed.

The aim of this study was to determine whether an 8-week period of backward walking (BW) training on the treadmill can improve the gross motor function measure (GMFM), weight-bearing symmetry, and temporospatial gait parameters in individuals with spastic cerebral palsy. Twelve participants aged 5-15 years with spastic cerebral palsy participated in this study. The BW training was conducted on a treadmill for up to 20 min, with three sessions per week for 8 consecutive weeks. Before each treadmill training session, lower limb stretching was included in the session. Interlimb differentials of vertical ground reaction force while standing, temporal-distance gait parameters, and scores of dimension D and dimension E of the GMFM were determined. Participants showed statistically significant improvements in a measure of GMFM (P<0.01) and weight-bearing symmetry value (P<0.05), forward walking velocity (P<0.05), and step/stride length (P<0.05 or P<0.01). This pilot study suggests that BW therapy on a treadmill may help to improve walking abilities and other gross motor skills in this sample of patients.
