In this market insights program, AMCP brought together a panel of experts representing various stakeholders: national and regional health plans, integrated health care systems, employer benefits groups, clinical experts, the Centers for Disease Control and Prevention, and patient advocacy organizations. The objectives were to gain insights into the current and evolving treatments in hemophilia, sickle cell disease, and ß-thalassemia; measure the effects of recently approved therapies on clinicians, payers, and patients; recognize emerging trends within the stop-loss market; address potential issues and obstacles related to monitoring and reporting outcomes; and identify concerns associated with both existing and emerging contracting and reimbursement models. This article aims to summarize expert perspectives on health care system challenges and strategies concerning the management of inherited blood disorders and to advance managed care professionals' understanding of their role in supporting care for these patients. The experts emphasized that when shaping coverage policies, a patient-centered approach is crucial, focusing on preserving organ function to maintain eligibility for future gene therapies among individuals with inherited blood disorders. These strategies, including benefit design modifications, specialized provider networks, and centralized mechanisms like registries, are vital for evaluating effectiveness, facilitating decision-making, and managing costs and risks associated with new and emerging treatment options for inherited blood disorders.

SUMMARY OBJECTIVE: Endogenous melatonin is produced from tryptophan which is an essential amino acid. Besides its role in the regulation of sleep patterns, melatonin has anti-inflammatory effects. In this case-control study, we aimed to compare tryptophan and melatonin levels and their relationship with the inflammatory response, specifically serum interleukin-1, interleukin-6, and c-reactive protein levels following major abdominal surgery in patients with food restriction and who receive parenteral nutritional therapy. METHODS: We enrolled 40 patients between the ages of 18 and 65 years in the study. We collected blood and urine samples 48 h before the operation and on postoperative days 1, 3, and 5. RESULTS AND CONCLUSION: The tryptophan levels in the experimental group were higher than in the control group but failed to reach any statistical difference. Melatonin levels were increased in both groups following the surgery compared with preoperative levels. The increase in the experimental group was statistically different 3 days after the surgery. The difference in the level of interleukin-1 between the control and the experimental groups was greatest on postoperative day 3. On postoperative day 3, the interleukin-6 level in the treatment group was slightly higher than in the control group. We did not find any difference in the levels of c-reactive protein between the groups. As a result, the levels of tryptophan and melatonin were increased in the parenteral nutrition group, irrespective of the postoperative inflammatory response.

Resumen Antecedentes: El diagnóstico de apendicitis aguda representa un reto en pacientes pediátricos. Objetivo: Establecer la importancia del índice neutrófilos-linfocitos (INL), índice plaquetas-linfocitos (IPL) y otros parámetros hematológicos ajustados por edad y sexo en la predicción de apendicitis aguda, así como describir un nuevo sistema de calificación. Material y métodos: Se analizaron retrospectivamente expedientes clínicos de 946 niños hospitalizados por apendicitis aguda. Se desarrolló un sistema de calificación basado en INL, IPL, ILM y proteína C reactiva (PCR) ajustados por edad y sexo. Resultados: Los pacientes se dividieron en grupo I de exploración negativa y grupo II de apendicitis aguda; las medias de edad correspondientes fueron 12.20 ± 2.31 y 11.56 ± 3.11. El recuento leucocitario, porcentaje de neutrófilos, INL, IPL, ILM y PCR fueron superiores en el grupo II. La calificación osciló entre 0 y 8 puntos; se determinó que 4.5 fue el mejor punto de corte para apendicitis aguda con mayor área bajo la curva (0.96), sensibilidad (94 %), especificidad (86 %), valor predictivo positivo (97.5 %), valor predictivo negativo (65 %), precisión (92.6 %) y tasa de clasificación errónea (7.4 %). Conclusión: El sistema de calificación que se propone, calculado por edad y sexo de los pacientes, se puede utilizar para evitar cirugías innecesarias.


Abstract Background: Acute appendicitis diagnosis can sometimes be a real challenge in pediatric patients. Objective: To establish the importance of neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), and other hematological parameters adjusted for age and sex in the prediction of acute appendicitis, as well as to describe a new scoring system. Material and methods: Medical records of 946 children hospitalized for acute appendicitis were retrospectively analyzed. A scoring system based on NLR, PLR, lymphocyte/monocyte ratio (LMR), and C-reactive protein (CRP) adjusted for age and sex was developed. Results: Patients were divided into group I, with negative examination, and group II, with acute appendicitis; mean ages were 12.20 ± 2.31 and 11.56 ± 3.11, respectively. Leukocyte count, neutrophil percentage, NLR, PLR, LMR and PCR were higher in group II. The scores ranged from 0 to 8 points; 4.5 was determined to be the best cut-off point for acute appendicitis with the highest area under the curve (0.96), sensitivity (94%), specificity (86%), positive predictive value (97.5%), negative predictive value (65%), accuracy (92.6%) and misclassification rate (7.4%). Conclusion: The proposed scoring system, calculated based on patient age and gender, can be used for unnecessary surgeries to be avoided.

Introducción: El presente artículo resume la guía de práctica clínica (GPC) para el manejo de la enfermedad renal crónica estadíos 3b, 4 y 5 en el Seguro Social del Perú (EsSalud). Objetivo: Proveer recomendaciones clínicas basadas en evidencia para el manejo de pacientes con enfermedad renal crónica estadíos 3b, 4 y 5 en EsSalud. Métodos: Se conformó un grupo elaborador de la guía (GEG) que incluyó especialistas y metodólogos. El GEG formuló 9 preguntas clínicas. Se realizó búsquedas sistemáticas de revisiones sistemáticas y estudios primarios en PubMed entre diciembre del 2020 y agosto del 2021. Se seleccionó la evidencia para responder a las preguntas clínicas planteadas. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). El GEG usó la metodología GRADE para revisar la evidencia y formular recomendaciones, los puntos de buena práctica clínica (BPC) y los flujogramas de manejo. Finalmente, la GPC fue aprobada con Resolución N° 88-IETSI-ESSALUD-2021. Resultados: La presente GPC abordó 9 preguntas clínicas. En base a dichas preguntas se formularon 17 recomendaciones (7 fuertes y 10 condicionales), 28 BPC, y 4 flujogramas de manejo. Conclusión: El presente artículo resume la metodología y las conclusiones basadas en evidencias de la GPC para el manejo de la Enfermedad Renal Crónica estadíos 3b, 4 y 5 en EsSalud.


Introduction: This article summarizes the clinical practice guideline (CPG) for the management of stage 3b, 4, and 5 chronic kidney disease (CKD) in the Social Security of Peru (EsSalud). Objective: To provide evidence-based clinical recommendations for the management of stage 3b, 4, and 5 CKD in EsSalud. Methods: A guideline development group (GDG) was formed, including specialists and methodologists. The GDG formulated 9 clinical questions. Systematic searches for systematic reviews and primary studies were conducted in PubMed from December 2020 to August 2021. Evidence was selected to answer the clinical questions posed. The certainty of the evidence was evaluated using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. The GDG used the GRADE methodology to review the evidence and formulate recommendations, points of good clinical practice (GPC), and management flowcharts. Finally, the CPG was approved with Resolution No. 88-IETSI-ESSALUD-2021. Results: This CPG addressed 9 clinical questions. Based on these questions, 17 recommendations (7 strong and 10 conditional), 28 GPC points, and 4 management flowcharts were formulated. Conclusion: This article summarizes the methodology and evidence-based conclusions of the CPG for the management of stage 3b, 4, and 5 CKD in EsSalud.

Soundscapes are characterized by a combination of natural and anthropogenic sounds. This study evaluated the stress effect of biological and anthropogenic sounds characterizing a Man and Biosphere UNESCO wetland, by assessing the protein content, oxidative biomarkers, and behavior of a key crab species (Neohelice granulata), through a tank-laboratory experiment. Biological sounds corresponded to predators of N. granulata (fish and crustacean stimuli), while anthropogenic ones belonged to motorboat passages (boat stimulus). Biochemical results showed differences depending on the sound stimuli used and the crab tissue analyzed. Protein content was higher in hemolymph when crabs were exposed to fish and boat stimuli, and in gills when exposed to boat stimulus. The enzymatic activity in hemolymph showed a decreased GST (fish stimulus) and CAT (fish and boat stimuli) activity, in hepatopancreas a higher GST (crustacean stimulus) and CAT (crustacean and boat stimuli) activity was found, and in gills a higher CAT activity was also observed (crustacean and boat stimuli). Lipid peroxidation was higher only in hemolymph (fish and crustacean stimuli). Protein oxidation was higher in gills (fish stimulus) and hepatopancreas (crustacean stimulus). Behavioral analysis demonstrated that the crab locomotion activity diminished when exposed to diverse sound stimuli. Thus, both sound sources caused physiological and behavioral stress in this species. The results contribute important data to be used in the development of management plans considering the habitat importance in terms of biodiversity, the ecosystem services provided and the role of the studied species.

RESUMO

El chupete es un dispositivo utilizado para calmar a los lactantes y niños pequeños; sin embargo, se lo asocia a efectos nocivos. Muchas veces en la consulta los cuidadores solicitan la opinión de los profesionales de la salud sobre su uso; por este motivo, decidimos realizar una búsqueda bibliográfica de la mejor evidencia disponible. En base a la información recabada y analizada, concluimos que la introducción precoz del chupete (antes de las dos semanas de vida) no afectaría el amamantamiento hasta los seis meses, y que en niños con lactancia materna bien establecida y madres motivadas para amamantar, no influye en el destete precoz. Además existe evidencia débil a favor del uso del chupete para reducir el riesgo del síndrome de muerte súbita del lactante, mientras que su uso durante la dentición primaria se asocia al desarrollo de mordida cruzada posterior. The pacifier is a device used to soo the infants and young children; however, it is associated with harmful effects. Many times during consultation, caregivers ask for the opinion of health professionals about its use; for this reason, we decided to perform a literature search of the best available evidence. Based on the collected and analyzed information, we concluded that the early introduction of the pacifier (before two weeks of life) would not affect breastfeeding until six months of age, and that in children with well-established breastfeeding and mothers motivated to breastfeed, it does not influence early weaning. Furthermore, there is weak evidence in favor of pacifier use to reduce the risk of sudden infant death syndrome, while its use during primary dentition is associated with the development of posterior crossbite. ABSTRACT Objective: To determine the growth reference value of children in our province and to compare these values with current values of the USA and other local values of our country. Methods: A total of 615 boys and 586 girls with different socioeconomic levels and ages of 0-5 years were included in the study. In all children, weight, height, head circumference, abdominal circumference, hip circumference, mid-arm circumference, and lower segment length were taken. All measurements were performed by the same paediatrician. The lambda-mu-sigma method was used for the preparation of percentile curves. Results: The body weight, head circumference and chest circumference measurements of boys were significantly higher than those in girls. Anthropometric measurements of children in high-income groups were higher than those in the low-income group. All of the anthropometric parameters except upper mid-arm circumference were higher in children fed with breast milk and formula milk than in children who only fed with breast milk. The relation between the mother's education level and anthropometric parameters was insignificant. Conclusion: Our results showed that the anthropometric measurements of our children were similar to those of west Turkey as well as those of the United States. On the other hand, our children's anthropometric measurements were higher than other local values. Relatively high values of height have been interpreted as a genetic feature.

El objetivo de este trabajo fue describir y profundizar la vivencia de las mujeres en su rol como amas de casa (AC). Es un estudio cualitativo, fenomenológico con mirada etnográfica. Se realizaron veinte entrevistas semiestructuradas analizadas en profundidad, familigramas y ecomapas, a AC de 29 y 61 años seleccionadas en forma intencional y por conveniencia en una población rural y otra urbana. Ama de casa y nada más: Las AC están pendientes de aquellos que las rodean, "sos maestra, cocinera, costurera, un montón de cosas…". Aunque su dedicación es de tiempo completo, su relato finaliza con "(…) nada más (…)". Nunca termina: Un día de las AC resulta agotador, "siempre hay cosas para hacer". ¿Atrapadas sin salida?: El tiempo libre es gestionado de diferente manera. Algunas no cuentan con este momento por la dificultad que tienen para delegar tareas, "sin mí no son nada". Quisiera ser…: varias no pudieron proyectarse más allá del día a día, otras expresaron sentimientos de resignación y algunas se vieron esperanzadas en realizar nuevas actividades. "La que no puede enfermarse soy yo": Nadie las sustituye en su trabajo, remarcando tener que estar 100 % disponibles para sus familias. "Se dice de mi": Se sienten valoradas por su familia, no socialmente, haciendo referencia al machismo predominante, "los hombres creen que es muy fácil". Es un trabajo invisible que se valora cuando está ausente. Creemos que, como Médicos de Familia podemos intervenir brindando un refuerzo positivo en cada consulta, empoderándolas, reconociendo que su trabajo es importante y valioso (AU)


The objective of this research was to describe and deepen into the experience of women in their role as housewives (HW). It is a qualitative, phenomenological research with an ethnographic view, based on 20 semi-structured interviews analyzed in depth, with family genograms and ecomaps, from HW between 29 and 61 years old, selected intentionally and for convenience, on a rural and a urban scenary. Housewife and nothing else: The HWs are aware of those around them, "you're a teacher, cooker, seamstress, a lot of things ...". Although their dedication is full time, their story ends with "(...) nothing else (...)". It never ends: A HW's day is exhausting, "there're always things to do." Caught with no way out?: Free time is managed in different ways. Some do not have this moment, because of the difficulty to delegate tasks, "without me they're nothing". I would like to be ...: many of them could not project beyond their rutine, others expressed feelings of resignation and some were hopeful at performing new activities. "The only one who can't get sick is me": No one substitutes them in their work, they have to be a 100% available for their families. "It's said of me": They feel valued by their families, not socially, referring to the predominant machismo, "men believe it's very easy." It's an invisible work that's valued when it's absent. We believe that, as Family Physicians, we can intervene by providing positive reinforcement in each consultation, empowering them, recognizing that their work is important and valuable (AU)

RESUMEN El presente artículo resume la guía de práctica clínica (GPC) para tamizaje, diagnóstico y manejo de los pacientes con enfermedad renal crónica (ERC) en los estadios 1 al 3 en el Seguro Social del Perú (EsSalud). Para el desarrollo de esta GPC, se conformó un grupo elaborador de la guía (GEG) que incluyó especialistas clínicos y metodólogos, el cual formuló ocho preguntas clínicas. Para responder cada pregunta se realizaron búsquedas sistemáticas de revisiones sistemáticas y, cuando fue considerado pertinente, de estudios primarios; y se seleccionó la evidencia pertinente. La certeza de la evidencia fue evaluada usando la metodología Grading of Recommendations Assessment, Development, and Evaluation (GRADE). En reuniones periódicas, el GEG usó la metodología GRADE para revisar la evidencia y emitir las recomendaciones. Se emitieron ocho recomendaciones (cuatro fuertes y cuatro condicionales), 29 puntos de buena práctica clínica, y tres flujogramas.

The certainty of the evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. In periodic work meetings, the GDG used the GRADE methodology for reviewing the evidence and for developing recommendations. At the end, this CPG formulated 08 recommendations (04 strong and 04 conditional), 29 points of good clinical practice, and 03 flowcharts were formulated.

OBJECTIVE To evaluate the effectiveness of adherence to a multidisciplinary renal health program in reducing mortality and progression to hemodialysis. METHODS We used a database that included patient monitoring (2013-2017), dialysis admissions and all cause of mortality in Peru. Adherence to the program was established by meeting minimum visits during the first year of monitoring. The outcome of interest was hemodialysis admissions or all cause-mortality. Kaplan-Meier curves, Log-Rank test and competing survival analysis methods were used to estimate the differential risk between adherent and non-adherent patients. RESULTS A total of 20,354 participants was evaluated; 54.1% were male, 72.1 years old in average, 2.2 years average follow-up, and 15,279 (75.1%) belonged to the early stages (1 to 3a) of Chronic Kidney Disease. Adherence decreased the risk of renal replacement therapy in 41.0% (HR = 0.59, 95%CI 0.41-0.85) in the low-risk group and mortality in the high-risk group was 31.0% (HR = 0.69, 95%CI 0.57-0.83). CONCLUSIONS The multidisciplinary care strategy with standardized assessments by stage is effective in reducing admission to .0when the patient is identified in early stages and in reducing mortality in advanced stages.

RESUMO

Replacement of regular salt with potassium-enriched substitutes reduces blood pressure in controlled situations, mainly among people with hypertension. We report on a population-wide implementation of this strategy in a stepped-wedge cluster randomized trial (NCT01960972). The regular salt in enrolled households was retrieved and replaced, free of charge, with a combination of 75% NaCl and 25% KCl. A total of 2,376 participants were enrolled in 6 villages in Tumbes, Peru. The fully adjusted intention-to-treat analysis showed an average reduction of 1.29 mm Hg (95% confidence interval (95% CI) (-2.17, -0.41)) in systolic and 0.76 mm Hg (95% CI (-1.39, -0.13)) in diastolic blood pressure. Among participants without hypertension at baseline, in the time- and cluster-adjusted model, the use of the salt substitute was associated with a 51% (95% CI (29%, 66%)) reduced risk of developing hypertension compared with the control group. In 24-h urine samples, there was no evidence of differences in sodium levels (mean difference 0.01; 95% CI (0.25, -0.23)), but potassium levels were higher at the end of the study than at baseline (mean difference 0.63; 95% CI (0.78, 0.47)). Our results support a case for implementing a pragmatic, population-wide, salt-substitution strategy for reducing blood pressure and hypertension incidence.

RESUMEN OBJETIVO Evaluar la efectividad de la adherencia a un programa de salud renal en la reducción de mortalidad y progresión a hemodiálisis. MÉTODOS Utilizamos una base de datos que condensaba el seguimiento de los pacientes (2013-2017), los ingresos a diálisis de los mismos y la mortalidad por todas las causas en Perú. La adherencia al programa se estableció con el cumplimiento de visitas mínimas durante su primer año de seguimiento. La efectividad de la adherencia al programa se midió en términos de debut a hemodiálisis o muerte por todas las causas. Se utilizaron curvas de Kaplan-Meier, test de diferencias en la distribución (Log-Rank test) y métodos de análisis de supervivencia. Los análisis se realizaron utilizando R estudio 3.5.0 RESULTADOS Fueron evaluados 20.354 participantes, 54,1% varones, edad media de 72,1 años, con un seguimiento medio de 2,2 años; 15.279 (75.1%) tuvieron ERC en estadios tempranos (estadio 1 al 3a). La adherencia disminuyó en un 41,0% el riesgo de terapia de reemplazo renal (HR = 0,59; IC95% 0,41-0,85) en el grupo de bajo riesgo y en un 31,0% (HR = 0,69; IC95% 0,57-0,83) la mortalidad en el grupo de alto riesgo. CONCLUSIONES La estrategia de cuidado multidisciplinario con evaluaciones estandarizadas según estadio es efectiva en reducir el ingreso a terapia de reemplazo renal cuando se identifica al paciente en estadios tempranos y en reducir la mortalidad en estadios avanzados.

Very few studies of sound production in the Brachyura have simultaneously identified the type of individuals (e.g., sex) producing acoustic signals, the structures involved in making sound and the social context. The emission and type of sound signals in Neohelice granulata were previously characterized, but the sex and the body structures involved in the sound production mechanism were not determined. In the present study, experiments conducted in the laboratory demonstrated that acoustic signals were produced by males through an up-down movement of the cheliped by rubbing the merus against the pterygostomial area of the carapace. The micromorphology of the merus showed that it has a ridge of tubercles which may act as a plectrum, while the pterygostomial area bears tubercles and might function as the pars stridens. Acoustic signals were displayed more frequently in the presence of receptive females. Agonistic encounters among males also occurred more often in the presence of receptive females. The authors propose that Neohelice granulata males use their chelipeds to produce sound signals in a mating context, probably to attract the receptive female and/or to repel other males when a receptive female is present. Thus, the display might have a reproductive function influencing mate choice.

Resumen Los objetivos de este estudio fueron: 1. Comparar la presencia de sobrepeso y obesidad en adolescentes de Argentina vs. España, 2. Comparar la personalidad según diagnóstico nutricional (DN) y lugar de residencia (LR), y 3. Identificar la presencia de riesgo psicopatológico (RP). Participaron 272 adolescentes, hombres y mujeres, de 11-16 años de edad, quienes completaron el Cuestionario de Personalidad para Niños de Eysenck (EPQ-J) y, para calcular el índice de masa corporal, fueron recabadas medidas de talla y peso. Presentaron sobrepeso u obesidad 39% de los adolescentes españoles y 43% de los argentinos, con quienes se conformaron dos grupos clínicos, uno por país; mientras que ambos grupos control estuvieron constituidos por los participantes normopeso. La combinación de las variables DN y LR no arrojó diferencias significativas en cuanto a las dimensiones de personalidad. Sin embargo, ambos grupos clínicos presentaron mayor neuroticismo (NE) que sus pares control. Respecto al LR, los adolescentes argentinos registraron mayor NE y dureza emocional (4% de alto riesgo) vs.3% de alto riesgo por NE en los españoles. Los datos obtenidos no permitieron establecer un perfil psicopatológico del adolescente con sobrepeso u obesidad, dado que prevaleció la influencia del país de procedencia.


Abstract The objectives of this study were: 1. To compare the presence of overweight and obesity in adolescents from Argentina vs. Spain, 2. To compare personality according to nutritional diagnosis (ND) and place of residence (PR), and 3. To identify the presence of psychopathological risk (PR). A total of 272 adolescents, men and women from 11 to 16 years completed the Personality Questionnaire for Children of Eysenck (EPQ-J) and body mass index was calculated through height and weight measures. The 39% of the Spanish adolescents and 43% of the Argentinians presented overweight or obesity, two clinical groups were formed, one per country and both control groups were constituted by the normal weight participants. The combination of the variables ND and PR did not show significant differences in terms of personality dimensions. However, both clinical groups presented greater neuroticism (NE) than their control peers. Regarding the PR, the Argentine adolescents registered higher NE and emotional hardness (4% of high risk) than 3% of high risk of NE in the Spanish. The data obtained did not allow to establish a psychopathological profile of the overweight or obese adolescent, this is because the influence of the country of origin prevailed.

Introducción.Desde un punto de vista integral, la sexualidad comprende aspectos de orden biopsicosocial. Los prejuiciosy el desconocimiento llevan a evitar hablar sobre este tema en la consulta con el adulto mayor (AM).Objetivo.Profundizar en la vivencia de la sexualidad del AM.Metodología.Estudio cualitativo, fenomenológico, realizado en dos centros de actividades recreativas para AM en SanMiguel de Tucumán, que incluyó 13 entrevistas semiestructuradas en profundidad a pacientes mayores de 60 años, 11 desexo femenino, seleccionados en forma intencional y por conveniencia en Agosto de 2018.Resultados.Existe una dicotomía entre quienes hablan del tema y quienes no; sin embargo, todos expresan el deseode naturalizarlo tanto en su círculo social como en la consulta médica. Los AM aceptan el proceso de envejecimiento,tienen una autopercepción positiva de su imagen y pese a los prejuicios, experiencias, comorbilidades y adversidades, seadaptan buscando formas para sentirse bien y disfrutar de una sexualidad plena.Conclusión.El envejecimiento y la sexualidad son conceptos dinámicos que confluyen con las experiencias de vida.Creemos que como médicos de familia es importante brindar una atención integral, abordando las diferentes dimensionesdel ser humano, incluyendo su sexualidad. Introduction.Sexuality from an integral point of view includes aspects of biopsychosocial order. Prejudice and ignorancelead to avoid talking about this topic in the medical consultation with the Eldery.Objective.To deepen the experience of the sexuality of the Eldery.Methodology.Qualitative, phenomenological study, conducted in two centres of recreational activities for Senior Citizensin San Miguel de Tucumán, which included 13 in-depth semi-structured interviews to patients over 60 years of age, 11female, selected intentionally and for convenience in August 2018.Results.There is a dichotomy between those who talk about the subject and those who do not; however, all of themexpress the desire to naturalize it in both, their social circle and the medical consultation. The eldery patients acceptthe ageing process, have a positive self-perception of their image and despite prejudices, experiences, comorbidities andadversities, they adapt looking for ways to feel good and enjoy a full sexuality.Conclusion.Aging and sexuality are dynamic concepts that converge with life experiences. We believe that, as familydoctors, it is important to provide comprehensive care, addressing the different dimensions of the human being, wheresexuality is included. (AU)

OBJECTIVES: High body mass index (BMI) and blood pressure (BP) are major contributors to the high burden of non-communicable diseases in adulthood. Individual high-risk and population approaches for prevention require newer strategies to target these risk factors and focusing on the family to introduce prevention initiatives appears as a promising scenario. Characterisation of the relationship between BMI and BP among the adult members of a given family merits evaluation. We conducted a secondary analysis of an implementation study in Tumbes, Peru, benefiting from data derived from families with at least one adult offspring. METHODS: The exposures of interest were the BMI, systolic BP (SBP) and diastolic BP (DBP) of the mother and father. The outcomes were the BMI, SBP and DBP of the offspring. Mixed-effects linear regression models were conducted. RESULTS: The mean age of the offspring, mothers and fathers was 29 (SD: 9.5), 54 (SD: 11.8) and 59 (SD: 11.6) years, respectively. Father's BMI was associated with a quarter-point increase in offspring BMI, regardless of the sex of the offspring. Mother's BMI had a similar effect on the BMI of her sons, but had no significant effect on her daughters'. Mother's SBP was associated with almost one-tenth of mmHg increase in the SBP of the adult offspring. There was no evidence of an association for DBP. CONCLUSIONS: In families with adult members, the higher the parents' BMI and SBP, the higher their adult offspring's levels will be.

ABSTRACT Objective To examine the effect of parental feeding styles and children's self-efficacy and social anxiety on adolescent obesity. Methods The samples of this descriptive and cross-sectional study were collected by using a simple sampling method. The study group was composed of 649 Turkish secondary schools students and their parents. Three secondary schools were selected from both rural and urban areas of the province. Data of the study were collected with the child-parent socio-demographic data collection form, the parenting feeding style questionnaire, the middle school self-efficacy scale and the social anxiety scale for the adolescent. To analyze the data of the study, we used percentages, mean, correlation, and regression analysis. Results The average age of the students was 11.58+1.21 years and percentages of female students 55.0%. The factors that significantly affect male adolescent obesity were fear of social situations in general, interpersonal relations, preventive healthcare self-efficacy, and strict dietary control, respectively. The factors affect male adolescent obesity at the rate of 35.0%. The factors that significantly affect female adolescent obesity were fear of social situations in general, preventive healthcare self-efficacy, emotional feeding, social evasion and distress in new situations, and tolerant dietary control, respectively. These factors affect female adolescent obesity at the rate of 32.8%. Conclusion It is indicated that the obesity of children is affected by self-efficacy levels, social anxiety, and the parental feeding style.


RESUMO Objetivo Analisar o efeito dos estilos de alimentação parental e da auto-eficácia das crianças e da ansiedade social na obesidade adolescente. Métodos As amostras deste estudo descritivo e transversal foram coletadas usando um método de amostragem simples. O grupo de estudo era composto por 649 estudantes de escolas secundárias turcas e seus pais. Três escolas secundárias foram selecionadas nas áreas rural e urbana da província. Os dados do estudo foram coletados com o formulário de coleta de dados sócio-demográficos filho-pai, o questionário de estilo de alimentação dos pais, a escala de auto-eficácia do ensino médio ea escala de ansiedade social para o adolescente. Para analisar os dados do estudo, utilizamos porcentagens, média, correlação e análise de regressão. Resultados A idade média dos alunos foi de 11,58+1,21 anos e percentual de estudantes do sexo feminino 55,0%. Os fatores que afetam significativamente a obesidade do adolescente masculino foram medo de situações sociais em geral, relações interpessoais, auto-eficácia preventiva de saúde e controle dietético rigoroso, respectivamente. Os fatores afetam a obesidade do adolescente masculino na taxa de 35,0%. Os fatores que afetam significativamente a obesidade do adolescente feminino foram o medo de situações sociais em geral, auto-eficácia preventiva da saúde, alimentação emocional, evasão social e angústia em novas situações, e controle dietético tolerante, respectivamente. Esses fatores afetam a obesidade da mulher adulta a uma taxa de 32,8%. Conclusão É indicado que a obesidade das crianças é afetada por níveis de auto-eficácia, ansiedade social e estilo de alimentação parental.
