RESUMO

Paraphrenia is a chronic psychotic disorder characterized by a strong delusional component with preservation of thought and personality. It was first introduced as a disorder associated with paranoid dementia and paranoia, but with less personality deterioration than schizophrenia and without fulfilling the clinical features of a delusional disorder. This classic diagnostic entity is not currently listed in main diagnostic systems, rendering delusional disorders difficult to classify in cases that resemble the concept of paraphrenia. We revisit the concept of paraphrenia through a critical review based on a clinical vignette of a patient followed at the psychiatry department of the University Hospital Center of São João.

This paper proposes a multiple-lens receiver scheme to increase the misalignment tolerance of an underwater optical wireless communications link between an autonomous underwater vehicle (AUV) and a sensor plane. An accurate model of photon propagation based on the Monte Carlo simulation is presented which accounts for the lens(es) photon refraction at the sensor interface and angular misalignment between the emitter and receiver. The results show that the ideal divergence of the beam of the emitter is around 15° for a 1 m transmission length, increasing to 22° for a shorter distance of 0.5 m but being independent of the water turbidity. In addition, it is concluded that a seven-lense scheme is approximately three times more tolerant to offset than a single lens. A random forest machine learning algorithm is also assessed for its suitability to estimate the offset and angle of the AUV in relation to the fixed sensor, based on the power distribution of each lens, in real time. The algorithm is able to estimate the offset and angular misalignment with a mean square error of 5 mm (6 mm) and 0.157 rad (0.174 rad) for a distance between the transmitter and receiver of 1 m and 0.5 m, respectively.

Spirulina is a filamentous microalga which is considered a promising alternative source of essential nutrients and active biomolecules. High production cost and the space required to install a photobioreactor are two of the greatest challenges in the industrial application of microalga-based products. Thus, this study aimed to improve Spirulina sp. LEB 18 biomass and phycocyanin content by combining the application of mixotrophic culture and magnetic fields (MF). Zarrouk medium was modified with 1 and 3 g/L liquid molasses and the application of 30 mT for 1·h/d was investigated. Mixotrophic culture with 1 g/L molasses showed the highest biomass concentration (1.62 g/L), carbohydrate content (25.6%), and lipid contents (8.7%) after 15 days. Although the combination of 30 mT and 1 g/L liquid molasses decreased biomass production (1.44 g/L), there was increase in protein yield (76.9%) and protein productivity (73.8 mg/L·d). The proposed method increased phycocyanin production by 145% and its purity from 0.584 in the control culture to 0.627. Data described by this study show that the combination of mixotrophic culture and MF application is a promising alternative to increase microalga protein and phycocyanin production.

Bipolar disorder is characterized by persistent and/or recurrent mood changes between depressive and manic poles. Rapid cycling is a frequent, although underrecognized, condition in bipolar disorder, and it is known to worsen the prognosis of the disease. With regard to the treatment of bipolar depression, there is a shortage of evidence-supported treatment choices, and the literature on the subject includes few references to cases of manic switch induced by lamotrigine. The authors describe a case of a rapid cycling bipolar patient who presented manic symptoms after initiating treatment with lamotrigine.

This study describes the physicochemical properties of soybean asolectin (ASO) liposomes loaded with phycocyanin (Phy) extracted from Spirulina sp. LEB 18. The effects of Phy in the liposomes' properties were investigated by Fourier transform infrared spectroscopy (FTIR), 1H and 31P nuclear magnetic resonance (NMR), zeta (ζ)-potential, dynamic light scattering (DLS) and ultraviolet-visible (UV-vis) techniques. Phy restricted the motion of ASO polar and interface groups and disrupted the package arrangement of the lipid hydrophobic regions, as a likely effect of dipolar and π interactions related to its amino acid residues and pyrrole portions. These interactions were correlated to antiradical/antioxidant Phy responses obtained by 2,2-diphenyl-1-picrylhidrazil (DPPH) assay, thiobarbituric acid reactive substances (TBARS) and ferric reducing antioxidant power (FRAP) methods, and discussed to bring new chemical perspectives about Phy-loaded liposomes-related nutraceutical applications in inflammatory and viral infection processes.

RESUMO

It is very important for healthcare professionals to have good clinical communication skills, especially dentists. Patient-centred care results in patient satisfaction, better outcomes, and less complaints from dental patients. Due to the onset of the COVID-19 pandemic, the clinical communications skills programme of the pre-graduate course in dental medicine at the University of Porto had to be transformed to an online format. Based on their role as faculty, the authors aimed to recount their initial reflections and concerns within this perspective, and to share how they adapted to the new reality of teaching clinical communication skills online, as well as the conclusions of our experience, in the hope that this will help others who may have to go through a similar process. The authors acknowledged that the students achieved the pre-established goals of the clinical communications skills programme through the new online format.

RESUMO

We report on the direct intensity modulation characteristics of a high-speed resonant tunneling diode-photodetector (RTD-PD) with an oscillation frequency of 79 GHz. This work demonstrates both electrical and optical modulation and shows that RTD-PD oscillators can be utilized as versatile optoelectronic/radio interfaces. This is the first demonstration of optical modulation of an RF carrier using integrated RTD-PD oscillators at microwave frequencies.

Os implantes imediatos têm sido utilizados com bastante frequência como uma forma de tratamento para a substituição de dentes condenados. Entretanto, quando a área a ser tratada pertence à zona estética, muito cuidado deve ser tomado. Nessa região, quando o objetivo é alcançar um peri-implante com estética satisfatória, o profissional deve utilizar técnicas que, associadas ao implante imediato, evitem a formação de um defeito decorrente da remodelação pós-exodontia, ou ainda, utilizar procedimentos de melhoria tecidual nos casos em que os dentes condenados já apresentem defeitos de tecido ósseo e/ou mole ao seu redor. O objetivo deste trabalho é, através da apresentação de um caso clínico de reposição do elemento 41 condenado, com perda óssea severa e defeito mucogengival, demonstrar e discutir uma modalidade de tratamento que utiliza o implante imediato de carga imediata não funcional associado ao enxerto de tecido conjuntivo e preenchimento com osso mineral bovino particulado.


Immediate implants have been used frequently as a treatment choice for the replacement of compromised teeth. However, when the area to be treated belongs to the aesthetic zone, great care must be exercised. In this region, when the objective is to reach a peri-implant with a satisfactory esthetics, the professional must use techniques that, associated to the immediate implant, prevent the formation of a defect due to the bone remodeling after tooth loss or to use procedures of tissue improvement for cases where the compromised teeth already have defects of bone and/or soft tissue around them. The objective of this study is to present a clinical case of replacement at tooth 41 with severe bone loss and mucogingival defect to demonstrate and discuss a treatment modality that uses the immediate implant with non-functional immediate loading associated with the connective tissue graft and particulate, deproteinized bovine bone filling.

INTRODUCTION: Doctor-patient communication is essential to the establishment of a therapeutic relationship and to the clinical improvement of the patient. Medical Interpreter Services (MIS) allow the doctors to communicate with patients that do not speak the same language of the clinical staff. However, MIS are not always available and when that happens it is of the upmost importance to find alternatives. OBJECTIVES AND METHODS: Presentation of a clinical case in which Google Translate^© (GT^©) was used as a translation tool to communicate with a patient. Discussion of the utility and validity of GT^© as a translation tool in Medicine. A Pubmed search for articles concerning the use of technological innovations as translation tools was performed using the key words: GT^©, language barriers, foreign language, and communication barriers. RESULTS: In the reported clinical case, GT^© allowed the translation and the establishment of communication with a foreign patient. The Pubmed search found two articles concerning the use and validity of GT^© as a translation tool in Medicine. CONCLUSIONS: GT^© is the most readily available and free initial mode of communication between a doctor and a patient when language is a barrier. Nonetheless, GT^

Digitized radio-over-fiber (D-RoF) transport schemes are being pointed as viable alternative solutions to their analog counterparts, in order to avoid distortion/dynamic range problems. Here we propose a novel D-RoF architecture that takes advantage of a bandpass sigma-delta modulator at the transmitter which subsequently permits the usage of a simpler/cheaper base station that avoids the employment of a digital to analog converter. The proposed architecture exploits the properties of the digital signal to enable the extraction of an higher carrier frequency through the employment of a bandpass filter. Furthermore, we present a comprehensive analysis regarding the impact of a low-cost electro-optic modulation on the quality of received demodulated signal. Finally, a comparison performance analysis between the conventional D-RoF and the proposed architecture is presented. We conclude that although the proposed architecture performs similarly to conventional D-RoF schemes, it is more competitive for either upgrading installed systems as well as for new deployments.

INTRODUCTION: Mental comorbidities, such as phobia, obsessive compulsive symptoms and anxiety disorders, are common in Williams syndrome. However, psychotic symptoms are rare in these patients. We report a case of psychosis in a patient with Williams syndrome. CASE PRESENTATION: A 23-year-old Caucasian woman with Williams syndrome arrived at the emergency room with persecutory delusions, auditory and verbal hallucinations, soliloquies and psychomotor agitation. These symptoms were consistently present for 2 months. No evidence of other medical illnesses or psychoactive substances was found. There was no evidence of prior psychiatric symptoms or family history of psychiatric or neurological disorders. She was treated with antipsychotics and her symptoms were resolved. CONCLUSION: We describe a rare case of a patient with Williams syndrome who experienced a nonorganic psychotic episode. Literature on this topic is scarce and, therefore, this case report intends to add further data about this comorbidity.

We report on photo-detectors consisting of an optical waveguide that incorporates a resonant tunneling diode (RTD). Operating at wavelengths around 1.55 µm in the optical communications C band we achieve maximum sensitivities of around 0.29 A/W which is dependent on the bias voltage. This is due to the nature of RTD nonlinear current-voltage characteristic that has a negative differential resistance (NDR) region. The resonant tunneling diode photo-detector (RTD-PD) can be operated in either non-oscillating or oscillating regimes depending on the bias voltage quiescent point. The oscillating regime is apparent when the RTD-PD is biased in the NDR region giving rise to electrical gain and microwave self-sustained oscillations Taking advantage of the RTD's NDR distinctive characteristics, we demonstrate efficient detection of gigahertz (GHz) modulated optical carriers and optical control of a RTD GHz oscillator. RTD-PD based devices can have applications in generation and optical control of GHz low-phase noise oscillators, clock recovery systems, and fiber optic enabled radio frequency communication systems.

We experimentally study the effect of ionizing radiation on the properties of long-period gratings fabricated in two pure-silica-core fibers with the arc-discharge technique. It is observed that the spectra of the gratings remain almost unchanged after being subjected to doses in excess of 0.5 MGy. The results also show that the gratings' temperature and strain sensitivities are not affected by gamma radiation.

RESUMO

Permanent long-period gratings were written using arc discharges in two aluminosilicate fibers, one of which was doped with erbium. Reversible gratings were also mechanically induced in both fibers. The thermal behavior of the arc-induced gratings was investigated at up to 1100 degrees C. It was found that the shift of the resonant wavelengths exhibited a well-defined linear dependence on temperature up to 700 degrees C.
