OBJECTIVE: To analyze the predictive capacity of the nephrolithometry scoring systems (GSS, STONE, CROES and S-ReSC) and stone surface regarding success and complications following percutaneous nephrolithotomy (PCNL). METHODS: We studied 392 patients who had undergone PCNL in our center. Only patients with a non-contrast CT (n = 240) were finally included for analysis. The predictive capacities for success and complications of the different scoring systems were evaluated using ROC curves and their area under the curve (AUC). RESULTS: Regarding success, the S-ReSC system had the highest predictive capacity with an AUC of 0.681 (95% CI 0.610-0.751), followed by the CROES with 0.667 (95% CI 0.595-0.738), the STONE with 0.654 (95% CI 0.579-0.728) and finally the GSS with 0.626 (95% CI 0.555-0.698). The stone surface as a single variable had an AUC of 0.641 (95% CI 0.565-0.718). As for complications, the S-ReSC had the highest AUC with 0.664 (95% CI 0.57-0.758), followed by STONE with 0.663 (95% CI 0.572-0.755), GSS with 0.626 (95% CI 0.555).-0.698) and CROES with 0.614 (95% CI 0.518-0.7). The stone surface alone had an AUC of 0.616 (95% CI 0.522-0.715). CONCLUSION: The nephrolithometry scales analyzed show a moderate predictive capacity for success and complications in patients undergoing PCNL in our center. Moreover, stone surface as an independent variable demonstrates moderate predictive capacity for both outcomes.

Antecedentes: Estudiar la posible relación entre la expresión inmunohistoquímica del receptor 1 del factor de crecimiento endotelial vascular (VEGFR1) y el valor máximo de captación estandarizada (SUVmáx) de la PET 18F-FDG en pacientes con cáncer de pulmón de células no pequeñas.Material y métodos:El estudio incluyó 39 pacientes con NSCLC (24 carcinomas de células escamosas y 15 adenocarcinomas). Según el estadio clínico, los pacientes se distribuyeron de la siguiente manera: 8 en estadio I, 7 en estadio II, 15 en estadio III y 9 en estadio IV. Se estudió la expresión inmunohistoquímica del VEGFR1 mediante la técnica de la matriz tisular utilizando el dispositivo de arreglo de tejidos (Beecher Instruments, Sun Prairie, WI), utilizando el anticuerpo policlonal contra el VEGFR1 (Santa Cruz Biotechnology, California, EE. UU.).Resultados: Se observó una expresión inmunohistoquímica positiva del VEGFR1 en 23 casos (59%). El número de tumores positivos no se relacionó con el estadio clínico pero hubo una asociación estadísticamente significativa diferente (p: 0,0009) entre la positividad de VEGFR1 y el tipo histológico, correspondiendo los mayores porcentajes de resultados positivos a los adenocarcinomas (93,3%) frente a los carcinomas escamocelulares (37,5%). Asimismo, los valores SUVmáx fueron mayores (p: 0,039) en los carcinomas VEGFR1 negativos que en los tumores VEGFR1 positivos (r: 4-32,1; 16,4+/-6,4 [mediana 16,1] vs. r: 3-47; 14,5+/-8,6 [12,8]).Conclusiones: Nuestros resultados nos llevaron a considerar que en el CPCNP, la expresión inmunohistoquímica negativa de VEGFR1 se asocia significativamente con el subtipo de carcinomas de células escamosas y con valores SUVmáx más altos en 18F-FDG-PET (AU)

BACKGROUND AND PURPOSE: Well-structured hallucinations in Parkinson's disease (PD) are associated with poor prognosis and dementia. However, the predictive value of minor psychotic phenomena in cognitive deterioration is not well known. Cross-sectional studies have shown that PD patients with minor hallucinations have more severe cortical atrophy than non-hallucinators, but baseline and longitudinal studies addressing the evolution of these brain differences are lacking. The impact of developing minor hallucinations on cognitive impairment and cortical atrophy progression in early PD was explored. METHODS: One hundred and thirty-one de novo PD patients from the Parkinson's Progression Marker Initiative for whom brain magnetic resonance imaging scans were available were included. Cognitive outcome at 5 years was compared between patients with and without minor hallucinations during follow-up. Additionally, using gray matter volume (GMV) voxel-based morphometry, cross-sectional (at baseline) and longitudinal (1- and 2-year GMV loss) structural brain differences between groups were studied. RESULTS: During follow-up, 35.1% of patients developed minor hallucinations. At 5 years, these patients showed an increased prevalence of subjective cognitive decline compared to non-hallucinators (44.1% vs. 13.9%; p < 0.001), but not formal cognitive impairment. Additionally, compared to non-hallucinators, they exhibited reduced GMV at baseline in visuoperceptive areas and increased GMV loss in left temporal areas (p < 0.05 corrected). CONCLUSIONS: Minor hallucinations seem to be an early clinical marker of increased neurodegeneration and are associated with mid-term subjective cognitive decline. Longer follow-up analyses would be needed to further define if these findings could reflect a higher risk of future cognitive deterioration.

BACKGROUND AND PURPOSE: Impulsivity is an aspect of personality and a major component of multiple neuropsychiatric conditions. In Parkinson's disease, it has been associated with the expression of impulse control disorders, a highly prevalent non-motor complication. Even though multiple tests of impulsivity have been used in this context, the impact of test choice has not been addressed. The aim was to evaluate whether different impulsivity measures in Parkinson's disease share substantial inter-scale and anatomical correlations or rather mirror different underlying phenomena. METHODS: In a consecutive sample of 89 Parkinson's disease patients without impulse control disorders, four common tests were evaluated assessing different aspects of impulsivity: impulsiveness trait, decisions under implicit risk with and without losses, and delay discounting. Correlations among test scores were analysed and each score was used as a regressor in a set of grey matter volume (GMV) voxel-based morphometry analyses to explore their brain structural correlates. RESULTS: No significant correlations were found between the different impulsivity tests. Furthermore, their structural brain correlates were divergent. Impulsiveness trait appeared to be associated with lower GMV in dorsal-lateral prefrontal cortices, implicit risk (with losses) with higher GMV in the left nucleus accumbens and lower left insular GMV, implicit risk (without losses) with higher GMV in the left lingual gyrus and lower GMV in the gyri recti and delay discounting with higher GMV in the left nucleus accumbens. CONCLUSIONS: In Parkinson's disease, different impulsivity measures reflect very dissimilar behavioural and brain structural correlates. Our results suggest that parkinsonian impulsivity is not a unitary phenomenon but rather a heterogeneous entity.

Salmonella spp. prevails as the main cause of raw meat foodborne illnesses. Implementation of food safety management systems such as Hazard Analysis and Critical Control Points in swine abattoirs can help to mitigate pathogen exposure. The objective of the present study was to evaluate the impact of the HACCP system in slaughterhouses in Colombia on reducing Salmonella spp. exposure due to the consumption of fresh pork meat. Two slaughtering plants with a different degree of HACCP implementation were selected and a quantitative microbiological mapping was built by collecting 820 samples of Salmonella spp. enumeration at different processing stages. The overall Salmonella spp. mean concentration was 1.15 ± 0.55 log MPN/g, with no significant differences among plants (P > 0.05). Deficiencies during carcass disinfection and temperature during distribution of meat cuts from the slaughterhouse lacking of HACCP resulted in a significant increase of Salmonella spp. prevalence (20-40%) (P < 0.05). Processing stages with the highest pathogen prevalence were transport (28-32%) and hanging (16-36%). The exposure assessment model estimated a higher degree of pathogen contamination at the time of consumption in meat cuts from the slaughterhouse without HACCP (3.36 versus 3.68 log MPN/g) and 10-fold increase in the probability a consumer would acquire a contaminated portion (0.011 versus 0.105). Implementation of the HACCP system in swine slaughterhouses represents tangible Salmonella spp. reduction control and public health protection measures.

BACKGROUND AND PURPOSE: Huntington disease is a devastating genetic neurodegenerative disorder for which no effective treatment is yet available. Although progressive striatal atrophy is its pathologic hallmark, concomitant cortical deterioration is assumed to occur, but it is poorly characterized. Our objective was to study the loss of cortical integrity and its association with clinical indicators throughout the course of the disease. MATERIALS AND METHODS: Using a cohort of 39 patients with Huntington disease and 25 controls with available MR imaging (T1WI and DTI), we compared cortical atrophy and intracortical diffusivity across disease stages. Intracortical diffusivity is a DTI-derived metric that has recently been suggested to detect incipient neuronal death because water can diffuse more freely in cortical regions with reduced neural density. RESULTS: We observed progressive thinning and increasing diffusivity within the cerebral cortex of patients with Huntington disease (P < .05, corrected for multiple comparisons). Most important, in the absence of pronounced atrophy, widespread increased diffusivity was already present in individuals with premanifest Huntington disease, correlating, in turn, with clinical and disease-specific progression markers. CONCLUSIONS: Intracortical diffusivity may be more sensitive than cortical thinning for tracking early neurodegeneration in Huntington disease. Moreover, our findings provide further evidence of an early cortical compromise in Huntington disease, which contributes to our understanding of its clinical phenotype and could have important therapeutic implications.

Introducción: El cáncer y las enfermedades degenerativas constituyen trastornos con algunos mecanismos compartidos que actúan en sentido opuesto, produciendo un fenómeno incontrolado de proliferación o pérdida de células. Observaciones diversas apuntan que los pacientes con enfermedad de Alzheimer tienen menor riesgo de desarrollar tumores y viceversa. En este artículo se expone la prevalencia de tumores (activos o superados) en pacientes de neurología cognitiva con y sin una enfermedad degenerativa demenciante. Pacientes y método: En 1.164 pacientes se analizó la frecuencia y topografía de tumores y la presencia o ausencia de enfermedad neurodegenerativa, que se clasificó en 4 grupos (enfermedad de Alzheimer, sinucleinopatía, enfermedad del complejo Pick y del complejo de poliglutamina). Se comparó la frecuencia de tumor en los subgrupos con y sin enfermedad degenerativa, y de esta en los pacientes con y sin trastorno tumoral. Resultados: Se registró proceso tumoral en el 12,1% de los pacientes con enfermedad neurodegenerativa y en el 17,3% del resto del grupo. En el grupo del estudio, un 14,8% de los que tienen antecedente tumoral fue diagnosticado de enfermedad neurodegenerativa, frente al 20,8% entre los que no tienen ese antecedente. Estas diferencias y las observadas en la comparación de subgrupos (tipo de enfermedad degenerativa y topografía del tumor) no alcanzaron significación estadística, excepto al contrastar enfermedades neurodegenerativas con tumores del sistema nervioso central y sinucleinopatías con neoplasias. Conclusiones: Las enfermedades neoplásicas y las neurodegenerativas demenciantes no son excluyentes, aunque muestran menor asociación de la esperada por su respectiva prevalencia

Conclusion: Dementing degenerative diseases and neoplastic disorders are not mutually exclusive. Nevertheless, the rate of co-occurrence is lower than would be expected given the prevalence rate for each group

Salmonella spp. continue to be a leading cause of foodborne morbidity worldwide. To assess the risk of foodborne disease, current national regulatory schemes focus on prevalence estimates of Salmonella and other pathogens. The role of pathogen quantification as a risk management measure and its impact on public health is not well understood. To address this information gap, a quantitative risk assessment model was developed to evaluate the impact of pathogen enumeration strategies on public health after consumption of contaminated ground turkey in the USA. Public health impact was evaluated by using several dose-response models for high- and low-virulent strains to account for potential under- or overestimation of human health impacts. The model predicted 2705-21 099 illnesses that would result in 93-727 reported cases of salmonellosis. Sensitivity analysis predicted cooking an unthawed product at home as the riskiest consumption scenario and microbial concentration the most influential input on the incidence of human illnesses. Model results indicated that removing ground turkey lots exceeding contamination levels of 1 MPN/g and 1 MPN in 25 g would decrease the median number of illnesses by 86-94% and 99%, respectively. For a single production lot, contamination levels higher than 1 MPN/g would be needed to result in a reported case to public health officials. At contamination levels of 10 MPN/g, there would be a 13% chance of detecting an outbreak, and at 100 MPN/g, the likelihood of detecting an outbreak increases to 41%. Based on these model prediction results, risk management strategies should incorporate pathogen enumeration. This would have a direct impact on illness incidence linking public health outcomes with measurable food safety objectives.

Introducción: Los tinnitus y las alucinaciones verbales o musicales (AVM) pueden deberse a lesiones neurológicas de naturaleza y topografía diversa. Método: Se han rastreado estos síntomas en 1.000 pacientes de una consulta de neurología cognitiva, anotando datos epidemiológicos y de neuroimagen. Resultados: Refirieron tinnitus el 6,9% y AVM el 0,9%. Hubo predominio femenino no significativo en el grupo con paracusias. La edad media fue menor en los pacientes con tinnitus y mayor en los que tenían AVM. La hipoacusia mostró mayor prevalencia en los enfermos con paracusias (significativo con AVM). No hubo diferencias en la prevalencia de trastorno psicótico u obsesivo-compulsivo, o de leucoaraiosis. El tratamiento con ácido acetilsalicílico mostró mayor frecuencia en el grupo con AVM, y otros analgésicos no opioides y benzodiacepinas en los pacientes con tinnitus. La presunta causa de las AVM fue demencia con cuerpos de Lewy (n = 2, uno con enfermedad vascular), enfermedad de Alzheimer (n = 2, uno con enfermedad vascular), enfermedad vascular cerebral pura (n = 3), lesión cerebral traumática (n = 1) y lesión quirúrgica en el tronco encefálico (n = 1). En los 9 casos había un elemento facilitador de la aparición de paracusias, como hipoacusia (n = 6) o medicación de riesgo (n = 9), además de polifarmacia (n = 9). Conclusiones: Los pacientes con tinnitus tomaban con frecuencia benzodiacepinas y analgésicos no opioides, y en los que tenían AVM había mayor prevalencia de hipoacusia y de tratamiento con ácido acetilsalicílico. Las causas de AVM fueron demencia con cuerpos de Lewy, enfermedad de Alzheimer y lesiones focales en mesencéfalo, protuberancia, lóbulo temporal izquierdo o claustro izquierdo (AU)

Hearing loss was more prevalent in the paracusia group (difference was significant in VMH subgroup). There were no intergroup differences in the prevalence of psychotic and obsessive-compulsive disorders, or of leukoaraiosis. Treatment with acetylsalicylic acid was more frequent in the VMH group, whereas other non-opioid analgesics and benzodiazepines were more commonly prescribed to patients with tinnitus. The suspected cause of VMH was dementia with Lewy bodies (n = 2, one with vascular disease), Alzheimer disease (n=2, one with vascular disease), isolated cerebrovascular disease (n = 3), traumatic brain injury (n = 1), and surgical brainstem lesion (n = 1). All VMH cases displayed an underlying factor that might prompt this symptom, eg, hearing loss (n = 6), a predisposing drug (n = 9), and polypharmacy (n = 9). Conclusions: Treatment with benzodiazepines and non-opioid analgesics was more frequent in the tinnitus group, whereas the VMH group showed a higher prevalence of hearing loss and treatment with acetylsalicylic acid. The causes of VMH were dementia with Lewy bodies, Alzheimer disease, and focal lesions in the mesencephalon, pons, left temporal lobe, or left claustrum (AU)

Objectives. The proposal and implementation of a computational framework for the quantification of structural renal damage from 99mTc-dimercaptosuccinic acid (DMSA) scans. The aim of this work is to propose, implement, and validate a computational framework for the quantification of structural renal damage from DMSA scans and in an observer-independent manner. Materials and methods. From a set of 16 pediatric DMSA-positive scans and 16 matched controls and using both expert-guided and automatic approaches, a set of image-derived quantitative indicators was computed based on the relative size, intensity and histogram distribution of the lesion. A correlation analysis was conducted in order to investigate the association of these indicators with other clinical data of interest in this scenario, including C-reactive protein (CRP), white cell count, vesicoureteral reflux, fever, relative perfusion, and the presence of renal sequelae in a 6-month follow-up DMSA scan. Results. A fully automatic lesion detection and segmentation system was able to successfully classify DMSA-positive from negative scans (AUC=0.92, sensitivity=81% and specificity=94%). The image-computed relative size of the lesion correlated with the presence of fever and CRP levels (p<0.05), and a measurement derived from the distribution histogram of the lesion obtained significant performance results in the detection of permanent renal damage (AUC=0.86, sensitivity=100% and specificity=75%). Conclusions. The proposal and implementation of a computational framework for the quantification of structural renal damage from DMSA scans showed a promising potential to complement visual diagnosis and non-imaging indicators (AU)


Objetivos. En el presente trabajo se propone, implementa y valida un entorno computacional de cuantificación de imágenes con 99mTc-ácido dimercaptosuccínico (DMSA) con el objetivo de obtener indicadores cuantitativos del daño renal subyacente. Estos indicadores se validan en un contexto de imágenes DMSA pediátricas, dada su relevancia en el diagnóstico de pielonefritis aguda y cicatrices renales. Materiales y métodos. Partiendo de un conjunto de 16 imágenes DMSA positivas para daño renal y 16 controles apareados por edad y sexo, se proponen y calculan una serie de indicadores cuantitativos basados en el área relativa lesionada y la distribución de su histograma. Se implementan aproximaciones manuales y automáticas para dicho cómputo. Los indicadores obtenidos se correlacionan con otras variables clínicas de interés en este contexto, como la proteína C reactiva, la cuenta leucocitaria, el reflujo vesicouretral, la fiebre, la perfusión relativa, y la presencia de secuelas renales en la imagen DMSA a los 6 meses de seguimiento. Resultados. El sistema implementado de detección y cuantificación de lesiones renales obtuvo un rendimiento significativo discriminando las imágenes DMSA positivas de las negativas (AUC=0,92, sensibilidad=81% y especificidad=94%). El indicador de área relativa de la lesión correlacionó con los niveles de proteína C reactiva y la presencia de fiebre (p<0,05). Finalmente, un indicador derivado de las propiedades del histograma de la lesión obtuvo un rendimiento significativo en la detección de la presencia de secuelas renales (AUC=0,86, sensibilidad=100% y especificidad=75%). Conclusiones. La propuesta e implementación de un entorno computacional para la obtención de indicadores cuantitativos a partir de imágenes DMSA muestra un potencial prometedor para complementar el diagnóstico visual (AU)

Los síntomas conductuales y psiquiátricos (SCP) son frecuentes en el enfermo neurológico, contribuyen a producir discapacidad y reducen la calidad de vida. Se ha observado, en pacientes de neurología cognitiva, la prevalencia y tipo de SCP y su asociación con diagnósticos, regiones cerebrales o tratamientos específicos. Método. Análisis retrospectivo de 843 pacientes consecutivos de neurología cognitiva, revisando SCP, diagnóstico, alteración sensorial, topografía lesional en neuroimagen y tratamiento. Se contempló el total y se comparó el grupo de pacientes con deterioro cognitivo objetivo (n = 607) y sin deterioro. Resultados. Hubo SCP en el 59,9% de los pacientes (61,3% en los deteriorados y 56,4% en el resto). Un 31,1% tenía un SCP, 17,4% dos y 11,4% más de dos. Los SCP son más frecuentes en mujeres, sobre todo depresión y ansiedad. En los mayores de 64 años predominan los síntomas psicóticos y conductuales, y en los menores de 65 la ansiedad. Las personas con alteración sensorial tienen más síntomas psicóticos. Se aprecian más síntomas conductuales y psicóticos en personas con demencia degenerativa, depresión y ansiedad en las que tienen enfermedad psiquiátrica o efecto nocivo de sustancias, labilidad emocional en relación con trastorno metabólico u hormonal, hipocondría en los síndromes dolorosos e irritabilidad en la hipoxia crónica. Hay más alteraciones de la conducta en pacientes con anomalía en lóbulos frontales o temporal o parietal derechos, y se tratan preferentemente con antipsicóticos. Aparte de los tratamientos estándar, se observó asociación de distimia con opioides, betahistina y estatinas, y síntomas psicóticos con levodopa, piracetam y vasodilatadores

El principal objetivo de este trabajo es brindar las bases para un esfuerzo coordinado con el fin de crear capacidad de análisis de riesgos para la inocuidad de los alimentos en la región de América Latina y el Caribe (ALC) reuniendo a organizaciones internacionales (Organización Panamericana de la Salud-OPS, Organización de las Naciones Unidas para la Alimentación y la Agricultura -FAO e Instituto Interamericano de Cooperación para la Agricultura-IICA) y universidades (Universidad de Nebraska-Lincoln, Universidad de Maryland, Universidad de Minnesota y Universidad Tecnológica de Texas) mediante la Alianza Estratégica para la Creación de Capacidades en Análisis de Riesgos para la Inocuidad de los Alimentos (AECAR). Esperamos que este trabajo, creado por la alianza, logre: a) generar confianza y fortalecer la comunicación entre todas las organizaciones que trabajan en la región; b) proporcionar las bases para enfoques coordinados, consistentes y efectivos para crear capacidad y desarrollar programas de estudio; y c) facilitar la implementación del marco para análisis de riesgos en la región. Este trabajo presenta los recursos actuales de los autores, considera algunos ejemplos exitosos de implementación de análisis de riesgos en la región (de los sectores académicos y gubernamentales), los desafíos experimentados al implementar el análisis de riesgos, y una hoja de ruta para la creación de capacidad propuesta por esta alianza para ampliar la adopción de análisis de riesgos en la región.


The main goal of this paper is to provide the foundation for a coordinated effort for food safety risk analysis capacity building in the Latin American and Caribbean (LAC) region by bringing together international organizations (Panamerican Health Organization-PAHO, Food and Agriculture Organization of the United Nations-FAO and the Interamerican Institute for Cooperation in Agriculture-IICA) and universities (University of Nebraska-Lincoln, University of Maryland, University of Minnesota and Texas Tech University) through the Strategic Alliance in Risk Analysis Capacity Building (SARAC). We expect that this paper, authored by the alliance, will: a) build trust and strengthen communication among all the organizations that work in the region; b) provide the foundation for coordinated, consistent, and effective approaches to capacity building and curriculum development; and c) facilitate the implementation of the risk analysis framework within the region. This paper provides the current resources by the authors, discusses some successful examples of risk analysis implementation in the region (from academia and government sectors), the challenges experienced on implementing risk analysis and a capacity building roadmap proposed by this alliance to enhance the adoption of risk analysis in the region.
