RESUMO
Should a nurse's report for patient discharge from clinical care be the end of the caretaking episode? The authors started by posing this question and then elaborated a poster which they submitted and won First Prize at the IV National Conference for Nurses of Internal Medicine. As an answer to the question posed, the authors conclude that the aforementioned patient discharge report ensures continuing care, thereby preventing possible complications and the need to readmit patients into hospital care.
