RESUMO

OBJECTIVE: To evaluate the effect of Helicobacter pylor (Hp) eradication in children with acute idiopathic thrombocytopenic purpura (ITP). METHODS: Ninety-three children with acute ITP and Hp infection were divided into two groups and treated with prednisone and Hp eradication (group A, 51 cases) or with prednisone without Hp eradication (group B, 42 cases). RESULTS: The Hp eradication rate was 94.1% in group A. No difference was found in the therapeutic effects on IPT between the two groups, but the recurrence rate in one year in group A was significantly lower than that in group B. CONCLUSION: NHp eradication does not obviously enhance the therapeutic effect on childhood acute ITP, but can decrease the relapse rate in one year. HP eradication therapy is recommended in children with acute ITP and Hp infection.

RESUMO

OBJECTIVE: To investigate the features of ischemic myocardial contracture after asphyxial cardiac arrest in rats. METHOD: Asphyxial cardiac arrest was induced in 8 Wistar rats, and the length and width of the heart were measured at the different time points after cardiac arrest. RESULTS: Obvious ischemic myocardial contracture occurred after the cardiac arrest, reaching the maximal contracture at 4-6 min after the arrest. CONCLUSIONS: Ischemic myocardial contracture induced by asphyxia may be an important factor affecting the outcome of cardiopulmonary resuscitation.

OBJECTIVE: To observe the changes in gene expression of cyclooxygenase (COX) during spontaneous recovery from stress ulcer in rats exposed to water immersion and restraint stress (WRS). METHODS: A rat model of stress ulcer was established by means of WRS, in which the changes in COX expression were detected with immunohistochemistry and reverse transcription (RT)-PCR. RESULTS: Very low levels of COX-2 expression were detected in the gastric mucosa of the control rats, and the expression increased significantly during the healing process of the stress ulcer (P<0.05). COX-1 expression in the gastric mucosa showed no significant difference between the control group and the stress ulcer groups during healing (P>0.05). CONCLUSION: COX-1 and COX-2 expressions in rat gastric mucosa during the recovery from stress ulcer participate in the recovery of the damaged mucosa possibly by mediating prostaglandin secretion.

RESUMO

OBJECTIVE: To explore the approaches for early diagnosis and effective management of closed abdominal injures. METHODS: The experience in diagnosis and management of 246 cases of closed abdominal injuries was analyzed. RESULTS: Of the 246 patients treated in the past 5 years, 221 underwent exploratory laparotomy and 25 received conservative treatment. Altogether 235 patients (95.53%) were cured and 11 died (4.47%). CONCLUSION: Systemic physical examination and early diagnoses are crucial for successful management of closed abdominal injuries. Surgeons should be aware of the importance of early-stage anti-shock treatment and timely surgical management, and the indications and procedures for exploratory laparotomy should be strictly followed.

RESUMO

A 23-year-old male patient, who was stabbed in the left chest at the fourth intercostal space leaving a 4-cm wound, was in agonal state with blood pressure of 0/0 kPa when admitted. The patient underwent emergency room thoractomy on stretcher through the left fourth intercostal anterior lateral incision 16 min after injury. The exploration revealed 2 500 ml blood in the plural cavity, 4-cm wound in the pericardium, 2-cm wound in the right ventricle of the heart without asystole, and 5-cm wound in the left upper lobe of the lung. Within 4 min the wound in the heart was sutured with the pericardium as the backing. The patient was discharged with full recovery 8 d after injury.
