RESUMO

The aim of this article is to analyze the serophobic content explicit in the publications published in Digital Social Networks in the context of HIV and AIDS in Brazil. This is a qualitative study of the descriptive exploratory type, based on documents. The data obtained were evaluated using the methodology of documentary analysis through Thematic Content Analysis with the aid of NVivo®12 Plus (Windows). A total of 187 codes were generated, subsequently grouped according to the semantics of the words, originating five thematic categories: #LivingWithHIV, #WeNeedtoTalkAboutIt, #WhatISSEROPHOBIA, #SerophobiaIsACrime, and #NoSerophobia. The results showed the main manifestations of HIV and AIDS-related serophobia on social networks. The shared content discussed the difficulties of living with a disease that has social dimensions; the relevance of talking and disseminating content about HIV and AIDS; the elements that make up the stigmatization process and, consequently, structure serophobia in society; the social and civil rights of people living with HIV; measures to combat serophobia in health institutions; and the implications of serophobia in the field of public health.


O objetivo do artigo é analisar o conteúdo sorofóbico explicitado nas publicações veiculadas nas redes sociais digitais no contexto do HIV e da Aids no Brasil. Trata-se de um estudo qualitativo do tipo exploratório descritivo, de base documental. Os dados obtidos foram avaliados utilizando a metodologia de análise documental por meio da análise de conteúdo temático com auxílio do software NVivo®12 Plus (Windows). Foram gerados 187 códigos, posteriormente agrupados conforme a semântica das palavras, originando cinco categorias temáticas: #VivendoComHIV, #PrecisamosFalarSobreIsso, #OQueÉSOROFOBIA, #SorofobiaéCrime e #SorofobiaNÃO. Os resultados evidenciaram as principais manifestações acerca da sorofobia relacionada ao HIV e à Aids nas redes sociais. O conteúdo compartilhado debateu as dificuldades de viver com uma doença que apresenta dimensões sociais; a relevância de falar e difundir conteúdo sobre o HIV e a Aids; os elementos que compõem o processo de estigmatização e, consequentemente, estruturam a sorofobia na sociedade; os direitos sociais e civis das pessoas vivendo com HIV; as medidas de combate à sorofobia nas instituições de saúde; e as implicações da sorofobia no âmbito da saúde pública.

Objetivo: Descrever como os enfermeiros atuantes na Atenção Primária identificam sua autonomia profissional no desenvolvimento das práticas de Enfermagem. Métodos: Trata-se de uma pesquisa exploratória, descritiva, de abordagem qualitativa. Participaram de entrevistas online 28 enfermeiros que atuam na Atenção Primária de um município do Sul do Brasil, entre o período de outubro de 2020 até fevereiro de 2021. Para tratamento dos dados, foi utilizada a análise de conteúdo temática. Resultados: Emergiram duas categorias: 1) Resolutividade das práticas da Enfermagem; 2) Respaldo nas regulamentações profissionais e evidências científicas. A Enfermagem dispõe de maior autonomia frente à atenção à saúde da mulher, Infecções Sexualmente Transmissíveis e no cuidado à pessoa com feridas, pois no momento da consulta do enfermeiro, despontam habilidades e competências para a tomada de decisão na prática clínica. Em relação à regulamentação para exercício profissional, os profissionais enfatizaram a importância dos protocolos para respaldar as ações. Conclusão: O protagonismo do enfermeiro e sucesso nas experiências indicam um caminho promissor para a discussão e implementação da Enfermagem de Prática Avançada no Brasil. (AU)


Objective: To describe how nurses working in Primary Care identify their professional autonomy in the development of Nursing practices. Methods: This is an exploratory, descriptive research with a qualitative approach. Twenty-eight nurses who work in Primary Care in a municipality in southern Brazil participated in online interviews, between October 2020 and February 2021. For data processing, thematic content analysis was used. Results: Two categories emerged: 1) Resolving nursing practices; 2) Support in professional regulations and scientific evidence. Nursing has greater autonomy in the care of women's health, Sexually Transmitted Infections and in the care of the person with wounds, because at the time of the nurse's consultation, skills and competences for decision-making in clinical practice emerge. Regarding regulation for professional practice, professionals emphasized the importance of protocols to support actions. Conclusion: The role of nurses and success in the experiences indicate a promising path for the discussion and implementation of Advanced Practice Nursing in Brazil. (AU)


Objetivo: Describir cómo los enfermeros que actúan en la Atención Primaria identifican su autonomía profesional en el desarrollo de las prácticas de Enfermería. Métodos: Se trata de una investigación exploratoria, descriptiva, con abordaje cualitativo. Veintiocho enfermeros que actúan en la Atención Primaria de un municipio del sur de Brasil participaron de entrevistas en línea, entre octubre de 2020 y febrero de 2021. Para el procesamiento de datos, se utilizó el análisis de contenido temático. Resultados: Emergieron dos categorías: 1) Prácticas resolutivas de enfermería; 2) Apoyo en normativa profesional y evidencia científica. La enfermería tiene mayor autonomía en el cuidado de la salud de la mujer, Infecciones de Transmisión Sexual y en el cuidado de la persona con heridas, porque en el momento de la consulta del enfermero emergen habilidades y competencias para la toma de decisiones en la práctica clínica. En cuanto a la regulación para la práctica profesional, los profesionales destacaron la importancia de los protocolos para apoyar las acciones. Conclusión: El papel de los enfermeros y el éxito de las experiencias indican un camino promisorio para la discusión e implementación de la Enfermería de Práctica Avanzada en Brasil. (AU)

BACKGROUND: Incidence of first-episode psychosis (FEP) varies substantially across geographic regions. Phenotypes of subclinical psychosis (SP), such as psychotic-like experiences (PLEs) and schizotypy, present several similarities with psychosis. We aimed to examine whether SP measures varied across different sites and whether this variation was comparable with FEP incidence within the same areas. We further examined contribution of environmental and genetic factors to SP. METHODS: We used data from 1497 controls recruited in 16 different sites across 6 countries. Factor scores for several psychopathological dimensions of schizotypy and PLEs were obtained using multidimensional item response theory models. Variation of these scores was assessed using multi-level regression analysis to estimate individual and between-sites variance adjusting for age, sex, education, migrant, employment and relational status, childhood adversity, and cannabis use. In the final model we added local FEP incidence as a second-level variable. Association with genetic liability was examined separately. RESULTS: Schizotypy showed a large between-sites variation with up to 15% of variance attributable to site-level characteristics. Adding local FEP incidence to the model considerably reduced the between-sites unexplained schizotypy variance. PLEs did not show as much variation. Overall, SP was associated with younger age, migrant, unmarried, unemployed and less educated individuals, cannabis use, and childhood adversity. Both phenotypes were associated with genetic liability to schizophrenia. CONCLUSIONS: Schizotypy showed substantial between-sites variation, being more represented in areas where FEP incidence is higher. This supports the hypothesis that shared contextual factors shape the between-sites variation of psychosis across the spectrum.

Objective: To assess the association between leptin/adiponectin ratio (LAR) and insulin resistance surrogates in prepubertal children. Materials and methods: Study based on data from the Growth and Obesity Chilean Cohort Study (GOCS) involving 968 Chilean prepubertal children. Plasma insulin, leptin, and adiponectin were determined by immunoassays. Several common insulin resistance surrogates were calculated, including the homeostasis model assessment of insulin resistance (HOMA-IR), triglyceride/HDL cholesterol index, triglyceride-glucose (TyG) index, and the TyG index corrected for body mass index (BMI; TyG-BMI) and waist circumference (WC; TyG-WC). Associations among variables were assessed using multiple linear and logistic regression analysis. Results: There was a significant direct association between plasma leptin and LAR with BMI z-score but no association between plasma adiponectin and adiposity. After adjustments for sex and age, LAR was significantly associated with all insulin resistance surrogates (which were categorized using the 75th percentile as the cutoff point), with the TyG-WC index emerging as the surrogate with the highest magnitude of association (odds ratio [OR] 2.44, 95% confidence interval [CI] 2.05-2.9). After additional adjustment for BMI z-score, only the association between LAR and TyG-WC remained significant (OR 1.64, 95% CI 1.27-2.12). Conclusion: Plasma leptin and LAR were strongly associated with several common insulin resistance surrogates in prepubertal children, most notably with the TyG-WC index. Associations between LAR and insulin resistance indexes were mainly driven by the effect of plasma leptin, which is also directly associated with increased adiposity.

Objetivo: demostrar las líneas de acción implementadas por gerentes de hospitales universitarios federales a partir planes de contingencia elaborados para hacer frente a la pandemia de COVID-19. Método: trabajo de investigación multicéntrico con enfoque cualitativo, basado en dieciséis entrevistas a gerentes de ocho hospitales universitarios que participaron en la elaboración de los planes de contingencia. Los datos se recolectaron entre abril y octubre de 2021, con análisis temático de contenido. Resultados: surgieron seis líneas de acción: Implementación y organización de unidades específicas para pacientes con COVID-19 y aumento en la cantidad de camas; Adquisición de equipos de protección personal e insumos; Elaboración de nuevos protocolos e implementación de programas de educación permanente; Contratación de profesionales; y Preocupación y apoyo en términos de la salud mental de los trabajadores. Conclusión: los hospitales universitarios fueron la principal vía de ingreso de los casos graves de COVID-19. Para brindar asistencia segura y eficiente, debieron reorganizar flujos de atención, implementar cambios estructurales, ofrecer programas de capacitación y proporcionar apoyo en términos de la salud mental de los profesionales.


Objective: to demonstrate the action strategies implemented by managers of federal university hospitals based on the COVID-19 pandemic contingency plan. Method: multicenter qualitative research based on sixteen interviews with managers from eight university hospitals involved in the development of contingency plans. Data collection took place from April to October 2021, with thematic content analysis. Results: six lines of action emerged: implementation and organization of specific COVID-19 patient units and increased bed capacity; acquisition of personal protective equipment and supplies; development of new protocols and continuous education; hiring of professionals; focus on and support for mental health of health workers. Conclusion: university hospitals served as the primary hate way for severe COVID-19 cases. For safe and efficient care, they needed to reorganize patient flow, implement structural changes, provide training and offer mental health support for professionals.

RESUMO Objetivo analisar as tendências investigativas de teses e dissertações sobre COVID-19 dos programas de pós-graduação da área da Enfermagem no Brasil. Métodos estudo bibliométrico realizado no Catálogo de Teses e Dissertações da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, com análise de dados bibliométricos por meio do IRAMUTEQ. Resultados 82 trabalhos foram selecionados, sendo a maioria dissertações (87,8%). Foram distribuídos em seis classes: 1) Saúde mental dos profissionais de enfermagem; 2) Saúde da mulher grávida e puérpera; 3) Adaptação do atendimento às pessoas em situação de vulnerabilidade; 4) Ser humano e ser enfermeiro: convivendo e enfrentando o vírus; 5) Reorganização do trabalho da enfermagem; e 6) Repercussões da COVID-19 para a enfermagem. Conclusão as pesquisas estão concentradas nas condições de trabalho da enfermagem, na saúde mental e na reorganização dos fluxos de atendimento. Contribuições para a prática: o estudo promove visibilidade à produção de conhecimento em Enfermagem no contexto da pandemia de COVID-19 no Brasil e identifica lacunas para futuros estudos.


ABSTRACT Objective to analyze the investigative trends of theses and dissertations on COVID-19 from graduate programs in Nursing in Brazil. Methods a bibliometric study conducted using the Theses and Dissertations Catalog from the Coordination for the Improvement of Higher Education Personnel, with bibliometric data analysis through IRAMUTEQ. Results 82 works were selected, with the majority being dissertations (87.8%). They were distributed into six classes: 1) Nursing professionals' mental health. 2) Pregnant and postpartum women health. 3) Adaptation of care for people in vulnerable situations. 4) Human being and being a nurse: living and facing the virus. 5) Nursing work reorganization, and 6) COVID-19 repercussions for nursing. Conclusion research is concentrated on nursing working conditions, mental health, and the care workflows reorganization. Contributions to practice: the study brings visibility to the knowledge production in Nursing within the context of the COVID-19 pandemic in Brazil and identifies gaps for future studies.

BACKGROUND: Childhood adversity and cannabis use are considered independent risk factors for psychosis, but whether different patterns of cannabis use may be acting as mediator between adversity and psychotic disorders has not yet been explored. The aim of this study is to examine whether cannabis use mediates the relationship between childhood adversity and psychosis. METHODS: Data were utilised on 881 first-episode psychosis patients and 1231 controls from the European network of national schizophrenia networks studying Gene-Environment Interactions (EU-GEI) study. Detailed history of cannabis use was collected with the Cannabis Experience Questionnaire. The Childhood Experience of Care and Abuse Questionnaire was used to assess exposure to household discord, sexual, physical or emotional abuse and bullying in two periods: early (0-11 years), and late (12-17 years). A path decomposition method was used to analyse whether the association between childhood adversity and psychosis was mediated by (1) lifetime cannabis use, (2) cannabis potency and (3) frequency of use. RESULTS: The association between household discord and psychosis was partially mediated by lifetime use of cannabis (indirect effect coef. 0.078, s.e. 0.022, 17%), its potency (indirect effect coef. 0.059, s.e. 0.018, 14%) and by frequency (indirect effect coef. 0.117, s.e. 0.038, 29%). Similar findings were obtained when analyses were restricted to early exposure to household discord. CONCLUSIONS: Harmful patterns of cannabis use mediated the association between specific childhood adversities, like household discord, with later psychosis. Children exposed to particularly challenging environments in their household could benefit from psychosocial interventions aimed at preventing cannabis misuse.

Objetivo: describir las competencias profesionales de los enfermeros que trabajan en urgencias y emergencias y comprender la percepción que tienen sobre las competencias esenciales para el desempeño y la actualización profesional. Método: estudio secuencial explicativo de métodos mixtos, realizado con enfermeros de urgencias y emergencias. Para obtener datos cuantitativos, se utilizó un cuestionario con 78 ítems, respondido por 39 enfermeros, que fueron analizados mediante estadística descriptiva y pruebas no paramétricas. Los datos cualitativos se obtuvieron a través de entrevistas semiestructuradas con 17 enfermeros, que fueron interpretados mediante el análisis temático de contenido inductivo. Los datos se combinaron por conexión. Resultados: se obtuvo un alto nivel de competencia en la autoevaluación de los enfermeros de urgencias y emergencias en el factor 2 - relaciones en el trabajo y un nivel más bajo en el factor 6 - excelencia profesional (p=0,036). Los datos cualitativos coincidieron positivamente con el factor relaciones en el trabajo, identificando la asociación entre conocimiento y experiencia práctica con las competencias, además de un escenario que carece de educación continua. Conclusión: a pesar de los altos niveles de competencia identificados en los enfermeros de urgencias y emergencias, el fortalecimiento de las estrategias educativas favorece el desarrollo y reconocimiento profesional.


Objective: to describe the professional competencies of nurses working in urgency and emergency services and to understand their perception of the essential competencies for professional performance and updating. Method: a sequential, mixed-methods and explanatory study, conducted with emergency nurses. To obtain the quantitative data, a questionnaire with 78 items was used, answered by 39 nurses and analyzed using descriptive statistics and non-parametric tests. In turn, the qualitative data were obtained through semi-structured interviews with 17 nurses, whose interpretation was based on inductive content thematic analysis. The data were combined by connection. Results: a high level of competence was obtained in the self-assessment of urgency and emergency nurses in Factor 2 - Relations at work and a lower level in Factor 6 - Professional excellence (p=0.036). The qualitative data corroborated positively with the "Relations at work" factor, identifying the association of knowledge and practical experience, with competencies beyond a scenario devoid of permanent education. Conclusion: despite the high levels of competence identified in emergency nurses, the strengthening of educational strategies favors professional development and recognition.

Já os dados qualitativos, foram obtidos por entrevista semiestruturada realizada com 17 enfermeiros, cuja interpretação deu-se pela análise temática de conteúdo indutiva. Os dados foram combinados por conexão. Resultados: obteve-se alto nível de competência na autoavaliação dos enfermeiros de urgência e emergência no fator 2 - relações no trabalho e menor nível no fator 6 - excelência profissional (p=0,036). Os dados qualitativos corroboraram de forma positiva com o fator relações no trabalho, identificando a associação do conhecimento e experiência prática com competências, além de um cenário desprovido de educação permanente. Conclusão: apesar de altos níveis de competência identificados nos enfermeiros de urgência e emergência, o fortalecimento de estratégias educacionais favorece o desenvolvimento e o reconhecimento profissional.

Objetivo: investigar los factores asociados a la infección por SARSCoV-2 en los profesionales de la salud de hospitales universitarios. Método: estudio multicéntrico, con abordaje mixto con estrategia incorporada concomitante, realizado con 559 profesionales en la etapa cuantitativa, y 599 en la etapa cualitativa. Fueron utilizados cuatro instrumentos de recolección de datos, aplicados a través un formulario electrónico. El análisis cuantitativo se realizó mediante estadística descriptiva e inferencial y los datos cualitativos mediante análisis de contenido Resultados: los factores asociados a la infección fueron: realización de la prueba "RT-PCR" (p<0,001) y unidades que atienden a pacientes con COVID-19 (p=0,028). Tener síntomas aumentó la prevalencia de infección 5,63 veces y cumplir la mayor parte del tiempo con el distanciamiento social en la vida privada la redujo un 53,9%. Los datos cualitativos mostraron las dificultades que enfrentaron los profesionales: escasez y baja calidad de equipos de protección personal, sobrecarga de trabajo, distanciamiento físico en el trabajo, procesos y rutinas inadecuados y la ausencia de una política de triage y testeo masivos. Conclusión: los factores asociados a la infección por SARS-CoV-2 en los profesionales de la salud se relacionaron mayormente con cuestiones laborales.


Objective: to investigate factors associated with the SARS-CoV-2 infection among health professionals from university hospitals. Method: a multicenter, mixed approach study with concomitant incorporated strategy, carried out with 559 professionals in the quantitative stage, and 599 in the qualitative stage. Four data collection instruments were used, applied by means of an electronic form. The quantitative analysis was performed with descriptive and inferential statistics and the qualitative data were processed by means of content analysis. Results: the factors associated with the infection were as follows: performance of the RT-PCR test (p<0.001) and units offering care to COVID-19 patients (p=0.028). Having symptoms increased 5.63 times the prevalence of infection and adhering to social distancing most of the time in private life reduced it by 53.9%. The qualitative data evidenced difficulties faced by the professionals: scarcity and low quality of Personal Protective Equipment, work overload, physical distancing at work, inadequate processes and routines and lack of a mass screening and testing policy. Conclusion: the factors associated with the SARS-CoV-2 infection among health professionals were mostly related to occupational issues.


Objetivo: investigar fatores associados à infecção por SARS-CoV-2 entre profissionais da saúde de hospitais universitários. Método: estudo multicêntrico, de abordagem mista com estratégia incorporada concomitante, realizado com 559 profissionais na etapa quantitativa, e 599 na etapa qualitativa. Foram utilizados quatro instrumentos de coleta de dados, aplicados via formulário eletrônico. A análise quantitativa foi realizada com estatística descritiva e inferencial e os dados qualitativos por meio de análise de conteúdo. Resultados: os fatores associados à infeção foram: realização de teste "RT-PCR" (p<0,001) e unidades com atendimento a pacientes com COVID-19 (p=0,028). Ter sintomas aumentou em 5,63 vezes a prevalência de infeção e aderir ao distanciamento social na maior parte do tempo na vida particular reduziu em 53,9%. Dados qualitativos evidenciaram dificuldades enfrentadas pelos profissionais: escassez e baixa qualidade de equipamentos de proteção individual, sobrecarga de trabalho, distanciamento físico no trabalho, processos e rotinas inadequadas e ausência de uma política de triagem e testagem em massa. Conclusão: os fatores associados à infecção por SARS-CoV-2 entre profissionais da saúde foram em sua maioria relacionados a questões ocupacionais.

Objetivo: comparar os sistemas de saúde e as implicações para a atuação do enfermeiro entre o Brasil e o Chile. Método: estudo documental com abordagem qualitativa, elaborado a partir da consulta a documentos oficiais de fonte aberta. Os dados foram analisados mediante análise de conteúdo. Resultados: os dois países respondem às orientações gerais da Organização Mundial da Saúde no que tange à promoção da saúde e prevenção de doenças ao longo do ciclo de vida, nos indivíduos, nas famílias e nas comunidades. Evidenciaram-se diferenças referentes aos sistemas de saúde, sobretudo a respeito da forma de financiamento, legislação e processo de trabalho do enfermeiro. Conclusão: embora existam semelhanças por serem países pertencerem à mesma região geográfica, pode-se constatar diferenças na Atenção Primária à Saúde, principalmente no que se refere à estruturação dos sistemas de saúde, financiamento e em algumas áreas, como a gestão do cuidado e atuação do enfermeiro.


Objective: to compare health systems and the implications for the role of nurses between Brazil and Chile. Method: documentary study with a qualitative approach, based on consultation with official open-source documents. Data were analysed using content analysis. Results: both countries respond to the general guidelines of the World Health Organization regarding health promotion and disease prevention throughout the life cycle, in individuals, families and communities. Differences related to health systems were evidenced, especially regarding the form of financing, legislation, and the nurses' work process. Conclusion: although there are similarities because the countries belong to the same geographic region, differences can be seen in Primary Health Care, especially regarding to the health systems' structure, financing and in some areas, such as care management and nurses' role.


Objetivo: comparar los sistemas de salud y las implicaciones para el trabajo del enfermero entre Brasil y Chile. Método: estudio documental con enfoque cualitativo, elaborado con base en la consulta de documentos oficiales de código abierto. Los datos se analizaron mediante análisis de contenido. Resultados: ambos países responden a los lineamientos generales de la Organización Mundial de la Salud en cuanto a la promoción de la salud y la prevención de enfermedades a lo largo del ciclo de vida, en los individuos, las familias y las comunidades. Se evidenciaron diferencias relacionadas con los sistemas de salud, especialmente en cuanto a la forma de financiación, la legislación y el proceso de trabajo de los enfermeros. Conclusión: si bien existen similitudes ya que los países pertenecen a la misma región geográfica, se perciben diferencias en la Atención Primaria de Salud, especialmente en lo que respecta a la estructura de los sistemas de salud, la financiación y en algunas áreas, como la gestión del cuidado y el papel del enfermero.

OBJECTIVE: To understand the experiences of mothers with early weaning. METHOD: Qualitative research with a theoretical-methodological contribution from Grounded Theory (Straussian perspective), carried out in the context of primary health care in a medium-sized municipality in the northeast of Brazil. 19 collaborators participated by theoretical sampling. Data collection took place between April and September 2018, with in-depth interviews, and was analyzed in three stages: open and axial coding, and integration. RESULTS: The central category "Women experiencing guilt and overload due to early weaning" was supported by three categories: a) conditions: "Showing the factors that limit breastfeeding"; b) actions/interactions: "Trying to balance motherhood and work during breastfeeding" and "Insufficient social support to keep exclusive breastfeeding"; and c) consequences: "Introducing formula and complementary foods before six months" and "Blaming oneself for the early weaning". FINAL CONSIDERATIONS: The theoretical model can give support to managers and health professionals to advocate for longer maternity leaves and confront gender disparities and inequities, professional performance with conflicts of interest, and abusive marketing regarding the use of formula.

OBJECTIVE: To develop a theoretical model for management of research groups in nursing graduate programs. METHOD: This is a Grounded Theory developed in a stricto sensu nursing graduate program of a public university. Data collection was conducted from April to October 2018 and the theoretical sample consisted of 21 participants arranged in three sample groups. RESULTS: Research activities are linked to research groups, which require human, material and financial resources to develop their studies. These conditions call for management strategies and national and international interactions that result in theoretical, scientific and technological development of the profession. FINAL CONSIDERATIONS: The theoretical model for management of research groups can serve as a guide for graduate programs in nursing and health for planning the work process and so that they can contribute with research of great impact for society.

BACKGROUND: A history of childhood adversity is associated with psychotic disorder, with an increase in risk according to the number of exposures. However, it is not known why only some exposed individuals go on to develop psychosis. One possibility is pre-existing polygenic vulnerability. Here, we investigated, in the largest sample of first-episode psychosis (FEP) cases to date, whether childhood adversity and high polygenic risk scores for schizophrenia (SZ-PRS) combine synergistically to increase the risk of psychosis, over and above the effect of each alone. METHODS: We assigned a schizophrenia-polygenic risk score (SZ-PRS), calculated from the Psychiatric Genomics Consortium (PGC2), to all participants in a sample of 384 FEP patients and 690 controls from the case-control component of the EU-GEI study. Only participants of European ancestry were included in the study. A history of childhood adversity was collected using the Childhood Trauma Questionnaire (CTQ). Synergistic effects were estimated using the interaction contrast ratio (ICR) [odds ratio (OR)exposure and PRS - ORexposure - ORPRS + 1] with adjustment for potential confounders. RESULTS: There was some evidence that the combined effect of childhood adversities and polygenic risk was greater than the sum of each alone, as indicated by an ICR greater than zero [i.e. ICR 1.28, 95% confidence interval (CI) -1.29 to 3.85]. Examining subtypes of childhood adversities, the strongest synergetic effect was observed for physical abuse (ICR 6.25, 95% CI -6.25 to 20.88). CONCLUSIONS: Our findings suggest possible synergistic effects of genetic liability and childhood adversity experiences in the onset of FEP, but larger samples are needed to increase precision of estimates.

This study investigated if the association between childhood maltreatment and cognition among psychosis patients and community controls was partially accounted for by genetic liability for psychosis. Patients with first-episode psychosis (N = 755) and unaffected controls (N = 1219) from the EU-GEI study were assessed for childhood maltreatment, intelligence quotient (IQ), family history of psychosis (FH), and polygenic risk score for schizophrenia (SZ-PRS). Controlling for FH and SZ-PRS did not attenuate the association between childhood maltreatment and IQ in cases or controls. Findings suggest that these expressions of genetic liability cannot account for the lower levels of cognition found among adults maltreated in childhood.

The COVID-19 pandemic has impacted the mental health of the youngest, worsening their emotional well-being. The demand for care in psychiatric emergencies may indirectly reflect the mental health state of children and adolescents and the emotional consequences of the pandemic. Moreover, suicidality can be considered a marker of severity in this population group. Therefore, we have aimed to longitudinally describe the number of children and adolescents attended in the psychiatry emergency department due to suicidal ideation or attempts and, to explore differences in suicidality according to gender and age. A retrospective study was carried out in the University Hospital of San Juan, Alicante, Spain, from January 01, 2018 to December 31, 2021. A total of 138 participants under 18 years requesting psychiatric care due to suicidal ideation or attempts were included. The sample was composed by 35% of males and the mean age was 14.8 years old (SD = 2.2). The number of cases per year range from 10 in 2018 to 88 in 2021. Attendances were significantly higher between 2021 and the three previous years. Besides, the number of attentions registered in the last 9 months of 2021 equals those that occurred in the entire previous period. Most of the cases were girls and middle adolescents. Suicide ideation or attempts have skyrocketed in children and adolescents. This alarming increase presents a one-year lag peak from the COVID-19 outbreak and continues until the end of 2021. Girls and those over 12 years have been identified as risk groups to present suicidal ideation or attempts.

OBJECTIVES: to analyze the quality of child and adolescent care transitions from hospital to home, considering the presence of chronic disease. METHODS: quantitative, cross-sectional study, carried out from February to September 2019 in two hospitals in the south of Brazil. We used an instrument to characterize participants and the Care Transitions Measure (CTM-15) for the legal tutors of children and adolescents that were discharged from the institutions. RESULTS: the general mean of the quality of transition of care was 87.9 (SD=13.4), in a scale from 0 to100). We found a significant difference in the quality of transition of care when comparing patients with and without chronic disease (90.0 and 84.3; p=0.001). CONCLUSIONS: we found the quality of the transition of care to be satisfactory, with better results for patients with chronic disease. This can help understand the most impactful aspects in the transition of care, especially in regard to children health.

BACKGROUND: While cannabis use is a well-established risk factor for psychosis, little is known about any association between reasons for first using cannabis (RFUC) and later patterns of use and risk of psychosis. METHODS: We used data from 11 sites of the multicentre European Gene-Environment Interaction (EU-GEI) case-control study. 558 first-episode psychosis patients (FEPp) and 567 population controls who had used cannabis and reported their RFUC.We ran logistic regressions to examine whether RFUC were associated with first-episode psychosis (FEP) case-control status. Path analysis then examined the relationship between RFUC, subsequent patterns of cannabis use, and case-control status. RESULTS: Controls (86.1%) and FEPp (75.63%) were most likely to report 'because of friends' as their most common RFUC. However, 20.1% of FEPp compared to 5.8% of controls reported: 'to feel better' as their RFUC (χ2 = 50.97; p < 0.001). RFUC 'to feel better' was associated with being a FEPp (OR 1.74; 95% CI 1.03-2.95) while RFUC 'with friends' was associated with being a control (OR 0.56; 95% CI 0.37-0.83). The path model indicated an association between RFUC 'to feel better' with heavy cannabis use and with FEPp-control status. CONCLUSIONS: Both FEPp and controls usually started using cannabis with their friends, but more patients than controls had begun to use 'to feel better'. People who reported their reason for first using cannabis to 'feel better' were more likely to progress to heavy use and develop a psychotic disorder than those reporting 'because of friends'.

to describe the professional competencies of nurses working in urgency and emergency services and to understand their perception of the essential competencies for professional performance and updating. a sequential, mixed-methods and explanatory study, conducted with emergency nurses. To obtain the quantitative data, a questionnaire with 78 items was used, answered by 39 nurses and analyzed using descriptive statistics and non-parametric tests. In turn, the qualitative data were obtained through semi-structured interviews with 17 nurses, whose interpretation was based on inductive content thematic analysis. The data were combined by connection. a high level of competence was obtained in the self-assessment of urgency and emergency nurses in Factor 2 - Relations at work and a lower level in Factor 6 - Professional excellence (p=0.036). The qualitative data corroborated positively with the "Relations at work" factor, identifying the association of knowledge and practical experience, with competencies beyond a scenario devoid of permanent education. despite the high levels of competence identified in emergency nurses, the strengthening of educational strategies favors professional development and recognition.

Los datos cualitativos se obtuvieron a través de entrevistas semiestructuradas con 17 enfermeros, que fueron interpretados mediante el análisis temático de contenido inductivo. Los datos se combinaron por conexión. se obtuvo un alto nivel de competencia en la autoevaluación de los enfermeros de urgencias y emergencias en el factor 2 - relaciones en el trabajo y un nivel más bajo en el factor 6 - excelencia profesional (p=0,036). Los datos cualitativos coincidieron positivamente con el factor relaciones en el trabajo, identificando la asociación entre conocimiento y experiencia práctica con las competencias, además de un escenario que carece de educación continua. a pesar de los altos niveles de competencia identificados en los enfermeros de urgencias y emergencias, el fortalecimiento de las estrategias educativas favorece el desarrollo y reconocimiento profesional.

Tener síntomas aumentó la prevalencia de infección 5,63 veces y cumplir la mayor parte del tiempo con el distanciamiento social en la vida privada la redujo un 53,9%. Los datos cualitativos mostraron las dificultades que enfrentaron los profesionales: escasez y baja calidad de equipos de protección personal, sobrecarga de trabajo, distanciamiento físico en el trabajo, procesos y rutinas inadecuados y la ausencia de una política de triage y testeo masivos. los factores asociados a la infección por SARS-CoV-2 en los profesionales de la salud se relacionaron mayormente con cuestiones laborales.


(1) Las cuestiones laborales influyeron en la protección de los profesionales durante la pandemia. (2) La alta adhesión a las precauciones estándar y al distanciamiento no disminuyeron los casos positivos. (3) La baja calidad de los EPP y las fallas en el triage dificultaron la protección en el trabajo. (4) La infraestructura hospitalaria no favoreció el distanciamiento físico entre los equipos.
