It has been hypothesized that the interprosthetic gap between ipsilateral hip and knee replacements acts as a stress riser affecting bone fracture behaviour. The aim of this study was to quantify femoral strength and fracture morphology for a wide range of interprosthetic gaps. Seven interprosthetic gaps (0-20cm) were created in artificial femora (N = 6-9/group). All specimens were loaded to failure following a compressive loading protocol. Fracture load and fracture morphology were recorded. Outcomes were compared to femora with a hip implant only (N = 6; reference group). Fracture load was highest for 0 cm gaps. All other interprosthetic gaps had fracture loads similar to that of the reference group. Fracture occurred most frequently with a medial butterfly fragment located at the tip of the hip stem.We conclude that small gaps do not act as stress risers. The specific fracture morphology may benefit from different treatment than peri-prosthetic hip fractures.

RESUMO

Laughing is recognized as a provoking factor for headache, certainly underestimated among the general population and few cases have been published to date. We report a single case of severe headache, provoked almost exclusively by outbursts of laughing, where venous magnetic resonance imaging revealed the presence of giant Pacchioni granulations in both right and transverse sinuses. Reviewing published cases of laughing headache, we discuss possible mechanisms of pain and the role of giant Pacchionian granulations.

AIM: Reducing variations by standardizing the key interventions in clinical processes is an effective tool to minimize the probability of medical errors. Thus, we determined whether variations exist in the key interventions included in care pathway documents (CPDs) of different organizations and in the timing of these interventions during the care process. METHODS: We invited members of the Belgian Dutch Clinical Pathway Network to send their CPDs for external review. We analysed 19 CPDs for total knee arthroplasty. International guidelines were consulted for definition of key interventions. Documentation of these interventions in the pathway documents was checked. RESULTS: From the 19 CPDs analysed, we retrieved 18 key interventions. Nine of these key interventions appeared in at least 80% of the care pathways. Only two key interventions appeared in all pathway documents. Nine out of 12 interventions that appeared in at least 80% of the pathway documents showed a variation of 1 day in the timing of care from the median timing. CONCLUSION: We conclude that important variation exists in the included interventions and in their timing. The results of this study suggest that, before use in daily patient care, CPDs should be reviewed by peers and checked to ensure that all evidence-based key interventions are included and properly planned. Only in this way can pathways documents be used interprofessionally during the entire perioperative period in the search for optimal quality and patient safety.

RESUMO

The history of a 17-year-old female patient with pseudoachondroplasia, with an extreme bilateral genu recurvatum due to tibial growth disturbance, is presented. She was treated by the Ilizarov method over a nine- month period for consecutive bilateral correction of the lower legs, leading to an excellent functional result. The authors advocate the Ilizarov method as the treatment of choice for severe juxtaarticular osseous deformities, where the technical limits of a standard osteotomy with internal fixation are being exceeded.
