BACKGROUND: Impairments in executive cognitive control, including a reduced ability to inhibit prepotent responses, have been reported in autism spectrum disorders (ASD). These deficits may underlie patterns of repetitive behaviors associated with the disorder. METHOD: Eighteen individuals with ASD and 15 age- and IQ-matched healthy individuals performed an antisaccade task and a visually guided saccade control task, each with gap and overlap conditions. Measures of repetitive behaviors were obtained using the Autism Diagnostic Inventory-Revised (ADI-R) and examined in relation to neurocognitive task performance. RESULTS: Individuals with an ASD showed increased rates of prosaccade errors (failures to inhibit prepotent responses) on the antisaccade task regardless of task condition (gap/overlap). Prosaccade error rates were associated with the level of higher-order (e.g. compulsions, preoccupations) but not sensorimotor repetitive behaviors in ASD. CONCLUSIONS: Neurocognitive disturbances in voluntary behavioral control suggest that alterations in frontostriatal systems contribute to higher-order repetitive behaviors in ASD.

A partnership of medical laboratories in Ontario, Canada has developed and made operational a centralized interactive database of cervical cytology (Pap) reports. This system automatically registers some 60,000 reports monthly, which are submitted electronically from geographically diverse sources. Patient histories are provided on-line to authorized pathologists to support ongoing diagnoses. A formalized coding system has been developed to represent elements of Pap reports in machine readable format, including diagnosis, clinical information about the patient, specimen adequacy, methods of specimen collection and follow-up recommendations. The system operates over a private network based on Internet standards. A review of operations, which commenced June 1, 1996, provides insights into applied EMR technology.

Acute leukemia is described in two patients treated with cytotoxic agents for a destructive, seropositive rheumatoid arthritis. Both patients had received longterm azathioprine therapy. In addition, one patient had been treated with cyclophosphamide, the other with melphalan. Chromosomal abnormalities were noted in both patients. Studies in one patient included colony forming units, ferrokinetics, electron microscopy of bone marrow, and autopsy examination. All reports of acute leukemia associated with cytostatic drugs in the literature to date are reviewed and the possible mechanisms discussed. It is suggested that patients with rheumatoid arthritis treated with azathioprine and alkylating agents may have an increased risk of developing a therapy-related acute leukemia.

Hemoglobin-xoygen affinity is known to vary in a number of disease states. The authors measured the continuous affinity of blood from healthy subjects and, using mathematical data reduction techniques, calculated coefficients for rational function models of average normal affinity, plus or minus 2 standard deviations, and 95 per cent confidence limits. Average normal P50 was 27.10 torr, with a two-standard-deviation range of 25.85 to 28.35; P50s of the 95 per cent confidence limits were 26.69 and 27.53 torr. The affinity usually accepted as standard lay between or very near to the 95% confidence limits of normal throughout its range. It is concluded that the range of normal affinity is narrow and that, for most practical purposes, standard affinity adequately represents normal affinity. There should be little difficulty in distinguishing from normal the shifts that occur in certain disease conditions.
