RESUMO

Until this day, deafness (and hearing loss) is still a major public health concern in our society. In Switzerland, congenital hearing loss affects about 0,1% of the population. Through a literature review, this article aims to highlight the health inequalities affecting the deaf population. It also reveals the shortcomings of a healthcare system made of practitioners who have limited knowledge about the deaf population and its specific needs, and whose care, particularly in the emergency department, still remains inappropriate. Lastly, we attempt to propose concrete measures that would bridge the communication gap in practice, with the goal of improving the quality of care and reducing mutual frustrations.


La problématique de la surdité et de la difficulté d'accès aux soins qui en découle constitue encore de nos jours un important enjeu de santé publique. En Suisse, la surdité congénitale concerne environ 0,1 % de la population. À travers une revue de littérature, cet article a pour but de faire un état des lieux des inégalités en santé dont est victime la population sourde. Il révèle également la défaillance d'un système de santé fait de professionnels qui méconnaissent largement cette population et ses besoins spécifiques, et dont la prise en charge aux urgences reste particulièrement inadaptée. Nous tentons enfin de proposer des aides concrètes qui permettraient de réduire le fossé communicationnel sur le terrain, dans le but d'améliorer la qualité des soins et de diminuer les frustrations réciproques.

RESUMO

When access to a patient is impossible by conventional means because of meteorological or technical conditions, complementary resources become required for optimal management. The Swiss alpine rescue, the regional rescue team in Valais and the Swiss cave rescue organization are all providing a complementary medical solution.


Lorsque des critères météorologiques ou techniques rendent l'accès à un patient par les moyens conventionnels impossible, la complémentarité des ressources engagées est indispensable à la prise en charge de celui-ci. Parmi celles-ci, le Secours alpin suisse (SAS), les Secours régionaux valaisans ainsi que le Spéléo-secours suisse (SSS) offrent une solution de médicalisation supplémentaire.

RESUMO

Chest x-ray and plain abdominal radiography are widely used in the daily practice of the emergency physician. The concept of « Choosing Wisely ¼ implies a more efficient medicine that encourages, among other things, the avoidance of investigations that do not have a significant impact on the therapeutic management of patient. In this context, indications for these two imaging examinations were reviewed, whether in the context of a consultation in the emergency department, a hospital admission or a preoperative check-up.


Les radiographies du thorax et de l'abdomen sans préparation sont largement utilisées dans la pratique quotidienne du médecin urgentiste. Le concept choosing wisely implique une médecine plus efficiente incitant, entre autres, à éviter les investigations n'ayant pas d'impact significatif sur la prise en charge thérapeutique des patients. Dans ce contexte, les indications à ces 2 examens, que ce soit dans le cadre d'une consultation aux urgences, d'une admission hospitalière ou d'un bilan préopératoire, ont été revues.

In an alpine region, the winter season has a major impact on the daily practice of both an emergency department and a general practice. During the 2017-2018 season, we have listed the consultations caused by winter sports, whether traumatological or medical. The vast majority are ambulatory. Initial care and follow-up can usually be performed by a primary care physician. In this paper, we will outline the management of the knee and acromioclavicular sprain, two frequent lesions in the winter sports setting.


Dans un canton alpin comme le Valais, la saison d'hiver a un impact important sur la pratique quotidienne aussi bien d'un service d'urgences que d'un cabinet de médecine interne générale. Durant la saison 2017-2018, nous avons répertorié les consultations engendrées par la pratique des sports d'hiver, que leur motif soit traumatologique ou médical. La grande majorité des consultations sont ambulatoires. Leur prise en charge initiale et le suivi peuvent souvent être assurés par un médecin de premier recours. Les grandes lignes de la prise en charge des traumatismes du genou et de l'entorse acromio-claviculaire, lésions parmi les plus fréquentes, seront détaillées dans cet article.

The emergency room is a stressful environment with poor privacy. Even so, sexually transmitted infections (STIs) need to be handled in such a setting. Some STIs can hide behind common clinical pictures. Signs and symptoms of acute retroviral syndrome, other STIs as well as the initiation of postexposure prophylaxis (PEP) need to be known and managed by an emergency physician and are discussed in this article.


Même dans le contexte stressant et parfois peu confidentiel des urgences, le sujet délicat des infections sexuellement transmissibles (IST) doit être abordé. Celles-ci peuvent se cacher derrière certains tableaux cliniques fréquemment rencontrés. De même, les signes et symptômes de la primo-infection VIH ou d'autres IST, ainsi que l'initiation d'une prophylaxie post-expositionnelle (PEP), qui reste une course contre la montre, sont des points-clés à maîtriser par l'urgentiste et sont abordés dans cet article.

RESUMO

Low back pain and sciatica are common causes of medical consultation. The medical history and the physical examination are essential in their management. The primary care physician initiates treatment and relies on his physical examination to identify "red flags" that require further investigations. Regarding sciatica, it is essential to identify 3 clinical entities that require prompt referral to the emergency department for prompt neurosurgical management. These include hyperalgic sciatica, sciatica with motor deficits and the cauda equina syndrome. Sciatica with sensory deficits do not require emergency department referral.
