RESUMO

Primary thyroid lymphoma is a rare malignancy that can grow rapidly and compress the airway. In such cases, a tracheostomy or thyroidectomy is generally required. Here, we report a case of a 76-year-old woman diagnosed with primary thyroid diffuse large B-cell lymphoma (DLBL), with dyspnea attributed to tracheal compression by the enlarged thyroid. The patient’s poor general condition meant that thyroidectomy was considered challenging. Therefore, a temporary endotracheal silicone stent was successfully inserted to maintain the airway, as tracheal stenting has been shown to be an effective treatment for both benign and malignant obstructive airway diseases. We conclude that, in patients with primary thyroid DLBL who present with airway compression but are not eligible for surgery, tracheal stent insertion can maintain the airway and allow time for chemotherapy to be administered.

RESUMO

As population ages, much more elderly patients have been diagnosed with head and neck cancer especially in Korea. Diverse factors such as comorbidities and disabilities make the establishment of evidence-based guidance more challenging. Chronological age, literally measured in days, months and years from the time the individual was born, is not an ideal predictor in determining personal health condition; individualization of treatment protocols with identifying the concept of frailty is the key to produce an advanced treatment plan of elderly patients suffering from head and neck malignancy. Frailty is a type of geriatric syndrome which refers to the status with age-associated declines in physiologic function and increased vulnerability to stress conditions independent of age. Therefore, screening the status of frailty may be beneficial for the elderly patients to determine decent treatment option and maximize the efficiency. In order to provide otolaryngologists prognostic information adapting the concept of frailty, research papers published from 2000 to 2018 are comprehensively reviewed in this article. For optimal care of elderly patients with head and neck cancer, frailty is definitely required to be assessed in order to establish an ideal treatment option and increase the efficacy of the treatment.

RESUMO

Aspiration, the entry of material into the airway below the true vocal folds, has been reported in various type of postsurgical head and neck patients. The important of correctly identifying the cause of aspiration in the head and neck surgical patient. Clearly each cause represents a different physiologic or anatomic disorder which requires different management strategy. We discuss about the cause of aspiration during head and neck treatment.

RESUMO

Ceruminous adenoma is a benign neoplasm of the ceruminous gland, and its development in human external auditory canal is very rare. Due to the limited number of cases, controversy still exists about nomenclature, classification, diagnosis and its treatment. Recently we experienced a 55 years old woman who was diagnosed with ceruminous adenoma of the external auditory canal, and who was treated with wide excision. So we report this uncommon case in combination with a review of the literature.

RESUMO

Small cell carcinoma commonly originates in the lung, with only about 4% of cases arising at extrapulmonary sites. Furthermore, small cell carcinoma of the sinonasal tract is extremely rare. In Korea, only 2 cases of primary sinonasal small cell carcinoma have been reported in the nasal cavity and the nasal septum, respectively. Recently, we have experienced a rare case of small cell carcinoma arising from the right maxillary sinus coexisting with a fungal ball lesion. Herein, we report this case with a review of the literature.

RESUMO

Marginal zone B-cell lymphomas are indolent lymphomas that arise from the memory B cells in the marginal zone of lymphoid tissue. They include mucosa-associated lymphoid tissue lymphoma, splenic marginal zone lymphoma, and nodal marginal zone lymphoma. These three subtypes share morphologic and immunophenotypic features, but can be distinguished by primary site at presentation. Recently, we experienced a rare case of primary nodal marginal zone lymphoma that arose from an intra-parotid lymph node. So we report this case with a review of literature.

RESUMO

Primary cartilaginous tumors of the laryngeal lesion are rare and it accounts for only 1% of laryngeal tumor pathology. They are most commonly located on the endolaryngeal surface of cricoid cartilage, and it is extremely rare to find them in vocal muscle lesion. In most cases, treatment of choice is based on surgical excision, and postoperative radiotherapy is typically recommended. We experienced a case of a 22-year-old young woman without underlying disease, presenting extraskeletal myxoid chondrosarcoma of vocal cord, especially in the vocal muscle. The patient underwent mass excision through suspension laryngoscope and radiation therapy.

RESUMO

Leiomyosarcoma is a malignant tumor originating from the smooth muscle tissue and is mostly found in the genitourinary and gastrointestinal tract and the peritoneum. A leiomyosarcoma involving the larynx is extremely rare because the smooth muscle is only present in the blood vessel in larynx. There are only few cases of pure laryngeal leiomyosarcoma that have been reported in modern medical literature. Recently, we experienced a 78-year-old male patient with laryngeal leiomyosarcoma. So we report this case with a review of literature.
