Introduction: In 2022, the US Food and Drug Administration enacted final regulations to establish the category of over-the-counter (OTC) hearing aids aimed at reducing barriers to access hearing health care for individuals with self-perceived mild to moderate hearing loss. However, given the infancy of this device category, the effectiveness of OTC hearing aids in real-world environments is not yet well understood. Methods and results: To gain insights into the perceived benefit of self-fitting OTC hearing aids, a two-pronged investigation was conducted. In the primary investigation, 255 active users of a self-fitting OTC hearing aid were surveyed on their perceived benefit using an abridged form of the Satisfaction with Amplification in Daily Living (SADL) scale. The mean global (4.9) and subscale scores (Positive Effect (PE): 4.3; Negative Features (NF): 4.3; Personal Image (PI): 6.1) were within the range of those previously reported for users of prescription hearing aids. In the secondary investigation, 29 individuals with self-reported hearing impairment but no prior experience with the investigational self-fitting OTC hearing aids used the devices and reported their perceived benefit and satisfaction following short-term usage. For this prospective group, the global SADL (5.4) and subscale scores (PE: 4.8; NF: 4.9; PI: 6.5) following a minimum of 10 weeks of real-world use were also within the range of those previously reported for traditional hearing aid users. In addition, this prospective group was also asked quality of life questions which assessed psychological benefits of hearing aid use. Responses to these items suggest hearing aid related improvements in several areas spanning emotional health, relationships at home and at work, social life, participation in group activities, confidence and feelings about one's self, ability to communicate effectively, and romance. Discussion: Converging data from these investigations suggest that self-fitting OTC hearing aids can potentially provide their intended users with a level of subjective benefit comparable to what prescription hearing aid users might experience.

RESUMO

A facile and efficient strategy for constructing anti-oil-fouling zwitterionic coatings on membranes is developed. The resultant membrane exhibits excellent anti-oil-fouling ability even in a dry state, and has a high efficiency for emulsion separation with a high flux of 5800 L m-2 h-1 bar-1 and an oil rejection of up to 99.6%.

Purpose: To examine the associations between higher order aberrations (HOAs), visual performance, demographics, and ocular characteristics in a young Asian population with high myopia. Methods: This was a retrospective review of military pre-enlistees conducted between March 2014 to September 2018. Visual acuity and contrast sensitivity were tested under photopic, mesopic and simulated night conditions. Ocular, corneal and internal HOAs were measured with a Hartmann-Shack wavefront aberrometer (KR-1W, Topcon Co., Tokyo, Japan). Results: 522 eyes of 263 consecutive subjects with severe high myopia (defined as spherical equivalent refraction [SER] ≤ -10.00D) in at least one eye, and high myopia (SER ≤ -6.00D) in the fellow eye, [mean (SD) SER -11.85 (2.03D)] were analysed. The mean (SD) age of subjects was 18.5 (1.6) years. Chinese eyes had significantly greater internal total HOA root-mean-square (RMS) compared to Malay eyes [mean difference (SD) 0.0246 (0.007) µm, p < 0.001). More negative SER was associated with greater ocular total HOA (p = 0.038), primary coma (p = 0.003) and tetrafoil (p = 0.025) RMS, as well as more positive ocular (p = 0.003) and internal primary spherical aberration (p = 0.009). Greater ocular total HOAs was associated with reduced visual acuity in simulated night conditions and low contrast, decreased contrast sensitivity under mesopic and simulated night conditions (all p < 0.05). Conclusions: Greater HOAs were associated with Chinese ethnicity and more negative SER in a young Asian population with high myopia. Greater HOAs were associated with poorer visual performance in low luminance and reduced contrast conditions.

An essential aspect of controlling and preventing mosquito-borne diseases is to reduce mosquitoes that carry viruses. We designed a smart mosquito trap system to reduce the density of mosquito vectors and the spread of mosquito-borne diseases. This smart trap uses computer vision technology and deep learning networks to identify features of live Aedes aegypti and Culex quinquefasciatus in real-time. A unique mechanical design based on the rotation concept is also proposed and implemented to capture specific living mosquitoes into the corresponding chambers successfully. Moreover, this system is equipped with sensors to detect environmental data, such as CO2 concentration, temperature, and humidity. We successfully demonstrated the implementation of such a tool and paired it with a reliable capture mechanism for live mosquitos without destroying important morphological features. The neural network achieved 91.57% accuracy with test set images. When the trap prototype was applied in a tent, the accuracy rate in distinguishing live Ae. aegypti was 92%, with a capture rate reaching 44%. When the prototype was placed into a BG trap to produce a smart mosquito trap, it achieved a 97% recognition rate and a 67% catch rate when placed in the tent. In a simulated living room, the recognition and capture rates were 90% and 49%, respectively. This smart trap correctly differentiated between Cx. quinquefasciatus and Ae. aegypti mosquitoes, and may also help control mosquito-borne diseases and predict their possible outbreak.

The popularization of long-term invasive tools for continuously monitoring blood pressure remains challenging. However, with the rising popularity of wearable personal health management devices, non-cuff blood pressure measurement technology that applies electrocardiography (ECG) and photoplethysmography (PPG) has gradually received increasing attention. In particular, whether blood pressure can be measured continuously by the PPG signal alone is of great interest. In this study, we aim to develop a device that includes systolic and diastolic blood pressure calculation formulas derived from characteristic waveform points in the PPG time domain and that can measure blood oxygenation and heart rate. This device applies empirical formulas developed by PPG waveforms in the PhysioNet MIMIC-II database to calculate blood pressure. The systolic and diastolic pressures are then compared with the actual blood pressures obtained from invasive blood pressure waveforms to verify the effectiveness and feasibility of the complete developed system. Overall, 263 waveforms with double peaks and 261 waveforms with only a single peak totaling 524 sets of data are used to derive the empirical formulas. The systolic blood pressure estimation result using single peak analysis has an excessively large error exceeding ±40 mmHg, providing no reference value. However, systolic blood pressure estimation is notably better in double peak analysis, with error values reducing to approximately 23 mmHg. Diastolic pressure estimation errors are low with both single (±7 mmHg) and double peak (±4 mmHg) analyses. The error is lower in double-peak analysis than in single-peak analysis for obtaining systolic pressure from PPG waves. We plan to use PPG to detect additional physiological parameters in the future, e.g., respiratory rate, heart rate variability, or irregular heartbeat, to further enhance the functionality of PPG-based wearable devices.

PURPOSE: Early confirmation or ruling out biliary atresia (BA) is essential for infants with delayed onset of jaundice. In the current practice, percutaneous liver biopsy and intraoperative cholangiography (IOC) remain the golden standards for diagnosis. In Taiwan, the diagnostic methods are invasive and can only be performed in selective medical centers. However, referrals from primary physicians and local pediatricians are often delayed because of lacking clinical suspicions. Ultrasounds (US) are common screening tools in local hospitals and clinics, but the pediatric hepatobiliary US particularly requires well-trained imaging personnel. The meaningful comprehension of US is highly dependent on individual experience. For screening BA through human observation on US images, the reported sensitivity and specificity were achieved by pediatric radiologists, pediatric hepatobiliary experts, or pediatric surgeons. Therefore, this research developed a tool based on deep learning models for screening BA to assist pediatric US image reading by general physicians and pediatricians. METHODS: De-identified hepatobiliary US images of 180 patients from Taichung Veterans General Hospital were retrospectively collected under the approval of the Institutional Review Board. Herein, the top network models of ImageNet Large Scale Visual Recognition Competition and other network models commonly used for US image recognition were included for further study to classify US images as BA or non-BA. The performance of different network models was expressed by the confusion matrix and receiver operating characteristic curve. There were two methods proposed to solve disagreement by US image classification of a single patient. The first and second methods were the positive-dominance law and threshold law. During the study, the US images of three successive patients suspected to have BA were classified by the trained models. RESULTS: Among all included patients contributing US images, 41 patients were diagnosed with BA by surgical intervention and 139 patients were either healthy controls or had non-BA diagnoses. In this study, a total of 1,976 original US images were enrolled. Among them, 417 and 1,559 raw images were from patients with BA and without BA, respectively. Meanwhile, ShuffleNet achieved the highest accuracy of 90.56% using the same training parameters as compared with other network models. The sensitivity and specificity were 67.83% and 96.76%, respectively. In addition, the undesired false-negative prediction was prevented by applying positive-dominance law to interpret different images of a single patient with an acceptable false-positive rate, which was 13.64%. For the three consecutive patients with delayed obstructive jaundice with IOC confirmed diagnoses, ShuffleNet achieved accurate diagnoses in two patients. CONCLUSION: The current study provides a screening tool for identifying possible BA by hepatobiliary US images. The method was not designed to replace liver biopsy or IOC, but to decrease human error for interpretations of US. By applying the positive-dominance law to ShuffleNet, the false-negative rate and the specificities were 0 and 86.36%, respectively. The trained deep learning models could aid physicians other than pediatric surgeons, pediatric gastroenterologists, or pediatric radiologists, to prevent misreading pediatric hepatobiliary US images. The current artificial intelligence (AI) tool is helpful for screening BA in the real world.

Objective To establish an animal model of sparganosis mansoni through oral administration of Cyclops infected with procercoids. Methods Domestic cats were infected with Sparganum mansoni under laboratory conditions, and fresh cat stool samples were collected, washed in dechlorinated water, and filtered. Spirometra mansoni eggs were collected and prepared into suspensions. Twenty C57BL/6j mice were randomly divided into the experimental group (n = 15) and the control group (n = 5). Wild Cyclops were infected with Spirometra mansoni coracidia to allow 3 to 5 procercoids in each Cyclop. Then, each mouse in the experimental group was given 15 Cyclops infected with procercoids by gavage, while mice in the control group were orally administered with the same volume of dechlorinated water. All mice were sacrificed after 5 months, and dissected, and suspicious Sparganum mansoni worms were collected. The serum specific IgG antibody against Sparganum mansoni was measured in mice using enzyme-linked immunosorbent assay (ELISA). Genomic DNA was isolated from suspicious Sparganum mansoni worms, and the specific Sparganum mansoni cytochrome oxidase I (COI) gene was amplified using PCR assay. Results Among the 15 mice in the experimental group, six were positive for the serum specific IgG antibody against Sparganum mansoni, and milky white worms were found and collected from the subcutaneous regions of 4 out of 6 mice. Only one worm was detected in each mouse, and the worm morphology was similar to Sparganum mansoni. Capillary electrophoresis of the PCR amplification products of COI gene presented a specific band with 151 bp in size, and sequencing analysis revealed 100% homology with Sparganum mansoni. Conclusions A mouse model of sparganosis mansoni is successfully created through oral administration of Cyclops infected with Spirometra mansoni procercoids.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a frequent problem of veterans receiving care and is often associated with cognitive deficits. The Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) is a well-validated cognitive screening measure often used in the US Department of Veterans Affairs (VA), particularly in neurorehabilitation settings. However, the influence of PTSD on RBANS performance is unclear, particularly within a heterogeneous VA outpatient population in which PTSD and traumatic brain injury (TBI) may not be the primary focus of care. METHODS: Participants included 153 veterans with complex deployment-related health problems, including a diagnosis of PTSD (n = 98) and a history of TBI (n = 92). All veterans completed a targeted cognitive battery that included the Wechsler Test of Adult Reading, the Wechsler Adults Intelligence Scale, measure assessing processing speed, attention, and cognitive flexibility, and RBANS. RESULTS: A diagnosis of PTSD was associated with worse performance on the Story Recall subtest of the RBANS, but not on any other cognitive measures. A diagnosis of mild TBI, or co-occurring PTSD and TBI did not predict cognitive performance on any measures. CONCLUSIONS: The RBANS best captured cognitive deficits associated with PTSD compared with a history of mild TBI or co-occurring mild TBI and PTSD. These findings may provide insight into the interpretation and attribution of cognitive deficits in the veteran population.

Objective To investigate the prevalence of Spirometra mansoni infections in hosts in Jiangsu Province, so as to provide the scientific basis for the management of sparganosis mansoni. Methods From 2018 to 2019, nine counties (cities, districts) were randomly selected from Jiangsu Province as the survey sites, and 100 healthy individuals were randomly selected to perform the serological test of S. mansoni infections and the detection of S. mansoni eggs. The procercoids were detected in the intermediate host Cyclops, and the S. mansoni eggs were identified in the stool samples of the definitive hosts cats and dogs. Results The prevalence of S. mansoni human infections was 0 (0/900) in the 9 survey sites of Jiangsu Province, and the sero-prevalence of the specific IgG antibody against S. mansoni was 1.22% (11/900). The positive rate of procercoids was 0.33% (3/900) in Cyclops. In addition, the S. mansoni egg-positive rate was 1.48% (2/135) in cats and dogs. Conclusions Sparganosis mansoni is prevalent in Jiangsu Province. Health education pertaining to the damages of sparganosis mansoni and the route of S. mansoni infections should be improved.

During the operation of subway vehicles, the vibration of air conditioning units is mainly transmitted to the vehicle body through the suspension support, which seriously affects the stability and comfort of the vehicle during operation. Therefore, the design and optimization of the suspension support of air conditioning units has become a hot topic in the research of the dynamic characteristics of subway vehicles. In this paper, the rigid and flexible coupling dynamic model of metro is firstly calculated to simulate the stress of the suspension point of air conditioning of the vehicle body when the vehicle is running. The initial structure design of the suspension support is carried out, and the stress of the air conditioning suspension point is taken as the load input to analyze the stiffness and strength of the initial structure of the suspension support. Then, the fatigue life is taken as the topology constraint, and the variable density method (SIMP) is used to optimize the topology of the suspension bracket. Finally, the optimized suspension support is validated. The results show that after topological optimization, the maximum displacement and maximum stress of the suspension support under vertical, horizontal, and vertical loads are reduced by 80%, 93%, and 99%, respectively, compared with the original structure model, and the maximum stress under vertical loads is reduced by 50%.

BACKGROUND: Continuous glucose monitoring (CGM) offers multiple data features that can be leveraged to assess glucose management. However, how diabetes healthcare professionals (HCPs) actually assess CGM data and the extent to which they agree in assessing glycemic management are not well understood. METHODS: We asked HCPs to assess ten de-identified CGM datasets (each spanning seven days) and rank order each day by relative glycemic management (from "best" to "worst"). We also asked HCPs to endorse features of CGM data that were important in making such assessments. RESULTS: In the study, 57 HCPs (29 endocrinologists; 28 diabetes educators) participated. Hypoglycemia and glycemic variance were endorsed by nearly all HCPs to be important (91% and 88%, respectively). Time in range and daily lows and highs were endorsed more frequently by educators (all Ps < .05). On average, HCPs endorsed 3.7 of eight data features. Overall, HCPs demonstrated agreement in ranking days by relative glycemic control (Kendall's W = .52, P < .001). Rankings were similar between endocrinologists and educators (R2 = .90, Cohen's kappa = .95, mean absolute error = .4 [all Ps < .05]; Mann-Whitney U = 41, P = .53). CONCLUSIONS: Consensus in the endorsement of certain data features and agreement in assessing glycemic management were observed. While some practice-specific differences in feature endorsement were found, no differences between educators and endocrinologists were observed in assessing glycemic management. Overall, HCPs tended to consider CGM data holistically, in alignment with published recommendations, and made converging assessments regardless of practice.

Growth arrest-specific 5 (GAS5) is an anti-oncogene that has been extensively studied in tumors. However, research on GAS5 in the context of nervous system disease is rare at present. This study aimed to investigate the role of the long non-coding RNA GAS5 in rat pheochromocytoma cells (PC12 cells). GAS5-overexpressing lentivirus was transfected into PC12 cells, and expression levels of GAS5 and C-myc were detected by real-time PCR. Ratios of cells in S phase were detected by 5-ethynyl-2'-deoxyuridine. Immunohistochemical staining was used to detect the immunoreactivity of neuron microtubule markers Tuj1, doublecortin, and microtubule-associated protein 2. Apoptosis was detected by flow cytometry, while expression of acetylcholine in cells was detected by western blot assay. We found that GAS5 can promote PC12 cells to differentiate into Tuj1-positive neuron-like cells with longer processes. In addition, cell proliferation and cell cycle were significantly suppressed by GAS5, whereas it had no effect on apoptosis of PC12 cells. Our results indicate that GAS5 could increase the expression of choline acetyltransferase and acetylcholine release. Thus, we speculate that GAS5 is beneficial to the recovery of neurons and the cholinergic nervous system.

OBJECTIVE: To establish an animal model of Sparganum mansoni (plerocercoid larva of S. mansoni) infection in mice and observe the changes of blood routine examinations and serum anti-sparganum antibody levels after the infection. METHODS: The spargana tapeworms were collected from frogs, and 25 Kunming mice were orally infected with the Sparganum tapeworms (3 tapeworms/mouse). Two days before the infection and 2, 7, 14, 21, 28, 35, 42 days and 49 days after the infection, the peripheral blood samples of mice were collected for the blood routine examinations and the detections of anti-S. mansoni IgG antibody with ELISA. Forty-nine days after the infection, all the mice were sacrificed to find out the Sparganum tapeworms in the bodies of mice. RESULTS: The count of the total white blood cells was significantly elevated on the second day of the mice infected with Sparganum. The serum anti-Sparganum antibody was detected on the 14th day of the infection in some mice, and on the 21st day of the infection, the serum anti-Sparganum antibody was detected in all the mice. After the mice were sacrificed, the Sparganum tapeworms were found out in many tissues and organs, and especially in the subcutaneous tissues and muscle. CONCLUSIONS: The establishment of animal model of Sparganum infection is successful in mice with the oral method, and white blood cells and serum specific IgG antibody detection can be used as auxiliary diagnosis methods of S. mansoni infection.

A diastereoselective α-sulfenylation of chiral α-aryl/alkyl N- tert-butanesulfinyl imidates has been developed. Suitable sulfur electrophiles can be used as sulfenylating reagents to intercept aza-enolates generated from imidate deprotonation, giving α-thiofunctionalized imidates in good yields with high diastereocontrol. This protocol for C-S bond formation can efficiently synthesize enantioenriched 1,2-sulfanyl amine derivatives such as sulconazole.

Many Veterans exposed to physical and psychological trauma experience symptoms of posttraumatic stress disorder (PTSD). As the etiology of PTSD symptoms is complex, a better understanding of the underlying biological mechanisms may improve preventative care and treatment for PTSD. Recent findings from the fields of neuroimaging and epigenetics offer important insights into the potential brain structures and biochemical pathways of modified gene expression associated with PTSD. We combined neuroimaging and epigenetic measures to assess current PTSD symptoms by measuring overall hippocampal volume and methylation of the glucocorticoid receptor (GR) gene (promoter region). Multiple regression analyses indicated that the hippocampal volume/GR methylation interaction was a predictor of PTSD symptoms. Our findings suggest that neuroimaging and epigenetic measures contribute interactively to PTSD symptoms. Incorporation of these metrics may aid in the identification and treatment of PTSD patients.

BACKGROUND: Diabetes is a chronic condition that requires constant self-management. As a consequence, several software platforms have been developed to facilitate the tracking of diabetes data to improve diabetes management. Our aim was to determine the real-world glycemic benefits of a mobile diabetes management platform used by individuals with type 1 and type 2 diabetes. METHODS: Mobile platform-using (n = 899) and control (n = 900) participants meeting specific minimum data criteria were randomly selected from a database of diabetes users. All results were modeled using different mixed effect generalized linear models, assigning random intercepts for each user, and adjusting the distribution assumption for each outcome. RESULTS: Users of the mobile platform increased their frequency of blood glucose monitoring (+8.8 tests per month, 95% CI [3.4, 14.1], P < .001) and had fewer hyperglycemic events and lower average glucose levels compared to the control group. In addition, a mobile user could expect a 3.5% drop in average BG (-6.4 mg/dL, 95% CI [-2.0, -10.7], P < .001) and a 10.7% decrease in hyperglycemia ( P < .001) after 2 months. CONCLUSION: Users of the mobile platform tested their BG more often and demonstrated greater improvement in blood glucose compared to users who did not use the mobile platform. This supports previous studies indicating that digital technologies can enhance diabetes care in a real-world setting.

RESUMO

Objective To establish an animal model of Sparganum mansoni (plerocercoid larva of S. mansoni) infection in mice and observe the changes of blood routine examinations and serum anti-sparganum antibody levels after the infection. Methods The spargana tapeworms were collected from frogs, and 25 Kunming mice were orally infected with the Sparganum tapeworms (3 tapeworms/mouse). Two days before the infection and 2, 7, 14, 21, 28, 35, 42 days and 49 days after the infection, the peripheral blood samples of mice were collected for the blood routine examinations and the detections of anti-S. mansoni IgG antibody with ELISA. Forty-nine days after the infection, all the mice were sacrificed to find out the Sparganum tapeworms in the bodies of mice. Results The count of the total white blood cells was significantly elevated on the second day of the mice infected with Sparganum. The serum anti-Sparganum antibody was detected on the 14th day of the infection in some mice, and on the 21st day of the infection, the serum anti-Sparganum antibody was detected in all the mice. After the mice were sacrificed, the Sparganum tapeworms were found out in many tissues and organs, and especially in the subcutaneous tissues and muscle. Conclusion The establishment of animal model of Sparganum infection is successful in mice with the oral method, and white blood cells and serum specific IgG antibody detection can be used as auxiliary diagnosis methods of S. mansoni infection.

Objective To explore the effect ofdexmedetomidine on the lipopolysaccharide (LPS) stimulated PC12 cells and its potential mechanism.Methods PC12 cells were treated by LPS with a concentration of 400μg/ml.The cell viability,the concentrations ofinterleukin-6 (IL-6) and tumor necrosis factor α (TNF-α) in the cell culture supernatant were measured after 3-,6-,or 12-h treatment.The expressions of toll-like receptor 4 (TLR4),myeloid differentiation factor 88 (MyD88) and phosphorylated p65 (p-p65) were measured.In the second part,PC12 cells were cultured under four different treatments,that is,normal culture media in first group,400μg/ml LPS in second group,100μmol/L dexmedetomidine in third group,400μg/ml LPS and100μmol/L dexmedetomidine in fourth group.The indexes mentioned above were measured 6 hours after LPS and DEX treatments.Results The cell viability was decreased after LPS treatment,and the concentrations of IL-6 and TNF-α were increased significantly.Compared with control group,the concentrations in 3-,6-,12-h groups showed statistically significant differences (P<0.05),especially after 6 hours.The TLR4/MyD88/NF-κB pathway was activated after LPS stimuli and reached the peak value.Compared with LPS treatment group,PC 12 cell apoptosis rate,the concentrations of IL-6 and TNF-α and the expressions of TLR4,MyD88 and p-p65 were decreased.The differences between LPS+DEX group and LPS group was statistically significant (P<0.05).Conclusion Dexmedetomidine has a protective effect on LPS stimulated PC 12 cells via the inhibition of inflammatory response.

PURPOSE: Currently, the test for useful field of view (UFOV) typically relies on subjective feedback for detecting peripheral targets while attending to a central target. Steady-state visual evoked potential (SSVEP) recorded from the occipital region of the brain has been frequently used as an objective alternative to replace subjective responses. Our aim was to investigate if a short screening test can approximate UFOV performance objectively. METHODS: We recorded SSVEPs during a simple peripheral task and a UFOV task with and without distractors as well as 30-second offline SSVEPs for eight participants. In all tasks, a peripheral annulus ring of checkered reversal pattern was concurrently presented to the participants to elicit SSVEPs. RESULTS: Orthogonal linear regression analysis found that normalized SSVEP during the UFOV task is predictive of peripheral target detection accuracy (F(1,6) = 16.250, P < 0.01, n = 8) for a task with peripheral distractors. However, this is not the case for a task without distractors (F(1,6) = 0.397, P = 0.552), possibly due to near-perfect accuracy across most participants. Offline normalized SSVEPs also only predicted the peripheral target detection accuracy (F(1,6) = 26.799, P < 0.01) for the UFOV task with peripheral distractors. CONCLUSIONS: Difficult peripheral target detection with distractors is required to differentiate UFOV performance in healthy young adults. The results suggested that offline normalized SSVEPs elicited by 30 seconds of pattern-reversal checkered annulus from the peripheral vision field can identify individuals with good UFOV performance under the stress of a difficult peripheral task.
