RESUMO

Background The serious air pollution of highway toll booths poses a high occupational exposure risk to toll collectors. It is urgent to develop purification methods suitable for airborne particles and microbial pathogens in highway toll booths. Objective To verify the purification effect of low temperature plasma air purifiers on airborne particles and microbes in highway toll booths. Methods Based on controlled-intervention design, we selected three toll booths in an expressway toll station as on-site experimental locations for 6 d (no-intervention period: the low-temperature plasma purifier was turned off in the first three days; intervention period: the purifier was turned on from 9:00 to 17:00 in the following three days). The indoor and outdoor PM2.5 and PM10 concentrations were continuously monitored during the study. At 9:00, 12:00, and 17:00 of every day during the experiment, indoor and outdoor air samples were collected to analyze the concentration of airborne culturable colonies with a plankton sampler. Airborne particle samples were collected in the outermost exit continuously from 9:00 to 17:00 every day during the experiment using a medium flow particulate sampler, and the species and relative abundance of fungi and bacteria contained in the samples were analyzed by gene sequencing. Independent-sample t test was used to compare the concentration of indoor PM2.5, PM10, and culturable colonies between the intervention period and the non-intervention period. α diversity analysis, β diversity analysis, and t test were used to compare the diversity and relative abundance of specific species of bacteria and fungi, as well as typical pathogenic bacteria and fungi in the samples between the non-intervention period and the intervention period to reflect the purification effect of low temperature plasma air purifier on airborne PM2.5, PM10, and microorganisms. Results During the intervention period, the mean indoor concentrations of PM2.5, PM10, and culturable colonies were lower than those of the no-intervention period (P<0.01 or P<0.001). The ratios of indoor to outdoor concentration (I/O) of PM2.5 and PM10 during the intervention period were significantly lower than those of the no-intervention period (P<0.001), except the I/O of culturable colonies. Compared with the average concentration at 9:00, the average cleaning rates at 12:00 and 17:00 for PM2.5 were 49.0% and 46.1%, for PM10 were 49.7% and 45.4%, for airborne culturable colonies were 50.8% and 49.9%, respectively. The β diversity analysis showed that there were significant differences in composition at the level of species of bacteria, and at the levels of genus and species of fungi between the intervention and the no-intervention periods. The relative abundances of 10 species of bacteria such as Lactobacillus and 7 species of fungi such as Torula in the intervention period were significantly lower than those in the non-intervention period, but the relative abundances of fungi such as unclassified_f_cladosporiaceae, trichomerium, and cercospora were higher (P<0.05). For typical pathogenic bacteria, the relative abundances of Lactobacillus and Clostridium_sensu_stricto_1 during the intervention period were 73.5% and 86.9% lower than those in the no-intervention period, and the relative abundance of Talaromyces was 53.5% lower (P<0.05). Conclusion Low temperature plasma air purifier has a good purification effect on indoor PM2.5, PM10, and culturable colonies in highway toll booths, and likely a limited effect on some fungi.

Objective:To study whether the excitability of facial nerve motor nucleus is increased in patients with facial spasm to further study the mechanism of facial spasm.Methods:Thirty patients with HFS underwent MVD in our hospital from April 2018 and March 2019 were chosen in our study. The amplitude and threshold voltage of facial nerve motor evoked potential (FNMEP) of the affected side and healthy side were monitored and compared by using monopulse stimulation and multi-pulse stimulation. The changes of FNMEP amplitude in patients before and after sevoflurane inhalation were monitored.Results:Stable facial motor evoked potential could be stimulated by monopulse in 26 patients (89.7%) on the affected side, while only 5 patients (17.2%) on the healthy side, showing statistical difference between the two sides ( P<0.05); there was no significant difference in FNMEP amplitude between the affected side and the healthy side ([207.2±62.1] μV vs. [180.2±55.0] μV, P>0.05); the average threshold voltage of neuromotor evoked potential on the affected side was (140.3±26.8) V, which was significantly lower than that in the healthy side ([177.0±23.2] V, P<0.05). The amplitude of FNMEP on the affected side was (207.2±62.1) μV, (133.0±36.5) μV, and (70.4±40.2) μV, respectively, before sevoflurane inhalation, and at 0.5 fold of minimun alveolar concentration (MAC) and 1.0 fold of MAC concentrations, with significant differences ( P<0.05). Conclusion:In patients with hemifacial spasm, there is increased excitability of facial nerve motor nucleus on the affected side, which to a certain extent supports the central theory of hemifacial spasm pathogenesis.

Objective To evaluate the efficacy and safety of nerve combing for trigeminal neuralgia without neurovascular decompression.Methods A retrospective review of clinical data of 281 patients with trigeminal neuralgia,admitted to our hospital from January 2008 to January 2016,was performed.The patients were divided into two groups:patients from group A were treated by nerve combing (n=32),and patients from group B were treated by microvascular decompression (n=249).The two groups were compared in terms of surgical outcomes and postoperative complications.Results The pain was completely relieved in 90.6% patients of group A (29/32) and 94.4% patients of group B (235/249),respectively;the mean follow-up duration was 62.5 months in group A,and 78.1% patients (25/32) were completely relieved;the mean follow-up duration was 59.8 months in group B,and 84.1% patients (201/239)were completely relieved;no statistically significant differences existed in the surgical outcomes and long-term outcomes between the two groups (P>0.05).No complications related to severe disability or death were noted in the two groups.The rate of facial numbness in patients from group A (71.9%) was significant higher than that in group B (2.5%,P<0.05).Conclusions Nerve combing is a safe and effective treatment for trigeminal neuralgia without neurovascular decompression.Most patients treated with nerve combing experienced some degrees of facial numbness.

Objective To observe the therapeutic effect and safety of the umbilical blood stem cell transplantation on patients with end-stage cirrhosis.Methods We chose 80 patients diagnosed with end-stage cirrhosis.All the re-lated contraindications were excluded .Communicated with all the patients, 50 patients chose the umbilical blood stem cell transplantation and conservative treatment , and 30 patients only chose conservative treatment .The 50 pa-tients were defined as observation group and the left were defined as control group .After transplantation, clinical symptoms and laboratory data were recorded at the 1 month, 6 month, and 12 month.Results ① 12 months after transplantation, clinical symptoms were improved in 38 cases(76%) in the observation group and that in 12 cases (40%) in the control group.② The laboratory data, including ALB, TBiL, ALT, AST, PTA were improved after transplantation in observation group (P <0.05).No significant difference was found in the control group after thera -py.③ After transplantation, the improvement of ALB, A/G, TBiL, PTA in observation group was higher than the control group(P <0.05).Conclusion After transplantation of the umbilical blood stem cell transplantation , the liver function and life quality of patients are significantly improved .This method is better than conservative treat -ment.The method is safe and effective in the treatment of patients with end -stage cirrhosis.

Objective To analyze the application value of CT in the diagnosis and invasive evaluation gastrointestinal stromal tumors (GIST).Methods The clinical data of 72 GIST cases examined by spiral CT scan and confirmed by surgery or pathology were analyzed retrospectively,evaluated the invasiveness by the tumor site,pattern,strengthen forms,plain and dual-phase enhanced CT scan value.Results GIST most commonly occurred in the stomach.Forteen cases were misdiagnosed in 72 cases of GIST patients,diagnosis of 5 cases was not made clearly.The differences between arterial,venous phase and scan difference in the same parts of different GIST classification were statistically significant (t =4.2,3.5,13.1,15.8,P < 0.05).The GIST in the same part of the different risk classification scan,arterial,venous CT values;difference between the arterial and scan,venous and scan,the difference was not statistically significant (t =34.8,28.5,39.4,32.2,40.1,P > 0.05).Reconstructed image could show the relationship among the tumor and its surrounding tissue and tumor feeding artery,the existence of which 33 cases of portal collateral,18 cases with the presence of arteriovenous fistula and 17 cases with liver metastasis.Conclusions Invasive different GIST dual-phase enhanced CT scans show that arteriovenous tumors are significantly enhanced;post-processing techniques can help clear the blood vessels and GIST relations with neighboring vessels and tissue imaging requires multi-disciplinary preoperative information analysis to make the right location and diagnosis.

RESUMO

Objective To analyze the clinical determination and mechanism of poststroke dementia.Methods 186 consecutively hospitalized patients who at admission after stroke,completed clinical valuation of neural-functional defect and intelligence assessments,and head CT scan.Their intelligene performances were assessed again after three months .Results Frequency of poststroke dementia was 32 5% .Poststroke dementia was related to age,level of education,residential area,hypertension ,hyperlipidemia,the location and severity of stroke,history of coronary heart disease and diabetes.But it unrelated to sex,smoking,alcohol drinking and type of stroke.Conclusions Dementia is frequently occurred after stroke,and multiple risk factors contribute to its occurrence

Objective To explore the relationship between the intelligence condition and the content of ?-aminobutyric acid (GABA) in cerebrospinal fluid (CSF) in epileptic patients.Methods We measured the Intelligence Quotient (IQ) of 40 cases of epileptic patients by using the Wechsler Intellectual Scaled, and tested their CSF-GABA contents by using HPLC.At the same time, factors which might have influence on patient’s IQ were analysed.Results In the 40 cases of epileptic patients, there were 14 cases of patients with FIQ≤89, and occupied 35%.The intelligence condition of epileptic patients was positive relation with the contents of CSF-GABA, education background and patterns of seizure,but was negative relation with the patient's age.
