RESUMO

Visceral artery aneurysms appear to belong to uncommon and potentially lethal vascular diseases. They are usually revealed accidentally during an ultrasonographic examination, magnetic resonance imaging, or computed tomography. Described in the article is a clinical case report concerning a sacciform aneurysm of the splenic artery, detected in a 53-year-old woman presenting with postcholecystectomy syndrome and followed up for abdominalgia by therapeutists and gastroenterologists. Timely performed radiodiagnosis (including multispiral computed tomography and angiography of the abdominal vessels) made it possible not only to detect the aneurysm, having thus verified the volumetric formation previously found on ultrasonographic examination, but to take adequate measures aimed at preventing rupture of the aneurysm and consisting in endovascular occlusion of the aneurysmatic cavity with metal spirals. Lack of complete clarity in the understanding of the mechanisms of the origin of and no distinctly defined therapeutic-and-diagnostic algorithm for visceral artery aneurysms dictate the necessity to continue collecting and generalizing clinical case reports regarding this rarely encountered vascular pathology.

Aneurysms of visceral arteries appear to belong to rare and potentially lethal vascular diseases. The most important role in the aetiology of aneurysms of the gastroduodenal artery is plaid by either acute or chronic pancreatitis. The article deals with a clinical case report concerning a saccular partially thrombosed aneurysm having developed in a 77-year-old woman presenting with the postcholecystectomic syndrome and detected on ultrasonographic examination of the abdominal-cavity vessels. Timely radiodiagnosis (SCT angiography of abdominal cavity vessels), adequate and timely performed endovascular occlusion of the cavity of the aneurysm with metal spirals performed in a timely manner made it possible to attain a favourable outcome. Little is known regarding the understanding of the aetiology and lack of full clarity in therapeutic approaches to aneurysms of visceral arteries predetermine the necessity to continue collecting clinical case reports concerning this rarely encountered vascular pathology in order to generalize and work out an appropriate therapeutic-and-diagnostic algorithm.

RESUMO

This study included 112 patients presenting with duodenal ulcer disease and 65 ones with chronic duodenitis treated with the use of sinusoidal modulated current (SMC) electrophoresis in combination with peloidotherapy (peat muds of the Uva health resort, Republic of Udmurtia). This treatment was shown to produce positive effect on the proliferative activity of duodenal epithelium.

RESUMO

Sinusoidal modulated current (SMC) electrophoresis of peat mud was used as monotherapy of primary chronic duodenitis in Uva sanatorium. A total of 65 patients with primary chronic duodenitis received SMC electrophoresis of peat mud diluted with distilled water. The effect was followed up with fibrogastroduodenoscopy and target biopsy of duodenal mucosa with further study of its morphological structure and element composition, acid-producing function of the stomach, hormonal profile, collagen metabolism. Positive changes were observed in clinical-endoscopic and morphological picture of the disease which is explained by favourable shifts in hormonal status and normalization of collagen metabolism in duodenal mucosa.

RESUMO

We studied the content of collagen metabolites in patients with duodenal ulcer (blood plasma, urea, and samples of the duodenal mucous tunic and mouth fluid). We also defined the role of an affected collagen metabolism in the formation and cicatrization of duodenal ulcer; finally, we ascertained that the nature of the detected changes in different biological substrate is unidirectional.

The purpose of the study was to define duodenal epithelial function in duodenal diseases and to assess the potentialities of correction of detected dysfunctions. The electrical properties (EPs) of the duodenal epithelium were explored in patients with primary chronic duodenitis, duodenal peptic ulcer (DPU) on an exacerbation and in the late postoperative period after selective proximal vagotomy. Mucosal biopsy specimens were obtained at fibrogastroduodenoscopy with biopsy forceps and placed in a sign-alternating field with the parameters, adequate bioelectric epitheliocytic characteristics: 28 V at a voltage sign change rate of 1 Hz, and a current strength of 200 microA. Bioelectrical cell reactions (nuclear fluctuation amplitude) were recorded by light microscopy using a MOB-1A ocular micrometer at minutes 10, 30, and 60 of the onset of electrical field action. A significant reduction was found in the amplitude of duodenal epithelial nuclear fluctuations in the examinees, which was most pronounced in DPU on an exacerbation. Duodenal epithelial EPs were shown to undergo regular changes during therapy and may serve as an additional test in evaluating the efficiency of therapy.

To study collagen metabolism in reparative regeneration of duodenal ulcer in response to peat mud electrophoresis of Uva sanatorium using sinusoidal modulated currents (SMC) as monotherapy in a day hospital, 82 duodenal ulcer patients in exacerbation or attenuation were examined. They received SMC-electrophoresis of peat mud diluted by sulfate sodium-calcium mineral water Uvinskaya of moderate mineralization. The dynamics of clinico-endoscopic and morphological picture of the disease was controlled with fibrogastroduodenoscopy with target biopsy of the periulcer zone and antral stomach, investigation of secretory and acid-producing function of the stomach, hormone profile and collagen metabolism. Positive changes were found attributed to favourable shifts in the hormone status and normalization of collagen metabolism in duodenal mucosa.

The electrokinetic properties of erythrocytes of the duodenal and buccal epitheliums were studied in patients with chronic duodenitis and with duodenal ulcer at exacerbation in the remote period after selective proximal vagotomy; the diagnostic value of the studied parameters was demonstrated, the presence of correlation bonds between them was shown.
