RESUMO

This is the biography of a deceased medical journal, the North American Medical and Surgical Journal, born in 1826 in Philadelphia. It was a publication of the Philadelphia Chapter of the Kappa Lambda Society. In the prospectus of the North American Medical and Surgical Journal the promoters observed that a well-conducted journal would achieve the object of elevating the medical profession to its legitimate rank which up to that time had been the recipient of low public opinion. The Journal hoped to inculcate 'a higher standard of excellence not merely in the professional or ministrative but also in the ethical relations and duties of physicians'. After several successful and productive years it passed into history in October 1831, the victim of financial difficulty.

This paper offers an inventory of Internet resources on the history of medicine. Some sites are bibliographic; they supply book and journal citations. Some provide leads (or links) to more specific information, and some include full-text articles or historical essays. Because this material is intended to furnish librarians with reference direction, more attention is given to bibliographic and "linked" sites. This paper updates a previous version published in Medical Reference Services Quarterly in Spring 1998.

RESUMO

The four-chamber view of the fetal heart is the most widely used screening examination for the detection of structural cardiac abnormalities during routine fetal ultrasonography. A prospective study of 520 consecutive routine fetal sonograms taken between 13 and 39 weeks of gestational age was performed to determine the frequency of obtaining a satisfactory four-chamber view, focusing on fetuses of less than 19 weeks' menstrual age. Menstrual age was a significant variable in procuring a satisfactory four-chamber view. Visualization of a normal four-chamber view on film progressively increased from five out of 15 (33%) at 14 weeks of menstrual age to 35 out of 38 (92%) at 19 weeks of menstrual age; real-time evaluation improved visualization to six out of 15 (40%) at 14 weeks of menstrual age and to 36 out of 38 (95%) at 19 weeks of menstrual age. Visualization in all fetuses scanned before 19 weeks' menstrual age improved from 62 (on film only) to 75% (real-time and film). Abnormalities on the four-chamber view were demonstrated before 19 weeks of menstrual age in four cases. Although not all serious cardiac defects can be detected, the four-chamber view remains a reliable screening examination of the fetal heart even early in gestation.

RESUMO

Although McClellan was the founder of a medical school and professor of surgery, his legacy of written material is pitifully small. Gross observed, "His reputation as a surgeon will be in great measure traditional, for he has left no adequate record of his observations and experience". Through notes such as these, we may develop a picture of the state of medical knowledge, the way in which it was taught and perhaps make inferences about the clinical expertise of Doctor McClellan.
