RESUMO

We present a patient with panfacial neurofibromatosis type 1 who underwent allogeneic transplantation of facial structures, which was complicated by severe rhabdomyolysis and temporary oligoanuria. Because of his underlying disease, this 35 year-old man, weighing 68 kg and with a body mass index (BMI) of 27, had undergone 17 operations for resection modeling of hypertrophied tissues, either alone or combined with static suspension techniques. He finally underwent allogeneic transplantation of facial structures. In the early hours of the postoperative period, in the context of a systemic inflammatory response syndrome, he experienced severe rhabdomyolysis, with elevation of the muscle enzyme creatine kinase producing a minor impact on kidney function. The patient was discharged home at 12 weeks after the transplantation.

BACKGROUND: The significant variation in the vascular anatomy of the abdominal wall makes preoperative imaging essential when raising a DIEP (deep inferior epigastric artery perforator) flap due to the potential for maximizing operative success, reducing intraoperative error and minimizing operative complications. Variability in perforator anatomy makes DIEP flap surgery a suitable candidate for computer and virtual reality bio models. In this context, a study was undertaken to determine the feasibility of CTA-guided by VirSSPA application. VirSSPA is a virtual reality tool developed in our Hospital for surgical planning and training. This application allows surgeons to generate the three-dimensional (3D) model of the patient. OBJECTIVE: In this paper, we present a study about VirSSPA tool for virtual reality navigation in DIEP flap surgery and compare findings with operative measurements. METHODS: We recruited 12 consecutive patients planned for an elective DIEP flap for breast reconstruction. Each patient underwent preoperative imaging of the anterior abdominal wall vasculature with both conventional CTA and VirSSPA 3D reconstruction. Imaging findings were compared to operative findings. RESULTS: 3D reconstruction of the abdominal wall with VirSSPA demonstrated a significant good correlation with perforator location compared to operative findings, showing an average error rate of 0.228 cm (95% CI, 0.17-0.30). The Pearson product-moment correlation coefficient was found to be 0.99 (p = 0.01), reflecting an almost linear relationship between the two distances, intraoperative and the one measured in the 3D reconstruction. CONCLUSION: VirSSPA provides additional and potentially more accurate data over conventional CTA with regard to the site of the best perforators and its course through the muscle for DIEP micro-vascular surgery.

The determination of antihistamines based on the measurement of the critical micelle concentration (c.m.c.) of mixed sodium dodecyl sulfate (SDS)-antihistamine aggregates is proposed. The dye Coomassie Brilliant Blue G (CBBG) was used as a photometric probe for the rapid determination of c.m.c.s. The micellar properties of these drugs permitted the determination of diphenhydramine, antazoline, tripelennamine, diphenylpyraline and clemizole at the micron level with detection limits ranging between 0.1 and 0.7 microns. Hence the proposed method surpassed existing alternative photometric methods used routinely in the quality control of these drugs in sensitivity and featured similar detection limits to fluorimetric methods. The relative standard deviation for 6 micron diphenhydramine was 3.7%. The method was applied to the determination of these drugs in pharmaceutical preparations (solutions, capsules, creams and pills). which were analyzed directly after dissolution of the samples in distilled water.

A new approach for the quantitation of drugs, based on the measurement of the critical micelle concentration of mixed surfactant-drug aggregates, is proposed. This methodology involves the photometric titration of drugs in an aqueous medium using a surfactant as titrant. The Coomassie Brilliant Blue G (CBBG) dye, negatively charged, is used as a photometric probe. The analytical applicability of this approach is demonstrated by quantifying tricyclic antidepressants (TCAs) such as imipramine, desimipramine, amitriptylin, and nortriptylin. Aggregates studied included TCA-sodium dodecyl sulfate (SDS) and TCA-Triton X-100 mixtures. Because of the opposite charge of TCAs and SDS, which causes strong synergistic effects in the mixture relative to the properties of their individual components, this anionic surfactant was selected for the quantitation of TCAs. Pharmaceutical preparations can be analyzed directly after dissolution of the samples in water or ethanol. The detection limit achieved for the studied drugs is about 0.12 mg L-1, so the proposed method surpasses existing alternative photometric methods in sensitivity and features a detection limit similar to fluorimetric methods. The relative standard deviation for 0.8 mg L-1 of TCA is 2.6%.

Formation of dye-induced mixed premicellar aggregates from binary surfactant solutions is proposed for the determination of alkyltrimethylammonium surfactants at the muM level. The Coomassie Brilliant Blue G (CBBG) dye, negatively charged, induces the formation of cationic surfactant aggregates at concentrations far below the cmc. The role of CBBG in the formation of premicelles was studied by using pyrene as a fluorimetric probe. Formation of CBBG-cationic surfactant aggregates of well-defined stoichiometries that depend on the total surfactant concentration added is demonstrated. Also, the influence of analytical parameters affecting the concentration at which a given aggregate is formed was studied. Linear calibrations for alkyltrimethylammonium surfactants were obtained by using different cationic surfactants as titrants; therefore, the previously derived measurement parameter for mixed micelles is applicable to premicellar aggregates as well.

A kinetic-photometric method for the determination of Hg(II) over the range 10-80 ng/ml is proposed. It is based on the accelerating effect of this ion on the reaction between hexacyanoferrate(II) and 1,10-phenanthroline which is monitored via the ferroin complex formed. Anionic sodium dodecyl sulphate (SDS) micelles, which catalyse the reaction, allow the ferroin complex to be formed under more acidic conditions. Combination of this pH shift and the development of the reaction in the vicinity of micelles results in improved selectivity in the determination of Hg(II) compared to the reaction occurring in an aqueous medium. Some observations on the effect of SDS on the reaction are reported.
