Introduction: The most crucial factor in improving animal reproduction efficiency is early pregnancy diagnosis. Early diagnosis not only reduces the time interval between two calvings but also aids farmers in identifying open animals, thereby preventing significant milk production losses. Therefore, the objective of this study was to discover circulatory miRNAs that would be useful for early pregnancy diagnosis in buffalo. Material and methods: Blood samples were taken on 0, 6th, 12th, and 18th day after artificial insemination from pregnant animals (n = 30) and non-pregnant animals (n = 20). During these stages of pregnancy, total RNA was extracted, and a small RNA library was subsequently generated and sequenced on the Illumina platform. Subsequently, Real-time PCR was used to validate the findings. Results and discussion: There were 4,022 miRNAs found during the pregnancy, with 15 of those lacking sequences and 4,007 having sequences already in the database. From the beginning of pregnancy until the 18th day, 25 of these miRNAs showed a substantial shift in expression levels in the maternal blood, with a change more than two logs. Furthermore, based on qPCR results, 19 miRNAs were found to be more abundant in pregnant animals than in non-pregnant animals. We used target prediction analysis to learn how maternally expressed miRNAs relate to fetal-maternal communication. In conclusion, miRNA based biomarkers that could be associated with the diagnosis of pregnancy were identified including miR-181a and miR-486 highly upregulated on the 18th day of pregnancy. This study also provides a comprehensive profile of the entire miRNA population in maternal buffalo blood during the early stages of pregnancy.

A biopsy-proven patient with prostate carcinoma aged 70 years was referred to the department of nuclear medicine for radionuclide-based therapy. His prostate-specific antigen levels were >1000 ng/mL, and prostatic magnetic resonance imaging showed an enlarged prostate with a heterogeneous signal and size 3.8x3.7x3.5 cm with few small heterogeneous nodular signals in the transition zone. He was scheduled for 18F prostate-specific membrane antigen (PSMA) positron emission tomography/computed tomography (PET/CT) scan before therapy. 18F PSMA PET/CT revealed PSMA-expressing prostate lesions (maximum standardized uptake value ~10.2) with extension into the urinary bladder along with bilateral supraclavicular, mediastinal, retrocrural, retroperitoneal, and pelvic lymph nodes and sclerotic lesions in the entire axial and appendicular skeleton.

Electrochemical ammonia synthesis, which couples oxygen evolution at the anode with nitrogen reduction at the cathode, holds great significance for future food and energy needs. Both of these half-cell reactions determine the overall cell potential and efficiency of the process. However, the employment of different catalysts on either side, due to discrete mechanisms, increases the complexity and material processing costs of the system, where the designing of a bifunctional catalyst active towards both the NRR and OER is of huge significance. Unfortunately, the initial screening of the designed catalysts via physical characterizations, optical methods and other techniques, does not provide details about the electrochemical activity. The scanning electrochemical microscopy (SECM) technique can be useful to screen multi-catalysts at the same time for their electrochemical activities. Herein, we employed the sample generation-tip collection (SG-TC) mode of SECM to screen the designed NixBy catalysts before half-cell investigations, which suggested that the catalyst synthesized via sonochemical reduction (SR), i.e. NixBy (SR), was a better catalyst. This inference was in accordance with the half-cell NRR and OER measurements (FE: 49% for NH3 production, OER overpotential: 300 mV). By virtue of this remarkable bifunctional activity, the NRR-OER coupled full cell was assembled, which initiated the NH3 production at just 1.7 V and produced NH3 (1.08 mg h-1 mgcat-1) at the cathode and O2 (0.81 mg h-1 mgcat-1) at the anode after 2 h of electrolysis at 1.9 V.

A 67-year-old man is presented with complaints of chest pain and productive cough for 1½ years. Chest X-ray was suggestive of right upper lobe Koch's lesion. Sputum was positive for mycobacterium tuberculosis. His symptoms got relieved partially by antitubercular treatment but the patient had an aggravation of symptoms for which he was evaluated. Computed tomography (CT) thorax revealed an endobronchial lesion in the right upper lobe bronchus. Bronchoscopy showed a mass in the right main bronchus and biopsy was suggestive of moderately differentiated squamous cell carcinoma (SCC). 18Fluoro-deoxy-glucose positron emission tomography/CT was performed for staging. There would have been chances of coexisting tuberculosis with SCC.

Aberrant regulation of ß-catenin signaling is strongly linked with cancer proliferation, invasion, migration, and metastasis, thus, small molecules that can inhibit this pathway might have great clinical significance. Our molecular modeling studies suggest that ormeloxifene (ORM), a triphenylethylene molecule that docks with ß-catenin, and its brominated analogue (Br-ORM) bind more effectively with relatively less energy (-7.6 kcal/mol) to the active site of ß-catenin as compared to parent ORM. Herein, we report the synthesis and characterization of a Br-ORM by NMR and FTIR, as well as its anticancer activity in cervical cancer models. Br-ORM treatment effectively inhibited tumorigenic features (cell proliferation and colony-forming ability, etc.) and induced apoptotic death, as evident by pronounced PARP cleavage. Furthermore, Br-ORM treatment caused cell cycle arrest at the G1-S phase. Mechanistic investigation revealed that Br-ORM targets the key proteins involved in promoting epithelial-mesenchymal transition (EMT), as demonstrated by upregulation of E-cadherin and repression of N-cadherin, Vimentin, Snail, MMP-2, and MMP-9 expression. Br-ORM also represses the expression and nuclear subcellular localization of ß-catenin. Consequently, Br-ORM treatment effectively inhibited tumor growth in an orthotopic cervical cancer xenograft mouse model along with EMT associated changes as compared to vehicle control-treated mice. Altogether, experimental findings suggest that Br-ORM is a novel, promising ß-catenin inhibitor and therefore can be harnessed as a potent anticancer small molecule for cervical cancer treatment.

The application of biocontrol agents in farm operations for pest control programs is gaining priority and preference globally. Effective delivery, infectivity of the biocontrol agents, and quality shelf-life products containing these bioagents are vital parameters responsible for the success of biopesticides under field conditions. In the present study, moisture-retaining bio-insecticidal dustable powder formulation (SaP) of Steinernema abbasi (Sa) infective juveniles (IJs) was developed and assessed for its shelf life, physicochemical profile, and bio-efficacy against subterranean termite under field conditions. Formulation exhibited free-flowing character, with pH of 6.50-7.50, and apparent density in the range 0.50-0.70 g cm-3. The bioefficacy study for two rabi seasons (2020-2021, and 2021-2022) in wheat and chickpea grown in an experimental farm heavily infested with subterranean termites (Odontotermes obesus) revealed a significant reduction in plant damage due to pest attack in formulation-treated plots, monitored in terms of relative number of infested tillers in wheat and infested plants in chickpea fields. The reduced damage to the crop caused by termite was reflected in the relative differences in the growth and yield attributes as well. The study establishes the potential of the developed product as a biopesticide suitable for organic farming and integrated pest management operations.

The thyroid cartilage metastatic involvement is an extremely rare entity. It can be asymptomatic at the earlier stage and can become symptomatic later on. Involvement of thyroid cartilage is frequent in melanoma and renal and rarely reported in an advanced stage of carcinoma prostate, breast, and lung. These cases were usually reported on positron emission tomography/computed tomography (PET/CT) as can often easily be missed on computed tomography scan alone. We present a case report of metastatic involvement of thyroid cartilage in squamous cell carcinoma of buccal mucosa detected on the whole-body 18F-fluorodeoxyglucose PET/CT.

Improving reproductive efficiency in livestock relies mainly on the ability to detect pregnancy quickly and accurately. Recently, circulating nucleic acids (CNAs) have been exploited for prenatal diagnosis in humans and animals. In the current investigation, serum samples were collected from pregnant animals (n = 30) and non-pregnant animals (n = 20) on 0th, 6th, 12th, and 18th day post artificial insemination. Total DNA was isolated from these serum samples. Two CNA tags (Bov-B and ART2A) derived from repetitive sections of the bovine genome were amplified using DNA extracted from serum samples. The expression analysis of these CNAs was done using real-time polymerase chain reaction assay, and copy number of each tag was calculated in pregnant and non-pregnant animals. The average number of copies of Art2A increased approximately threefold (P < 0.01) from day zero of pregnancy (7,000 copies) to the day 18 of pregnancy (> 21,000). Similarly, BovB levels in the pregnant group increased significantly (approximately 2.9-fold) from day zero (93,900 copies) till day 18 (> 2, 72,310 copies) (P < 0.01). There was no significant change observed on the 6th and 12th day of pregnancy and on the 18th day in the non-pregnant animals. In conclusion, based on these findings, the defined cut-off value can distinguish between pregnant and non-pregnant animals with a sensitivity of nearly 80% and specificity of nearly 70%. It is possible to employ these two CNA tags as biomarkers for early detection of pregnancy in buffaloes.

Crop evapotranspiration is essential for planning and designing an efficient irrigation system. The present investigation assessed the capability of four machine learning algorithms, namely, XGBoost linear regression (XGBoost Linear), XGBoost Ensemble Tree, Polynomial Regression (Polynomial Regr), and Isotonic Regression (Isotonic Regr) in modeling daily reference evapotranspiration (ETo) at IARI, New Delhi. The models were developed considering full and limited dataset scenarios. The efficacy of the constructed models was assessed against the Penman-Monteith (PM56) model estimated daily ETo. Results revealed the under full and limited dataset conditions, XGBoost Ensemble Tree gave the best results for daily ETo modeling during the model training period, while in the testing period under scenarios S1(Tmax) and S2 (Tmax, and Tmin), the Isotonic Regr models yielded superior results over other models. In addition, the XGBoost Ensemble Tree models outperformed others for the rest of the input data scenarios. The XGBoost Ensemble Tree algorithms reported the best values of correlation coefficient (r), mean absolute error (MAE), mean square error (MSE), root mean square error (RMSE), and mean absolute percentage error (MAPE). Thus, we recommend applying the XGBoost Ensemble Tree algorithm for precisely modeling daily ETo in semi-arid climatic conditions.

The 0.2-0.8mg% @ 0.2 mg% bovine serum albumin (BSA 66), hemoglobin (Hb 64.5), and double stranded deoxyribonucleic acid (dsDNA 130 kDa) with water and 0.25, 0.50, and 1.00 g% aqueous carboxymethyl cellulose sodium salt (NaCMC) thermodynamically, kinetically, and tentropically stable homogeneous solutions via resonating energy transfer were designed. Density (ρ), viscosity (η), surface tension (γ), friccohesity (σ), Gibbs free energy (ΔGb), chemical potential (µ), frictional volume (ϕ), apparent molar volume (V2), hydrodynamic radius (Rh), and isentropic compressibility (ksϕ) at 298.15, 304.15, and 310.15 K were studied. UV-Vis spectrophotometry, fluorescence spectroscopy, circular dichroism (CD), MALDI TOF, and in-silico study via reorientational activities have elucidated structural recognition. The ρ, η, γ, σ, ΔGb, µ, V2, ϕ, Rh, and ksϕ physicochemical properties (PCPs) have studied the interacting activities of salt bridges (peptide bonds) of proteins and base pairs (adenine, thymine, cytosine, guanine) of dsDNA on developing nanohydration sphere (NHS) with water and 0.25, 0.50, and 1.00 g% aq-NaCMC. Regression constants of PCPs have elaborated the interacting mechanism of internal linkages of BSA, Hb, and dsDNA for solubilizing them without unfolding assisted by glycosidic bonds of glucopyranose units of NaCMC. Magnitudes of regression constants analyse the inner salt bridges, electrostatic dipoles, and hydrogen bonds (HB) to interact with H2O dipoles and NaCMC affecting solubilization of biomolecules (biomols) in areas of biochemical, biophysical, bioengineering, tissue, and genetic engineering. PCPs, UV-Vis, fluorescence, CD, and in-silico study analyse their structurally interacting abilities with NaCMC via respective NHS to be extended to pharmaceutical and cosmetic processes by minimizing quantum energy barrier (QEB) as Newtonian behaviour of structural sensors.

We synthesized graphene oxide (GO) doped with transition metal ions and characterized it using XPS, FT-IR, TGA/DTG, XRD, SEM, AFM, ICP-OES, UV/vis, and Raman spectroscopy. An intrinsic viscosity [η] of 0.002-0.012 g% @ 0.002 aq-GO was determined for viscosity average molecular weight (M v) of GO at 288.15, 298.15, and 308.15 K. Mark-Houwink (M-H) constants k (cm3 g-1) and a (cm3 mol g-2) were calculated for 5-15 mg/100 mL polyvinylpyrrolidone (PVP), using 29, 40, 55 kg mol-1 as markers for calculating M v by fitting the [η] to the Mark-Houwink-Sakurada equation (MHSE). We obtained 48 134.19 g mol-1 M v at 298.15 K, and the apparent molar (V ϕ m , cm3 mol-1), limiting molar volumes (V 0 GO)GO⃑0, enthalpy (ΔH m, J mol-1), entropy (ΔS m, J mol-1 K-1), viscosity (η m, mPa s mol-1), surface tension (γ m, mN m-1 mol-1), friccohesity (σ m, scm-1 mol-1), fractional volume (ϕ m, cm3 mol-1), isentropic compressibility (K sϕ,m, 10-4 cm s2 g-1 mol), infer GO molar consistency throughout the chemical processes. Molar properties (MPs) infer a GO monodispersion producing negative electrons (e-) and positive holes (h+) under sunlight. The transition metal ions (Fe2+, Mn2+, Ni2+, Cr3+, TMI) doped onto GO (TMI-GO), can photodegrade methylene blue (MB) in 60 min compared with 120 min using GO alone. The 4011 C atoms, 688 hexagonal sheets, 222 π-conjugations, and 4011 FE were calculated from the 48 134.19 g mol-1. The functional edges are the negative and positive holes generating centres of the GO 2D sheets.

The 7-nitro-2'-phenyl-5',6',7',7a'-tetrahydrospiro[indeno[1,2-b]quinoxaline-11,3'-pyrrolizine]-1',1'(2'H)-dicarbonitrile (SIQPI), 2'-(4-cyanophenyl)-7-nitro-5',6',7',7a'-tetrahydrospiro[indeno[1,2-b]quinoxaline-11,3'-pyrrolizine]-1',1'(2'H) dicarbonitrile (SIQPII), and 2'-(4-methoxyphenyl)-7-nitro-5',6',7',7a'-tetrahydrospiro[indeno[1,2-b]quinoxaline-11,3'-pyrrolizine]-1',1'(2'H)-dicarbonitrile (SIQPIII) were used to photocatalyze quinonoid phenolphthalein (QHIn) in aq-ACN-EtOH (mixed solvent) with NaCl and KCl electrolytes. SIQPI, II, and III spiroindenoquinoxaline pyrrolidines (SIQPs) as spiroheterocyclic photocatalysts alone could not reduce QHIn, but with the addition of electrolytes they are reduced via π cationic interactions (PCI). SIQPI, II, and III with NaCl reduced QHIn in 120, 28, and 50 min, unlike in 138, 58, and 63 min with KCl in mixed solvent. SIQPI, II, and III alone have reduced methylene blue (MB) in 120, 45, and 70 min, unlike in 110, 27, and 55 min with graphene oxide (GO), whereas with NaCl and KCl hey are reduced in 82, 36, and 44 min and 89, 43, and 50 min, respectively. SIQPs with GO had reduced MB in less time than the SIQPs alone, and SIQPs with NaCl had reduced QHIn in a shorter time than KCl. The electrolytes have cocatalyzed a reduction of dyes under sunlight (SL). The electrolytes have reduced a quinonoid structure (QS) and dyes by generating negative and positive (e - and h +) holes in a shorter time. SIQPII and magnetic nanoparticles (MNPs) of 58 nm with NaCl photocatalyzed the QHIn in 2880 min. The SIQPs also reduced methyl orange (MO) and brilliant blue R (BBR) at variable temperature (T) and pH range, whereas SIQPs have developed a molecular organic framework (MOF) with transition-metal salts (NiCl2, CrO3, KMnO4, CuSO4, and MnCl2) on photocatalysis.

OBJECTIVE: To evaluate the clinical and radiological outcome of Gamma Knife radiosurgery (GKS) in treatment of intracranial dural arteriovenous fistula (DAVF) with cortical venous drainage (CVD) and compare it with the outcome of endovascular therapy. METHODS: Patients who underwent GKS or endovascular therapy for intracranial DAVF with CVD over 10 years (January 2007 to December 2016) at the All India Institute of Medical Sciences, New Delhi, were included. Demographics, clinical presentation, imaging details, and follow-up clinical status were reviewed retrospectively. Clinical follow-up was conducted once every 6 months. Radiological follow-up using digital subtraction angiography was performed at a mean 24 months after intervention. Patients with clinical follow-up of <1 year were excluded from the study. RESULTS: The study included 35 patients (26 in embolization group and 9 in GKS group) who had intracranial DAVF with CVD were included in the study. Clinical improvement was seen in 77.78% of the patients who received GKS and 57.7% of the patients who underwent embolization (P = 0.431). Complete obliteration of DAVF was seen in 55.56% of the patients in the GKS group and 57.7% of the patients in the embolization group (P = 1). GKS was at least as effective as embolization in terms of clinical and radiological outcome in treatment of intracranial DAVF with CVD. CONCLUSIONS: Contrary to popular perception, GKS should be considered as an effective first-line treatment alternative of intracranial DAVF with CVD.

OBJECTIVE: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of corona virus-induced disease 19 (COVID-19) that was declared as a global pandemic in March 2020 by the world health organization (WHO). Two vaccines were granted for emergency use by the Central Drugs Standard Control Organization (CDSCO) in India, Covishield® (AstraZeneca's vaccine manufactured by Serum Institute of India) and Covaxin® (manufactured by Bharat Biotech Limited). Sputnik - V has been granted EUA in the month of April 2021. The purpose of this study is to determine the association of COVID-19 infection, its severity and outcome in COVID-19 vaccinated people. METHODS: This was a hospital based prospective cohort study done between March to June 2021 at PBM Associated Group of Hospitals (AGH), Bikaner, Raj. Total 1028 COVID suspected cases consulted in COVID OPD or hospitalized under department of medicine, out of which 146 satisfied the inclusion and exclusion criteria, out of these 146, first 100 cases who gave consent for part of study were selected. RESULTS: Among 100 COVID-19 infected cases, 49 received first dose while rest got both doses. After first dose of vaccination 42.86% had mild and 32.65% had severe clinical infection while after both doses 80.39% had mild and 11.76% had severe clinical infection. On evaluation of HRCT Chest, after first dose 8.16% had normal 40.82% were in severe category while those who got both doses it was 52.82% 3.92% respectively. Among 49 who got first dose, 10.20% recovered on just home based treatment without any need of hospitalization, while 89.8% got admitted in dedicated COVID hospital out of which 73.47% got recovered and 16.33% died. Among 51 who got both the doses, 66.67% recovered on just home based treatment, while 33.33% required hospitalization out of which 25.49% got recovered and 7.84% died. CONCLUSION: After 2nd dose of vaccine there is a significant risk reduction in need of hospitalization and getting severe infection and mortality when compared with first dose only.

Spiroindenoquinoxaline pyrrolizidines (SIQPs)-7-nitro-2'-phenyl-5',6',7',7a'-tetrahydrospiro[indeno[1,2-b]quinoxaline-11,3'-pyrrolizine]-1',1'(2'H)-dicarbonitrile (SIQP I), 2'-(4-cyanophenyl)-7-nitro-5',6',7',7a'-tetrahydrospiro[indeno[1,2-b]quinoxaline-11,3'-pyrrolizine]-1',1'(2'H)-dicarbonitrile (SIQP II), and 2'-(4-methoxyphenyl)-7-nitro-5',6',7',7a'-tetrahydrospiro[indeno[1,2-b]quinoxaline-11,3'-pyrrolizine]-1',1'(2'H)-dicarbonitrile (SIQP III)-have been synthesized through a one-pot cascade Knoevenagel condensation reaction in acetonitrile (ACN) with 91, 98, and 87% yields, respectively. Structures are characterized by 1H NMR and 13C NMR spectroscopy, nuclear Overhauser enhancement spectroscopy (NOESY), Fourier transform infrared (FT-IR) and UV-vis spectroscopy, thermogravimetric analysis (TGA), high-resolution mass spectroscopy (HRTEM), fluorescence and Raman spectroscopy, and energy-dispersive analysis by X-ray (EDX) spectroscopy. SIQPs in ACN photocatalyzed methylene blue (MB) but not phenolphthalein (HIn). SIQPs distinguished the quaternary atoms and dipoles of the fluorescent dye (MB) contrary to the quinonoid HIn structure. In sunlight, SIQPs without electricity input acted as a photonic sensor to detect fluorescent dyes in waste effluents of textile, paper, dyes, and other industries. Activation energy (E a), enthalpy (ΔH), entropy (ΔS), and Gibbs free energy (ΔG) calculated from UV-vis absorption spectra show photocatalytic reduction (PCR) activities in the order SIQP II > III > I. The N-atom of pyrrolizidine and -NO2 of nitro-indenoquinoxaline (NIQ) induced the highest occupied molecular orbital (HOMO) to the lowest unoccupied molecular orbital (LUMO) electrodynamics to enable the SIQPs to catalyze biochemical activities.

Lanthanide nanoparticles and nanorods especially their biocompatible oxide forms like cerium oxide nanoparticles (CNPs) with therapeutic applications are used to cure neurological oxidative stresses. Thus it tempts to study their biocompatible aspects by interactions with several biologically significant molecules. In-Vitro interactions of 15-240 µM CNPs with water, Phosphate buffered saline (PBS), DMEM media, Insulin (Ins) hemoglobin (Hb) and ds-DNA at 37 °C were studied. Their physicochemical properties study by Borosil Mansingh Survismeter (BMS) showed the first order interaction with the protein-protein structure breaking behaviour of CNPs with Hb, Ins, and DNA. Zeta potential measurements of CNPs in different biological medium show a net increase in negative charge magnitude with good colloidal stability. Kb = 4 × 106 mM-1 of CNPs-DNA infer noncovalent interactions. Circular dichroism and FTIR revealed a loss of secondary conformation with increasing CNPs concentration. In-silico molecular docking depicts CNP interaction via conventional hydrogen bonding, carbon hydrogen bonding and electrostatic interactions at the minor groove of DNA. The study reports in-dept unfolding functional mechanism investigated by physicochemical, spectroscopic, and In-Silico approaches of protein on interactions with CNPs for safer-by-design use in medicine and pharmaceutics. Fundamentally the CeO2 in ~62% and Ce2O3 in ~38% with Ce4+ and Ce3+ oxidation potentials develop a unique case of electronic configurations with 4f05d06s0 and 4f15d06s0 electrons respectively which these studies a novel one.

Tri-o-tolyl benzene-1,3,5-tricarboxylate (TOBT (T0)), tri-4-hydroxyphenyl benzene-1,3,5-tricarboxylate (THBT (T1)), and tri-3,5-dihydroxyphenyl benzene-1,3,5-tricarboxylate (TDBT (T2)), a series of 1st tier dendrimers with a common 1,3,5-benzenetricarbonyl trichloride/trimesoyl chloride (TMC) core, are reported. T0 does not have any replaceable H+ on its terminal phenyl group, acting as a branch. T1 has one phenolic -OH at the para position and T2 has two phenolic -OH groups at the 3 and 5 positions of each terminal phenyl group. During synthesis, these -OH groups at the terminal phenyl groups were protected through tert-butyldimethylsilyl chloride (TBDMSCl) assisted with t-BuOK in DCM, THF, indazole, 4-dimethylaminopyridine (DMAP), and tertiary-n-butyl ammonium fluoride (TBAF). MTBDMSP (mono-tertiary butyl dimethylsilane phloroglucinol), DTBDMSP (di-tertiary butyl dimethylsilane phloroglucinol), and TTBDMSP (tri-tertiary butyl dimethylsilane phloroglucinol) were obtained with >90% yield, and TTBDMSP phenolic derivatives (PDs) were developed to synthesize T0, T1, and T2 dendrimers by deprotecting with TBAF. T0 showed superhydrophobic properties as it did not dissolve in methanol, contrary to T1 and T2, but dissolved in acetone. Their structures were determined using 1H and 13C NMR spectroscopies, and mass spectrometry. Their scavenging activities were studied using UV-Vis spectrophotometry compared with ascorbic acid and protein binding was studied with bovine serum albumin (BSA) and lysozyme (lyso). T0 exhibited exceptional optical activity contrary to T1 and T2, which acted as antioxidants to scavenge free radicals.

RESUMO

PURPOSE: To compare the effect of 'intermittent' central venous oxygen saturation (ScvO2) monitoring with 'continuous' ScvO2 monitoring on shock resolution and mortality in children with septic shock. METHODS: Primary outcome was the achievement of therapeutic goals or shock resolution in the first 6 h. We randomly assigned children < 17 years' age with septic shock to 'intermittent ScvO2' or 'continuous ScvO2' groups. All children were subjected to subclavian/internal jugular line insertion and managed as per Surviving Sepsis Campaign Guidelines. To guide resuscitation, we used ScvO2 estimated at other clinical and laboratory parameters were monitored similarly in both groups. RESULTS: We enrolled 75 and 77 children [median (IQR) age: 6 (1.5-10) years] in the 'intermittent' and 'continuous' groups, respectively. Baseline characteristics were comparable between the groups. When compared to the 'continuous' group, fewer children in the 'intermittent' group achieved shock resolution within first 6 h [19% vs. 36%; relative risk (RR) 0.51; 95% CI 0.29-0.89; risk difference - 18.0%; 95% CI - 32.0 to - 4.0]. The lower bound of confidence interval, however, crossed the pre-specified non-inferiority margin. There was no difference in the proportion of children attaining shock resolution within 24 h (63% vs. 69%; RR 0.86; 95% CI 0.68-1.08) or risk of mortality between the groups (47% vs. 43%; RR 1.06; 95% CI 0.74-1.51). CONCLUSIONS: Given that a greater proportion of children attained therapeutic end points in the first 6 h, continuous monitoring of ScvO2 should preferably be used to titrate therapy in the first few hours in children with septic shock. In the absence of such facility, intermittent monitoring of ScvO2 can be used to titrate therapy in these children, given the lack of difference in the proportion of patients achieving shock resolution at 24 h or in risk of mortality between the intermittent and continuous groups.
