BACKGROUND: Cervical cancer is a preventable but highly prevalent cancer in many low -and middle-income countries including South Africa. Cervical cancer outcomes can be improved with improved vaccination, a well-coordinated and efficient screening programme, increased community awareness and uptake, and increased knowledge and advocacy of health professionals. This study therefore aimed to ascertain the knowledge, attitudes, practices and barriers of cervical cancer screening among nurses of selected rural hospitals in South Africa. METHODS: A quantitative cross-sectional study was conducted in five hospitals in the Eastern Cape Province of South Africa between October and December 2021. A self-administered questionnaire was used to assess demographic characteristics of nurses and cervical cancer knowledge, attitudes, barriers and practices. A knowledge score of 65% was deemed adequate. Data were captured in Microsoft Excel Office 2016 and exported to STATA version 17.0 for analysis. Descriptive data analyses were used to report the results. RESULTS: A total of 119 nurses participated in the study with just under two thirds (77/119, 64.7%) being professional nurses. Only 15.1% (18/119) of participants were assessed as having obtained a good knowledge score of ≥ 65%. The majority of these (16/18, 88.9%) were professional nurses. Of the participants with a good knowledge score, 61.1% (11/18) were from Nelson Mandela Academic Hospital, the only teaching hospital studied. Cervical cancer was deemed to be a disease of public health importance by 74.0% (88/119). However, only 27.7% (33/119) performed cervical cancer screening. Most of the participants (116/119, 97.5%) had an interest of attending more cervical cancer training. CONCLUSION: The majority of nurse participants did not have adequate knowledge about cervical cancer and screening, and few performed screening tests. Despite this, there is a high level of interest in being trained. Meeting these training needs is of utmost importance to implementing a comprehensive cervical cancer screening programme in South Africa.

Despite interventions, cancer remains a global public health concern. Cancer burden continues to grow globally, demanding the implementation of important preventative and control initiatives. Informative reports on existing routine cancer data are therefore required. This study describes the distribution and trends of cancer in Buffalo City (BFC) population, Eastern Cape (EC) Province for the period 1991-2009. Cancer patients were retrospectively surveyed in the database of Frere Hospital Oncology-Radiation Unit. Proportion by sex, race, geographical distribution in the province and top cancer sites were calculated. Direct method of standardisation was used to calculate Age Standardised Rates (ASR) for a subpopulation of patients residing at BFC; age-specific rates were applied to the reference population (World Standard Population). Trends in rates with 95% Confidence Intervals (CI) for two most common cancers in males and females over time adjusted for age and sex and interactions between time and race were assessed using Poisson Regression. A total of 19 737 patients´ records were analysed; 38.8% (7 656) males and 61.2% (12 081) females. Most patients were Black Africans (81.5%), followed by Whites (13.5%), Mixed Race (4.5%) and Asians (0.5%). A larger proportion (46.0%) of the patients were from Buffalo City, while the rest were distributed in other municipalities served by Frere Hospital. Top five cancers in males were lung [22.5%, ASR 21.0], prostate [14.7%, ASR 9.2], larynx [5.8%, ASR 5.0], mouth [4.4%, ASR 3.7] and tongue [3.8%, ASR 2.9 per 100 000] in females; cervix [20.9%, ASR 23.0], breast [23.6%, ASR 20.2], lung [3.4%, ASR 4.7], ovary [2.1%, ASR 3.0] and corpus uteri [3.4%, ASR 2.8]. Trends showed a decrease in lung and prostate cancers in males, while cervix and breast remained stable in females. White males were two times (95% CI: 1.87-2.49) more likely to have lung cancer and five times (95% CI: 3.90-6.21) more likely to have prostate cancer than Black Africans. White females were 0.43 (95% CI: 0.44-0.73) less likely to have cervical cancer and three times (95% CI: 2.45-3.14) more likely to have breast compared to Black Africans. In conclusion, the availability of routine cancer data at Frere Hospital contributes to strengthening of the national cancer notification regulation, as the results of this study suggest that the burden of cancer in the EC Province remains high.

Background: Globally, prostate cancer is rated the second most common cancer and the sixth leading cause of death. In South Africa, it is ranked as leading cancer among men. This study describes prostate cancer patterns and trends in the rural Eastern Cape Province population. Methodology: Secondary data were used from which a sample of 723 prostate cancer (C61) patients was extracted from the database into STATA version 14.0 for descriptive analysis. A direct standardization method was used to estimate age-specific and age-standardized incidence rates. Keyfitz method was used to calculate the standard error and confidence interval, whereas the Joinpoint program the annual percentage change. Results: The mean age was 64 years, with a standard deviation of 9.9. Trends in prostate cancer incidence increased significantly (p = 0.026) from 7.4% in 2010 to 12.6% in 2017. Incidence rates varied across the region, with the lowest of 4.5 per 100,000 in 1998 to the highest of 21.4 per 100,000 in 2017 period. Lusikisiki had the highest incidence rates of 53.4 per 100,000 population (95% CI 0.8-61.4), while Centane with 21.7 per 100,000 (95% CI 2.3-27.6) rated the second. Other magisterial areas showed a constant increase (p > 0.05) throughout the observation period except for Idutywa and Willowvale, with no apparent increase. Conversely, in Butterworth, incidence rates decreased from 15.2 per 100 000 (95% CI 8.6-21.9) to 11.5 per 100,000 (95% CI 6.2-16.7). Conclusion: As experienced globally and regionally, prostate cancer has become a public health concern in this population. Incidence variations across the surveillance area in the Eastern Cape were noted with hotspots.

The quality and accuracy of the data provided by cancer registries has a significant impact on decision making. Over decades, high-income countries have been successful in monitoring their cancer burden because of well-established data abstraction techniques such as digital systems. Conversely, in low- and middle-income countries, sparsely distributed cancer registries, using alternative less costly, but imprecise methods are struggling to capture all cancer cases. A population-based cancer registry in South Africa covering a resource-limited rural population is faced with challenges in case finding yet the quality and accuracy of the data provided has a significant impact on decision making. The objective of this study was to assess data quality using two data quality attributes 'completeness and accuracy' and also to determine the benefits of using active and passive case-finding methods for cancer registration in this population. Data used were collected between January 2014 and December 2015 from four hospitals to compare the quality of both active and passive case-finding methods. From all four hospitals during the same period, a first set of data obtained through passive reporting was compared with a second set of data obtained through active case finding. Covering multiple facilities during active case finding can significantly improve quality of data, while passive case finding is challenged by data collection being confined to one specific health facility, only. Better investment in active case finding is recommended in settings with resource-distribution disparities.

BACKGROUND: The poor health of South Africans is known to be associated with a quadruple disease burden. In the second National Burden of Disease (NBD) study, we aimed to analyse cause of death data for 1997-2012 and develop national, population group, and provincial estimates of the levels and causes of mortality. METHOD: We used underlying cause of death data from death notifications for 1997-2012 obtained from Statistics South Africa. These data were adjusted for completeness using indirect demographic techniques for adults and comparison with survey and census estimates for child mortality. A regression approach was used to estimate misclassified HIV/AIDS deaths and so-called garbage codes were proportionally redistributed by age, sex, and population group population group (black African, Indian or Asian descent, white [European descent], and coloured [of mixed ancestry according to the preceding categories]). Injury deaths were estimated from additional data sources. Age-standardised death rates were calculated with mid-year population estimates and the WHO age standard. Institute of Health Metrics and Evaluation Global Burden of Disease (IHME GBD) estimates for South Africa were obtained from the IHME GHDx website for comparison. FINDINGS: All-cause age-standardised death rates increased rapidly since 1997, peaked in 2006 and then declined, driven by changes in HIV/AIDS. Mortality from tuberculosis, non-communicable diseases, and injuries decreased slightly. In 2012, HIV/AIDS caused the most deaths (29·1%) followed by cerebrovascular disease (7·5%) and lower respiratory infections (4·9%). All-cause age-standardised death rates were 1·7 times higher in the province with the highest death rate compared to the province with the lowest death rate, 2·2 times higher in black Africans compared to whites, and 1·4 times higher in males compared with females. Comparison with the IHME GBD estimates for South Africa revealed substantial differences for estimated deaths from all causes, particularly HIV/AIDS and interpersonal violence. INTERPRETATION: This study shows the reversal of HIV/AIDS, non-communicable disease, and injury mortality trends in South Africa during the study period. Mortality differentials show the importance of social determinants, raise concerns about the quality of health services, and provide relevant information to policy makers for addressing inequalities. Differences between GBD estimates for South Africa and this study emphasise the need for more careful calibration of global models with local data. There are few cancer trend data reported in sub-Saharan Africa notably due to the scarcity of population-based cancer registries (PBCRs). The Eastern Cape Province PBCR is amongst the few registries in sub-Saharan Africa that reports data for a rural population. Trends in cancer incidence are reported for the period 1998-2012. Registered cases, age-standardized rates (ASRs) and standardized rate ratios are presented for the most common cancers in both males and females in three periods (1998-2002, 2003-2007 and 2008-2012). In males, the most commonly diagnosed cancer during the 15 year period was cancer of the oesophagus; incidence rates showed a significant decline over the 15 year period, entirely due to a 30% decrease between 2003-2007 and 2008-2012, to an ASR of 23.2 per 100,000 population. This was followed by prostate cancer, the incidence of which was more than doubled to a level of 9.9/100,000. In women, cancer of the cervix uteri has become the most common malignancy, with a significant increase in incidence during the period to 29.0/100,000. Oesophageal cancer is second in frequency, with (as in males) a significant decline in the final 10 years to an incidence of 14.5/100,000 in 2008-2012. The incidence of breast cancer increased by 61%, although the absolute rate remains low (12.2/100,000). The incidence rates of colorectal cancer are low, and the increases in incidence, although relatively large (35% in men, 63% in women) were not statistically significant. Kaposi sarcoma showed a dramatic increase in incidence in both sexes (3.5-fold in men, 11-fold in women) although the incidence remains relatively low by southern African standards. Cancer prevention and control activities in the area need to be informed by these data and strengthened.
