In recent years, the development of antitumor drugs has been dedicated to natural products. Amygdalin is a natural herbal cyanoglycoside that has anticarcinogenic effect on many types of cancers once hydrogen cyanide (HCN) is released. The main objective of the present study is to synthesize and investigate the potential of carboxymethyl chitosan nanoparticles (CMC NPs) as drug delivery agents for amygdalin encapsulation and its delivery to cancer and normal cell lines. In this study, carboxymethyl chitosan nanoparticles encapsulated with amygdalin (CMC-Am NPs) were prepared and characterized through their particle size, surface charge, chemical structure and dielectric properties. Also, the invitro drug release of amygdalin from CMC NPs was studied. Additionally, the cytotoxcity of the amygdalin and CMC-loaded amygdalin NPs were evaluated through MTT assay. The results showed that the prepared CMC-loaded amygdalin NPs exhibited a small particle size of 129 nm, high zeta potential value of - 43 mV and confirmed the amygdalin stability and compatibility with CMC NPs. Furthermore, the CMC NPs demonstrated sustained release of amygdalin during 24 h. Moreover, compared to free amygdalin, amygdalin-loaded CMC NPs have significant anti-cancerous effect on human colon HCT-116 and breast MCF-7 cancer cell lines while being safe on normal cells BJ1. In conclusion, CMC NPs can be employed as an efficient drug delivery vehicle for controlled and sustained amygdalin release with enhanced cytotoxicity on malignant cells without harming normal cells.

Objectives. Clinical estimation of malignant breast tumor size is critical for preoperative planning and is crucial for following up the tumor's response to the therapy in case she receives a neoadjuvant chemotharapy. Ductal carcinoma in situ (DCIS) accompanies about 25.4% of detected invasive breast cancers. The aim of this study was to examine the effect of the presence of DCIS on the accuracy of the ultrasonographic measuring malignant breast tumor size using B-mode and real time elastography. Materials and Methods. We recruited histologically confirmed breast cancer patients in a prospective observational study. Results. We recruited 50 breast cancer patients with a median age of 57.5 years. DCIS was confirmed to accompany 42% (n = 21) of the cases. Tumor size estimation using B-mode sonography (P < 0.001) as well as using real time elastography (P < 0.001). was statistically significant correlated to the actual tumor size. Presence of DCIS in 42% of our recruited patients affected the tumor size estimation using both methods thus losing the correlation between both estimations (P = 0.794). Conclusion. This study shows that the presence of DCIS significantly affects the accuracy of measuring the sizes of malignant breast tumors when using either B-mode ultrasonography or real time elastography.

BACKGROUND: The aim of this study is to detect possible risk factors for development of short- and long-term local complications after inguinofemoral lymphadenectomy for vulval cancer. METHODS: This retrospective cohort study included 34 vulval cancer patients that received inguinofemoral lymphadenectomy. The detected complications were wound cellulitis, wound seroma formation, wound breakdown, wound infection, and limb lymphoedema. Followup of the patient ran up to 84 months after surgery. RESULTS: Within a total of 64 inguinofemoral lymphadenectomies, 24% of the inguinal wounds were affected with cellulitis, 13% developed a seroma, 10% suffered wound breakdown, 5% showed lower limb edema within a month of the operation, and 21.4% showed lower limb edema during the long-term followup. No significant correlation could be found between saphenous vein ligation and the development of any of the local complications. The 3-year survival rate in our cohort was 89.3%. CONCLUSIONS: Local complications after inguino-femoral lymphadenectomy are still very high, with no single pre-, intra-, or postoperative factor that could be incriminated. Saphenous vein sparing provided no significant difference in decreasing the rate of local complications. More trials should be done to study the sentinel lymph node detection technique.

OBJECTIVE: Chicken skin mucosa (CSM) is a common finding around juvenile polyps in children. Its ultrastructural features and fate after polypectomy are not yet clear. The aim was to study ultra-structural features and outcome of this CSM compared to that of the polyps and distant endoscopically normal mucosa. MATERIAL AND METHODS: From 240 children with juvenile polyps, 45 needed a second colonoscopy. Thirty six patients showing CSM represented the cohort of this study. One polyp only was studied in each patient. The histologic features of the CSM were compared to normal and polyp mucosa. The fate of CSM was evaluated in the second colonoscopy. RESULTS: The mean numbers of intraepithelial lymphocytes, as well as lamina propria inflammatory cellular infiltrates were significantly higher in polyp mucosa than in CSM. Goblet cells were significantly higher in CSM compared to normal mucosa with marked depletion in the polyp mucosa. The muscularis mucosae thickness was significantly higher in CSM compared to polyps (p<0.0001) and both showed higher values than the normal mucosa. The CSM almost disappeared within a month period following polypectomy. CONCLUSIONS: The polyp showed the most intense mucosal inflammatory reaction. CSM with the unique thickening of muscularis mucosae especially around larger polyps almost disappeared after polypectomy. So these results suggest that CSM is a benign compensatory reaction induced by the mechanical effect of the polyp.

RESUMO

Airborne particulate material was monitored as total suspended particles and particulate matter less than 10 microm at selected sites in Cairo. The selected sites represent heavily industrial and industrial-residential areas of Cairo, Egypt. The filters were further analyzed for lead, cadmium, zinc and nickel using ion chromatography. The chosen method was modified to improve resolution and decrease the retention time. The data obtained were treated statistically using one-way analysis of variance and correlated with the anthropological and industrial activities of the sites. The concentrations of the heavy metals are studied in terms of particle size, time dependence, and safety.

Five malonanilide derivatives (M1-M5) were prepared by the reaction of ethyl malonate with aniline derivatives. These compounds were investigated as new stabilizers for double-base propellants (DBPs). The evaluation process has been performed through thermal stability tests, thermal analyses measurements (TGA and DSC) and kinetic parameters calculations (E(a)). The results of the new stabilizers were compared with the results of the classical stabilizer N, N-diethyldiphenyl urea. It has been found that o- and p-dinitromalonanilides, in particular, showed better stability effect for DBPs than the classical one.

The diagnosis of tuberculous ascites is often difficult because of the subtle clinical clues, poorly discriminative biochemical assays, delayed results of bacteriological studies and hazards of laparoscopy. Therefore, the role of ascites adenosine deaminase (ADA) activity and interferon-gamma (IFN-delta) level in distinguishing tuberculous from other causes of ascites was examined in 50 patients with ascites. Following bacteriologic culture, seventeen (34%) patients were found to have tuberculous ascites; nine (59.9%) of them had also schistosomal hepatic fibrosis (SHF). Therefore, 36% (9 out of 25) of all patients with SHF included in the study, had coexistent peritoneal tuberculosis despite the presence of transudative ascites and unrecognized clinical features. Ascites ADA activity was significantly higher in tuberculous than in other causes of ascites (P < 0.001) regardless of the presence of an underlying liver disease. A cut-off of 28 U/L reached a sensitivity of 94.4% and a specificity of 100%. A direct correlation was found between ascites ADA activity and total proteins in the tuberculous group (r = 0.613) and the only false-negative result occurred in a patient with SHF and low-ascites protein. Ascites IFN-delta level was also significantly higher in tuberculous ascites with or without SHF than in other causes of ascites (P < 0.05). A cut-off of 26 pg/ml reached a sensitivity of 81% and a specificity of 100%. There was no correlation between ascites ADA activity and IFN-delta level in the tuberculous group (r = 0.329). Based on the results of the present study, it can be concluded that tuberculous ascites should be considered as an important cause of ascites particularly in patients with underlying liver disease. Ascites ADA activity was more sensitive than ascites IFN-delta in diagnosing tuberculosis (TB). It has proved to be an easy, rapid, safe and reliable method for routine use in the early diagnosis of tuberculous ascites.

Sixty patients having schistosomiasis mansoni infection were studied for lactase deficiency by different methods including lactase assay in intestinal biopsies. Thirty patients suffering from simple intestinal (Group I) and 30 patients suffering from hepatointestinal (Group II) schistosomiasis were compared to 60 controls (Group III). Lactase deficiency was evaluated by symptoms of lactose intolerance, stool pH and osmolarity, oral lactose tolerance test (OLLT), oral lactose hydrogen breath test (OLHBT) and lactase activity (LA) in small intestinal biopsies. OLTT showed intolerance in 36.7% of Group I and 40% of Group II compared to 23.3% of the controls. OLHBT showed intolerance in 26.7%, 30% and 18% for the three groups respectively, indicating a significance in Groups I and II. Abdominal symptoms of hypolactasia had higher scores in the patient groups compared to the controls, and was even higher in Group I than Group II. The mean lactase activity measured by micromol/gm wet wt/min was not statistically higher in Group I but higher in Group II than the controls. No correlation was found between lactase activity in the mucosa and the symptoms of hypolactasia in Group I. Hence, other factors in intestinal schistosomiasis may affect lactose hydrolysis. The study showed also that OLHBT is the most sensitive, specific and efficient among the non-invasive tests.

Recent investigations show that nontechnical interventions influence patients' ratings of the quality of health care, and that these aspects of the medical encounter are as important to the patient as the technical aspects; perhaps more important. This paper adopts a consumerist approach and measures patients' perceptions of health care quality using a scale adapted from the consumer behavior literature (SERVQUAL). The study measures health care quality as well as five of its individual dimensions. The findings indicate that, for the whole sample, patients' ratings of overall quality as well as the ratings of four of the five dimensions of care are negative. Further analysis indicates that many individual aspects (scale items) are rated negatively by each of two age groups (25-65 and over 65 years old), but the gap between perceptions of the younger group and their expectations is greater than that of the senior group. The two dimensions of "assurance" and "empathy" are found to be the most discriminating dimensions between the two groups. Other analyses indicate that age, annual household income, and work status significantly relate to overall quality rating. Marketing and strategic planning implications of the results are discussed.

Serum immunoglobulin, secretory IgA in stools and leucocyte migration inhibition test were done for 75 patients suffering from bilharzial and amoebic dysentery as well as 20 normal controls. Sigmoidoscopic examination with immunofluorescent staining of colonic biopsy was done in patients with bilharzial dysentery. Humoral immune response increased significantly as detected by significant increase in serum IgG and IgM. Secretory IgA decreased significantly in bilharzial cases leucocyte migration inhibition test decreased significantly as well as plasma cells secreting IgA. This change in systemic and local immune response may predispose to infection or occur secondary to the disease.

Milled black pepper (Piper nigrum) force-fed to Egyptian toads as a suspension in amphibian saline or injected subcutaneously in the dorsal lymph sac as an ethanol extract, induced primary tumours in the liver and secondary tumours in other organs (kidney and spleen). When applied to the skin of experimental animals as an ethanol extract, black pepper induced primary tumours in the liver and secondary tumours in the ileum and stomach. Tumours of the liver were diagnosed as hepatocellular carcinomas and those of the other organs as metastases of the primary liver tumours. It is speculated that one or more constituents of black pepper may be responsible for tumour induction in the organs of the Egyptian toad Bufo regularis.

Investigations on the effect of colostrum feeding in 1--4-day-old newborn infants on serum proteins and their immunoglobulins were carried out. The values for serum total proteins, albumin and gamma globulins are higher in colostrum-fed infants than the artificially fed group. Immunoglobulin G and immunoglobulin A levels were significantly higher, while immunoglobulin M level was only slightly elevated in the former group. On the other hand, serum total proteins as well as albumin and gamma globulins levels were decreased in lactating compared with non-lactating females, while the alpha and beta globulins levels were higher for lactating than controls. For individual immunoglobulins; IgG level was lower and IgA, IgM levels were higher for lactating than non-lactating females.
