AIMS: Pneumonitis is a common and potentially deadly complication of combined chemoradiation and immune checkpoint inhibition (CRT-ICI) in patients with locally advanced non-small cell lung cancer (LA-NSCLC). In this study we sought to identify the risk factors for pneumonitis with CRT-ICI therapy in LA-NSCLC cases and determine its impact on survival. MATERIALS AND METHODS: We conducted a retrospective chart review of 140 patients with LA-NSCLC who underwent curative-intent CRT-ICI with durvalumab between 2018 and 2021. Pneumonitis was diagnosed by a multidisciplinary team of clinical experts. We used multivariable cause-specific hazard models to identify risk factors associated with grade ≥2 pneumonitis. We constructed multivariable Cox proportional hazard models to investigate the impact of pneumonitis on all-cause mortality. RESULTS: The median age of the cohort was 67 years; most patients were current or former smokers (86%). The cumulative incidence of grade ≥2 pneumonitis was 23%. Among survivors, 25/28 patients had persistent parenchymal scarring. In multivariable analyses, the mean lung dose (hazard ratio 1.14 per Gy, 95% confidence interval 1.03-1.25) and interstitial lung disease (hazard ratio 3.8, 95% confidence interval 1.3-11.0) increased the risk for pneumonitis. In adjusted models, grade ≥2 pneumonitis (hazard ratio 2.5, 95% confidence interval 1.0-6.2, P = 0.049) and high-grade (≥3) pneumonitis (hazard ratio 8.3, 95% confidence interval 3.0-23.0, P < 0.001) were associated with higher all-cause mortality. CONCLUSIONS: Risk factors for pneumonitis in LA-NSCLC patients undergoing CRT-ICI include the mean radiation dose to the lung and pre-treatment interstitial lung disease. Although most cases are not fatal, pneumonitis in this setting is associated with markedly increased mortality.

Previous descriptions of Corynosoma bullosum (Linstow, 1892) show that specimens vary greatly in the proportions of different body structures, measurements of females and males, number of rows of hooks, and egg measurements, among other features. We redescribe this species from specimens found in southern elephant seal faeces from King George Island. We also provide a molecular characterization, in addition to 5.8S and internal transcribed spacer (ITS) existing sequences. We examined 41 elephant seals, and 30 adult acanthocephalans were found in 15 of them. The specimens were identified as belonging to the genus Corynosoma due to each having a tubular body with an inflated anterior part forming a thorny disk and the posterior part bearing somatic spines on the ventral surface, and genital spines surrounding the genital pore. Individual morphology corresponded to C. bullosum: large size, marked sexual dimorphism, and proboscis with 16-18 rows of spines with 11-15 spines per row. The molecular profile of three specimens of C. bullosum was analysed using 18S rDNA. We inferred phylogenetic relationships of the family Polymorphidae using maximum likelihood and Bayesian inference. We provide an updated morphological redescription for C. bullosum including electron microscopy photographs and molecular data. The 18S gene sequences showed low genetic variation and supported that C. bullosum is a sister to Corynosoma australe.

Current medical guidelines consider pregnant women with COVID-19 to be a high-risk group. Since physiological gestation downregulates the immunological response to maintain "maternal-fetal tolerance", SARS-CoV-2 infection may constitute a potentially threatening condition to both the mother and the fetus. To establish the immune profile in pregnant COVID-19+ patients, a cross-sectional study was conducted. Pregnant women with COVID-19 (P-COVID-19+; n = 15) were analyzed and compared with nonpregnant women with COVID-19 (NP-COVID-19+; n = 15) or those with physiological pregnancy (P-COVID-19-; n = 13). Serological cytokine and chemokine concentrations, leucocyte immunophenotypes, and mononuclear leucocyte responses to polyclonal stimuli were analyzed in all groups. Higher concentrations of serological TNF-α, IL-6, MIP1b and IL-4 were observed within the P-COVID-19+ group, while cytokines and chemokines secreted by peripheral leucocytes in response to LPS, IL-6 or PMA-ionomicin were similar among the groups. Immunophenotype analysis showed a lower percentage of HLA-DR+ monocytes in P-COVID-19+ than in P-COVID-19- and a higher percentage of CD39+ monocytes in P-COVID-19+ than in NP-COVID-19+. After whole blood polyclonal stimulation, similar percentages of T cells and TNF+ monocytes between groups were observed. Our results suggest that P-COVID-19+ elicits a strong inflammatory response similar to NP-COVID19+ but also displays an anti-inflammatory response that controls the ATP/adenosine balance and prevents hyperinflammatory damage in COVID-19.

Resumen La presencia de alteraciones de señal en resonancia magnética (RM) cerebral durante o posterior a un evento epiléptico es cada vez más reconocida en la literatura. Los cambios de señal peri-ictales se considera que sería el resultado de edema cerebral localizado, defecto de la autorregulación cerebral y disrupción de la barrera hemato-encefálica que ocurre durante una crisis epiléptica sostenida. Reportamos el caso de un hombre de 62 años diagnosticado de un síndrome parietal de instalación subaguda cuyo estudio con RM de cerebro mostró una lesión tumefacta con edema cortico-subcortical de ubicación temporo-occipital derecha. El estudio con electroencefalograma mostró actividad ictal en la misma localización. Se inició terapia con fármacos anticonvulsivantes mostrando franca mejoría clínica y electrofisiológica. El control con RM diferido mostró resolución completa de las alteraciones descritas.


The presence seizure-induced signal changes on brain magnetic resonance imaging (MRI) have been increasingly recognized in the literature. The reversible MRI changes in epileptic patients may be the result of a local brain swelling, a defect of cerebral autoregulation and a blood-brain barrier disruption during sustained epileptogenic activity. We report a 62 years old man diagnosed with a subacute right parietal syndrome. MRI shows a tumefactive lesion in right temporo-occipital lobes mimicking a structural lesion. Electroencephalogram (EEG) exhibits continuous ictal activity in the same region. Antiepileptic drugs were started achieving progressive electro-clinical improvement. Subsequent MRI showed remission of signal changes.

Seals (Phocidae) undergo an annual cycle of moulting that implies hair regeneration, and in the case of southern elephant seals, it also involves the superficial strata of the epidermis. Therefore, surviving the moulting period is crucial for their obligate and permanent ectoparasites. Throughout evolutionary time, sucking lice (Echinophtiriidae) have developed morphological, behavioural and ecological adaptations to cope with the amphibious lifestyle of their hosts. Lepidophthirus macrorhini, the Southern elephant seal louse species, faces the additional challenge of surviving attached to the host during the moulting period. Since lice live on the skin, L. macrorhini has developed a unique survival strategy by piercing the skin of their host, thus keeping them protected from moulting. During fieldwork in Patagonia and Antarctica, skin samples with lice within were collected for histological analysis to assess whether these parasites caused damage to the host. Lice generate an inflammatory process in the host's dermis, and these lesions could alter the normal chemical and mechanical protective properties of the skin facilitating secondary infections. Further studies that analyse the potential pathogens in those skin lesions are necessary to properly assess the real impact of ectoparasites on their host health.

Aging can be associated with decreasing muscle strength, and related factors are comorbidities, sex, physical activity, and possibly genetic factors. Among genetic factors the renin-angiotensin system is of interest, but data on the Peruvian population is lacking. The objective of our study was to evaluate the association of grip strength and angiotensin convertase enzyme (ACE) polymorphism in Peruvian older people. A cross-sectional study in a convenience sample of 104 participants over 60 years in Lima, Perú, with analysis of the ACE polymorphism, was performed. We studied 104 participants, 46 men (44,2%) and 58 women (55,8%), with a mean age and standard deviation (SD) of 73,7 (7,4) years, range between 60-90 years. The frequency of D/D, I/D and I/I genotypes was 12,7; 43,7 and 43,7% respectively. The genotype distribution of ACE polymorphism agreed with the Hardy-Weinberg equilibrium (p=0,746). The mean (SD) of grip strength in the D/D, I/D and I/I polymorphisms were 24,8 (7,2); 22,8 (7,2) and 23,4 (7,6) kg respectively; no significant difference was observed (p=0,41) between genetic groups. In this small convenience sample of older Peruvians, no association was found between grip strength and ACE genotype.

Lice from family Echinophthiriidae are of the few insects that have successfully colonized marine environment living as ectoparasites of pinnipeds, i.e., sea lions, seals, and the walrus. They have developed unique adaptations to cope with the amphibious lifestyle of their hosts. Because eggs do not survive underwater, lice could only reproduce when their host remains on pack ice enough time. Consequently, lice generations per year are limited by host haul-out behavior. The objective of this work is to study the effect of host sex and age class, and the annual variation on the prevalence and mean abundance of Antarctophthirus lobodontis in crabeater seals from the Antarctic Peninsula. During three consecutive field-seasons, we collected lice from 41 crabeater seals (23 females, 16 males, 2 indeterminate, being 24 adults, and 17 juveniles). We investigated this effect on the prevalence and mean abundance by a generalized linear model formulation in a Bayesian framework. According to the lowest Deviance Index Criterion model, sex host does not affect prevalence nor mean abundance. We found that juveniles present greater abundance and prevalence than adults, possibly due to foraging habits. They spent more time on the ice than adults in groups of dozens of animals. This behavior would favor both egg development and lice transmission. We do not find adult females with lice, which suggests that transmission of A. lobodontis should be horizontal. The high mean abundance of lice in 2014 could be associated with an unusual increase in Lobodon carcinophaga population, probably related to the pack-ice availability and zooplankton abundance.

RESUMO

We report on the first computation of the strong running coupling at the physical point (physical pion mass) from the ghost-gluon vertex, computed from lattice simulations with three flavors of domain wall fermions. We find α_{MS[over ¯]}(m_{Z}^{2})=0.1172(11), in remarkably good agreement with the world-wide average. Our computational bridge to this value is the Taylor-scheme strong coupling, which has been revealed of great interest by itself because it can be directly related to the quark-gluon interaction kernel in continuum approaches to the QCD bound-state problem.

Intraoperative somatosensory evoked potentials (SEPs) provide dorsal somatosensory system functional and localizing information, and complement motor evoked potentials. Correct application and interpretation require in-depth knowledge of relevant anatomy, electrophysiology, and techniques. It is advisable to facilitate cortical SEPs with total intravenous propofol-opioid or similarly favorable anesthesia. Moreover, SEP optimization is recommended to enhance surgical feedback speed and accuracy by maximizing signal-to-noise ratio (SNR); it consists of selecting highest-SNR peripheral and cortical derivations while omitting low-SNR channels. Confounding factors causing non-surgical SEP reduction should be excluded before issuing a warning. It is advisable to facilitate their identification with peripheral SEP controls and cortical SEP systemic controls whenever possible. Warning criteria should adjust for baseline drift and reproducibility. The recommended adaptive warning criterion is visually obvious amplitude reduction from recent pre-change values and clearly exceeding trial-to-trial variability, particularly when abrupt and focal. Acquisition and interpretation should be done by qualified technical and professional level personnel. Indications for SEP monitoring include intracranial, posterior fossa, and spinal neurosurgery, as well as orthopedic spine, cerebrovascular, and descending aortic surgery. Indications for SEP mapping include sensorimotor cortex and dorsal column midline identification. Future advances could modify current recommendations.

This study aimed to molecularly survey Bartonella in dogs from Chile. Quantitative real-time PCR (qPCR) for Bartonella spp. based on nuoG gene was performed in 139 blood samples taken from dogs belonging to rural localities of the Valdivia Province, Los Ríos region, southern Chile. nuoG qPCR-positive samples were submitted to conventional PCR assays for ftsZ, gltA, rpoB and nuoG genes and sequencing for speciation and phylogenetic analysis. Based upon qPCR results, Bartonella spp. occurrence in dogs was 4.3% (6/139). Out of six nuoG qPCR-positive samples, six, three, two and none showed positive results in cPCR assays based on gltA, ftsZ, rpoB and nuoG genes, respectively. Consistent sequencing results were obtained only for the ftsZ gene from sample #1532 (GeneBank accession number: MG252491), and gltA gene from samples #1535 (MG252490) and #1532 (148 bp fragment that was not deposited in GenBank). Phylogenetic analysis of ftsZ and gltA genes allowed speciation of two nuoG-positive samples, one as Bartonella vinsonii subsp. berkhoffii and the other as B. henselae. Bartonella vinsonii subsp. berkhoffii and B. henselae are detected for the first time in dogs from Chile, highlighting the importance of the canine population as a source of zoonotic agents and potential infection risk to humans.

Resumen: ANTECEDENTES: la inducción del trabajo de parto es un procedimiento que se indica cuando existe riesgo de continuar el embarazo, en lugar de interrumpirlo. Esta maniobra enfrenta nuevas presiones, por lo que es necesario mantener actualizado el conocimiento sobre su indicación. En los últimos años se han desarrollado protocolos clínicos de mayor eficacia y seguridad, que han hecho más accesible este procedimiento. OBJETIVO: emitir un consenso actualizado y analizar los diferentes aspectos de la práctica cotidiana relacionada con la inducción del trabajo de parto. MATERIALES Y MÉTODOS: se integró un grupo de especialistas de trece instituciones nacionales para analizar diferentes aspectos de la práctica cotidiana de la inducción del trabajo de parto. Se siguió una metodología tipo Delphi de cuatro etapas, con bibliografía de normas clínicas internacionales de apoyo. CONCLUSIONES: la inducción del trabajo de parto considera los siguientes criterios: establecer con certeza que el procedimiento ofrece el mejor desenlace para la madre y el feto, confirmar la edad gestacional, realizar la evaluación obstétrica completa y contar con infraestructura para enfrentar las posibles complicaciones. Existen diferentes opciones para la inducción del trabajo de parto; sin embargo, en los últimos años se ha generalizado la prescripción de análogos de prostaglandinas (misoprostol) en todo el mundo. El especialista debe efectuar la evaluación individualizada de la paciente y el feto, con la finalidad de descartar situaciones que comprometan la salud de ambos. La complicación más común de la inducción del trabajo de parto es la taquisistolia, que puede asociarse con desprendimiento prematuro de placenta, rotura uterina y sufrimiento fetal agudo. Los óvulos vaginales de liberación controlada representan la única opción para retirar el estímulo con dinoprostona o misoprostol ante efectos adversos.


Abstract: BACKGROUND: Induction of labor is a maneuver indicated when there is a greater risk of continuing the pregnancy, than interrupting it. The induction of labor faces new pressures that make it necessary for the doctor to be permanently updated. In recent years, clinical protocols of greater efficiency and safety have been developed, which have made this procedure more accessible. OBJECTIVE: To present an updated consensus and to analyze the different aspects related to the labor induction. MATERIAL AND METHODS: A Delphi-type of consensus was conducted with participation of active obstetricians and gynecologists specialists from thirteen national institutions. Major clinical-oriented topics of induction of labor were addressed. CONCLUSIONS: To induce labor, it is necessary that at least the following situations coexist: to establish with certainty that the procedure offers the best outcome for the mother and her child, to confirm the gestational age, to make a complete obstetric evaluation and to have the infrastructure Necessary to deal with possible complications. There are several options to induce labor, although in recent years the indication of prostaglandin analogues (misoprostol) has become the most common option worldwide. The specialist must make an individualized evaluation of the patient and the fetus, in order to rule out situations that may endanger the health of any of them. The most common complication of labor induction is tachysystole, which can be complicated by premature placental abruption, uterine rupture and acute fetal distress, requiring urgent attention. Controlled-release vaginal ovules are the only option available to withdraw the stimulus with dinoprostone or misoprostol in the presence of adverse effects.

A non-surgical sterilant (NSS) was used with the objective of assessing the zootechnical performance, carcass and meat quality, and hormone levels. 90 male piglets were selected with birthweights varying between 1.5kg to 2.0kg. The control group was constituted of 45 males castrated by the conventional surgical method, at the seventh day of age, and the treated group was constituted of 45 males castrated with NSS (active principle of zinc gluconate in the concentration of 26.2mg/mL associated to the dimethyl sulfoxide at 0.5%), with the application of the first dose on the seventh day of age and the second dose on the fourteenth day of age. The zootechnical assessments were carried out on the farm and consisted: weight gain in the periods per animal and feed conversion. The slaughter of the animals and the assessment of the carcass and meat quality and hormone levels were carried out in a slaughterhouse and the municipality of Campinas (SP). The study demonstrated that the use of NSS was a viable alternative in relation to the zootechnical performance and the carcass quality. In relation to the meat quality there was no significant difference in the majority of the assessed parameters.(AU)


Foi utilizado esterilizante não cirúrgico (ENC) com o objetivo de se avaliar o desempenho zootécnico e a qualidade de carcaça, de carne e de níveis hormonais. Foram selecionados 90 leitões machos de peso entre 1,5kg e 2,0kg ao nascimento. O grupo controle foi constituído por 45 machos castrados pelo método cirúrgico convencional, ao sétimo dia de idade, e o grupo tratado foi constituído por 45 machos castrados com ENC (princípio ativo de gluconato de zinco na concentração de 26,2mg/mL associado ao dimetil sulfóxido a 0,5%), com primeira dose de aplicação ao sétimo dia de idade e a segunda dose ao 14° dia de idade. As avaliações consistiram de: ganho de peso nos períodos por animal e conversão alimentar. O abate dos animais e a avaliação da carne e de níveis hormonais foram realizados em um frigorífico, na cidade de Campinas (SP). O uso de ENC foi uma alternativa viável em relação ao desempenho zootécnico e à qualidade da carcaça. Em relação à qualidade da carne, não houve diferença significativa na maioria dos parâmetros avaliados.(AU)

Abstract: Introduction. Ankylosing spondyloarthritis (AS) is the most common and representative of seronegative spondyloarthropathy. It is characterised by chronic inflammation of the axial skeleton, leading to chronic back pain and progressive stiffness. It typically occurs in young patients, with symptoms often appearing with peak onset between 20 and 30 years of age, and is strongly associated with human leukocyte antigen B-27 (HLA B-27). Current imaging techniques are central in the management of these patients. The recognition of its classic imaging findings and its complications is essential for radiologists. Objectives. To present a pictorial review of the spinal imaging findings in patients with AS. Methods and materials. An analysis will be made of the spinal imaging findings in conventional radiography, CT and MRI in multiple patients from our medical centre. The most representative findings, complications, differential diagnoses, and some radiological key points will be shown.


Resumen: Introducción. La espondilitis anquilosante (EA) es la artropatía seronegativa más común y representativa. Se caracteriza por inflamación crónica del esqueleto axial, dolor referido a la columna vertebral de tipo inflamatorio y rigidez progresiva. Típicamente se manifiesta en pacientes jóvenes que inician su sintomatología entre los 20-30 años de edad y está ligada al antígeno leucocitario humano B-27 (HLA B-27). Las imágenes son fundamentales en el manejo de estos pacientes. Es deber del radiólogo reconocer los hallazgos imagenológicos presentes. Objetivos. Realizar una revisión pictográfica de hallazgos imagenológicos presentes en la columna vertebral de pacientes portadores de EA. Material y método. Análisis de las manifestaciones imagenológicas de columna más representativas de la EA en pacientes de nuestro centro en radiografía convencional, TC y RM, junto con posibles complicaciones y diagnósticos diferenciales.

RESUMO

We determined the zero-temperature phase diagram of D2 physisorbed on graphane using the diffusion Monte Carlo method. The substrate used was C-graphane, an allotropic form of the compound that has been experimentally obtained through hydrogenation of graphene. We found that the ground state is the δ phase, a commensurate structure observed experimentally when D2 is adsorbed on graphite, and not the registered √3 x √3 structure characteristic of H2 on the same substrate.

RESUMO

This Letter reports on the first computation, from data obtained in lattice QCD with u, d, s, and c quarks in the sea, of the running strong coupling via the ghost-gluon coupling renormalized in the momentum-subtraction Taylor scheme. We provide readers with estimates of α(MS[over ¯])(m(τ)(2)) and α(MS[over ¯])(m(Z)(2)) in very good agreement with experimental results. Including a dynamical c quark makes the needed running of α(MS[over ¯]) safer.

In patients with chronic obstructive pulmonary disease (COPD), pulmonary rehabilitation has been demonstrated to increase exercise capacity and reduce dyspnea. In the most disabled patients, the intensity of exercise during the training sessions is limited by ventilatory pump capacity. Non-invasive ventilation (NIV) support has been used successfully in patients with exacerbation of COPD. However, its benefit in patients with stable COPD or in a pulmonary rehabilitation program is under discussion. This chapter therefore evaluated the scientific evidence regarding the beneficial effect of NIV support in the respiratory rehabilitation on exercise tolerance. Features of the NIV as to when they do it-whether during training or night-and modality were reviewed. Use of noninvasive ventilation in pulmonary rehabilitation was recommended in selected patients with severe COPD or advanced stage, and in those with suboptimal response to exercise training. The use of NIV should be preferably at night, using inspiratory pressure support, which can help achieve a better exercise tolerance, an improvement in gas exchange and a reduction in the overhead of the respiratory muscles and dyspnea (quality evidence B, moderate strength recommendation).


Se ha demostrado que la rehabilitación respiratoria en pacientes con enfermedad pulmonar obstructiva crónica (EPOC) aumenta la capacidad de ejercicio y reduce la disnea. En la mayoría de los pacientes con discapacidad, la intensidad del ejercicio durante las sesiones de entrenamiento se ve limitada por la capacidad de la bomba ventilatoria. La ventilación no invasiva (VNI) ha sido utilizada con mucho éxito en pacientes con EPOC que presentan una exacerbación de su enfermedad. No obstante, su beneficio en pacientes con EPOC estable o en rehabilitación respiratoria está en discusión. En este capitulo se evaluó la evidencia científica que existe en cuanto al beneficio de la ventilación no invasiva (VNI) en la rehabilitación respiratoria. Se revisó la modalidad y las características de la VNI en cuanto a cuando realizarla -si durante el entrenamiento o nocturna-. Se recomendó la utilización de la ventilación no invasiva en rehabilitación respiratoria en pacientes seleccionados, con EPOC en etapa grave o avanzada, y en aquellos con respuestas subóptimas al entrenamiento. La utilización de la ventilación no invasiva debe ser preferentemente nocturna, y utilizando presión de soporte inspiratorio, la cual puede permitir alcanzar una mejor tolerancia al ejercicio, una mejoría del intercambio gaseoso y una disminución de la sobrecarga de los músculos respiratorios y la disnea. (calidad de la evidencia B, fuerza de la recomendación: moderada).

El embarazo cervical supone un 1% de los embarazos ectópicos. En el pasado se asociaba a una alta incidencia de hemorragias intensas e histerectomías. En la actualidad el diagnóstico precoz ecográfico y el tratamiento con metotrexate posibilita la preservación de la fertilidad (AU)


Cervical pregnancy represents less than 1% of all ectopic pregnancies. In the past, this event was associated with a high incidence of hysterectomies due to intense hemorrhage. Currently, due to early ultrasonographic diagnosis and medical treatment with metotrexate, future fertility can usually be preserved (AU)
