RESUMO

OBJECTIVE: Early detection and treatment of cervical precancers can prevent disease progression. However, in low-resource communities with a high incidence of cervical cancer, high equipment costs and a shortage of specialists hinder preventative strategies. This manuscript presents a low-cost multiscale in vivo optical imaging system coupled with a computer-aided diagnostic system that could enable accurate, real-time diagnosis of high-grade cervical precancers. METHODS: The system combines portable colposcopy and high-resolution endomicroscopy (HRME) to acquire spatially registered widefield and microscopy videos. A multiscale imaging fusion network (MSFN) was developed to identify cervical intraepithelial neoplasia grade 2 or more severe (CIN 2+). The MSFN automatically identifies and segments the ectocervix and lesions from colposcopy images, extracts nuclear morphology features from HRME videos, and integrates the colposcopy and HRME information. RESULTS: With a threshold value set to achieve sensitivity equal to clinical impression (0.98 [p = 1.0]), the MSFN achieved a significantly higher specificity than clinical impression (0.75 vs. 0.43, p = 0.000006). CONCLUSION: Our findings show that multiscale optical imaging of the cervix allows the highly sensitive and specific detection of high-grade precancers. SIGNIFICANCE: The multiscale imaging system and MSFN could facilitate the accurate, real-time diagnosis of cervical precancers in low-resource settings.

OBJECTIVE: to analyze the effects of a Mindfulness-based intervention on nurse leaders' emotional intelligence and resilience. METHOD: a pilot study of a randomized crossover clinical trial. The sample (n=32) was randomized into Group A (n=18) and Group B (n=14) and evaluated at the pre-test, post-test and follow-up moments. The outcomes were assessed using the Emotional Intelligence Measure, the Connor-Davidson Resilience Questionnaire and the Five Facet Mindfulness Questionnaire, and analyzed using Generalized Linear Mixed Models. RESULTS: a total of 32 nurses with a mean age of 42.6 years old were evaluated. The analyses showed significant interactions between the effects of the group x moment factors on the Self-motivation (p=0.005), Sociability (p<0.001), Self-control (p=0.013), and Total (p=0.002) emotional intelligence skill scores; as well as on the Observe (p=0.042), Describe (p=0.008), Non-judgment (p<0.001), Act with awareness (p=0.004) and Total (p<0.001) mindfulness facets. Post-test: there was a statistically significant increase in the Sociability (p=0.009) and Self-control (p=0.015) emotional intelligence skills; as well as in the Non-judgment (p=0.022) and Total (p=0.002) mindfulness facets. Follow-up: a significant increase was observed in the Non-judgment (p=0.024) and Total (p=0.026) mindfulness facets. The "resilience" variable did not present statistical significance in the "group x moment" factor, both in the post-test and during follow-up. CONCLUSION: the Mindfulness-based intervention used proved to be effective in increasing nurse leaders' emotional intelligence and dispositional mindfulness skills. PURPOSE: To evaluate sex differences in operating room (OR) time and case volumes among comprehensive cataract surgeons in Ontario, Canada's most populated province. DESIGN: Retrospective, population-based cohort study. METHODS: Physician billing data of active comprehensive cataract surgeons between 2010 and 2019 were analyzed to identify all cataract surgeries in this timeframe. The number of OR days and case volume were the primary outcomes. Data were stratified by surgeon sex and career stage. RESULTS: Between 2010 and 2019, approximately 1.05 million cataract surgeries were performed in Ontario. There were an average of 195 ± 3 comprehensive cataract surgeons per year, of which 39 ± 5 were female. The proportion of female surgeons increased from 16.8% of all surgeons in 2010 to 24.4% in 2019. The greatest proportion of male surgeons were in the late phase of their career, whereas the greatest proportion of female surgeons were in the early stage of their career. On average, male surgeons had 44.9 ± 1.90 OR days per year and females had 32 ± 1.90 OR days per year, resulting in female surgeons averaging 12.45 ± 1.90 fewer OR days per year. This OR distribution remained consistent across career stages. Average case volumes per OR day were similar across sexes, but male surgeons performed on average 172.7 ± 30.6 more surgeries per year. CONCLUSIONS: Despite performing similar average case volumes per OR day, female surgeons had less OR time compared to their male counterparts per year, and this remained consistent across career stages and over the 10-year period. Metrics for OR allocation and use should be well defined and transparent.

Los resultados fueron evaluados mediante la Medida de Inteligencia Emocional, el Cuestionario de Resiliencia de Connor-Davidson y el Cuestionario Mindfulness de Cinco Facetas, y analizados por el Generalized Linear Mixed Models . Resultados: fueron evaluadas 32 enfermeras con una edad promedio de 42,6 años. Postest: hubo un aumento estadísticamente significativo de las habilidades de sociabilidad (p=0,009) y autocontrol (p=0,015) de inteligencia emocional; de las facetas no juzgar (p=0,022) y total (p=0,002) de mindfulness . Seguimiento: se observó un aumento significativo en las facetas no juzgar (p=0,024) y total (p=0,026) de mindfulness . La variable resiliencia no presentó significación estadística en el factor grupo vs. momento en el postest y seguimiento. Conclusión: la intervención basada en mindfulness utilizada demostró ser eficaz para aumentar la inteligencia emocional y las habilidades de mindfulness disposicional de los enfermeros líderes. Objetivo: analisar a estrutura da representação social da saúde para umbandistas com vistas à abordagem do cuidado transcultural. Método: estudo qualitativo descritivo-exploratório, fundamentado na abordagem estrutural da Teoria das Representações Sociais e Teoria Transcultural de Madeleine Leininger. Os participantes foram captados pela técnica Snowball, que responderam a um questionário online, contendo caracterização sociodemográfica; escala DUREL; e instrumento de evocações livres. Os dados foram analisados por estatística descritiva e as evocações pela análise prototípica e de similitude. Resultados: participaram 110 invíduos, predominando mulheres (70%), entre 29 e 39 anos, e com alta religiosidade. A análise prototípica evidenciou vida, bem-estar e ter-fé como provavelmente centrais e hospital e médico como elementos de contraste. A análise de similitude demonstrou além dos elementos previamente e provavelmente centrais, cuidado e corpo com comportamento de centralidade. Considerações finais: concepções multidimensionais de bem-estar e qualidade de vida marcam esta representação, embasada pela alta religiosidade e o cuidado transcultural e integral em saúde(AU)


Objective: to analyze the structure of the social representation of health for Umbanda fans with a view to approaching transcultural care. Method: qualitative descriptive-exploratory study, based on the structural approach of the Theory of Social Representations and Transcultural Theory by Madeleine Leininger. Participants were captured using the Snowball technique, who responded to an online questionnaire containing sociodemographic characteristics; DUREL scale; and instrument of free evocations. Data were analyzed using descriptive statistics and the evocations using prototypical and similarity analysis. Results: 110 individuals participated, predominantly women (70%), between 29 and 39 years old, and with high religiosity. The prototypical analysis highlighted life, well-being and having faith as probably central and hospital and doctor as contrasting elements. The similarity analysis demonstrated, in addition to the previously and probably central elements, care and the body with centrality behavior. Final considerations: multidimensional conceptions of well-being and quality of life mark this representation, based on high religiosity and transcultural and comprehensive health care(AU)

The CDK4/6 inhibitors significantly increase progression-free survival (PFS) in ER+/HER2- advanced breast cancer patients. In clinical trials, overall survival (OS) improvement has been demonstrated for ribociclib and abemaciclib but not for palbociclib. We undertook a real-world evaluation of PFS and OS in 227 post-menopausal patients who received first-line CDK4/6 inhibitors. There is no significant difference in median PFS (27.5 months vs. 25.7 months, p = 0.3) or median OS (49.5 months vs. 50.2 months, p = 0.67) in patients who received either palbociclib or ribociclib, respectively. De novo disease is significantly associated with prolonged median PFS and median OS compared with recurrence disease (47.1 months vs. 20.3 months (p = 0.0002) and 77.4 months vs. 37.3 months (p = 0.0003), respectively). PR- tumours have significantly reduced median PFS and OS compared with PR+ disease (19.2 months vs. 38 months (p = 0.003) and 34.3 months vs. 62.6 months (p = 0.02), respectively). In the very elderly (>80 years), median PFS and OS are significantly shorter compared with patients who are 65 years or below (14.5 months vs. 30.2 months (p = 0.01), and 77.4 months vs. 29.6 months (p = 0.009), respectively) in the palbociclib group. Our data suggest that the benefit in the very elderly is limited, and PR+/de novo disease obtains the maximum survival benefit.

OBJECTIVE: The present study aims to establish a risk profile for high-grade cervical lesions and cervical cancer (CIN2 + ) in women undergoing colposcopy at the Hospital do Câncer de Barretos, through the analysis of Human Papillomavirus (HPV) infection, cervical cytology, and patient's age. METHODS: Retrospective cross-sectional study based on a computerized database of women aged ≥ 18 years old who underwent colposcopy at the Prevention Department of the Hospital do Câncer de Barretos from 2017 to 2019. RESULTS: A total of 3,411 women were included, 58.0% were positive for high-risk-HPV test, with a higher prevalence of CIN2+ for HPV16 (30.3%) and other HPV (45.0%). Cytological findings that suggest invasive cervical cancer (squamous cells or adenocarcinoma), regardless of the status of HPV test, showed 100% diagnosis of CIN2 + , while atypias that suggest high-grade lesions, HSIL and ASC-H, positive for HPV test, showed in 86 and 55.2%, respectively, diagnosis of CIN2 + . ASC-H cytological results among women aged > 40 years old and negative HPV were mainly associated with benign findings. We observed that ≤ CIN1 has a higher prevalence among older women with negative HPV, while for high-grade lesions there is an increase among young women HPV16- and/or 18-positive. In cancer diagnosis, we observed a predominance of HPV 16/18 regardless of the age group. CONCLUSION: The highest risks of precursor lesions and cervical cancer were found among women with positive HPV 16/18 tests and severe cytological atypia in population screening tests. In addition, cytological findings of ASC-H HPV negative in women > 40 years old usually represent benign findings in histological investigation.


OBJETIVO: Estabelecer um perfil de risco de lesões intraepiteliais de alto grau e câncer do colo do útero (NIC2 + ) em mulheres submetidas a colposcopia considerando-se a infecção pelo papilomavírus humano (HPV), citologia cervical e idade. MéTODOS: Estudo retrospectivo transversal em banco de dados informatizado de mulheres com idade ≥ 18 anos que realizaram colposcopia no departamento de Prevenção de Câncer no Hospital do Câncer de Barretos/SP no período de 2017 a 2019. RESULTADOS: Foram incluídas 3.411 mulheres, sendo 58,0% positivas para HPV de alto risco, e maior prevalência de NIC2+ para HPV16 (30,3%) e outros HPV (45,0%). Resultados citológicos sugestivos de lesões invasivas (epidermoide ou adenocarcinoma), independente do teste de HPV, apresentaram 100% de diagnóstico NIC2 + , enquanto atipias sugestivas de lesões de alto grau, HSIL e ASC-H, associados a HPV positivo, apresentaram 86 e 55,2%, respectivamente. Resultados citológicos de ASC-H entre mulheres > 40 anos e HPV negativo foram associados principalmente a achados benignos. Observamos que ≤ NIC1 apresenta uma maior prevalência entre mulheres mais velhas com HPV negativo, enquanto para lesões de alto grau, há um aumento entre mulheres mais jovens positivas para HPV16/18. Para diagnóstico de câncer, observamos que há um predomínio de HPV16/18 independente da faixa etária. CONCLUSãO: Foi identificado maior risco de lesões precursoras e câncer entre mulheres com HPV 16/18 positivo e atipias citológicas graves em testes de rastreio populacional. Além disso, resultados citológicos de ASC-H quando associados a HPV negativo com idade > 40 anos habitualmente representam achados benignos em investigação histológica.

Endothelial dysfunction is commonly associated with a cardiovascular event, such as myocardial infarction. Myocardial infarction is marked by an ischemia/reperfusion (IR) phenomenon associated with endothelial dysfunction, contributing even more to future cardiovascular events. Although the supplementation with L-citrulline and nitrate from watermelon and beetroot have been used to improve vascular function, the effect of microencapsulated watermelon rind (WR) or its co-ingestion with beetroot (WR + B) on endothelial IR injury has not been addressed. Therefore, this study aimed to investigate the effect of a single dose of WR and WR + B on IR-induced macro-and microvascular dysfunction. In a randomized, crossover, placebo-controlled study, 12 volunteers underwent macro (flow-mediated dilation) and microvascular (muscle oxygen saturation) assessment and blood collection (to measure L-citrulline, L-arginine, nitrate and nitrite) before and after 20 min of blood occlusion in WR, WR + B and placebo conditions. Prolonged ischemia induced endothelial dysfunction in the macro but not in the microvasculature. The WR and WR + B supplementation significantly restored FMD after IR injury compared to the placebo (p < 0.05). However, there was no significant difference between WR and WR + B in the macrovascular function (p > 0.05). Plasma L-citrulline, L-arginine, nitrate, and nitrite significantly increased (p > 0.05) after WR and WR + B supplementation compared to the placebo. A single dose of WR and WR + B effectively minimizes IR-induced macrovascular endothelial dysfunction in healthy individuals. Beetroot co-ingestion with watermelon did not provide an additional effect of endothelial dysfunction induced by IR (NCT04781595, March 4, 2021).

Assessment of senior medical students is usually calibrated at the level of achieving expected learning outcomes for graduation. Recent research reveals that clinical assessors often balance two slightly different perspectives on this benchmark. The first is the formal learning outcomes at graduation, ideally as part of a systematic, program-wide assessment approach that measures learning achievement, while the second is consideration of the candidate's contribution to safe care and readiness for practice as a junior doctor. The second is more intuitive to the workplace, based on experience working with junior doctors. This perspective may enhance authenticity in assessment decisions made in OSCEs and work-based assessments to better align judgements and feedback with professional expectations that will guide senior medical students and junior doctors' future career development. Modern assessment practices should include consideration of qualitative as well as quantitative information, overtly including perspectives of patients, employers, and regulators. This article presents 12 tips for how medical education faculty might support clinical assessors by capturing workplace expectations of first year medical graduates and develop graduate assessments based on a shared heuristic of 'work-readiness'. Peer-to-peer assessor interaction should be facilitated to achieve correct calibration that 'merges' the differing perspectives to produce a shared construct of an acceptable candidate.

Objetivo: analisar as evidências científicas em relação ao uso da aromaterapia na redução de sintomas relacionados ao estresse na equipe de Enfermagem. Método: revisão integrativa da literatura realizada em 01 de julho de 2022, a partir dos descritores "Aromatherapy", "Stress" e "Nursing", com utilização da plataforma Rayyan para seleção dos estudos realizados por dois pesquisadores independentes. Foram incluídos documentos de acesso digital na íntegra e gratuitos nos idiomas português, inglês ou espanhol, que analisaram a aromaterapia e a população de interesse separadamente. Resultados: doze estudos foram incluídos com predominância de ensaios clínicos randomizados e estudos quase experimentais. Destacou-se aromaterapia inalatória com o óleo essencial de Lavandula angustifolia. A utilização de grupo-controle e múltiplas medições dos desfechos foram os critérios que apresentaram maior fragilidade entre os estudos analisados. Considerações finais: a aromaterapia demonstrou efetividade e segurança na redução de sintomas de estresse para a equipe de Enfermagem.


Objetivo: analizar las evidencias científicas en relación con utilizar Aromaterapia para reducir síntomas relacionados con el estrés en equipos de Enfermería. Método: revisión integradora de la literatura realizada el 1 de julio de 2022 a partir de los siguientes descriptores: "Aromatherapy", "Stress" y "Nursing", y empleando la plataforma Rayyan para seleccionar los estudios a cargo de dos investigadores independientes. Se incluyeron documentos con acceso digital a sus textos completos y gratuitos en portugués, inglés o español, y que analizaran la Aromaterapia y la populación de interés en forma separada. Resultados: se incluyeron doce estudios, con predominio de ensayos clínicos aleatorizados y estudios cuasiexperimentales. Se destacó la Aromaterapia por inhalación con aceite esencial de Lavandula angustifolia. Emplear un Grupo Controle y de múltiples mediciones de los desenlaces fueron los criterios que presentaron mayor fragilidad entre los estudios analizados. Consideraciones finales: la Aromaterapia demostró ser efectiva y segura para reducir síntomas de estrés en equipos de Enfermería.


Objective: to analyze the diverse scientific evidence regarding the use of aromatherapy in reducing stress-related symptoms in the Nursing team. Method: an integrative literature review carried out on July 1st, 2022, based on the "Aromatherapy", "Stress" and "Nursing" descriptors and using the Rayyan platform to select the studies, all in charge of two independent researchers. Full-text and free digital access documents in Portuguese, English or Spanish were included, which analyzed aromatherapy and the population of interest separately. Results: twelve studies were included, with predominance of randomized controlled trials and quasi-experimental studies. Inhalation aromatherapy with Lavandula angustifolia essential oil stood out. The use of a Control Group and multiple outcome measures were the criteria that presented the greatest weakness among the studies analyzed. Final considerations: Aromatherapy showed effectiveness and safety in reducing stress symptoms for the Nursing team.

Abstract Objective The present study aims to establish a risk profile for high-grade cervical lesions and cervical cancer (CIN2 + ) in women undergoing colposcopy at the Hospital do Câncer de Barretos, through the analysis of Human Papillomavirus (HPV) infection, cervical cytology, and patient's age. Methods Retrospective cross-sectional study based on a computerized database of women aged ≥ 18 years old who underwent colposcopy at the Prevention Department of the Hospital do Câncer de Barretos from 2017 to 2019. Results A total of 3,411 women were included, 58.0% were positive for high-risk-HPV test, with a higher prevalence of CIN2+ for HPV16 (30.3%) and other HPV (45.0%). Cytological findings that suggest invasive cervical cancer (squamous cells or adenocarcinoma), regardless of the status of HPV test, showed 100% diagnosis of CIN2 + , while atypias that suggest high-grade lesions, HSIL and ASC-H, positive for HPV test, showed in 86 and 55.2%, respectively, diagnosis of CIN2 + . ASC-H cytological results among women aged > 40 years old and negative HPV were mainly associated with benign findings. We observed that ≤ CIN1 has a higher prevalence among older women with negative HPV, while for high-grade lesions there is an increase among young women HPV16- and/or 18-positive. In cancer diagnosis, we observed a predominance of HPV 16/18 regardless of the age group. Conclusion The highest risks of precursor lesions and cervical cancer were found among women with positive HPV 16/18 tests and severe cytological atypia in population screening tests. In addition, cytological findings of ASC-H HPV negative in women > 40 years old usually represent benign findings in histological investigation.


Resumo Objetivo Estabelecer um perfil de risco de lesões intraepiteliais de alto grau e câncer do colo do útero (NIC2 + ) em mulheres submetidas a colposcopia considerando-se a infecção pelo papilomavírus humano (HPV), citologia cervical e idade. Métodos Estudo retrospectivo transversal em banco de dados informatizado de mulheres com idade ≥ 18 anos que realizaram colposcopia no departamento de Prevenção de Câncer no Hospital do Câncer de Barretos/SP no período de 2017 a 2019. Resultados Foram incluídas 3.411 mulheres, sendo 58,0% positivas para HPV de alto risco, e maior prevalência de NIC2+ para HPV16 (30,3%) e outros HPV (45,0%). Resultados citológicos sugestivos de lesões invasivas (epidermoide ou adenocarcinoma), independente do teste de HPV, apresentaram 100% de diagnóstico NIC2 + , enquanto atipias sugestivas de lesões de alto grau, HSIL e ASC-H, associados a HPV positivo, apresentaram 86 e 55,2%, respectivamente. Resultados citológicos de ASC-H entre mulheres > 40 anos e HPV negativo foram associados principalmente a achados benignos. Observamos que ≤ NIC1 apresenta uma maior prevalência entre mulheres mais velhas com HPV negativo, enquanto para lesões de alto grau, há um aumento entre mulheres mais jovens positivas para HPV16/18. Para diagnóstico de câncer, observamos que há um predomínio de HPV16/18 independente da faixa etária. Conclusão Foi identificado maior risco de lesões precursoras e câncer entre mulheres com HPV 16/18 positivo e atipias citológicas graves em testes de rastreio populacional. Além disso, resultados citológicos de ASC-H quando associados a HPV negativo com idade > 40 anos habitualmente representam achados benignos em investigação histológica.

Cervical cancer remains a leading cause of cancer death among women in low-and middle-income countries. Globally, cervical cancer prevention programs are hampered by a lack of resources, infrastructure, and personnel. We describe a multimodal mobile colposcope (MMC) designed to diagnose precancerous cervical lesions at the point-of-care without the need for biopsy. The MMC integrates two complementary imaging systems: 1) a commercially available colposcope and 2) a high speed, high-resolution, fiber-optic microendoscope (HRME). Combining these two image modalities allows, for the first time, the ability to locate suspicious cervical lesions using widefield imaging and then to obtain co-registered high-resolution images across an entire lesion. The MMC overcomes limitations of high-resolution imaging alone; widefield imaging can be used to guide the placement of the high-resolution imaging probe at clinically suspicious regions and co-registered, mosaicked high-resolution images effectively increase the field of view of high-resolution imaging. Representative data collected from patients referred for colposcopy at Barretos Cancer Hospital in Brazil, including 22,800 high resolution images and 9,900 colposcope images, illustrate the ability of the MMC to identify abnormal cervical regions, image suspicious areas with subcellular resolution, and distinguish between high-grade and low-grade dysplasia.

BACKGROUND AND AIMS: To evaluate the effect of lifestyle modification by adopting a DASH diet, with and without physical activity guidance, on blood pressure, glycemic control, lipid profile, weight, and body composition in older patients with type 2 diabetes mellitus (T2DM) and hypertension. METHODS AND RESULTS: For this randomized clinical trial, we recruited patients aged 60 years or older with T2DM and uncontrolled hypertension. One group (DASH) received only DASH dietary guidance, while the other group (DASHPED) received dietary guidance and encouragement to walk with a pedometer. Outcomes of interest were (1) blood pressure, (2) physical activity, (3) weight, body mass index (BMI), and body composition, and (4) biochemical variables. Measurements were taken at baseline and 16 weeks after the intervention. We included 35 patients in the analysis. At the end of the study, the DASHPED group had an mean increase in physical activity of 1721 steps/day. Both groups displayed significantly reduced weight, BMI, and waking diastolic pressures on ambulatory blood pressure monitoring after the intervention. A trend of reduced sleeping diastolic pressure was found in the DASHPED group. Changes in weight, BMI, muscle mass, body fat, waist-hip ratio, glycemic control, lipid profile, and insulin sensitivity did not differ between the groups. CONCLUSION: There was no difference in outcomes between the group that only dieted and the group that also performed increased physical activity, despite a significant increase in exercise. This reinforces the importance of dietary changes in immediate blood pressure control.

A fisioterapia obstétrica surge com a necessidade de atender a gestante em suas variadas semanas de gestação, no trabalho de parto e no puerpério. Tem como propósito promover a prevenção de complicações, desconfortos e disfunções musculoesqueléticas e uroginecológicas, além do alívio das dores, orientação postural e percepção corporal. O objetivo deste artigo é traçar o nível de conhecimento das gestantes atendidas nas unidades básicas de saúde (UBS) do município de Teresópolis (RJ), na tentativa de verificar a existência e operacionalização dos mecanismos de acesso à informação dessas usuárias quanto à fisioterapia obstétrica. Trata-se de uma pesquisa transversal, de caráter qualiquantitativo, baseada na aplicação de um questionário sobre o tema. O questionário buscou avaliar o grau de conhecimento das gestantes a respeito da atuação da fisioterapia no período gestacional, parto e pós-parto. Ao analisar os dados coletados, alcançamos os seguintes resultados: 27 gestantes responderam ao questionário. Destas, 74,1% não conheciam a atuação fisioterapêutica na disfunção musculoesquelética durante a gestação, 51,9% não sabiam que o fisioterapeuta atua durante o trabalho de parto e 77,8% não conheciam a atuação fisioterapêutica no pós-parto. Os resultados sugerem que as gestantes atendidas na Atenção Básica do município de Teresópolis têm pouco conhecimento sobre a fisioterapia obstétrica e sua importância durante a fase gestacional, parto e pós-parto.


Obstetric physiotherapy emerged to care for pregnant women throughout gestation, labor, and puerperium. It aims to prevent complications, discomforts, and musculoskeletal and urogynecological disfunctions, as well as pain relief, postural orientation, and body perception. Hence, this study sough to outline the level of knowledge of pregnant women attended in Basic Health Units in Teresópolis, Rio de Janeiro, Brazil, to verify the existence and operationalization of mechanisms of access to information for these users regarding obstetric physiotherapy. This is a cross-sectional, qualitative and quantitative research. Data were collected by means of a questionnaire, which sought to assess the degree of knowledge of pregnant women about the role of physical therapy during pregnancy, childbirth and postpartum. A total of 27 pregnant women answered the questionnaire, showing that 74.1% ignored the physiotherapeutic role in musculoskeletal dysfunction during pregnancy, 51.9% did not know that physiotherapists are involved in labor, and 77.8% did not know about the physiotherapeutic role in postpartum. The results suggest that pregnant women assisted in primary care in the municipality of Teresópolis have little knowledge regarding obstetric physiotherapy and its importance during pregnancy, childbirth and postpartum.


La fisioterapia obstétrica surge con la necesidad de atender a la mujer embarazada en su gestación, en el trabajo de parto y en el puerperio. Su objetivo es promover la prevención de complicaciones, molestias y trastornos musculoesqueléticos y uroginecológicos, además del alivio del dolor, la orientación postural y la percepción corporal. El objetivo de este artículo es rastrear el nivel de conocimiento de las embarazadas atendidas en las Unidades Básicas de Salud del municipio de Teresópolis, en Rio de Janeiro (Brasil), para verificar la existencia y operacionalización de mecanismos de acceso a la información de esas usuarias sobre fisioterapia obstétrica. Se caracteriza por ser una investigación transversal, de carácter cualicuantitativo, a partir de la aplicación de un cuestionario sobre el tema. El cuestionario buscó evaluar el grado de conocimiento de las mujeres embarazadas sobre la realización de la fisioterapia en el período gestacional, parto y puerperio. Al analizar los datos recolectados, se llegó a los siguientes resultados: 27 embarazadas respondieron al cuestionario. De las cuales el 74,1% desconocía el papel de la fisioterapia en el trastorno musculoesquelético durante el embarazo, el 51,9% desconocía que el fisioterapeuta trabaja durante el parto y el 77,8% no conocía el papel de la fisioterapia posparto. Los resultados muestran que las mujeres embarazadas atendidas en la atención primaria de la ciudad de Teresópolis tienen poco conocimiento sobre la fisioterapia obstétrica y su importancia durante la fase de gestación, parto y puerperio.

Decision-making in clinical assessment, such as exit-level medical school Objective Structured Clinical Examinations (OSCEs), is complex. This study utilized an empirical phenomenological qualitative approach with thematic analysis to explore OSCE assessors' perceptions of the concept of a "prototypical intern" expressed during focus group discussions. Topics discussed included the concept of a prototypical intern, qualities to be assessed, and approaches to clinical assessment decision-making. The thematic analysis was then applied to a theoretical framework (Cultural Historical Activity Theory-CHAT) that explored the complexity of making assessment decisions amidst potentially contradicting pressures from academic and clinical perspectives. Ten Australasian medical schools were involved with 15 experienced and five less experienced assessors participating. Thematic analysis of the data revealed four major themes in relation to how the prototypical intern concept influences clinical assessors' judgements: (a) Suitability of marking rubric based on assessor characteristics and expectations; (b) Competence as final year student vs. performance as a prototypical intern; (c) Safety, trustworthiness and reliability as constructs requiring assessment and (d) Contradictions in decision making process due to assessor differences. These themes mapped well within the interaction between two proposed activity systems in the CHAT model: academic and clinical. More clinically engaged and more experienced assessors tend to fall back on a heuristic, mental construct of a "prototypical intern," to calibrate judgements, particularly, in difficult situations. Further research is needed to explore whether consensus on desirable intern qualities and their inclusion into OSCE marksheets decreases the cognitive load and increases the validity of assessor decision making.

OBJECTIVE: to analyze the effects of Cognitively Based Compassion Training (CBCT®) among people in situations of social vulnerability. METHOD: a mixed, sequential and transformative study with the same QUAL→QUAN weight. Focus Groups were applied at the beginning (n=24) and three months (n=11) after CBCT®, to understand the participants' knowledge about emotions, (self)care and stress situations. Content analysis was performed in the WebQDA software. The participants (n=65) were randomized into control (n=31) and intervention (n=34) to assess self-compassion, perceived stress, and positive and negative affects at three time moments. The mixed factorial ANOVA analysis considered within-participants (time) and between-participants (place and group) factors. RESULTS: mean age (37), female gender (88%), single (51%) and black-skinned people (77%). The following thematic categories emerged before the course: "Reducing others' suffering as a bridge to conscious self-care" and "Social vulnerability as a potentiator of low emotional literacy". Subsequently, self-compassion and awareness of the mental states for social activism. The quantitative analysis showed a significant increase in self-compassion within-participants (p=0.003); group factor (p<0.001); perceived stress reduction (p=0.013); negative affects group factor (p=0.005); and increase in positive affects (p<0.001) within-participants. CONCLUSION: CBCT®ï¸ exerted a positive effect on individual well-being and a positive impact on community engagement to promote social well-being in the outskirts. Abstract Background This study aims to validate the single-platform method for enumeration of CD34+ cells, by comparing the performance of two different commercial kits, as well as to evaluate the efficiency of the AccuriTM C6 cytometer in providing direct counts of absolute cell numbers. Method We evaluated 20 samples from umbilical cord blood (UCB), comparing the two different methodologies for enumeration of CD34+ cells: single and dual-platform. For the assessment of the single-platform, Procount and SCE kits were used, both of which use fluorescent beads as a counting reference to obtain absolute CD34+ cells numbers. Moreover, after the acquisition of samples in flow cytometer AccuriTM C6, following the protocol established for each kit, the number of CD34+ cells was recalculated, considering the cell count provided by the AccuriTM C6. Main Results In our analysis, the results showed a strong correlation between the number of CD34+ cells/μL (r2 = 0.77) when comparing the SCE kit and the current dual-platform method. On the other hand, the comparison between Procount kit and dual-platform results showed a moderate correlation for the number of CD34+/μL cells (r2 = 0.64). Conclusion Our results showed that the AccuriTM C6 flow cytometer can be used safely, applying both the dual and single platform analysis strategy. Considering the ISHAGE protocol-based single-platform approach, as the most appropriate methodology for CD34+ cells enumeration, our results demonstrated that the SCE kit has great potential for national standardization of UCB samples analysis methodology.

BACKGROUND: With an aging population in Ontario, ophthalmologists provide most of their care to older adults, which has prominent human resource implications. In this study, we sought to investigate the supply and demographic characteristics of Ontario's ophthalmologists. METHODS: In this retrospective, population-based analysis, we evaluated cohort demographics, including sex and career stage, of Ontario's ophthalmologists from 2010 to 2019, which we reported using descriptive statistics. Similarly, we detailed ophthalmologist supply within different areas of care using descriptive statistics. RESULTS: Over the study period, a median of 464 ophthalmologists were practising in Ontario each year. The proportion of female ophthalmologists increased from 18.7% in 2010 to 24.1% in 2019. The proportion of late-career ophthalmologists (aged > 55 yr) significantly increased by 6.4% over the study period and constituted 45.3% of the workforce in 2019. Comprehensive cataract surgery was the most common area of care. Although the number of ophthalmologists per 100 000 people remained stable over the study period (3.27 ophthalmologists/100 000 people in 2019), the number of ophthalmologists per 100 000 people aged 65 years and older fell by 18.4% from 2010 to 2019. The greatest supply reduction was among moderate-volume comprehensive cataract surgeons (-20.2% overall and -35.4% relative to the population aged ≥ 65 yr). INTERPRETATION: Between 2010 and 2019, the overall number of ophthalmologists in Ontario remained stable; however, we observed declines in the number of ophthalmologists per 100 000 people aged 65 years and older for most areas of care. Nearly half of the ophthalmology workforce is now older than 55 years and female representation is increasing.

Resumo Objetivo Comparar a aptidão funcional e as dimensões da qualidade de vida de idosos participantes e não participantes das práticas orientadas Lian Gong, Tai Chi e Qigong. Métodos Estudo caso-controle, desenvolvido no município de São Paulo - SP, Brasil, com 118 idosos, pareados por sexo e idade: 59 no grupo caso, participantes das práticas corporais, divididos em subgrupos por tempo de adesão:< 24 meses e ≥ 24 meses; e 59 no grupo controle, não participantes das práticas. Para a coleta de dados, aplicaram-se: questionário com variáveis sociodemográficas e de saúde, testes funcionais e o instrumento Medical Outcomes Study 36- Item Short-Form Health Survey (SF36). As análises estatísticas foram realizadas com o programa R versão 3.3.2. Os dados foram inicialmente analisados de forma descritiva e, em seguida, efetuou-se regressão logística univariada e o teste Kruskal Wallis. Os aspectos éticos da pesquisa com seres humanos foram obedecidos. Resultados Verificou-se que, no grupo caso, idosos com tempo de adesão às práticas ≥ 24 meses apresentaram resultado superior no teste funcional de levantar e sentar da cadeira (p=0,006), bem como melhor desempenho nos seguintes domínios da qualidade de vida: dor (p= 0,003), vitalidade (p=0,021), aspectos emocionais (p=0,034) e saúde mental (p=0,020). Conclusão A participação nas práticas corporais, orientadas, Lian Gong, Tai Chi e Qigong pode contribuir para a qualidade de vida e a aptidão funcional de idosos.

Para la recopilación de datos, se aplicó un cuestionario con variables sociodemográficas y de salud, pruebas funcionales y el instrumento Medical Outcomes Study 36- Item Short-Form Health Survey (SF36). Los análisis estadísticos se realizaron con el programa R versión 3.3.2. Los datos se analizaron inicialmente de forma descriptiva y, a continuación, se realizó regresión logística univariada y la prueba de Kruskal Wallis. Los aspectos éticos de la investigación con seres humanos fueron cumplidos. Resultados Se verificó que, en el grupo caso, los adultos mayores con tiempo de participación en las prácticas ≥ 24 meses presentaron un resultado superior a la prueba funcional de levantarse y sentarse en la silla (p=0,006), como también un mejor desempeño en los siguientes dominios de calidad de vida: dolor (p= 0,003), vitalidad (p=0,021), aspectos emocionales (p=0,034) y salud mental (p=0,020). Conclusión La participación en las prácticas corporales orientadas Lian Gong, Tai Chi y Qigong puede contribuir para la calidad de vida y para la aptitud funcional de adultos mayores.


Abstract Objective To compare functional fitness and quality of life dimensions of elderly participants and non-participants of Lian Gong, Tai Chi and Qigong guided practices. The data were initially analyzed in a descriptive way and, then, a univariate logistic regression and the Kruskal Wallis test were performed. The ethical aspects of research with human beings were obeyed. Results It was found that, in the case group, elderly people with a time of compliance with practices ≥ 24 months showed a superior result in the functional 30-second chair stand test (p=0.006), as well as better performance in the following quality of life domains: bodily pain (p=0.003); vitality (p=0.021); role emotional (p=0.034); and mental health (p=0.020). Conclusion Participation in guided body practices, Lian Gong, Tai Chi and Qigong, can contribute to elderly people's quality of life and functional fitness.
