RESUMO

Astroglia play crucial neuroprotective roles by internalizing pathogenic aggregates and facilitating their degradation. Here, we show that α-SYN protofibril-induced organelle toxicities and reactive oxygen species (ROS) cause premature cellular senescence in astrocytes and astrocyte-derived cancer cells, resulting in a transient increase in the biogenesis of tunneling nanotubes (TNTs). TNT-biogenesis and TNT-mediated cell-to-cell transfer lead to clearance of α-SYN-induced organelle toxicities, reduction in cellular ROS levels, and reversal of cellular senescence. Enhanced cell proliferation is seen in the post-recovered cells after recovering from α-SYN-induced organelle toxicities. Further, we show that α-SYN-induced senescence promotes the transient localization of focal adhesion kinase (FAK) in the nucleus. FAK-mediated regulation of Rho-associated kinases plays a significant role in the biogenesis of TNTs and their subsequent proliferation. Our study emphasizes that TNT biogenesis has a potential role in the clearance of α-SYN-induced cellular toxicities, the consequences of which cause enhanced proliferation in the post-recovered astroglia cells.

Background: Proteins are essential for the maintenance, repair, and growth of muscle mass. This is particularly important for master athletes because aging has been associated with loss of muscle mass, function, and strength. Moreover, the timing of intake has been shown important for the best protein utilization. Aim: To analyze timing, quantity, and source of dietary protein in competitive master athletes according to current recommendations. Methods: Twenty-one male master swimmers (47.9 ± 10.0 years; 79.2 ± 6.5 kg; 179.1 ± 5.5 cm; 23.5 ± 4.9% body fat; 73.3 ± 4.2% lean mass) participated in this cross-sectional study. Protein intake was analyzed based on 7-day food records, regarding quantity, timing, and sources of intake. Protein intake was evaluated according to current international sports nutrition guidelines, including the International Society of Sports Nutrition Position Stand. Body fat (%) and lean mass (%) were evaluated using dual-energy X-ray absorptiometry. Results: Participants' mean protein intakes were 1.9 ± 0.5 g/kg/day, 0.6 ± 0.2 g/kg/meal post-training, and 33.5 ± 23.9 g during the pre-sleep period. Daily intake was within the recommended values of 1.4 and 2.0 g/kg/day (p = 0.01 and 0.147, respectively). Mean pre-sleep intake was within the recommendation values of 30-40 g (p = 0.28 and 0.147, respectively). Most of the daily protein intake was consumed at lunch (66.7 ± 6.9 g) and dinner (48.0 ± 4.5 g). Regarding protein sources, intakes from animal, vegetal, and supplements were, respectively, 65.7%, 29.2%, and 5.1%. Conclusion: Master swimmers presented a total protein intake within the recommendations for a daily basis, but the majority of intake was at lunch and dinner. Protein intake could be better distributed throughout the day to optimize protein synthesis. Guidance on daily protein intake distribution should be reinforced in clinical practice.

BACKGROUND: HIV Pre-Exposure Pophylaxis (PrEP) is provided free of charge by the Brazilian national health system. Though effective in preventing HIV infection, little is known about its impact on the health-related Quality of Life (QoL) of users. OBJECTIVE: The present study aimed at assessing the impact of PrEP on the QoL of its users. METHODS: Prospective cohort study with 114 HIV-negative participants aged 18 years or older. Participants' QoL was assessed before starting PrEP and after 7 months of use, using the self-responsive WHOQOL-bref questionnaire. Sociodemographic and behavioral aspects were described and the Wilcoxon signed-rank test with p ≤ 0.05 was considered statistically significant. RESULTS: Improvement was seen in QoL scores for the environment domain (p = 0.02), which addresses feeling of physical safety, access to information and health services, and participation in leisure activities. Furthermore, participants reported improved satisfaction with their sex life, when questioned about the social relationships domain. There was no statistically significant change in the global QoL score, in the global health score, in the physical and psychological domains, nor in the total score for the social relationships domain. As for their socio-demographic profile, most participants were white and highly educated young cisgender men who have sex with men. 76.3% had unprotected sex in the 3 months before starting PrEP. 60.5% had reported substance use: marijuana (42.1%), club drugs (35.1%), and poppers (20.2%). CONCLUSIONS: This study unveiled that PrEP benefited our cohort beyond its effectiveness in preventing HIV infection, having improved environmental aspects of QoL and self-satisfaction with sex life.

OBJECTIVE: To identify and analyze the features and quality of self-management support of mobile applications available in Brazil for chronic low back pain in adults. METHOD: A systematic review on the Apple Store® and Google Play® digital platforms. The Self-Management Support Assessment Tool scale was used to assess self-management support and the Institute for Healthcare Informatics Functionality Score scale was used to assess functionality. RESULTS: Seventeen applications were selected, which included around seven self-management skills. The applications that met the majority of self-management support skills were Pathways, Branch, Pancea, Pain Navigator, and Curable. The Curable, Branch and MoovButh applications had the highest scores, with ten features on the functionality scale. CONCLUSION: Some applications have the potential to complement in-person treatment in terms of validity, acceptability and clinical usefulness in pain management. However, barriers such as lack of partnership between healthcare providers and patients, limited evidence-based content, social support, cultural relevance, cost, language, security and privacy can limit their sustained use. Global biodiversity gradients are generally expected to reflect greater species replacement closer to the equator. However, empirical validation of global biodiversity gradients largely relies on vertebrates, plants, and other less diverse taxa. Here we assess the temporal and spatial dynamics of global arthropod biodiversity dynamics using a beta-diversity framework. Sampling includes 129 sampling sites whereby malaise traps are deployed to monitor temporal changes in arthropod communities. Overall, we encountered more than 150,000 unique barcode index numbers (BINs) (i.e. species proxies). We assess between site differences in community diversity using beta-diversity and the partitioned components of species replacement and richness difference. Global total beta-diversity (dissimilarity) increases with decreasing latitude, greater spatial distance and greater temporal distance. Species replacement and richness difference patterns vary across biogeographic regions. Our findings support long-standing, general expectations of global biodiversity patterns. However, we also show that the underlying processes driving patterns may be regionally linked.

RESUMO

Oral mucosal and other head and neck conditions in children have a variety of presentations. The joint oral medicine and paediatric (JOMP) dental clinic is a specialised unit within a London teaching hospital, developed to manage a wide range of oral conditions with an absolute commitment to a child-centred care approach. The authors present eight cases from the JOMP clinic experience at Guy's and St Thomas' NHS Foundation trust, over a nine-year period. Each case is unique in its presentation, diagnosis and bespoke management, tailored to the nuance of each individual patient and their unique position. The eight clinical cases demonstrate the success of the JOMP team in achieving good patient outcomes, in terms of providing accurate diagnoses for their oral conditions and for appropriately tailored management/ treatment. The cases also serve to raise awareness of some of the more unusual oral conditions affecting paediatric patients among our professional colleagues.

Se utilizó la escala Self-Management Support Assessment Tool para evaluar el apoyo a la autogestión y la escala Institute for Healthcare Informatics Functionality Score para evaluar la funcionalidad. Resultados: Se seleccionaron 17 aplicaciones, que incluían alrededor de siete habilidades de autogestión. Las aplicaciones que cumplieron con la mayoría de las habilidades de apoyo a la autogestión fueron Pathways, Branch, Pancea, Pain Navigator y Curable. Las aplicaciones Curable, Branch y MoovButh obtuvieron las puntuaciones más altas, con diez características en la escala de funcionalidad. Conclusión: Algunas aplicaciones tienen potencial para complementar el tratamiento presencial en términos de validez, aceptabilidad y utilidad clínica en el manejo del dolor. Background: The diagnosis and treatment of breast cancer can significantly affect a woman's health and well-being. Several studies have identified factors contributing to disparities in breast reconstruction among ethnic groups, but few have examined the experience of Hispanic women. Methods: Hispanic women who had undergone breast reconstruction were interviewed in retrospective focus groups. Data were collected to identify themes that affected decision-making regarding the type and timing of reconstruction. Results: Most participants chose to undergo reconstruction to regain normalcy and maintain their previous "feminine" appearance. Most (75%) received silicone breast implants, and 25% received autologous reconstruction. Safety was the most important consideration when choosing the type of reconstruction. All but 1 participant relied on a spouse or other family member for support during recovery. Although most found the experience overwhelming, they felt their community was supportive and encouraging throughout the process. Conclusions: Several themes underlying the decision to undergo reconstruction were identified, including maintaining femininity and choosing the safest procedure. Overall, participants described the experience as overwhelming but found support from community and family. These findings, which could be applied broadly to women regardless of ethnicity, can be used to improve communication between surgeon and patient throughout the reconstruction process.

This article examines the United Kingdom Supreme Court decision in Whittington Hospital NHS Trust v XX [2020] UKSC 14. The case centred on whether damages could be awarded for the cost of a commercial surrogacy arrangement in California, following clinical negligence by the hospital that left the plaintiff unable to carry her own children. After examination of this case, the article outlines and compares the United Kingdom and Australian surrogacy laws. It then discusses how a similar case would be decided in Australia and argues that the result would be the same in some Australian States. It also discusses the concept of reproductive autonomy and the importance of this concept when considering cases involving the loss of fertility.

Resumo Objetivo Avaliar os efeitos da acupuntura no alívio da dor osteomuscular em pessoas idosas e a possibilidade de melhora da funcionalidade para as atividades básicas de vida diária. Método Estudo quase experimental, com avaliação antes e depois da intervenção utilizando a Escala Visual Analógica e o Índice de Katz. Amostra por conveniência, constituída de 31 pessoas idosas. O período de intervenção comportou quatro sessões de atendimentos semanais. A intervenção com acupuntura abordou um protocolo de tratamento sistêmico por intermédio de cinco pares de pontos: IG4 (Hegu), F3 (Taichong), E36 (Zusanli), PC6 (Neiguan) e BP6 (Sanyinjiao). Resultados 100% (31) informando sentir dor, sendo 64,52% (20) em intensidade moderada (3-7); 25,81% (8) dor intensa (8-10); e 9,68% (3) dor leve (0-2). O protocolo de tratamento mostrou-se positivo em 4 sessões, com redução da dor ao grau leve, e média geral reduzida abaixo de 2 no score. A análise dos resultados da Escala Visual Analógica e os do índice de Katz, mostrou possibilidades do uso da acupuntura no alívio da dor osteomuscular e na melhora da funcionalidade da pessoa idosa. Conclusão O uso da técnica de acupuntura sistêmica apontou resultados satisfatórios no alívio da dor osteomuscular e na melhoria da funcionalidade para as atividades básicas de vida diária da pessoa idosa.


Abstract Objective To assess the effects of acupuncture on the relief of osteomuscular pain in older adults and the potential improvement in functionality for activities of daily living. Method A quasi-experimental study, with pre- and post-intervention assessment using the Visual Analog Scale and the Katz Index. Convenience sample comprising 31 older adults. The intervention period consisted of four weekly treatment sessions. Acupuncture intervention targeted a systemic treatment protocol utilizing five pairs of acupoints: LI4 (Hegu), LIV3 (Taichong), ST36 (Zusanli), PC6 (Neiguan), and SP6 (Sanyinjiao). Results 100% (31) reported experiencing pain, with 64.52% (20) reporting moderate pain (3-7), 25.81% (8) reporting severe pain (8-10), and 9.68% (3) reporting mild pain (0-2). The treatment protocol demonstrated positive outcomes after 4 sessions, with a reduction of pain to a mild degree and an overall average score reduced below 2. The analysis of results from the Visual Analog Scale and the Katz Index showed potential for the use of acupuncture in relieving osteomuscular pain and improving the functionality of the older adult. Conclusion The use of systemic acupuncture techniques yielded satisfactory results in the relief of osteomuscular pain and the enhancement of functionality for activities of daily living of the older adult.

The Fragile-X Mental Retardation Protein (FMRP) is an RNA binding protein that regulates translation of mRNAs essential for synaptic development and plasticity. FMRP interacts with a specific set of mRNAs, aids in their microtubule-dependent transport and regulates their translation through its association with ribosomes. However, the biochemical role of FMRP's domains in forming neuronal granules and associating with microtubules and ribosomes is currently undefined. We report that the C-terminus domain of FMRP is sufficient to bind to ribosomes akin to the full-length protein. Furthermore, the C-terminus domain alone is essential and responsible for FMRP-mediated neuronal translation repression. However, dendritic distribution of FMRP and its microtubule association is favored by the synergistic combination of FMRP domains rather than individual domains. Interestingly, we show that the phosphorylation of hFMRP at Serine-500 is important in modulating the dynamics of translation by controlling ribosome association. This is a fundamental mechanism governing the size and number of FMRP puncta that contain actively translating ribosomes. Finally through the use of pathogenic mutations, we emphasize the hierarchical contribution of FMRP's domains in translation regulation.

Intellectual disability (ID) and autism spectrum disorder (ASD) are neurodevelopmental disorders that have become a primary clinical and social concern, with a prevalence of 2-3% in the population. Neuronal function and behaviour undergo significant malleability during the critical period of development that is found to be impaired in ID/ASD. Human genome sequencing studies have revealed many genetic variations associated with ASD/ID that are further verified by many approaches, including many mouse and other models. These models have facilitated the identification of fundamental mechanisms underlying the pathogenesis of ASD/ID, and several studies have proposed converging molecular pathways in ASD/ID. However, linking the mechanisms of the pathogenic genes and their molecular characteristics that lead to ID/ASD has progressed slowly, hampering the development of potential therapeutic strategies. This review discusses the possibility of recognising the common molecular causes for most ASD/ID based on studies from the available models that may enable a better therapeutic strategy to treat ID/ASD. We also reviewed the potential biomarkers to detect ASD/ID at early stages that may aid in diagnosis and initiating medical treatment, the concerns with drug failure in clinical trials, and developing therapeutic strategies that can be applied beyond a particular mutation associated with ASD/ID.

Although Pakistan has rich biodiversity, many groups are poorly known, particularly insects. To address this gap, we employed DNA barcoding to survey its insect diversity. Specimens obtained through diverse collecting methods at 1,858 sites across Pakistan from 2010-2019 were examined for sequence variation in the 658 bp barcode region of the cytochrome c oxidase 1 (COI) gene. Sequences from nearly 49,000 specimens were assigned to 6,590 Barcode Index Numbers (BINs), a proxy for species, and most (88%) also possessed a representative image on the Barcode of Life Data System (BOLD). By coupling morphological inspections with barcode matches on BOLD, every BIN was assigned to an order (19) and most (99.8%) were placed to a family (362). However, just 40% of the BINs were assigned to a genus (1,375) and 21% to a species (1,364). Five orders (Coleoptera, Diptera, Hemiptera, Hymenoptera, Lepidoptera) accounted for 92% of the specimens and BINs. More than half of the BINs (59%) are so far only known from Pakistan, but others have also been reported from Bangladesh (13%), India (12%), and China (8%). Representing the first DNA barcode survey of the insect fauna in any South Asian country, this study provides the foundation for a complete inventory of the insect fauna in Pakistan while also contributing to the global DNA barcode reference library.

BACKGROUND: Traditionally, the provision of comfort care and support during the dying process for infants born with life-limiting diagnoses has occurred in the neonatal intensive care unit (NICU). A major goal for the families of these infants is often the opportunity to spend as much time as possible with their infant in order to make memories and parent their infant. PURPOSE: The objective of the Mother Baby Comfort Care Pathway is to implement a program of family-centered care with logistically flexible care delivery, allowing mothers and their families to share as normal a postpartum care experience as possible with a focus on quality of life, memory making, and time spent together. METHODS: The program was developed with the nucleus of care coordination and provision on the Mother Baby Unit (postpartum unit), with involvement from the labor and delivery room, NICU, and other units as necessary to provide the postpartum mother, her dying infant, as well as possible additional siblings (in the case of multiple gestation), postpartum care while rooming-in. The program was rolled out with training workshops for postpartum nurses. FINDINGS: Nurses who took part in the workshops and the patient care program rated both highly. IMPLICATIONS FOR PRACTICE: The Mother Baby Comfort Care Pathway aims to offer a framework for providing multidisciplinary family-centered comfort care to newborns during the postpartum period in a compassionate, evidence-based, and individualized manner in order to maximize quality time together for families with a dying infant.

OBJECTIVES: This study was designed to investigate whether coronary computed tomography angiography assessments of coronary plaque might explain differences in the prognosis of men and women presenting with chest pain. BACKGROUND: Important sex differences exist in coronary artery disease. Women presenting with chest pain have different risk factors, symptoms, prevalence of coronary artery disease and prognosis compared to men. METHODS: Within a multicenter randomized controlled trial, we explored sex differences in stenosis, adverse plaque characteristics (positive remodeling, low-attenuation plaque, spotty calcification, or napkin ring sign) and quantitative assessment of total, calcified, noncalcified and low-attenuation plaque burden. RESULTS: Of the 1,769 participants who underwent coronary computed tomography angiography, 772 (43%) were female. Women were more likely to have normal coronary arteries and less likely to have adverse plaque characteristics (p < 0.001 for all). They had lower total, calcified, noncalcified, and low-attenuation plaque burdens (p < 0.001 for all) and were less likely to have a low-attenuation plaque burden >4% (41% vs. 59%; p < 0.001). Over a median follow-up of 4.7 years, myocardial infarction (MI) occurred in 11 women (1.4%) and 30 men (3%). In those who had MI, women had similar total, noncalcified, and low-attenuation plaque burdens as men, but men had higher calcified plaque burden. Low-attenuation plaque burden predicted MI (hazard ratio: 1.60; 95% confidence interval: 1.10 to 2.34; p = 0.015), independent of calcium score, obstructive disease, cardiovascular risk score, and sex. CONCLUSIONS: Women presenting with stable chest pain have less atherosclerotic plaque of all subtypes compared to men and a lower risk of subsequent MI. However, quantitative low-attenuation plaque is as strong a predictor of subsequent MI in women as in men. (Scottish Computed Tomography of the HEART Trial [SCOT-HEART]; NCT01149590).

Twenty-nine species are treated, most of which have host caterpillar and food plant records, and all but one are new to science. The first host record for the agathidine genus Amputoearinus is given. Gnathopleurajosequesadai Sharkey, sp. nov. is reported as a hyperparasitoid of fly larvae, the first such record for the genus. Resumo Introdução Este trabalho discute a possibilidade de utilização de informações sobre a vacinação infantil reportada pelas mães em inquéritos domiciliares para a construção de indicadores de cobertura vacinal no Brasil. Objetivo Avaliar o potencial das informações declaradas pelas mães sobre a imunização das crianças em inquéritos domiciliares como uma fonte para o cálculo do indicador de cobertura vacinal. Método Analisaram-se os indicadores de confiabilidade (precisão) e validade (concordância) entre as informações disponíveis nos cartões de vacina das crianças menores de 2 anos e as informações reportadas pelas mães em pesquisas domiciliares realizadas nos anos de 2013 e 2015 no município de Santo Antônio do Monte, Minas Gerais. Resultados O principal resultado mostra que, em um contexto de alta cobertura, as mães tendiam a informar sobre a vacinação de seus filhos de forma aleatória, ou seja, informações reportadas pelas mães apresentavam baixa validade. Conclusão Embora a coleta de informações dos cartões de vacina das crianças seja um procedimento mais custoso, essa é a forma mais adequada e confiável de se mensurar a cobertura vacinal no contexto brasileiro.


Abstract Background This study discusses the possibility of using information on child vaccination reported by mothers through household surveys to construct indicators of immunization coverage in Brazil. Objective Evaluate the potential of information reported by mothers on the immunization of children in household surveys as a source to calculate a vaccination coverage indicator. Method The indicators of reliability (accuracy) and validity (concordance) between the information available on the vaccination cards of children under two years of age and that reported by mothers in two household surveys conducted in 2013 and 2015 in the municipality of Santo Antônio do Monte, state of Minas Gerais, Brazil. Results The main result shows that, in a context of high coverage, the mothers tended to inform randomly about the vaccination of their children, that is, the information reported by mothers presented low validity. Conclusion Although collecting information from children's vaccination cards is a more costly procedure, it is the most adequate and reliable way to measure vaccination coverage in the Brazilian context.

Objetivo: identificar os comportamentos agressivos e as reações à agressão entre escolares e verificar se há associação com as variáveis idade, sexo, escolaridade, situação conjugal dos pais e ao tipo de instituição escolar. Método: estudo descritivo, transversal e quantitativo. População composta por 492 crianças, a qual gerou uma amostra de 26 crianças da escola privada e 86 da escola pública, que responderam ao Questionário de Comportamentos Agressivos e Reativos entre Pares. Os dados passaram pela análise descritiva e o teste exato de Fisher. Resultados: a maioria das crianças apresentou agressividade e reações agressivas abaixo da média. E no que se refere as reações a agressividade as mais frequentes foram as respostas internalizadas. Conclusão: os dados obtidos podem nortear intervenções de combate e prevenção da agressividade escolar. Porém, ficou evidente que será necessário desenvolver estudos com designs metodológicos mais robustos


Objective: to identify aggressive behaviors and reactions to aggression among schoolchildren and to verify if there is an association with the variables age, sex, schooling, parents' marital status and type of school institution. Method: descriptive, cross - sectional and quantitative study. A population composed of 492 children, which generated a sample of 26 children from the private school and 86 from the public school, who answered the Questionnaire on Aggressive and Reactive Behaviors among Peers. Data were subjected to descriptive analysis and Fisher's exact test. Results: most children presented aggression and aggressive reactions below average. And the most frequent reactions to aggression were the internalized responses. Conclusion: the data obtained can guide interventions to combat and prevent school aggression. However, it has become evident that it will be necessary to develop studies with more robust methodological designs

Método: estudio descriptivo, transversal y cuantitativo. Población compuesta por 492 niños, la cual generó una muestra de 26 niños de la escuela privada y 86 de la escuela pública, que respondieron al Cuestionario de Comportamientos Agresivos y Reactivos entre Pares. Los datos pasaron por el análisis descriptivo y la prueba exacta de Fisher. Resultados: la mayoría de los niños presentó agresividad y reacciones agresivas por debajo de la media. Y en lo que se refiere a las reacciones a agresividad las más frecuentes fueron las respuestas internalizadas. Conclusión: los datos obtenidos pueden orientar intervenciones de combate y prevención de la agresividad escolar. Sin embargo, resulta evidente que será necesario desarrollar estudios con diseños metodológicos más robustos
