The enteric parasite Cryptosporidium is spread through the fecal-oral pathway, most commonly by the consumption of contaminated water but also through food. Because eating raw or barely cooked shellfish might put consumers at risk for cryptosporidiosis, identifying the parasite in oysters is important for public health. A total of 240 oysters, collected from two shellfish aquaculture sites in Thailand's Gulf coast, Nakhon Si Thammarat and Surat Thani, were tested for the presence of Cryptosporidium. Escherichia coli, enterococci, and thermotolerant coliform total levels were measured to assess seawater quality in the shellfish production regions. Oocysts of Cryptosporidium spp. were detected in 13.8% of the samples processed by immunofluorescence analyses. The detection of Cryptosporidium spp. oocysts in oysters obtained from Surat Thani (17.5%) was higher than in those obtained from Nakhon Si Thammarat (9.2%). The difference in detection of positive samples obtained from Nakhon Si Thammarat and those obtained from Surat Thani may be attributed to the effects of physical, ecological, and anthropogenic conditions, resulting in an increased level of marine water contamination by Cryptosporidium spp. oocysts. These findings demonstrate that native commercial oysters obtained from Thailand's southern Gulf coast contained Cryptosporidium spp. oocysts which might serve as a source of human infection. Consequently, these findings pose a serious public health concern and suggest that more quality control measures need to be implemented by the oyster aquaculture business to ensure the safety of seafood.

Helicobacter pylori infects approximately 50% of the world's population and is considered the major etiological agent of severe gastric diseases, such as peptic ulcers and gastric carcinoma. Increasing resistance to standard antibiotics has now led to an ever-decreasing efficacy of eradication therapies and the development of novel and improved regimens for treatment is urgently required. Substantial progress has been made over the past few years in the identification of molecular mechanisms which are conducive to resistant phenotypes as well as for efficient strategies to counteract strain resistance and to avoid the use of ineffective antibiotics. These involve molecular testing methods, improved salvage therapies, and the discovery of novel and potent antimicrobial compounds. High rates of prevalence and gastric cancer are currently observed in Asian countries, including Japan, China, Korea, and Taiwan, where concomitantly intensive research efforts were initiated to explore advanced eradication regimens aimed at reducing the risk of gastric cancer. In this review, we present an overview of the known molecular mechanisms of antibiotic resistance and discuss recent intervention strategies for H. pylori diseases, with a view of the research progress in Asian countries.

Many of the medicinally active molecules in the flavonoid class of phytochemicals are being researched for their potential antiviral activity against various DNA and RNA viruses. Quercetin is a flavonoid that can be found in a variety of foods, including fruits and vegetables. It has been reported to be effective against a variety of viruses. This review, therefore, deciphered the mechanistic of how Quercetin works against some of the deadliest viruses, such as influenza A, Hepatitis C, Dengue type 2 and Ebola virus, which cause frequent outbreaks worldwide and result in significant morbidity and mortality in humans through epidemics or pandemics. All those have an alarming impact on both human health and the global and national economies. The review extended computing the Quercetin-contained natural recourse and its modes of action in different experimental approaches leading to antiviral actions. The gap in effective treatment emphasizes the necessity of a search for new effective antiviral compounds. Quercetin shows potential antiviral activity and inhibits it by targeting viral infections at multiple stages. The suppression of viral neuraminidase, proteases and DNA/RNA polymerases and the alteration of many viral proteins as well as their immunomodulation are the main molecular mechanisms of Quercetin's antiviral activities. Nonetheless, the huge potential of Quercetin and its extensive use is inadequately approached as a therapeutic for emerging and re-emerging viral infections. Therefore, this review enumerated the food-functioned Quercetin source, the modes of action of Quercetin for antiviral effects and made insights on the mechanism-based antiviral action of Quercetin.

This study aimed to develop simple diagnostic guidelines which would be useful for the early detection of severe dengue infections. Retrospective data of patients with dengue infection were reviewed. Patients with diagnosed dengue infection were categorized in line with the International Statistical Classification of Diseases (ICD-10): A90, dengue fever; A91, dengue hemorrhagic fever; and A910, dengue hemorrhagic fever with shock. A total of 302 dengue-infected patients were enrolled, of which 136 (45%) were male and 166 (55%) were female. Multivariate analysis was conducted to determine independent diagnostic predictors of severe dengue infection and to convert simple diagnostic guidelines into a scoring system for disease severity. Coefficients for significant predictors of disease severity generated by ordinal multivariable logistic regression analysis were transformed into item scores. The derived total scores ranged from 0 to 38.6. The cut-off score for predicting dengue severity was higher than 14, with an area under the receiver operating curve (AUROC) of 0.902. The predicted positive value (PPV) was 68.7% and the negative predictive value (NPV) was 94.1%. Our study demonstrates that several diagnostic parameters can be effectively combined into a simple score sheet with predictive value for the severity evaluation of dengue infection.

BACKGROUND: Tumour necrosis factor-alpha (TNF-α) levels are reportedly altered during malaria. In this systematic review and meta-analysis, we aimed to collect and compare data on TNF-α levels between patients with malaria of varying severity and healthy asymptomatic positive controls. METHODS: We searched PubMed, Scopus and Web of Science for studies that reported TNF-α levels in malaria cases of different severity and healthy asymptomatic positive controls using a combination of search terms. The quality of the included studies was assessed using the Strengthening the Reporting of Observational Studies in Epidemiology checklist. To compare the TNF-α levels among fatal cases, severe cases, uncomplicated cases and healthy asymptomatic positive controls, we applied the random-effects model that assumed the existence of variations between studies. The effect estimate was pooled mean difference (MD) with a 95% confidence interval (CI). RESULTS: From 1694 studies, we included 31 studies that met our eligibility criteria for systematic review and meta-analysis. Patients with severe malaria showed higher mean TNF-α levels than those with uncomplicated malaria (P < 0.001, pooled MD = 79.02 pg/ml, 95% CI: 63.68-94.35 pg/ml, I2: 99.5%, n = 26 studies). Furthermore, fatal cases had no difference in the mean TNF-α levels in comparison with survived cases (P = 0.055, pooled MD = 82.38 pg/ml, 95% CI: -1.93 to 166.69 pg/ml, I2: 99.54%, n = 5 studies). Finally, patients with uncomplicated malaria showed higher mean TNF-α levels than those with asymptomatic malaria (P < 0.001, pooled MD = 45.10 pg/ml, 95% CI: 18.45-71.76 pg/ml, I2: 97.09%, n = 5 studies). CONCLUSION: This systematic review and meta-analysis confirmed the increase of TNF-α levels in patients with severe malaria. Therefore, TNF-α may be alternatively used as a prognostic biomarker of severe malaria. TRIAL REGISTRATION: Not applicable.

Peganum harmala, a well-known medicinal plant, has been used for several therapeutic purposes as it contains numerous pharmacological active compounds. Our study reported an anti-parasitic activity of P. harmala seed extract against Acanthamoeba triangularis. The stress induced by the extract on the surviving trophozoites for Acanthamoeba encystation and vacuolization was examined by microscopy, and transcriptional expression of Acanthamoeba autophagy-related genes was investigated by quantitative PCR. Our results showed that the surviving trophozoites were not transformed into cysts, and the number of trophozoites with enlarged vacuoles were not significantly different from that of untreated control. Molecular analysis data demonstrated that the mRNA expression of tested AcATG genes, i.e., ATG3, ATG8b, and ATG16, was at a basal level along the treatment. However, upregulation of AcATG16 at 24 h post treatment was observed, which may indicate an autophagic activity of this protein in response to the stress. Altogether, these data revealed the anti-Acanthamoeba activity of P. harmala extract and indicated the association of autophagy mRNA expression and cyst formation under the extract stress, representing a promising plant for future drug development. However, further identification of an active compound and a study of autophagy at the protein level are needed.

BACKGROUND: Polycyclic aromatic hydrocarbons (PAHs) are environmental pollutants that cause cancers in humans. Exposure results from incomplete burning of organic materials. Grilled meat vendors are occupationally exposed to PAHs in smoke from continuously burning charcoal and meat, which may have chance to get cancer. Individual risk perception is a critical antecedent of preventive behavior However relatively little is known about the cancer risk perception and preventive behaviors among grilled meat vendors. OBJECTIVE: The present study aimed to examine cancer risk perception and preventive behaviors and factors associated with preventive behaviors among grilled meat vendors in Bangkok, Thailand. MATERIAL AND METHOD: A cross-sectional survey was used in the present study. The subjects consisted of 40 males and 82 females grilled meat vendors who conducted their jobs at Victory Monument, Bang Lumpoo, Pratunam, Jakkawat or Patumwan. They were interviewed regarding cancer risk perception and preventive behaviors. Multiple logistic regression analysis was used to analyze data. RESULTS: Average cancer risk perception score was 23.3 +/- 3.3 out of 32. About 79.5% of the subjects had high risk perception. The prevalence of good preventive behaviors was 86.1%. The three most frequent preventive behaviors among the subjects during grilling meat were using good quality charcoal (65.6%), ventilation fan (41.0%) and wearing long sleeves shirt (36.1%). Only 10.7% always used gloves. Multiple logistic regression analysis showed 2 factors were independently associated with preventive behaviors: educational level and risk perception. The subjects who had higher educational level or risk perception were more likely to develop better behaviors than those who had a lower educational level and perception (p < 0.05). CONCLUSION: In conclusion, the present study found that the educational background and cancer risk perception were associated with preventive behaviors among grilled meat vendors. To improve the preventive behaviors among grilled meat vendors, interventions focusing on cancer risk perceptions and considering educational background among target group are recommended.

The aim of this study was to investigate water samples collected in coastal areas of Southern Thailand in the years of 2005 and 2008 for their contamination by the protozoan parasites Cryptosporidium and Giardia. One hundred eighteen water samples of different origin were collected from six Tsunami affected southern provinces of Thailand in early 2005, and they have been analyzed using standardized methodology. Fifteen out of 118 samples (12.7%) were positive for Cryptosporidium spp. and nine (7.6%) positive for Giardia spp. Additional 42 samples from two same areas were examined 3 years later, in the early 2008. Five out of 42 (11.9%) samples were positive for Cryptosporidium spp., and three out of 42 (7.1%) were positive for Giardia spp.. Both protozoans were found in reservoir, river/canal, and pond waters. It appears no significant differences (p < 0.05) between Cryptosporidium and Giardia (oo)cysts levels during the two monitoring periods; however, the number of the investigated areas and samples in the second period was significantly less than in the first period. This is the first description on Cryptosporidium and Giardia (oo)cysts in water sources of Thailand, and it suggests the need for water quality control in the interest of public health safety.

BACKGROUND & OBJECTIVES: Dengue fever (DF), dengue hemorrhagic fever (DHF) and dengue shock syndrome (DSS) are the re-emerging infectious diseases caused by the four serotypes of dengue (DEN) virus, type 1 to 4, belonging to the family Flaviviridae and genus Flavivirus. In the absence of a safe and effective mass immunisation, the prevention and control of dengue outbreaks depend upon the surveillance of cases and mosquito vector. The aim of this work is to test enzyme-linked immunosorbent assay (ELISA) tool for the virological surveillance of dengue. METHODS: Virus-infected Aedes mosquitoes were collected from the field in order to serve as an early warning monitoring tool for dengue outbreaks. In a prospective field study conducted from April to September 2000, female adult Aedes mosquitoes were caught from selected dengue-sensitive area in Chombung district, Ratchaburi province and assayed by ELISA. RESULT: Approximately 18.3% were found positive for dengue virus. CONCLUSION: This can imply that ELISA can be an alternative tool for epidemiological surveillance for dengue in mosquitoes.

Border bancroftian filariasis caused by Wuchereria bancrofti nocturnally subperiodic mainly exists in Karens residing alongside the Thailand-Myanmar border. Imported bancroftian filariasis caused by W. bancrofti nocturnally periodic mainly exists in cross-border Myanmar migrants. We analyzed seroprevalence data based on W. bancrofti adult worm antigen (Ag) loads and human immunodeficiency virus (HIV) immunoglobulins in the sentinel population samples which were studied prior to the start of the diethylcarbamazine (DEC) mass treatment phase in the PELF during fiscal years 2002-2006. In the Karens, the cumulative infection prevalence (36.8% serological antigen positivity or SAP) was specific for age (p < 0.001) but universal for gender (p = 0.77). The infection intensity (median Ag load = 60,827 antigen units or AU/ml) was specific for age (p = 0.031) and for males (p = 0.016). In the Myanmars, infection prevalence (24.0% SAP) was universal for age (p = 0.961) and for gender (p = 0.676). The infection intensity (median Ag load = 19,068 AU/ml) was universal for age (p = 0.433) but specific for females (p = 0.027). Overall, the Ag loads between the groups were significantly different (p = 0.014). In analysis of concomitant HIV and W. bancrofti infections, 7 (3.2%) Myanmars infected with HIV 1 and 3 (5.7%) with concomitant infections, subjected to biannual DEC treatment with 300 mg oral-dose FILADEC, were prevalent. The antigenemia clearance in the concomitant infections (r = -0.732, p = 0.039) as well as in the single W. bancrofti infection (r = -0.781, p = 0.022) was correlated with time required to clear antigenemias. We reemphasize that W. bancrofti adult worm Ag loads in the sentinel population samples would be beneficial for the PELF's implementers at the provincial level to probe the disease burdens in target areas and to evaluate and monitor the DEC treatment efficacy and effectiveness in those sentinel populations, including those with concomitant HIV eligible for the DEC mass treatment phase in the PELF.

A Geographic Information System (GIS) was used as analysis tool to study the spatial distribution of dengue virus-infected Aedes mosquitos in Thailand. Global Positioning System (GPS) instruments were used to map villages involved in dengue epidemiological studies in Ratchaburi Province, Thailand. Differentially processed GPS data, with a spatial resolution of approximately 1 meter, were incorporated into a GIS for analysis and mapping. Databases associated with a village GIS included village number, Aedes aegypti populations, and test results. Epidemiological surveillance for dengue infection through the detection of the dengue virus type(s) infecting Aedes mosquitos during epidemic periods constitutes a reliable sentinel system for dengue outbreaks. Various techniques were applied including: enzyme linked immunosorbent assay (ELISA), indirect immunofluorescent assay (IFA), and reverse transcriptase-polymerase chain reaction (RT-PCR) assay for the virologic surveillance of the type-specific detection of dengue viruses in artificially infected and in field-caught adult Aedes mosquitos. In laboratory experiments, all assays showed sufficient sensitively to detect one virus infected mosquito and the rapid RT-PCR clearly showed serotype-specificity with very high detection sensitivity. In the field study conducted from April to September 2000, female adult Aedes mosquitos were collected from selected dengue-sensitive areas in Chom Bung district, Ratchaburi Province and assayed by ELISA, IFA and RT-PCR with 18.3% (44/240), 28.98% (20/69) and 15% (3/20) positive for dengue virus, respectively. Geographic distribution of the virus-infected Aedes mosquitos and household locations were demonstrated by the GPS and the GIS. The development of disease mapping data coupled with RT-PCR laboratory-based surveillance of dengue virus infection can successfully serve as epidemiologic tools in an early warning system for dengue hemorrhagic fever (DHF) epidemics.

In total, 117 HIV-1 infected patients from several provinces in Northeastern Thailand were analysed. All blood samples collected from individuals were confirmed by EIA and Western blot and partially by HIV-1 gag-, pol- and env-PCR. By serotyping with a V3-peptide ELISA, 108 (92.3%) of the sera samples belonged to subtype E, 9 (7.7%) were serotype B. For 10 Thai HIV-1 infections, the serotype and genotype were determined. The genotype was determined by phylogenetic analysis of directly sequenced PCR amplicons, 8 were subtype E, 2 subtype B. For these patients the serotype did correlate with the genotype. Tracing back the origin of Thai patients, it seems that most were infected within early years of the epidemic and the Thai subtype B infected patients have been imported directly from foreign countries via sexual contact. The findings suggest there are two district subtypes in Thailand with the majority being subtype E. The relatively high prevalence of subtype B in Northeastern Thailand may be due to the increasing intermix of the two strains (subtypes E and B) and the migration for employment from foreign countries. This may lead to public health concerns regarding surveillance of HIV-1 subtypes and the regulation of potentially infected workers returning from abroad to the country.
