RESUMO

OBJECTIVE: Measurement of cardiac biomarker troponin T plays a vital role in the diagnosis of myocardial infarction. A quick turnaround time (TAT) for cardiac troponin T is crucial in managing patients in the emergency department and critical care unit. The goal of the study was to implement Lean Six Sigma methods to improve stat troponin T TAT compliance rate (the time specimen received in the laboratory to the reporting of results through the laboratory information system [LIS]) from 86% to 95% in an 8-month period. METHODS: We conducted a quality and process improvement project to reduce stat troponin T TAT in the core laboratory. We used a 5-stage Six Sigma methodology to simplify the laboratory work process and decrease the TAT by eliminating non-value-added steps. Data from April 2021 (baseline) and January 2022 (improved) are included in the analysis. RESULTS: In the core laboratory, we improved the TAT for the preanalytical and analytical process by eliminating the batch processing and prioritizing the stat samples. We improved the TAT for the postanalytical process by replacing manual result verification with auto result verification via an LIS. Improved stat troponin T TAT compliance rate has the potential to enhance the overall quality of patient care, especially in the emergency and critical care departments. CONCLUSION: Using Lean Six Sigma methodologies in the core laboratory, we successfully improved the stat troponin T TAT compliance rate from 86% to 95% in an 8-month period.

RESUMO

Isolated brain metastasis from cancers of urothelial origin are rare, especially after a long recurrence-free interval with few reports in the literature. We herein present the case of a 62 year old male with history of recurrent bladder cancers treated in 2004 and 2005 and a left distal ureteral high grade pT3aN1M0 urothelial cancer treated with distal ureterectomy and reimplant followed by adjuvant chemotherapy in 2014 who presented after a 5 year recurrence-free interval with tonic-clonic seizure. Further workup revealed an isolated 12.0 mm x 18.0 mm x 8.0 mm mass overlying the left parietal lobe with no other metastatic sites. The patient was treated with resection of the mass and adjuvant radiation therapy with pathology confirming metastatic carcinoma of urothelial origin. Though prognosis for patients with such a presentation remains poor, we provide a review of the current literature with respect to incidence, presentation and therapeutic considerations for such patients.

INTRODUCTION: A 28-year old female presented with a non-radiating persistent left upper quadrant pain and tenderness for 5 weeks. METHODS: A preliminary CT scan displayed bony structures in the spleen. A delayed scanning subsequently showed the bones to have changed position, consistent with a life fetus. RESULTS: Ultrasound confirmed the CT findings, and ascertained the fetus to be consistent with 13 weeks of gestation. Laparoscopic splenectomy was performed and examination of the intact spleen confirmed a male fetus that was morphologically normal. CONCLUSION: This is the first report of fetal bony structures in the spleen associated with an advanced intra-splenic pregnancy.
