The role development of assistive personnel has become more important in rural hospitals because of the limited professional nursing supply. St. Mary's Hospital has fostered interdepartmental collaboration to reallocate duties to personnel who are best qualified to perform them. The dyad model, uniquely tailored to this hospital's needs, has resulted in the more efficient and effective use of personnel.

The nursing departments of three hospitals and a community college in southern Maryland collaborated to develop a solution to the area's nursing shortage. "Launch Your Nursing Career" (LYNC) brings high school students into contact with working nurses during a 2-year period. Participants attend summer classes at the college and work with nurse preceptors in the community hospitals. In addition, nurses in administrative or teaching positions establish mentor relationships with the participants.

Until nurses at St. Mary's Hospital took the lead in establishing an outpatient infusion unit, oncology patients had to travel up to 100 miles to receive chemotherapy. Today, they need travel only ten minutes to receive the best and latest of a variety of high-tech services.

The authors describe the process that bridges the gap between concept and reality by developing a successful system of charging for nursing services. By determining the cost of nursing services and extracting the nursing charge from the traditional room rate, nursing units are converted to revenue producers, with the ability to negotiate for managed care contracts, justify budgets and staffing, equate nursing with other revenue producing centers, and eliminate opinion about perceptions of the cost of nursing services.
