INTRODUCTION: Applying to general surgery residency is undoubtedly a competitive process. Participation in scholarly activity (SCA) has been cited as a criterion when selecting applicants for interview and in the ranking process. This study aims to evaluate the association between gender of applicants to surgery residency and SCA and to characterize trends in SCAs over time. METHODS: We analyzed the SCA of applicants interviewed at a general surgery residency program over 6-interview cycles (2016-2021). Eight SCA categories were included: (1) Poster Presentation, (2) Oral Presentation, (3) Peer-Reviewed (PR) Journal Articles/Abstracts, (4) PR Journal Articles/Abstracts (Other than Published), (5) PR Online Publication, (6) PR Book Chapter, (7) Nonpeer reviewed Online Publication, and (8) Other Articles/Scientific Monograph. RESULTS: Of a total of 335 interviewed applicants, 288 (86%) had at least one count of SCA. Overall, no difference between male and female applicants was noticed (n = 178, 84.8% versus n = 110, 88%, P = 0.409) and no change in percentage of SCA over the six cycles (P = 0.239). The most reported SCAs were poster presentations (n = 242, 72.2%), oral presentations (n = 159, 47.5%), PR journal articles/abstracts (n = 159, 47.5%). Female applicants have marginally higher median (interquartile range) for SCAs compared to male applicants (5 [3, 8] versus 4 [3, 8], P value 0.272). CONCLUSIONS: No association between gender and SCA among applicants for general surgery residency positions was observed. While more than three-fourths of applicants have at least one SCA, only a small fraction of applicants were published. Students should be made aware of the importance of SCA early in graduate medical education.

Stygofauna are aquatic fauna that have evolved to live underground. The impacts of anthropogenic climate change, extraction and pollution on groundwater pose major threats to groundwater health, prompting the need for efficient and reliable means to detect and monitor stygofaunal communities. Conventional survey techniques for these species rely on morphological identification and can be biased, labour-intensive and often indeterminate to lower taxonomic levels. By contrast, environmental DNA (eDNA)-based methods have the potential to dramatically improve on existing stygofaunal survey methods in a large range of habitats and for all life stages, reducing the need for the destructive manual collection of often critically endangered species or for specialized taxonomic expertise. We compared eDNA and haul-net samples collected in 2020 and 2021 from 19 groundwater bores and a cave on Barrow Island, northwest Western Australia, and assessed how sampling factors influenced the quality of eDNA detection of stygofauna. The two detection methods were complementary; eDNA metabarcoding was able to detect soft-bodied taxa and fish often missed by nets, but only detected seven of the nine stygofaunal crustacean orders identified from haul-net specimens. Our results also indicated that eDNA metabarcoding could detect 54%-100% of stygofauna from shallow-water samples and 82%-90% from sediment samples. However, there was significant variation in stygofaunal diversity between sample years and sampling types. The findings of this study demonstrate that haul-net sampling has a tendency to underestimate stygofaunal diversity and that eDNA metabarcoding of groundwater can substantially improve the efficiency of stygofaunal surveys.

Groundwater biota are crucial for the ecological functioning of subterranean ecosystems. However, while knowledge of the taxonomic diversity of groundwater invertebrates (stygofauna) is increasing, functional ecological information is still limited. Here, we investigate seldom empirically tested assumptions around stygofaunal trophic plasticity in coping with oligotrophic habitats. We focus on Barrow Island (Western Australia), an ideal natural laboratory due to the occurrence of natural oil seeps in association with aquifers. The trophic position and food source use of the endemic atyid shrimp Stygiocaris stylifera (Holthuis, 1960) were assessed via δ13C and δ15N stable isotope analysis (SIA). Background information on the environmental conditions was gathered through hydrochemical data and δ13C SIA combined with 14C data from dissolved inorganic/organic carbon and particulate organic carbon from groundwater samples. Our results indicate carbon enrichment in proximity to the natural oil seepage coupled with changes in trophic positions of S. stylifera from higher consumers/predators to biofilm grazers/decomposers. These results are consistent with an increased involvement of hydrocarbon seeps and associated microbial communities in the carbon flows and confirm potential for the trophic flexibility in stygofauna. Further investigations involving other trophic groups will help elucidate the functioning of the ecosystems at a community level.

PURPOSE: The broad aim of sport-science research is to enhance the performance of coaches and athletes. Despite decades of such research, it is well documented that sport-science research lacks empirical evidence, and critics have questioned its scientific methods. Moreover, many have pointed to a research-practice gap, whereby the work undertaken by researchers is not readily applied by practitioners. The aim of this study was to use a systems thinking analysis method, causal loop diagrams, to understand the systemic issues that interact to influence the quality of sport-science research. METHODS: A group model-building process was utilized to develop the causal loop diagram based on data obtained from relevant peer-reviewed literature and subject-matter experts. RESULTS: The findings demonstrate the panoply of systemic influences associated with sport-science research, including the existence of silos, a focus on quantitative research, archaic practices, and an academic system that is incongruous with what it actually purports to achieve. CONCLUSIONS: The emergent outcome of the interacting components is the creation of an underperforming sport-science research system, as indicated by a lack of ecological validity, translation to practice, and, ultimately, a research-practice gap.

The coronavirus disease 2019 (COVID-19) pandemic has had a dramatic impact on the way in which the community interacts within public spaces. Consequently, the design of these urban environments requires new approaches. It is our view that Human Factors and Ergonomics approaches can be used to provide these insights. This article explores the opportunities for making public spaces safer and more accessible for community use under pandemic conditions. The study utilizes a sociotechnical systems model of an archetype public space, developed pre-COVID-19, to explore the infrastructure and activities that are impacted by the introduction of the virus to the public space system. The aim was to identify those elements of the system that are completely unavailable under pandemic conditions, those elements that become limited in use, and those which remained unaffected by the presence of the virus. The findings show that under pandemic conditions pre-COVID-19 public spaces were surprisingly resilient with proportionally few elements within the model completely unavailable. They also demonstrate that overall, the public spaces system, while still operating, is significantly constrained in achieving and optimizing community and individual health and well-being. The insertion of five (5) urban design interventions within the system model presented unique insights into the possibilities for optimizing adaptive capacity. These interventions revealed design opportunities across several levels of the systems model. Such insights are argued to assist in not only re-establishing community access under pandemic conditions but also more inclusive access to a broader range of the community under all conditions.

Following strict "lockdown" restrictions designed to control the spread of the COVID-19 virus, many jurisdictions are now engaged in a process of easing restrictions in an attempt to stimulate economic and social activity while continuing to suppress virus transmission. This is challenging and complex, and in several regions, new outbreaks have emerged. We argue that systems Human Factors and Ergonomics methods can assist in understanding and optimizing the return from lockdown. To demonstrate, we used work domain analysis to develop an abstraction hierarchy model of a generic "return from lockdown restrictions" system. The model was assessed to identify (a) issues preventing a successful return from lockdown; and (b) leverage points that could be exploited to optimize future processes. The findings show that the aim of continuing to suppress virus transmission conflicts with the aims of returning to pre-virus economic and social activity levels. As a result, many functions act against each other, ensuring that the system cannot optimally achieve all three of its primary aims. Potential leverage points include modifying the goals and rules of the system and enhancing communications and feedback. Specifically, it is argued that moderating economic aims and modifying how social and community activities are undertaken will result in longer term suppression of the virus.

Crashes at intersections represent an important road safety problem. Interactions between different road user types, such as between vehicles and vulnerable road users, are a particular concern. It has been suggested driver-centric road design plays a role in crashes. A multi-road user evaluation of three novel intersection designs is described. The designs were generated using the Cognitive Work Analysis Design Toolkit, underpinned by sociotechnical systems theory. The desktop evaluation involved drivers, motorcyclists, cyclists and pedestrians rating the design concepts against alignment with design goals, sociotechnical systems theory and usability, and providing feedback on the positive and negative aspects. Two concepts received more positive ratings and feedback in comparison to a concept that provided more user autonomy. The evaluation results also highlight clear differences in needs across road user groups. The design and evaluation process demonstrates how sociotechnical systems values and principles can be applied in the design of public spaces. Practitioner Summary: This study involved a participatory evaluation of novel road intersection designs, based on sociotechnical systems theory. The results identified important differences in needs and preferences across road user groups and demonstrate the value of sociotechnical systems theory and user participation in road transport design and evaluation processes. In the current study, social network analysis (SNA) and notational analysis (NA) methods were applied to examine the goal scoring passing networks (GSPN) for all goals scored at the 2016 European Football Championships. The aim of the study was to determine the GSPN characteristics for the overall tournament, between the group and knock out stages, and for the successful and unsuccessful teams. The study also used degree centrality (DC) metrics as a novel method to determine the relative contributions of the pitch locations involved in the GSPN. To determine changes in GSPN characteristics as a function of changing score line, the analysis considered the match status of the game when goals were scored. There were significant differences for SNA metrics as a function of match status, and for the DC metrics in the comparison of the different pitch locations. There were no differences in the SNA metrics for the GSPN between teams in the group and knock out stages, or between the successful and unsuccessful teams. The results indicate that the GSPN had low values for network density, cohesion, connections, and duration. The networks were direct in terms of pitch zones utilised, where 85% of the GSPN included passes that were played within zones or progressed through the zones towards the goal. SNA and NA metrics were significantly different as a function of changing match status. The current study adds to the previous research on goal scoring in football, and demonstrates a novel method to determine the prominent pitch zones involved in the GSPN. These results have implications for match analysis and the coaching process.

Despite significant progress, road trauma continues to represent a global safety issue. In Queensland (Qld), Australia, there is currently a focus on preventing the 'fatal five' behaviours underpinning road trauma (drug and drink driving, distraction, seat belt wearing, speeding, and fatigue), along with an emphasis on a shared responsibility for road safety that spans road users, vehicle manufacturers, designers, policy makers etc. The aim of this article is to clarify who shares the responsibility for road safety in Qld and to determine what control measures are enacted to prevent the fatal five behaviours. This is achieved through the presentation of a control structure model that depicts the actors and organisations within the Qld road transport system along with the control and feedback relationships that exist between them. Validated through a Delphi study, the model shows a diverse set of actors and organisations who share the responsibility for road safety that goes beyond those discussed in road safety policies and strategies. The analysis also shows that, compared to other safety critical domains, there are less formal control structures in road transport and that opportunities exist to add new controls and strengthen existing ones. Relationships that influence rather than control are also prominent. Finally, when compared to other safety critical domains, the strength of road safety controls is brought into question.

An increasing intensity of operations means that the longstanding safety issue of rail level crossings is likely to become worse in the transport systems of the future. It has been suggested that the failure to prevent collisions may be, in part, due to a lack of systems thinking during design, crash analysis, and countermeasure development. This paper presents a systems analysis of current active rail level crossing systems in Victoria, Australia that was undertaken to identify design requirements to improve safety in future rail level crossing environments. Cognitive work analysis was used to analyse rail level crossing systems using data derived from a range of activities. Overall the analysis identified a range of instances where modification or redesign in line with systems thinking could potentially improve behaviour and safety. A notable finding is that there are opportunities for redesign outside of the physical rail level crossing infrastructure, including improved data systems, in-vehicle warnings and modifications to design processes, standards and guidelines. The implications for future rail level crossing systems are discussed.

Footpaths provide an integral component of our urban environments and have the potential to act as safe places for people and the focus for community life. Despite this, the approach to designing footpaths that are safe while providing this sense of place often occurs in silos. There is often very little consideration given to how designing for sense of place impacts safety and vice versa. The aim of this study was to use a systems analysis and design framework to develop a design template for an 'ideal' footpath system that embodies both safety and sense of place. This was achieved through using the first phase of the Cognitive Work Analysis framework, Work Domain Analysis, to specify a model of footpaths as safe places for pedestrians. This model was subsequently used to assess two existing footpath environments to determine the extent to which they meet the design requirements specified. The findings show instances where the existing footpaths both meet and fail to meet the design requirements specified. Through utilising a systems approach for footpaths, this paper has provided a novel design template that can inform new footpath design efforts or be used to evaluate the extent to which existing footpaths achieve their safety and sense of place requirements.

RESUMO

We describe and report the results of an evaluation of a smart alarm algorithm for post coronary artery bypass graft (CABG) patients. The algorithm (CABG-SA) was applied to vital sign data streams recorded in a surgical intensive care unit (SICU) at a hospital in the University of Pennsylvania Health System. In order to determine the specificity of CABG-SA, the alarms generated by CABG-SA were compared against the actual interventions performed by the staff of the critical care unit. Overall, CABG-SA alarmed for 55% of the time relative to traditional alarms while still generating alarms for 12 of the 13 recorded interventions.

Previous research has demonstrated that manipulating vision influences balance. Here, we question whether manipulating balance can influence vision and how it may influence vision--specifically, the perception of width. In Experiment 1, participants estimated the width of beams while balanced and unbalanced. When unbalanced, participants judged the widths to be smaller. One possible explanation is that unbalanced participants did not view the stimulus as long as when balanced because they were focused on remaining balanced. In Experiment 2, we tested this notion by limiting viewing time. Experiment 2 replicated the findings of Experiment 1, but viewing time had no effect on width judgments. In Experiment 3, participants' level of arousal was manipulated, because the balancing task likely produced arousal. While jogging, participants judged the beams to be smaller. In Experiment 4, participants completed another arousing task (counting backward by sevens) that did not involve movement. Again, participants judged the beams to be smaller when aroused. Experiment 5A raised participants' level of arousal before estimating the board widths (to control for potential dual-task effects) and showed that heightened arousal still influenced perceived width of the boards. Collectively, heightened levels of arousal, caused by multiple manipulations (including balance), influenced perceived width.
