RESUMO

The Mantel-Haenszel estimator is one of the most popular techniques for measuring differential item functioning (DIF). A generalization of this estimator is applied to the context of DIF to compare items by taking the covariance of odds ratio estimators between dependent items into account. Unlike the Item Response Theory, the method does not rely on the local item independence assumption which is likely to be violated when one item provides clues about the answer of another item. Furthermore, we use these (co)variance estimators to construct a hypothesis test to assess DIF for multiple items simultaneously. A simulation study is presented to assess the performance of several tests. Finally, the use of these DIF tests is illustrated via application to two real data sets.

BACKGROUND: Acute kidney injury (AKI) is common. An episode of AKI may modify the risk of developing kidney stones by potential long-term effects on urine composition. We aimed to investigate the association between AKI and the risk of kidney stone presentations. METHODS: The retrospective cohort study used patient data (1 January 2008-31 December 2017), from an Australian Local Health District, which included AKI diagnosis, demographics, comorbidities and kidney stone admissions. Time-varying Cox proportional hazards and propensity-matched analysis were used to determine the impact of AKI on the risk of kidney stones. To address possible population inhomogeneity in comparisons between no AKI and hospitalized AKI, sub-group analysis was done comparing inpatient and outpatient AKI versus no AKI, to assess consistency of association with future stones. Sensitivity analysis was undertaken to capture the impact of a known AKI status and AKI severity. RESULTS: Out of 137 635 patients, 23 001 (17%) had an AKI diagnosis and 2295 (2%) had kidney stone presentations. In the unadjusted analysis, AKI was associated with kidney stones, with AKI used as a time-varying exposure, [hazard ratio (HR) 1.32, 95% confidence interval (CI) 1.16-1.50)]. Both inpatient-AKI (HR 1.19, 95% CI 1.01-1.39) and outpatient-AKI (HR 1.59, 95% CI 1.30-1.94) were significantly associated with future stones compared to no AKI subjects. This association persisted in the adjusted analysis (HR 1.45, 95% CI 1.26-1.66), propensity-matched dataset (HR 1.67, 95% CI 1.40-1.99) and sensitivity analysis. There was a dose-response relationship with higher stages of AKI being associated with a greater risk of kidney stones. CONCLUSIONS: In a large cohort of patients, AKI is associated with a greater risk of kidney stones, which increases with higher stages of AKI. This association should be examined in other cohorts and populations for verification.

This repeat cross-sectional study investigated the impact of lockdown in Europe in Winter (January and February 2021) on children's and adolescent's physical activity (PA) and recreational screen time (RST), and compared PA to the lockdown in Spring 2020. An online survey was administered (n = 24 302; 6-18 years; 51.7% boys) in nine countries. PA and RST were assessed by 7-day recall. In total, 9.3% of children (95% confidence interval: 6.9-11.7) met WHO PA recommendation, which was half of the proportion observed in Spring 2020 [19.0% (18.2-19.9)]. Sixty percent exceeded the RST recommendations. This suggests that winter lockdown could have a more negative impact on PA than in spring.

To date, few data on how the COVID-19 pandemic and restrictions affected children's physical activity in Europe have been published. This study examined the prevalence and correlates of physical activity and screen time from a large sample of European children during the COVID-19 pandemic to inform strategies and provide adequate mitigation measures. An online survey was conducted using convenience sampling from 15 May to 22 June, 2020. Parents were eligible if they resided in one of the survey countries and their children aged 6-18 years. 8395 children were included (median age [IQR], 13 [10-15] years; 47% boys; 57.6% urban residents; 15.5% in self-isolation). Approximately two-thirds followed structured routines (66.4% [95%CI, 65.4-67.4]), and more than half were active during online P.E. (56.6% [95%CI, 55.5-57.6]). 19.0% (95%CI, 18.2-19.9) met the WHO Global physical activity recommendation. Total screen time in excess of 2 h/day was highly prevalent (weekdays: 69.5% [95%CI, 68.5-70.5]; weekend: 63.8% [95%CI, 62.7-64.8]). Playing outdoors more than 2 h/day, following a daily routine and being active in online P.E. increased the odds of healthy levels of physical activity and screen time, particularly in mildly affected countries. In severely affected countries, online P.E. contributed most to meet screen time recommendation, whereas outdoor play was most important for adequate physical activity. Promoting safe and responsible outdoor activities, safeguarding P.E. lessons during distance learning and setting pre-planned, consistent daily routines are important in helping children maintain healthy active lifestyle in pandemic situation. These factors should be prioritised by policymakers, schools and parents. HighlightsTo our knowledge, our data provide the first multi-national estimates on physical activity and total screen time in European children roughly two months after COVID-19 was declared a global pandemic.Only 1 in 5 children met the WHO Global physical activity recommendations.Under pandemic conditions, parents should set pre-planned, consistent daily routines and integrate at least 2-hours outdoor activities into the daily schedule, preferable on each day. Schools should make P.E. lessons a priority. Decision makers should mandate online P.E. be delivered by schools during distance learning. Closing outdoor facilities for PA should be considered only as the last resort during lockdowns.

Eastern Australia has a climate characterised by extreme variability and the occurrence of multiple years of drought conditions. Arguably one of the severest droughts on record - the Big Dry ended in many areas with the La Niña of 2009/2010. A succession of subsequent dry years brought a return to drought conditions across much of eastern Australia in 2018 and 2019, ending with the catastrophic fires of 2019/2020. An analysis of river gauges in eastern Australia demonstrates that unregulated rivers have been subject to reduced monthly and total annual flow for far longer than the recent multi-year droughts. A breakpoint regression model on the annual streamflow data shows statistically significant declines in total annual flow (by up to a factor of three) since 1992/93 on the far South coast of New South Wales (NSW). In the monthly data, fifteen of the nineteen gauges analysed exhibit modelled breakpoints, but with statistically significant differences in monthly mean discharge between consecutive periods only occurring in three of these gauges (occurring between 1972 and 1993 in both the North and South coast). The trend toward reduced flow over the last few decades has, for many rivers, coincided with land use and river management changes resulting in increases in woody riparian vegetation. To show this we use a remote sensing technique and estimate the magnitude of vegetation change along all major rivers and their tributaries on the eastern seaboard of NSW (28 catchments with total river length assessed of 19,750 km) using a normalized difference vegetation index (NDVI) analysis of woody vs non-woody riparian vegetation extent. Predicted vegetation change between 1987 and 2020 is spatially variable across catchments but the mean increase in woody riparian vegetation across all catchments is 9-51% (0.2 and 0.1 NDVI increases). Such increases are perhaps the largest biogeomorphic change the SE Australian drainage network has experienced since the initial clearance of vegetation associated with European colonisation in the late 18th and early 19th centuries.

INTRODUCTION: 24-hour movement behaviours (physical activity, sedentary behaviour and sleep) during the early years are associated with health and developmental outcomes, prompting the WHO to develop Global guidelines for physical activity, sedentary behaviour and sleep for children under 5 years of age. Prevalence data on 24-hour movement behaviours is lacking, particularly in low-income and middle-income countries (LMICs). This paper describes the development of the SUNRISE International Study of Movement Behaviours in the Early Years protocol, designed to address this gap. METHODS AND ANALYSIS: SUNRISE is the first international cross-sectional study that aims to determine the proportion of 3- and 4-year-old children who meet the WHO Global guidelines. The study will assess if proportions differ by gender, urban/rural location and/or socioeconomic status. Executive function, motor skills and adiposity will be assessed and potential correlates of 24-hour movement behaviours examined. Pilot research from 24 countries (14 LMICs) informed the study design and protocol. Data are collected locally by research staff from partnering institutions who are trained throughout the research process. Piloting of all measures to determine protocol acceptability and feasibility was interrupted by COVID-19 but is nearing completion. At the time of publication 41 countries are participating in the SUNRISE study. ETHICS AND DISSEMINATION: The SUNRISE protocol has received ethics approved from the University of Wollongong, Australia, and in each country by the applicable ethics committees. Approval is also sought from any relevant government departments or organisations. The results will inform global efforts to prevent childhood obesity and ensure young children reach their health and developmental potential. Findings on the correlates of movement behaviours can guide future interventions to improve the movement behaviours in culturally specific ways. Study findings will be disseminated via publications, conference presentations and may contribute to the development of local guidelines and public health interventions.

BACKGROUND: The restrictions associated with the 2020 COVID-19 pandemic has resulted in changes to young children's daily routines and habits. The impact on their participation in movement behaviours (physical activity, sedentary screen time and sleep) is unknown. This international longitudinal study compared young children's movement behaviours before and during the COVID-19 pandemic. METHODS: Parents of children aged 3-5 years, from 14 countries (8 low- and middle-income countries, LMICs) completed surveys to assess changes in movement behaviours and how these changes were associated with the COVID-19 pandemic. Surveys were completed in the 12 months up to March 2020 and again between May and June 2020 (at the height of restrictions). Physical activity (PA), sedentary screen time (SST) and sleep were assessed via parent survey. At Time 2, COVID-19 factors including level of restriction, environmental conditions, and parental stress were measured. Compliance with the World Health Organizations (WHO) Global guidelines for PA (180 min/day [≥60 min moderate- vigorous PA]), SST (≤1 h/day) and sleep (10-13 h/day) for children under 5 years of age, was determined. RESULTS: Nine hundred- forty-eight parents completed the survey at both time points. Children from LMICs were more likely to meet the PA (Adjusted Odds Ratio [AdjOR] = 2.0, 95%Confidence Interval [CI] 1.0,3.8) and SST (AdjOR = 2.2, 95%CI 1.2,3.9) guidelines than their high-income country (HIC) counterparts. Children who could go outside during COVID-19 were more likely to meet all WHO Global guidelines (AdjOR = 3.3, 95%CI 1.1,9.8) than those who were not. Children of parents with higher compared to lower stress were less likely to meet all three guidelines (AdjOR = 0.5, 95%CI 0.3,0.9). CONCLUSION: PA and SST levels of children from LMICs have been less impacted by COVID-19 than in HICs. Ensuring children can access an outdoor space, and supporting parents' mental health are important prerequisites for enabling pre-schoolers to practice healthy movement behaviours and meet the Global guidelines.

Growing body of research recognizes the importance of green spaces on the perinatal outcomes however, further evidence from different geographies are warranted. We aimed to investigate association between, and differential responses to, maternal exposure to green space and birthweight. Birth records (n = 82,221) were extracted from the Perinatal Data Collection (PDC) in Sydney's metropolitan area between January 2016 and December 2017. Association between green space quantity and birthweight, term birthweight, low birthweight, term low birthweight and preterm were assessed using linear and logistic regressions. Potential modification by area-level socioeconomic status and maternal country of birth were tested using interaction terms. Difference in birth weight for the ≥40% versus <20% green space within SA2s was 59.0 g (95%CI: 42.9, 75.3) in unadjusted models which dropped to 25.6 g (95%CI: 13.0, 38.2) in adjusted models. Stratified analysis suggested stronger associations for babies of mothers from affluent neighbourhoods, while statistically significant association was not observed in deprived areas. Furthermore, the association was more pronounced among babies to mothers who were born overseas. Associations were consistent for term births. Higher levels of green space were associated with lower odds of preterm birth in adjusted models. However, we did not identify statistically significant association between green space quantity and the risk of low birthweight (LBW). Our study suggests that green space may support healthier birth outcomes and help to reduce the birthweight gap between newborns of mothers born in Australia and overseas. However, disproportionate benefits among women in affluent neighbourhoods may widen socioeconomic inequities in birthweight.

Maximum likelihood estimation of the model parameters for a spatial population based on data collected from a survey sample is usually straightforward when sampling and non-response are both non-informative, since the model can then usually be fitted using the available sample data, and no allowance is necessary for the fact that only a part of the population has been observed. Although for many regression models this naive strategy yields consistent estimates, this is not the case for some models, such as spatial auto-regressive models. In this paper, we show that for a broad class of such models, a maximum marginal likelihood approach that uses both sample and population data leads to more efficient estimates since it uses spatial information from sampled as well as non-sampled units. Extensive simulation experiments based on two well-known data sets are used to assess the impact of the spatial sampling design, the auto-correlation parameter and the sample size on the performance of this approach. When compared to some widely used methods that use only sample data, the results from these experiments show that the maximum marginal likelihood approach is much more precise.

Previous studies suggest that green and blue spaces may promote several health outcomes including birth outcomes. However, no synthesis of previous work has specifically asked policy-relevant questions of how much and what type is needed in every neighborhood to elicit these benefits at the population level. A systematic review and meta-analyses were conducted to synthesize thirty-seven studies on the association between residential green and blue spaces and pregnancy outcomes. Meta-analyses were performed for birth weight (BW), small for gestational age (SGA), low birth weight (LBW) and preterm birth (PTB). Increase in residential greenness was statistically significantly associated with higher BW [ß = 0.001, 95%CI: (<0.001, 0.002)] and lower odds of SGA [OR = 0.95, 95%CI: (0.92, 0.97)]. Associations between green space and LBW and PTB were as hypothesized but not statistically significant. Associations between blue spaces and pregnancy outcomes were not evident. No study explicitly examined questions of threshold, though some evidence of nonlinearity indicated that moderate amounts of green space may support more favorable pregnancy outcomes. Policy-relevant green and blue space exposures involving theory-driven thresholds warrant testing to ensure future investments in urban greening promote healthier pregnancy outcomes.

The cumulative logit or the proportional odds regression model is commonly used to study covariate effects on ordinal responses. This paper provides some graphical and numerical methods for checking the adequacy of the proportional odds regression model. The methods focus on evaluating functional misspecification for specific covariate effects, but misspecification of the link function can also be dealt with under the same framework. For the logistic regression model with binary responses, Arbogast and Lin (Statist. Med. 2005; 24:229-247) developed similar graphical and numerical methods for assessing the adequacy of the model using the cumulative sums of residuals. The paper generalizes their methods to ordinal responses and illustrates them using an example from the VA Normative Aging Study. Simulation studies comparing the performance of the different diagnostic methods indicate that some of the graphical methods are more powerful in detecting model misspecification than the Hosmer-Lemeshow-type goodness-of-fit statistics for the class of models studied.

Surveys often contain qualitative variables for which respondents may select any number of the outcome categories. For instance, for the question "What type of contraception have you used?" with possible responses (oral, condom, lubricated condom, spermicide, and diaphragm), respondents would be instructed to select as many of the outcomes that apply. This situation is known as multiple responses. When the data includes stratification variables, we discuss two approaches: (1) the "GEE" approach which uses logit models directly applying the generalized estimating equations (GEE) method (Liang and Zeger, 1986); and (2) the "GMH" approach which extends the generalized Mantel-Haenszel type estimators (Greenland, 1989) to make inferences across multiple responses. These approaches can also be used for data with dependent observations across strata.
