BACKGROUND AND AIMS: Commercial plasma donation was introduced in China in the 1970s. Cases of non-A, non-B hepatitis (hepatitis C) continued to occur, with multiple outbreaks among plasma donors in Guan county, Hebei province between 1972 and 1990. The outcomes of hepatitis C virus (HCV) infection in these paid plasma donors from six villages of Guan county were followed up for 12-19 years. METHODS: A total of 402 plasma donors with HCV infection were enrolled since anti-HCV-positive in 1991 or 1998. Follow up was maintained until death or the end of the observation period. No antiviral treatment was applied during the period of infection. RESULTS: Follow up was lost in 23 cases. After a 12-19-year follow up, 31 donors died, with the cause of death directly related to liver disease in 15 cases, and an overall mortality of 8.18% (31/379). The incidence of liver cirrhosis was 10.03%, and hepatocellular carcinoma (HCC) was 2.90%. The rate of viral spontaneous clearing was 20.32% (77/379), and 13.69% (23/168) in males and 25.59% (54/211) in females. In May 2010, detections were performed in 348 cases. Abnormality of liver function was related to HCV viremia. Sex and alcohol intake impacted the outcome of HCV infection. There was no correlation between the viral spontaneous clearance with age of infection and genotype. CONCLUSIONS: This area has a high rate of chronicity in HCV infection due to plasma donation. Twenty-five years after virus infection, liver cirrhosis or HCC developed in one-tenth of patients, with an overall mortality of 8.18%.

BACKGROUND/AIMS: Hepatitis C virus is a hepatotrophic flavivirus infecting 170 million people worldwide and 3.2% people in China. We aim to investigate the clinical outcomes and hepatic histology of patients with hepatitis C in the north of China. METHODOLOGY: Nine patients with hepatitis C, 4 males and 5 females, aged 44 +/- 7 years old, who were infected during transfusion or blood donation underwent follow-up for 13-27 years. We performed liver histological examination by biopsy, ultrasonography, and biochemical examination of alanine transaminase (ALT), aspartate transaminase, and total bilirubin. The serum anti-HCV was detected by ELISA, and HCV RNA quantification by PCR. RESULTS: ALT were higher than the normal value at every time point and HCV RNA remained positive (3.57 x 10(5) - 7.21 x 10(6) copies/mL) in all patients. Ultrasonography showed mild and moderated hepatitis (3 and 6 cases respectively). The modified histological activity indices (HAI) were 5.0-8.5 and fibrosis scores were 1-4. The ALT value was higher in the moderate cases than in the mild cases, and higher in those with higher HAI. The viral load was higher in the patients infected by the virus of the genotype 1b than in the patients infected by the virus of genotype 2. CONCLUSIONS: The clinical outcomes and hepatic histology of patients with hepatitis C in north of China seem not very severe.

Based on the CROSSBAR network (CN) and the BANYAN network (BN), two new rearrangeable nonblocking constructions of extended BANYAN network (EBN) were proposed for implementing 8x8 optical matrix switch. The interconnection characteristics of these two types of rearrangeable nonblocking EBN were studied, and the logic program for driving switching units was provided. The calculated insertion loss is 3.3 dB for 8x8 optical matrix switch. Silica waveguide 8x8 matrix optical switch was designed and fabricated according to the calculated results. The silica waveguide propagation loss of 0.1dB/cm and waveguide-fiber coupling loss of 0.5dB/point were measured. With the fabricated 8x8 matrix optical switch, optical insertion loss of 4.6 dB, cross-talk of -38 dB, polarization dependent loss of 0.4 dB, averaged switching power of 1.6 W, and switching time of 1 ms were obtained. A basic agreement between experimental results and theoretical calculated values was achieved.

OBJECTIVE: To investigate the clinical outcomes and hepatic histology of patients with hepatitis C. METHODS: Nine patients with hepatitis, 4 males and 5 females, aged 44 +/- 7, who were infected during transfusion or blood donation underwent follow-up for 13 approximately 14 years: liver histological examination by biopsy, ultrasonography, biochemical examination of alanine transaminase (ALT), apartate aminotransferase (AST), and total bilirubin (TBIL), serum anti-HCV by ELISA, and HCV RNA quantification by PCR. RESULTS: ALT were higher than the normal value at every time point and HCV RNA remained positive (3.57 x 10(8) approximately 7.21 x 10(9) copies/L) in all patients. Ultrasonography showed mild and moderate hepatitis (3 and 6 cases respectively). The modified histological activity indices (HAI) were 5.0 approximately 8.5 and the fibrosis scores were 1 approximately 4. The ALT value was higher in the moderate cases than in the mild cases, and higher in those with higher HAI. The viral load was higher in the patients infected by the virus of the genotype 1b than in the patients infected by the virus of the genotype 2. CONCLUSION: The clinical outcomes and hepatic histology of patients with hepatitis C seem not very severe.

RESUMO

A 2x2 digital optical switch using two symmetrical unidirectional Bragg grating couplers is proposed and studied in this paper. A low-loss polymer is used as waveguide material, and the Bragg grating coupling efficiency is optimized to be 22%, then the unidirectional coupling efficiency of 99.9% is achieved in theory. The performance of the switch based on the unidirectional couplers with Bragg gratings is theoretically modeled and simulated. Finally, the 2.4dB insertion loss, the -17dB crosstalk between two output ports, the 28 dB extinction ratio, the 1.5ms response speed and the 87mW power consumption are experimentally demonstrated with this regime.

BACKGROUND: To investigate natural history of hepatitis C virus (HCV) infection and related factors in China. METHODS: Totally 41 women infected with HCV 10-15 years ago were evaluated in this study. Their clinical manifestations were recorded and sera tested for alamine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyltransferase (gamma-GT), alkaline phosphatase (ALP), total bilirubin (TBIL), direct bilirubin (DBIL), total protein (TP), albumin (ALB), and alpha-fetoprotein (alpha-FP), as well as virus markers. The B-type ultrasound examination was performed on the liver, gall bladder, spleen and pancreas. For virus markers, HCV RNA was detected with fluorogenic-quantitative polymerase-chain-reaction, HBsAg and anti-HCV antibody, with enzyme-linked immunosorbent assay (ELISA). RESULTS: Ten to 15 years after infection, the infection was cleared spontaneously in 8 cases (19.51%); 73%(30/41) cases had signs at present, 32% (13/41) cases had mild ALT and/or AST elevation. On ultrasound examination, 83%(34/41) had mild degree 17%(7/41) moderate degree changes of the viral hepatitis. CONCLUSION: Women with HCV infection for 13 +/- 1 (10-15) years after transfusion had slight hepatic inflammation. A part of them had cleared the virus spontaneously.

AIM: To investigate the effect of interleukin-12 p40 gene (IL12B) 3'-untranslated region polymorphism on the outcome of HCV infection. METHODS: A total of 133 patients who had been infected with HCV for 12-25 (18.2+/-3.8) years, were enrolled in this study. Liver biochemical tests were performed with an automated analyzer and HCV RNA was detected by fluorogenic quantitative polymerase chain reaction. B-mode ultrasound was used for liver examination. Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was used for the detection of IL12B (1188A/C) polymorphism. RESULTS: Self-limited infection was associated with AC genotype (OR = 3.48; P = 0.001) and persistent infection was associated with AA genotype (OR = 0.34; P = 0.014) at site 1188 of IL12B. In patients with persistent HCV infection, no significant differences were found regarding the age, gender, duration of infection and biochemical characteristics (P>0.05). According to B-mode ultrasound imaging and clinical diagnosis, patients with persistent infection were divided into groups based on the severity of infection. No significant differences were found in the frequency of IL-12 genotype (1188A/C) between different groups (P>0.05). CONCLUSION: The polymorphism of IL12B (1188A/C) appears to have some influence on the outcome of HCV infection.

OBJECTIVE: To evaluate the health-related quality of life (HRQOL) in chronic hepatitis C (CHC) female patients, and to identify factors associated with impairment in HRQOL. METHODS: We enrolled 41 women cases with hepatitis C for 10 to 15 years and 51 health women in Guan area of Hebei province, China. HRQOL was assessed by the SF-36 by an interview. RESULTS: CHC female patients had significantly lower SF-36 scors in all subscales when compared to those of the non-HCV infected female population. CONCLUSION: CHC female patients have a reduced HRQOL. CHC alone is associated with significant impairment in HRQOL.

OBJECTIVE: To study the outcome of hepatitis C virus (HCV) vertical transmitted infants. METHODS: Thirteen HCV vertical infected infants were followed up for 10 years. HCV antibody and HCV RNA in the blood samples from them were tested using second generation HCV antibody EIA kits and RT-PCR, respectively. RESULTS: Among the 13 infants, one developed clinical hepatitis C, and serum HCV antibody and HCV RNA could be detected for 7 and 8 years, respectively. Three were subclinical hepatitis C, serum HCV antibody continued to be positive for 12 months (2 infants) and 24 months (1 infant), respectively, and serum HCV RNA turned to be negative at the 24th month (2 infants) and the 60th month (1 infant), respectively. Nine were HCV insidious infection, whose serum HCV antibody and HCV RNA turned to be negative in 12 months. During the eight to ten years, there was no infants with anti-HCV or HCV RNA positive again. CONCLUSIONS: It is rarely happened that vertical transmitted HCV induce chronic HCV carrying state and chronic viral hepatitis, and most of the infected infants have good outcome.

OBJECTIVE: To investigate the evolution of HCV quasispecies in persistent and self-limited infection patients. METHODS: Peripheral blood HCV preserved for 10 years from 8 naive patients were analyzed. HCV core and E2 genes were cloned and sequenced. Genetic distances of different isolates at different time were analyzed by phylip software. RESULTS: The genetic distances of HCV quasispecies were higher in persistent infection patients than that of self-limited patients. In each patient the genetic distance of HCV E2 region isolates were higher than that of HCV C region which has some relationship between each other. In the 4 persistent patients there exist no differences between within-group and between-group genetic distance in HCV C region and also exist no differences in HCV E2 region in 2 of the 4 persistent patients. CONCLUSIONS: The diversity degree of HCV quasispecies may affect the outcome of hepatitis C. Research on HCV C and E2 region to study HCV quasispecies may obtain different results. The distribution of HCV quasispecies may be stable in hepatitis C natural history.
