RESUMO

Contemporary computer technology allows precise and fast large database analysis. Widespread and common use depends on appropriate, user friendly software, usually lacking in special medical applications. The aim of this work was to develop an integrated system designed to store, explore and analyze data of patients treated for pancreatic cancer. For that purpose the database administration system MS Visual Fox Pro 3.0 was used and special application, according to ISO 9000 series has been developed. The system works under MS Windows 95 with possibility of easy adaptation to MS Windows 3.11 or MS Windows NT by graphic user's interface. The system stores personal data, laboratory results, visual and histological analyses and information on treatment course and complications. However the system archives them and enables the preparation reports of according to individual and statistical needs. Help and security settings allow to work also for one not familiar with computer science.

They presented a valuation of exocrine pancreatic function using pancreolaurinic test in 15 patients with chronic pancreatitis and in 34 patients after acute pancreatitis. In all patients with chronic pancreatitis they noted an insufficiency of exocrine pancreatic function with association of disorders of tolerance for carbohydrates during a year after a Puestow's operation. They noted a handicap of exocrine pancreatic function in 42% patients just after acute pancreatitis, in 18% patients a year acute pancreatitis and in 26% patients after 2-6 years from illness with association of disorders of tolerance for carbohydrates.

Fifteen patients with chronic pancreatitis were qualified for surgical treatment on the basis of the results of endoscopic retrograde cholangiopancreatography, pancreolauryl test and glucose tolerance test. Significant changes according to Cambridge scale correlated with exocrine-endocrine pancreatic failure. In patients with Wirsung duct of diameter above 6 mm Puestow operation was performed while in patients with narrow Wirsung duct resection procedures were carried out. Control examinations carried out one year after Puestow operation demonstrated in all 10 patients lack of improvement both in the exocrine and endocrine pancreatic function. In 70% of the patients after anastomotic operation and in 80% of the patients after resections, complete regression of preoperative pain was observed.

The authors discuss their own 14-year experience with surgical treatment of the chronic pancreatitis. They treated 64 patients of a mean age of 43 years. An abdominal pain, persisting in all patients, constituted the main indication to surgery. Forty patients underwent pancreatic enterostomy with Puestow technique, 19 patients resections of various extension, and 5 patients underwent pancreatic duodenostomy. Mortality rate in pancreatic enterostomy and pancreatic duodenostomy was 2.5% whereas mortality rate in resections was 5.3%. Complications were noted in 27.5% and 36.8% of patients, respectively. Pain was completely abolished or markedly reduced in 90% of patients. Forty eight (77.4%) out of 62 patients were identified and followed up. According to commonly used criteria, positive late results were achieved in 56.2% of patients, moderate in 29.2%, and poor result in 14.6% of the treated patients.

It is supposed that the excess of fatty tissue exerts a diabetogenic effect. In obese subjects changes in the reactivity of the enlarged adipocytes to insulin might play a significant role in the pathogenesis of diabetes. Using the method of Rodbell in our own modification the responses of isolated adipocytes obtained from 10 lean and 15 obese subjects to a) insulin, b) theophylline and c) insulin and theophylline jointly (metabolism of glucose and glycerol) were determined. The dose-effect relationship curve was plotted against the effect of glucose utilization caused by increasing concentrations of insulin in the suspensions of adipocytes of lean and obese subjects. In adipocyte donors insulin sensitivity was also determined by Himsworth's test. It was found that adipocytes of 15 subjects with hyperthrophic obesity showed a significantly decreased effect of insulin regulating glucose utilization and glycerol release. They maintained a normal pattern of response to theophylline. The curve of relationship between insulin concentration in the incubation medium and its effect on glucose utilization by the adipocytes was changed. The adipocytes of these donors were relatively refractory to insulin in vivo as determined by the test of Himsworth. The cause of these disturbances may lie in the changed function of insulin receptors in the enlarged adipocytes. The observed disturbances may play a significant role in the pathophysiology of the diabetogenic effect of obesity.
