STATEMENT OF THE PROBLEM: Oral mucositis (OM) is a common inflammatory complication consequent to chemotherapy or radiotherapy in cancer patients that affects their quality of life. Therefore, finding an effective treatment for OM is always imperative. PURPOSE: The purpose of this experimental study was to measure the stereological, anti-inflammatory and antioxidant markers of hydro-alcoholic extract of Pistacia Atlantica (P. atlantica) leaves, Trachyspermum Ammi (T. ammi) fruits and their mixture on induced OM in hamsters. MATERIALS AND METHOD: In Experimental study, OM was generated by the method employed by Tanideh et al., in 90 hamsters, by 5-FU (5-fluorouracil 65 mg/kg, IP). All the animals were divided into 5 groups: control, topical gel base, hydro-alcoholic extract 3% of P. atlantica, hydro-alcoholic extract 1% of T. ammi, and a mixture of the extracts of the two plants. On days 13, 15, and 17, the cheek pouch samples were harvested. Stereological scores and the levels of malondialdehyde (MDA), Superoxide dismutase (SOD), myeloperoxidase (MPO), and interleukin-1B (IL-1B) in the pouch tissue were measured. RESULTS: Lower levels of MPO, MDA, and IL-1B and an increase of SOD level were discovered in the mixture-treated group in comparison with other groups. As an aspect of MDA, no significant differences were detected between the topical gel form of P. atlantica and T. ammi groups on days 13 (p= 0.648), 15 (p= 0.981) and 17 (p= 0.540). No variations of MPO were detected among P. atlantica and T. ammi groups on days 13 (p= 0.159) and 15 (p= 0.694); however, the MPO activity of the T. ammi group was significantly lower than the P. atlantica group on day 17 (5.57±0.56 vs. 6.48±0.35 U/ml, p< .001). The volume density of the epithelium increased in the mixture- treated group (p< 0.05). CONCLUSION: The results of this study indicated that the mixture of P. atlantica leaves and T. Ammi fruits might be effective in the treatment of OM.

OBJECTIVE: Pulp regeneration within the root canal of necrotic teeth is considered an ideal treatment to allow for continued root development and recover teeth vitality. This study aims to evaluate the inductive effect of platelet-rich plasma (PRP) on expression of angiogenesis factors and pulpal revascularization of immature necrotic teeth. MATERIALS AND METHODS: In this experimental animal study, we randomly divided 28 immature premolars from two mixed breed dogs into four groups, two experimental, negative and a positive control. Premolars in negative control group were left intact to develop normally. In the positive control and experimental groups, we removed the pulps and induced pulp necrosis, after which the chambers were sealed. Then, we applied the revascularization protocol in the experimental teeth located in the right quadrant. Two months later, the same protocol was applied to the left quadrant. The root canals were disinfected by irrigation with sodium hypochlorite (NaOCl) solution and application a triple antibiotic past. Following the induction of a blood clot (BC) inside the canal space, the coronal portion of the canals was assigned to either of two experimental groups: group 1 [BC+PRP+ mineral trioxide aggregate (MTA)], group 2 (BC+MTA). Access cavities were sealed with a Glass Ionomer. The jaws that held the teeth were processed for histologic analysis of newly formed tissue and immunohistochemical evaluation according to vascular endothelial growth factor (VEGF) and factor VIII expressions in the canals. RESULTS: Histological analysis demonstrated no significant difference in the formation of new vital tissue inside the root canals between groups1 (42.8%) and 2 (43.5%, P>0.05). Based on immunohistochemical evaluation, micro-vessel density (MVD) of the granulation tissues in both groups were similar and were higher compared with the normal pulp. We observed strongly positive expressions of VEGF and factor VIII in the stromal and endothelial cells, with severe intensity after one month. Both factors showed downregulation at three months postoperative. CONCLUSION: PRP could not increase the formation of new vital tissue. The immunohistochemical results showed that VEGF and factor VIII played a pivotal role in the formation of new vessels inside the root canals of immature, non-vital teeth.

Introduction: The aim of this study was to determine the histologic effect of low level laser therapy (LLLT) with or without the use of mineral trioxide aggregate (MTA) on exposed pulp tissues of dogs. Methods: Twenty-five premolar teeth in three healthy mature dogs were randomly divided into five groups. In group 1, the pulp tissue was capped with MTA. In groups 2 and 3, before capping with MTA, the exposure site was irradiated by low power lasers at 630 nm (20 mW, 150 seconds, 7.5 J/cm2) and 810 nm (peak power 80 W, average power 50 mW, 1500 Hz, 50 seconds, 6.25 J/cm2) wavelengths, respectively. In groups 4 and 5, the exposure site was irradiated similar to that described in groups 2 and 3, but the pulp tissue was covered by gold foil instead of MTA. After 2 months, the animals were sacrificed and the samples were prepared for histologic evaluation. Results: There were differences in pulpal response and dentinal barrier formation among the study groups. The morphology of pulpal tissue and the integrity of dentinal barrier and formation of odontoblastic layer were more favorable in the first three groups. The occurrence of extensive and intense inflammation and necrosis was more frequent in groups 4 and 5. Conclusion: Under the conditions used in this study, the presence of MTA as a pulp capping material was more important than laser therapy in the success of direct pulp capping (DPC) treatment. MTA proved to be an effective material either alone or in combination with laser irradiation in vital pulp therapy.

INTRODUCTION: Disinfection and subsequent obturation of the root canal space require adequate mechanical enlargement of the canals. The purpose of this in vitro study was to compare the shaping ability of Mtwo, RaCe and Medin rotary instruments during the preparation of curved root canals. MATERIALS AND METHODS: Sixty mesiobuccal root canals of mandibular molars with severe curvatures between 25-35(°) and radius of 4-9 mm were randomly divided into three groups of 20 canals each. Using pre- and post-instrumentation radiographs, straightening of the canal and the apical transportation were determined with AutoCAD software. The data were analyzed using Chi square, analysis of variance, and post-hoc tests and the significance level was set at P<0.05. RESULTS: Mtwo instruments maintained the canal curvature significantly better than Race and Medin instruments (P<0.05). There was significant difference between the rotary instruments for iatrogenic transportation of the major foramen (P>0.05). CONCLUSIONS: Under the conditions of this in vitro study, Mtwo instruments seemed superior to the two other rotary instruments.

BACKGROUND: The aim of this study was to evaluate the antimicrobial activity of Mineral Trioxide Aggregate (MTA) and Calcium Enrich Mixture) CEM (mixed with different concentrations of chlorhexidine (CHX). MATERIALS AND METHODS: Cements used in this in vitro study included Gray proRoot MTA and CEM with the microorganisms being entrococcus faecalis, streptococcus muntas, Candida albicans, Actinomyces, Escherichia coli, and a mixture of these microorganisms. CHX was used in the form of liquid at 0.2%, 2%, and 0.12% concentrations. Contact dilution and colony count method was used to evaluate the antibacterial activity of these cements. After 0, 24, 48, 72, and 96-hour intervals, we cultured the samples on blood agar medium. Colonies were counted after incubation at 37°. Data were statistically analyzed by a Kruskal-Wallis test to compare the antimicrobial activity of MTA and CEM. RESULTS: All concentrations of CHX were mixed with MTA and the CEM had antibacterial activities on all microorganisms' strains except for the Enterococcus faecalis and the mixture group. MTA had better antibacterial activity than the CEM, but this difference was not significant (P = 0.13). The mixing of MTA and the CEM with CHX significantly increased the antibacterial properties of both cements (P < 0.03). There was no statistically significant difference between the different concentrations of CHX. The antibacterial activity of the materials increased through time. CONCLUSION: The mixture of MTA and CEM with different concentration of CHX significantly increased the antibacterial activity.

A hermetic seal of the root canal space following canal preparation is important in endodontics. This study evaluated the effect of canal curve on adaptation of gutta-percha to dentinal walls of canals obturated using the Herofill system. For this in vitro study, 80 mesial roots of mature human first molars with length of 16 mm, curve between 5° and 45°, and no caries or resorption of the root surface were selected. A cone beam computed tomography system was used to evaluate the presence or absence of a gap in the samples. Photographs were taken in three sections: 2 mm above the curve, at the curve and 2 mm below the curve. The gap area was identified using Photoshop and AutoCAD software. Kruskal Wallis and Mann-Whitney tests were used to analyze the data mounted in a castcurve. There were significant differences between the two lateral condensation groups with various curves; roots with more curve had more gaps. When the Herofill and lateral condensation groups were compared without considering the sections and curve, there was a significant difference between these groups and the Herofill group had less gaps. The Herofill system is an alternative to the lateral condensation technique in severely curved canals.

INTRODUCTION: Open-apex teeth with irreversible pulpitis require complex and difficult treatment. Providing the right environment for apexogenesis and pulp protection is vital for their long term prognosis. The young pulp of open apex tooth, however, is better equipped against irritation and assault. The aim of this study was to evaluate pulp inflammation in open apex canine teeth of cats. MATERIALS AND METHODS: In this in vivo study, twelve cats with open apex canines were used. Pulps were iatrogenically exposed and the animals were sacrificed at one, seven, thirty and ninety days after pulp exposure. Samples were prepared for histological evaluations. RESULTS: During the first and seventh day, changes were limited to acute inflammation in the coronal pulp. During the first month pulp changes in 45.5% of samples were similar to the seventh day. In the other samples necrosis and abscess spread to the end of the root, and internal resorption and periapical abscess were observed. In 45.5% of samples in the apical region vital tissue was barely observed (during 90 days); in 54.5% of samples however, complete pulp necrosis, internal resorption and a large periapical lesion was observed. CONCLUSION: In one and seven-day periods pulp tissue was vital, however, in the thirty and ninety-day periods, minority of the pulp samples were vital.

INTRODUCTION: The aim of this study was to compare centering ability and dentin removal of three rotary systems in curved root canals of extracted teeth. MATERIALS AND METHODS: Sixty root canals of mandibular first molars with curvatures ranging between 25-35(o) were divided into three groups of 20 teeth each. Based on pre-instrumentation radiographs that assessed the angle and the radius of canal curvatures, teeth with curvatures were equally spread between the three groups. The root canals were sectioned horizontally at two levels before preparation and then remounted onto the muffle. All root canals were prepared using a low-torque control motor with Mtwo or Medin or Race instruments. Cross sectional images were obtained before and after instrumentation. Cross-sectional area and centering ability were evaluated. The data were analyzed using the one-way ANOVA and Tukey tests. RESULTS: Neither instrument fracture nor permanent deformation occurred during preparations. The best centering ability was obtained by Mtwo instruments compare to Race and Medin instruments. In the coronal and middle sections, Mtwo removed less dentin than Race and Medin; while the difference in the apical section was not significant. CONCLUSION: Under the conditions of this study, the debridement of root canals was more conservative with Mtwo. The canals prepared with these instruments were better centered in all three regions of the root.

RESUMO

Forced eruption can be performed in teeth with caries, fracture, resorption or perforation in the cervical third of the root or isolated teeth with one- or two-walled vertical periodontal defects. The purpose of this case report is to introduce an innovative orthodontic appliance which enables forced eruption. This appliance is easy to fabricate, cost-effective and very effective in forced eruption of non-restorable teeth.

INTRODUCTION: Traumatic injuries may affect the vitality and development of tooth. So divergence or parallelism of canal walls induces an open apex which is very susceptible to fracture. The aim of this study was to determine the stress distribution pattern in immature teeth and compare it with mature teeth. MATERIALS AND METHODS: In order to analyze stress distribution using finite element method, models were first designed according to actual samples by "ANSYS" software. Two models of the maxillary central incisors were designed, one mature and one with open apex. Mature tooth was designed as having undergone root canal therapy with gutta-percha and the immature one with both MTA plug and gutta-percha. Samples were loaded in seven stages and then, the stress distribution in each model was measured, using the "ANSYS" software. RESULTS: During gutta-percha condensation, the immature tooth transfers the stress directly to the external root surface where force is directly applied, whereas during masticatory occlusal forces, the stress is transferred to middle third of buccal and lingual surfaces and to buccal cervix in lunar shape. During traumatic forces, stress concentration was on the cervical region of buccal surface and middle third of buccal and lingual surfaces. MTA plug prevents stress distribution toward apical region and forces concentrate in dentin at the point where stress terminates. CONCLUSION: Within the limitations of this simulated mechanical analysis, it was confirmed that the pattern of stress distribution in mature and immature teeth is different. Cervical area of the buccal surface in immature teeth is one of the stress concentration areas, which contribute to the high rate of fracture in this area.

INTRODUCTION: Failure to achieve adequate and profound anesthesia in teeth with acute pulp inflammation is a common condition during emergency visits in root canal therapy. Many different anesthetic solutions such as morphine and capsaicin have accordingly been examined. Mannitol- an alcoholic sugar with high osmotic pressure level- is applicated for reducing intracranial and post retinal pressure in medicine. It has also been used for its diuretic effect. In combination with local anesthetic solution, it increases permeability of the nerve fiber sheath and leads to influx of the local anesthetic through cytoplasmic membrane .The purpose of the present study was to compare the efficacy of routine local anesthesia with or without using mannitol in teeth with inflamed pulps. MATERIALS AND METHODS: one hundred patients with acute dental pain in posterior teeth were selected. Vials with 3 ml anesthetic solution containing 2.5% lidocaine with 1/80000 epinephrine or 2.5% lidocaine with 1/80000 epinephrine and 0.5 mol mannitol were used for anesthesia. For each patient, the routine injection technique was applied, during the removal of decay and dentine. Depth of anesthesia was evaluated and the supplementary injection was done in case of pain feeling and then pulpotomy was done. The analysis of data was done using chi-square statistical test. RESULTS: The results showed that complete anesthesia after the first injection was obtained with lidocaine mannitol in 46% and with lidocaine alone in 38% of cases. However, the difference was not significant. CONCLUSION: These finding suggest that the addition of mannitol to the standard anesthetic solution could insignificantly increase the level of anesthesia in teeth with inflamed pulps.
