RESUMO

PURPOSE: The foveal function of patients with retinitis pigmentosa (RP) has been estimated by visual acuity (VA) or visual field (VF) tests. In the present study, the potential of optical coherence tomography (OCT) and focal electroretinogram (fERG) for monitoring macular function in RP patients was investigated. DESIGN: Cross-sectional observational study. METHODS: A total of 56 eyes of 56 patients with RP underwent ophthalmic examination including VA, VF, fERG, and OCT. Patients were morphologically divided into three groups by the appearance of photoreceptor inner/outer segment junction (IS/OS) that were depicted with OCT; type 1: no IS/OS visible, type 2: IS/OS was visible but the length was < or =2 mm, and type 3: IS/OS >2 mm was confirmed. Functional results for VA and fERG were compared and analysed based on the three groups. RESULTS: The average VA of type 1 patients was significantly lower than that of types 2 or 3 patients (P<0.001). There were no significant VA differences detected between types 2 and 3 patients. While most of the type 1 patients (21/22) showed non-recordable fERG, 3 out of 18 type 2 patients and none of type 3 patients showed non-recordable fERG. Significant differences of the fERG amplitudes were observed among the three groups (a-wave, b-wave, and OP, P<0.001 in all three components). However, the implicit time showed no difference between type 2 and 3. CONCLUSIONS: Analysing the IS/OS with OCT and the amplitudes of fERG may be helpful for monitoring RP patients in addition to VA and VF.

BACKGROUND: Little information is available on the kinematics of the normal knee in deep flexion. The purpose of this study was to use magnetic resonance imaging to analyze the patellofemoral articulation in deep flexion. METHODS: Axial scans were made of the patellofemoral joint of twenty healthy Japanese volunteers with the knee in approximately 90 degrees of flexion, in maximum active flexion (mean [and standard deviation], 140 degrees +/- 10 degrees ), and in maximum passive flexion (mean, 156 degrees +/- 5 degrees ). A fat-suppressed, three-dimensional, fast low-angle shot sequence was used to visualize the articular cartilage. The patellofemoral contact area was determined on sequential images and was reconstructed three-dimensionally. RESULTS: At 90 degrees of flexion, the contact area on the patella was continuous over the medial and lateral facets in fourteen knees and was located in the proximal half of the articular surface. At maximum active and passive flexion, the odd facet engaged in fifteen and eighteen knees, respectively. At maximum passive flexion, the contact area of the lateral facet moved distally and decreased significantly (p = 0.0002). From 90 degrees of flexion to maximum active flexion, the mean total contact area remained constant (3.43 +/- 0.70 and 3.62 +/- 0.72 cm (2), respectively); it then decreased significantly in maximum passive flexion (2.96 +/- 0.78 cm (2), p = 0.04). CONCLUSIONS: The contact area on the patella was divided into two parts (the odd and lateral facets) and moved distally in deep knee flexion. The size of the contact area on the lateral facet significantly decreased in maximum passive flexion.

Cerebral blood flow (CBF) during silent verb generation was measured at four Japanese PET centers. To minimize the variance of the measurement, speakers of a single language (Japanese) served as subjects and experimental conditions at the four PET centers were controlled as much as possible. Two types of activation patterns were observed: activations in the left dorsolateral prefrontal cortices and the medial frontal cortex (at the two centers with a 2D PET scanner) and additional activation in the left posterior temporal cortex (at the two centers with a 3D scanner). This suggests either a difference in the sensitivity of the two types of PET scanners (viz., a 3D scanner is generally more sensitive than a 2D scanner) and/or subject bias due to the small number of subjects at the individual centers. The pooled activation pattern was fundamentally similar to activation patterns obtained in the previous studies for verb generation in English and other European languages, suggesting that regions for verb generation are independent of particular languages. Regions relevant to verb generation are discussed.

We have created images of z value, error, and variation components for a PET activation study using various ANOVA designs and anatomical standardization methods. Data were acquired in four PET centers. In each center, CBF was measured on six normal male subjects under resting and covert verb generation, three times for each. The images were anatomically standardized with LINEAR transformation, SPM (Ver. 95), HBA (Karolinska/Tohoku), or MICHIGAN (Minoshima). ANOVA was performed pixel by pixel to compute t (and z) for the task main effect (Verb vs Rest) in four different designs: (i) two way (subject and task) (2W), (ii) two-way with interaction (2WI), (iii) subject considered a random factor (2WI-MX), and (iv) three-way (subject, task, and replication) (3W). A large area extending from the Broca to the left premotor cortex was activated. The localization of the highest peak depended both on the anatomical standardization and on the ANOVA design, the variation ranging 3-4 cm. Smoothing reduced the variation while erasing possible subfoci. The z images of 2W, 2WI, and 3W looked alike, whereas 2WI-MX presented lower peak z values. SPM tended to present higher z values than the other methods. The error was high in the gray and low in the white matter. The root mean square for the subject effect was high on the border of gray matter especially in LINEAR and HBA, revealing intersubject mismatch in the gray matter distribution. The root mean square for the subject-by-task interaction effect revealed individual variation in activation.

Correspondences between spelling and sound for Japanese kanji are complex and deep. The meaning of kanji words has generally been assumed to be accessed directly from orthography without phonological mediation. Experiment 1, however, replicated the findings of Van Orden (1987) that subjects made more false-positive errors on homophone foils than they did on nonhomophone controls in a semantic decision task, although they did so only when the foils were orthographically similar to the correct exemplars, which indicates both orthographic and phonological activations of meaning. Experiment 2 showed the same results when subjects were not required to pronounce the target words after semantic decisions, which indicates automatic phonological activation of kanji words. In Experiment 3, under pattern-masking conditions, this homophony effect was reduced but remained on errors, and the orthographic-similarity effect remained strong on both homophone and nonhomophone foils. These results suggest that both orthography and phonology play an important role in the comprehension of kanji words.

RESUMO

This study examines identification of the synthetic speech stimuli [ga] and [ka] of varying voice onset times (VOTs) in 17 Japanese aphasic subjects. Approximately two-thirds of the aphasic subjects showed deterioration in performance of VOT identification of the stimuli. A quantitative analysis of the results indicates that the problems of the majority of these subjects may be attributable not only to difficulty in labeling the stimuli but also to difficulty in processing the acoustic and/or phonetic information of the stimuli.
