RESUMO

The paper describes an investigation of phase decomposition of apatite lattice doped with rare earth ions (cerium, samarium, and holmium) at temperatures ranging from 25 to 1200 ºC. The rare-earth ion-doped apatite minerals were synthesized using sol-gel method. In situ high-temperature powder X-ray diffraction (XRD) was used to observe phase changes and the lattice parameters were analyzed to ascertain the crystallographic transformations. The expansion coefficient of the compounds was determined, and it was found that the c-axis was the most expandable due to relatively weak chemical bonds along the c-crystallographic axis. Differential scanning calorimetry (DSC) and thermogravimetric analysis (TGA) were used to examine the decomposition properties of the materials. Due to rare earth ion doping, the produced materials had slightly variable decomposition behaviour. The cerium and samarium ions were present in multiple oxidation states (Ce3+, Ce4+, Sm3+, Sm2+), whereas only Ho3+ ions were observed. Rare earth ion substitution affects tri-calcium phosphate proportion during decomposition by regulating concentrations of vacancies. X-ray photoelectron spectroscopy (XPS) analysis indicated that cerium and samarium ion-doped apatite yielded only 25% tricalcium phosphate during decomposition. This finding advances our understanding of apatite structures, with implications for various high-temperature processes like calcination, sintering, hydrothermal processing, and plasma spraying.

PURPOSE: We aimed to assess the early outcome following revision arthroplasty for failed metal-on-metal (MoM) total hip replacements (THR) due to adverse reaction to metal debris (ARMD). METHODS: We reviewed 106 consecutive revision arthroplasties. Case notes and radiological investigations were reviewed to assess the complications. Oxford Hip Score (OHS) and Euroqol (EQ-5D-3L) scores were used to assess the functional outcome and improvement of quality of life. RESULTS: At a mean follow-up of 20 months (12-48 months), the mean OHS was 28.7. Pain improved in 61% patients. A majority of patients were in level 2 for all the EQ-5D-3L dimensions. The overall complication rate was 16%. Survivorship free from further revision for any cause was 94.3% at 48 months. There was no correlation between pre-revision blood metal ions and the final outcome. CONCLUSIONS: Revision surgery for failed MoM hip replacement due to ARMD is associated with a relatively higher rate of complications and risk of chronic pain. There is poor correlation between serum metal ions and development of ARMD and outcome following revision surgery.

BACKGROUND: Analysis of exhaled breath condensates (EBC) is a non-invasive technique to evaluate biomarkers such as antioxidants in the pediatric population, but limited data exists of its use in intubated patients, particularly newborns. Currently, tracheal aspirate (TA) serves as the gold standard collection modality in critically ill newborns, but this method remains invasive. We tested the hypothesis that glutathione status would positively correlate between EBC and TA collections in intubated newborns in the Newborn Intensive Care Unit (NICU). We also hypothesized that these measurements would be associated with alveolar macrophage (AM) glutathione status in the newborn lung. METHODS: Reduced glutathione (rGSH), glutathione disulfide (GSSG), and total GSH (rGSH + (2 X GSSG)) were measured in sequential EBC and TA samples from 26 intubated newborns via high performance liquid chromatography (HPLC). Additionally, AM glutathione was evaluated via immunofluorescence. Pearson's correlation coefficient and associated 95% confidence intervals were used to quantify the associations between raw and urea-corrected concentrations in EBC and TA samples and AM staining. Statistical significance was defined as p ≤ 0.05 using two-tailed tests. The sample size was projected to allow for a correlation coefficient of 0.5, with 0.8 power and alpha of 0.05. RESULTS: EBC was obtainable from intubated newborns without adverse clinical events. EBC samples demonstrated moderate to strong positive correlations with TA samples in terms of rGSH, GSSG and total GSH. Positive correlations between the two sampling sites were observed in both raw and urea-corrected concentrations of rGSH, GSSG and total GSH. AM glutathione staining moderately correlated with GSSG and total GSH status in both the TA and EBC. CONCLUSIONS: GSH status in EBC samples of intubated newborns significantly correlated with the GSH status of the TA sample and was reflective of cellular GSH status in this cohort of neonatal patients. Non-invasive EBC sampling of intubated newborns holds promise for monitoring antioxidant status such as GSH in the premature lung. Further studies are necessary to evaluate the potential relationships between EBC biomarkers in the intubated premature newborn and respiratory morbidities.
