RESUMO

The aim of the work was a comparative study of Tocilizumab and Thymalin effects on the morphological composition and indicators of the blood clotting system in COVID-19 of middle aged and elderly patients. Severe COVID-19 patients were divided into 3 groups: 1st - control (basic therapy), 2nd - basic therapy +Tocilizumab, 3rd - basic therapy +Thymalin. Hospital mortality in COVID-19 patients after standard therapy, Tocilizumab and Thymalin application was 40,9; 28,4 and 20,6% accordingly. The number of platelets increased by 1,5 times, the concentration of fibrinogen in blood decreased by 78% and activated partial thromboplastin time decreased by 9,3% in patients taking Tocilizumab. Under the influence of Tocilizumab, the platelet/white blood cell and platelet/lymphocyte ratios increased by 1,6 and 1,4 times, which may be a predictor of an unfavorable outcome of COVID-19. Thymalin increased the number of lymphocytes and monocytes by 2 times, the number of leukocytes - by 1,3 times, the number of platelets in the blood - by 1,5 times. Thymain decreased the platelet/lymphocyte and neutrophil/lymphocyte ratios by 1,4 times and 2 times. The use of Thymalin decreased the level of fibrinogen, lactate dehydrogenase and D-dimer in the blood by 1,2; 1,8 and 1,7 times, respectively. Thymalin, compared with Tocilizumab, meets the principles of pathogenic therapy for severe COVID-19 of middle aged and elderly patients to a greater extent, having a normalizing effect on the morphological composition and indicators of the blood clotting system.

OBJECTIVE: To study plasma adipokines (adiponectin, leptin, adipsin) in patients with the first episode of schizophrenia treated with haloperidol and risperidone. MATERIAL AND METHODS: Two hundred and twelve patients with the first episode of paranoid schizophrenia (F20.09) were examined. Patients were divided into the haloperidol group (n=107) and the risperidone group (n=105). The control group included 132 healthy people. Body mass index was recorded at baseline and in 2, 4, 6, 8 week of treatment. Adiponectin, adipsin and leptin were determined by the multiplex analysis. Blood test was conducted prior to therapy and after 8 weeks of treatment. RESULTS: An increase in body mass index is recorded in both clinical groups, with no statistically significant differences between groups. Before treatment with antipsychotics, the levels of adiponectin and adipsin increase in both clinical groups. In the course of therapy, the levels of adiponectin and adipsin significantly increase in the haloperidol group and the levels of adiponectin and leptin in the risperidone group. CONCLUSION: The increase in body weight is not the only mechanism of changes in adipokines in blood during treatment with antipsychotics. Further studies on the relationship between the levels of adipokines and pharmacogenic metabolic disorders are required.

In present study we performed gas-liquid chromatographic analysis of exhaled breath condensate to measure volatile fatty acids (C2 - acetic, C3 - propionic, C4 - butanoic, isoC4 - isobutyric, C5 - valerianic, C6 - caproic, C7 - heptanoic) and fatty acid with a long aliphatic chain (C14:0 - myristic, C15:0 - pentadecanoic, C16:0 - palmitic, C16:1 - palmitooleic, C17:0 - heptadecanoic, C17:1 - heptadecenoic, C18:0 - stearic, C18:1 - oleic, C18:2 - linolenic, C18:3ω3 - α-linolenic, C20:4ω6 - arachidonic) in patients suffering from moderate chronic obstructive pulmonary disease (2nd stage, GOLD). We revealed the increase of the total amount of short chain fatty acids (C2, C3, C4, C5) and polyunsaturated (C18:2, C20:4ω6) fatty acids, meanwhile the level of saturated fatty acids (C16:0, C17:0, C18:0) decreased.

OBJECTIVE: The aim of the study was to evaluate the content of the vitamin D metabolite (25(OH)D3) in the blood serum, the level of some indicators of the immune and peroxidic status in the oral fluid in people with carious processes of different intensities while taking the native solution of vitamin D (international non-proprietary name Cholecalciferol). MATERIAL AND METHODS: The study included 47 people aged 20 to 22 with caries and low levels of 25(OH)D3 in blood serum. The control group consisted of 13 people with DMFT index of 0.00 [0.00; 0.00] and a normal serum content of 25(OH)D3. The amount of antimicrobial peptide cathelicidin LL-37, secretory immunoglobulin A, lipopolysaccharide binding protein (LBP) was determined in the oral fluid using ELISA reagent kits, and the content of intermediate lipid peroxidation products such as malondialdehyde (TBA-active products) and the level of total antioxidant activity were spectrophotometrically evaluated. RESULTS: Reduced serum concentrations of 25(OH)D3 were recorded predominantly in individuals with a DMFT index of more than 9.0. In patients with caries, at a concentration of 25(OH)D3 in blood serum below 30 ng/ml, a decrease in the values of secretory immunoglobulin A, lipopolysaccharide-binding protein, cathelicidin, and the level of total antioxidant activity was observed, accompanied by an increase in the number of intermediate products of lyoperoxidation in the oral fluid. The course intake of the native AquaDtrim vitamin D solution (Cholecalciferol) led to normalization of the values of certain immune and biochemical parameters (cathelicidin LL-37, secretory immunoglobulin A, LBP of the oral fluid) and restoration of balance in the system «lipid peroxidation - antioxidants¼. CONCLUSION: The results indicate the advisability of assessing the level of 25(OH)D3 in the body for the diagnosis of pathological processes in the oral cavity and the use of vitamin D preparations in the prevention and complex therapy of carious process.

AIM: To study the content of circulating endothelial cells in the blood of patients with alcohol withdrawal syndrome (uncomplicated and complicated delirium). MATERIAL AND METHODS: Thirty patients, aged from 18 to 40 years, with the diagnosis of 'alcohol withdrawal, uncomplicated' (F10.302) or 'alcohol withdrawal complicated by delirium' (F10.40) were examined in the acute state and after 2 weeks of therapy. The control group consisted of 22 normals. The content of circulating endothelial cells (CEC) in the blood was measured by flow-cytometry. RESULTS AND CONCLUSION: Significant changes in the CEC content were observed in alcohol withdrawal syndrome: a multiple increase in the number of CECs due to the CEC-Living subpopulation, the appearance of CEC-Dead, which characterized the pronounced damage to the endothelium; the compensatory growth of ECP (CD146+CD34+) and AEC (CD146+CD105+). Patients with uncomplicated abstinence after 2 weeks of therapy showed positive changes in the form of a decrease in CEC level due to the CEC-Living. The CEC level remained high in patients with alcohol delirium after 2 weeks of treatment, the decrease in the content CEC-Living with a significant growth CEC-Dead and a slight compensatory increase in ECP (CD146+CD34+) was observed.

Oral fluid is a unique biological environment, containing a wide range of substances, coming from local and systemic sources, which makes it possible to use it as an object for assessing pathological changes in the body both at the local and systemic levels. In comparison with the traditional method of blood analysis, the advantage of evaluating the parameters of the oral fluid is the non-invasive of this method of obtaining material. All patients underwent oral fluid sampling using special plastic containers with a swab, which facilitate the selection of material, eliminating the penetration of mucin into a clean test sample, which helps to obtain more accurate analysis results. The amount of secretory IgA, lipopolysaccharide-binding protein (LBP), TBA-active products, the level of total antioxidant activity in the oral fluid in individuals with a low level of 25(OH)D before and after taking the native solution of vitamin D "Aqua Trim" were determined. The concentrations of secretory immunoglobulin A, lipopolysaccharide, binding protein and the level of total antioxidant activity are reduced in the oral fluid of people with vitamin D deficiency, but the number of intermediate products of lyoperoxidation increases. The course intake of the native solution of vitamin D (International Nonproprietary Name - Colecalciferol) normalizes the functioning of the immunity of the oral cavity and restores the balance of the "lipid peroxidation-antioxidants" system.

To investigate the consequences of the endothelium dysfunction caused by the experimental hyperhomocysteinemia. Methods: The experiment included the similar aged 42 white non-pedigree male rats with average weight of 150 grams. The rats were divided in two groups each of these having 21 species. Hyperhomocysteinemia was induced in experimental rats by intraperitoneal injection of 0,1 mmol homocysteine per 1 gr of weight once a day during 14 days. The equal intraperiotenal dose of the physiological solution was injected to the rats of the control group. The level of homocysteine was determined in the blood serum with high-yield liquid chromatography (HYLC), the content of IFNg, TNFa, IL-17A с was measured with the system of multiplex analysis FlowCytomix 5 plex (BMS826FF) combined with Simplex Kit (BMS8635FF) of appropriate analyts for «Bender Medsystems¼ rats (Austria). Coagulation indices were determined in the blood plasm of subclavian vein. TF expression of tissue factor was estimated with immune histochemical method. Results: Fivefold increase of endothelin level and decrease of nitrates and nitrites levels were found in rats with hyperhomocysteinemia. Expression of tissue factor, shortening of activated partial thromboplastin and thrombin times, increase of soluble fibrin monomeric complex level, significant increase of tumor necrosis factor and interferon gamma levels were identified in endothelial cells of myocardial vessels. Conclusion: Thus, experimental hyperhomocysteinemia (HHC) proved to be accompanied by expressed endothelium dysfunction which is marked by thrombogenic activity of blood as well as endothelium and cytokine level increasing.

RESUMO

The article presents comparative evaluation of aggregative activity of blood corpuscles in norm and in patients with ischemic stroke derived using flow cytofluorometry and light microscopy. It is noted that in patients with stroke in peripheral blood absolute content of leukocyte thrombocyte and erythrocyte thrombocyte aggregates is drastically increased as compared with healthy examined persons. Besides, more the number of aggregates is formed during the first day of onset of disease more the severe cerebral stroke progresses.

RESUMO

The article considers the technique of simultaneous detection of free catecholamines and free metanephrines in urine using inverse phase highly effective liquid chromatography with fluorimetric detection. The solid phase extraction was implemented on cartridges with 30 mg of hyper cross-linked polystyrene (Purosep-200). The simplicity, reproducibility and sufficient sensitivity of technique permit applying it in clinical practice to diagnose pheochromocytoma.

RESUMO

Moderate exogenous hyperhomocysteinemia is associated with an increase in lymphocyte count (mainly at the expense of cytotoxic T cells), a 2-fold drop of the immunoregulatory index, and increase in TNF-α, IFN-γ, and IL-17α concentrations in the serum and in mononuclear cells. Homocysteine concentration in the mononuclears remains unchanged.

RESUMO

The sampling consisted of 98 women (average age 71.2 ± 8.6 years) with ischemic heart disease. The combination of ischemic heart disease and severe osteoporosis was diagnosed in 48 out of them. All female patients were analyzed for level of IL-1ß, TNF-α and their soluble receptors in blood serum. The technique of immune enzyme analysis was applied. It was revealed that in women with associated pathology the level of TNF-α was higher and concentration of soluble receptors reliably lower than in women with isolated ischemic heart disease. It was established that direct correlation relationship exists between occurrence of bone fractures, coronary atherosclerosis and level of TNF-α. The occurrence of bone fractures was inversely correlated with level of soluble receptors of TNF (γ = 0.3; p = 0.0029), and occurrence of ischemic heart disease with soluble receptors of IL-1.

The aim of this work was to study the qualitative and quantitative composition of circulating endotheliocytes, P-selectin-associated leukocyte and platelet of coogregation blood levels of endothelin, interleukin-6 students on the background of exam stress. The research was made on the group conditionally healthy volunteers aged MT 18.3 ± 1 years that before participating in the study had a medical examination (n = 15). Blood sampling was made three times: 1--three months before the session (in a normal school day, two hours after training sessions), 2--for 25 ± 10 min before the exam, 3 - 15 ± 10 min after the exam. Approved the development of endothelial dysfunction in the background of exam stress, manifested by increased levels of circulating endothelial cells and endothelin. The total number of leukocyte and platelet of coogregation during the semester, before and after the exam has not changed, while there was an increase outlets, due to the expression of P-selectin on red blood cells before the exam and precipitous decline after the exam.

RESUMO

The study was carried out to identify concentrations of vessel epithelium growth factor (VEGF), souble forms of VEGF receptor type I and II, chemo-toxic protein I and metalloproteinase-9 in lachrymal fluid. The immune enzyme analysis was applied. He sampling included patients with myopia without complications while using silicone hydro-helium and hydro-helium contact lenses and patients with hypoxic complications of contact correction. It is established that application of hydro-helium and silicone hydro-helium contact lenses more than one year is followed by increasing of concentration of detected substances in lachrymal fluid. Under hypoxic complications of contact correction the levels of substances are significantly higher than in cases without complications. This occurrence makes it possible to substantiate possibility of application of immunologic indicators as diagnostic markers.

RESUMO

Proposed modified HPLC method for determination of valproic acid in biological fluids. Created solid-phase extraction of valproic and heptanoic acids (internal standard, IS) on the cartridges packed hyper cross-linked polystyrene which maintain some tens extractions without losses of efficiency. Carboxylic acids are derivative with 1-(bromoacetyl)pyrene in acetone at presence of triethylamine. Chromatographic separation of derivatives is performed on Chromolith Perfonnance RP-18e columns, which packed unique monolithic sorbent. UV detection at 360 nm. Mobile phase acetonitrile - water (90:10, v/v) plus 1% isopropanol, speed flow 2000 microL/min, pressure 21 bar. Complete chromatographic cycle less than 3 minutes. Yield of IS and valproic acid (extraction plus derivatization) was 101-106%. Sensitivity (limit detection) was near 1 ng for valproic and near 0.6 ng for heptanoic acid during signal/noise ratio = 3.

RESUMO

The article deals with the results of analysis of specters of short-chained fat acids and cytokines in affected skin of patients with psoriasis. The study revealed the significant decrease of short-chained fat acids level, the shift of cytokine profile in the direction of anti-inflammatory factors (interleukins 1L-1beta, IL-8, tumor necrosis factor TNF-alpha, interferon IFN-alpha) and mytogenetic factors (EGF) on the background of stable values of anti-inflammatory cytokine IL-4. The direct pathogenically significant correlation relationships are established between the IL-4 level and the amount of most analyzed short-chained fat acids. The negative correlation relationships were established between content of C2 and IL-1beta.

The relative content of higher fatty acids (FA) was studied in lachrymal fluid of 10 virtually healthy subjects and of 9 patients with maculodystrophy aged 30 to 40. The FA methyl ethers were assayed with the "Chrystall-2000 M" gas chromatograph. The content of C(15:0), C(15:1), C(16:0) and C(16:1) as well as the content of such oversaturated FA as C(18:3omega6) and C(20:4omega6) was found to be decreased in patients with macoludystrophy. Simultaneously, a higher level of FA with a longer carbon chain (C(17:0), C(18:0), C(18:1), C(18:3omega3) and C(20:3omega6) was registered in these patients. The results testify to changes in the composition of lachrymal fluid in ophthalmopathy and point out the expedience of further research in the discussed field.
