RESUMO

Lipomas are common benign mesenchymal tumours, whereas lipomatoses are uncommon. Familial multiple lipomatosis (FML) is a rare syndrome characterized by multiple usually painless lipomas which may be associated with other conditions. FML is considered to be genetic, with various patterns of inheritance suggested. In this case report, we described a case of multiple familial lipomatosis that was misdiagnosed as dercum's disease.

RESUMO

We present an unusual case of placental chorangioma in a 32-year-old female with poor antenatal care. Abdominopelvic ultrasound revealed polyhydramnios with no fetal cardiac activity and suspected placental cyst. An emergency caesarean was performed, and she had still birth of male baby. The cut specimen of the placenta revealed a well-circumscribed marginal mass of 4 cm. Our case emphasizes the importance of regular antenatal screening for early detection of placental abnormalities. While chorangiomas are rare, they should be considered in the differential diagnosis of placental masses. Prompt diagnosis and appropriate management are essential to reduce the maternal and fetal complications associated with chorangiomas. Histological examination of the placenta plays a vital role in differentiating chorangioma from other placental abnormalities with different clinical implications.

Diabetes mellitus is one of the most debilitating diseases, diabetic neuropathy happens to be the most common and perhaps the most serious complication of diabetes mellitus, often leading to morbidity and mortality. A 60 year old female presented with disorientation, history of vomiting, shortness of breath, respiratory failure initially. Blood reports revealed that she was positive for ketone bodies with elevated HbA1c and general random blood sugar. Chest radiogram revealed atelectasis of the right lung with prominent involvement of right middle and lower lobes. High-resolution computed tomography of chest confirmed the findings and unilateral diaphragmatic paralysis due to phrenic nerve neuropathy due to undetected type 2 diabetes was diagnosed. Although phrenic nerve paralysis is a rare occurrence with diabetes, the possibility shouldn't be overlooked as a presentation of diabetes mellitus.

RESUMO

Cryptophthalmos is a rare congenital eye anomaly characterized by the absence of the palpebral fissure. Cryptophthalmos is often associated with Fraser's syndrome. We present a case of 3 days old female Asian neonate with complete unilateral cryptophthalmos, with the absence of a right eyelid. On inspection, there is an absence of eyelid, eyebrow and eyelashes in the right eye, collectively known as adnexal structures. The left eye was apparently normal. As per the parent's decision, surgical intervention was not pursued due to the poor visual prognosis. We advised prenatal genetic screening and testing for future pregnancies. These findings suggest the importance of genetic counseling and testing in cases of cryptophthalmos to identify potential genetic mutations and facilitate appropriate management.

RESUMO

The most common day-to-day antiseptic preparation used before any surgery usually involves povidone-iodine. Any irritant reaction to it can be devastating for the patient's aspect and would raise the need for a preliminary investigation prior to going through any antiseptic preparation. In literature, very few cases were reported on irritant dermatitis with povidone-iodine in Indian setting. We present a case of an 18-year-old female presented with irritant contact dermatitis due to povidone-iodine following a surgical intervention.

RESUMO

Intestinal obstruction is a common surgical emergency that requires prompt intervention. We present a case report of a 30-year-old male with recurrent intestinal obstruction due to sigmoid volvulus. Our case highlights the challenges associated with managing recurrent intestinal obstructions caused by adhesions following surgical interventions for sigmoid volvulus. It emphasizes the need for careful evaluation and meticulous surgical techniques to minimize the risk of adhesion formation and subsequent complications.

RESUMO

Aplastic Anemia (AA) is a condition where the body's immune system attacks and destroys the hematopoietic stem cells, leading to pancytopenia and an empty bone marrow. AA can be effectively treated using immunosuppressive therapy or hematopoietic stem-cell transplantation. The damage to stem cells in the bone marrow can be due to several causes, such as autoimmune diseases, medication like cytotoxic drugs and antibiotics, and exposure to toxins or chemicals in the environment. In this case report, we present the diagnosis and treatment of a 61-year-old man who was diagnosed with Acquired Aplastic Anemia, which could have developed following his serial immunizations with the SARS-CoV-2 COVISHIELD viral vector vaccine. The patient received immunosuppressive therapy consisting of cyclosporine, anti-thymocyte globulin, and prednisone, resulting in a significant improvement in his condition.

Dermoid cysts of the head and neck region are very rare, with about 7% occurrence and parotid being an extremely rare location. In this case report, we presented a case of a 23-year-old man with a recurrent parotid dermoid cyst and the clinical presentation and diagnostic difficulties are discussed.

Succinylcholine is a short-acting depolarizing neuromuscular blocking agent. We describe a case where the above drug was employed for self-harm by a health-care worker. The patient, a 28-year-old female, was brought to the emergency department (ED) in impending respiratory arrest and altered mental status. On arrival, she had hypoxia, bradycardia, and hypotension. Although the cause for rapid deterioration in this patient was unknown, the ED physician still went ahead by resuscitating the patient's airway, breathing, and circulation. During the course of resuscitation, information was received that an empty ampoule of succinylcholine was recovered from her bathroom. Further clinical examination and laboratory investigations led the treating physicians to suspect deliberate intravenous injection of succinylcholine. She was mechanically ventilated and monitored in the critical care unit. Targeted temperature management was initiated in the ED and was continued for 24 h. The patient was discharged from the hospital without any neurological deficits after 4 days. Patients with acute poisoning are one of the major encounters in ED, and this case highlights the possibility of anesthetic drug misuse in any health-care workers coming to the ED with sudden cardiac arrest, altered sensorium, or abnormal vitals. This is the first report describing the survival of a patient following intentional succinylcholine injection for self-harm.

BACKGROUND AND OBJECTIVES: Over the past few years, ultrasonography is increasingly being used to confirm the correct placement of endotracheal tube (ETT). In our study, we aimed to compare it with the traditional clinical methods and the gold standard quantitative waveform capnography. Two primary outcomes were measured in our study. First was the sensitivity and specificity of ultrasonography against the other two methods to confirm endotracheal intubation. The second primary outcome assessed was the time taken for each method to confirm tube placement in an emergency setting. METHODS: This is a single-centered, prospective cohort study conducted in an emergency department of a tertiary care hospital. We included 100 patients with indication of emergency intubation by convenient sampling. The intubation was performed as per standard hospital protocol. As part of the study protocol, ultrasonography was used to identify ETT placement simultaneously with the intubation procedure along with quantitative waveform capnography (end-tidal carbon dioxide) and clinical methods. Confirmation of tube placement and time taken for the same were noted by three separate health-care staffs. RESULTS AND DISCUSSION: Out of the 100 intubation attempts, five (5%) had esophageal intubations. The sensitivity and specificity of diagnosis using ultrasonography were 97.89% and 100%, respectively. This was statistically comparable with the other two modalities. The time taken to confirm tube placement with ultrasonography was 8.27 ± 1.54 s compared to waveform capnography and clinical methods which were 18.06 ± 2.58 and 20.72 ± 3.21 s, respectively. The time taken by ultrasonography was significantly less. CONCLUSIONS: Ultrasonography confirmed tube placement with comparable sensitivity and specificity to quantitative waveform capnography and clinical methods. But then, it yielded results considerably faster than the other two modalities.
