The article presents an overview of foreign publications reflecting approaches to the definition of the essence of the concept of «professionalism¼ in relation to representatives of the field of health and medicine. The main personal and professional qualities of the doctor, which are associated with the concept of «professionalism¼ in patients, are noted. It is noted that the formation of professional behavior is influenced not only by the personal qualities of a medical worker, but also by the working environment of the medical organization in which he carries out professional activities. It is concluded that the formation of the professionalism of a medical worker should be carried out with the participation of both the employee himself and the public health system and the system of medical professional education.

The article deals with organizational and managerial problems of projects in the field of medical charity. The development of the sector of non-profit organizations in the medical field today is due to the fact that the effective activity of the state is ensured by the stable development of the social sphere. The importance of charitable projects in the field of medicine is one of the important resources aimed at increasing the availability of medical care to needy categories of citizens around the world. Accessibility of medical care is achieved due to a complex of factors, among which a significant role is given to the capabilities of medical organizations themselves. Due to the fact that financial support has its limitations, charitable projects in the medical field become a resource that allows you to regulate the availability of medical care. The activities of charitable organizations in the medical field in modern conditions require the involvement of significant resources and the formation of an organizational and managerial mechanism for the development of projects in the field of assistance to those in need. The analysis of organizational and managerial aspects of the development of projects in the field of medical charity and the development of proposals for improving the activities of charitable organizations are the purpose of this work.

RESUMO

The article considers essential theoretical methodological approaches of studying problematics of social capital presented in corresponding modern publications. The key results of conducted empirical studies were analyzed too. It is noted that in conditions of spread of COVID-19 pandemic, occurs increasing of risk of "social distancing" that affects intensification of psychological anxiety feeling and decreasing of level of interpersonal and institutional trust. The presented results testify that topic of social capital becomes one of the most important in the study of processes of pandemic course. The conducted studies permitted to identify relationships between various components of social capital and scale of pandemic spread, forms of population's counteraction to pandemic. The review of scientific publications on problematics of social capital and functioning of organizations in conditions of intensification of risks of pandemic spread and developing crisis processes in economics also permits to establish particular trends in respect of developing corresponding strategies of adaption to new reality.

RESUMO

The article is devoted to the research of problems and tasks of state management under the conditions of threat of spread of coronavirus infection COVID-19 and consideration of approaches to optimization of processes in the system of state management. The social and economic risks of the pandemic are identified and the directions of anti-crisis measures are considered. Public administration is a system consisting of citizens, business structures and government employees. State authorities, while implementing the directions of public policy, must take into account the interests of all participants of the system, building the channels of interaction between them in the most effective way. The pandemic is a serious challenge to the state administration system of any country, the prevention of the consequences of which requires timely and effective measures to be taken. Under the conditions of the pandemic in Russia, as well as all over the world, the role of the state has significantly increased, both in terms of prevention of pandemic spread, and in the form of measures aimed at supporting citizens and businesses, the most affected spheres of the economy. The purpose of this work is to consider new approaches to optimize the processes in the system of public administration, caused by the need to revise the directions of state policy under the threat of the spread of COVID-19.

An important area of medical media is the preservation of health and a healthy lifestyle, the preservation and maintenance of adaptive properties of the body - the basis of content strategies of a large number of modern Russian media. To a greater or lesser extent and by virtue of their professional duties, it is the publications of doctors or interviews with them, as well as official research published in public and non-professional media, that popularize medical knowledge. The article focuses on the fact that the functions of popularizing health and a healthy lifestyle are inherent in the media, both special and for a wide range of the population. In addition to mass media that publish articles on preventive medicine and medicine of innovations and introduction of medical technologies, there are a number of mass media that cover medical knowledge for the population, in order to familiarize society with the achievements of medicine in the direction of maintaining health, maintaining a healthy lifestyle. This paper also concludes that all publications, all materials of Internet portals and messages, both in non-specialized and specialized media, most of the medical topics have a message of interpretation of the popularization of medical topics related to the preservation, maintenance of health and maintaining a healthy lifestyle. Popularization of medical knowledge in the media is an important direction for medical professionals and the public.

Inhibition of hydrolysis of palmitic and oleic triglycedires (TG) in very low density lipoproteins (VLDL), slow formation of active apoВ-100 conformation, blockade of апоЕ/В-100 ligand formation in VLDL and their reduced uptake by insulin-dependent cells cause hypertriglyceridemia (HTG). Palmitic and oleic VLDL (>80% total VLDL) are not converted in low density lipoproteins (LDL). Atherosclerosis is not an alimentary deficiency of polyenic fatty acids (PFA), but results from low in vivo bioavailability of PFA in LDL against the background of high dietary palmitic FA and palmitic LDL. Plasma PFA content and cellular PFA deficiency are as high as LDL cholesterol (CL). Primary prevention of atherosclerosis should be based on a decrease in dietary content of palmitic saturated FA, trans FA and a moderate increase in PFA. It seems highly unlikely that the xeobiotics statins, fibrates and probucol produce pleiotropic biological effects in vivo. These effects are brought about by phylogenetically early humoral mediators eicosanoids: prostacyclins, prostaglandins, thromboxanes, leukotrienes, and resolvins. It is reasonable to suggest that all preparations which act according to the same algorithm activate TG hydrolysis in VLDL and normalize cellular uptake of PFA in linoleic and linolenic LDL via apoВ-100 endocytosis. Atherosclerosis is a syndrome of cellular deficiency of essential polyenic FA.

The deficiency of energy substrates in the biological function of trophology and biological reaction of exotrophy is formed by two factors. Excess of meat in the diet leads to high content of palmitic fatty acid (FA) in hepatocytes and formation of palmitic triglycerides (TG). Post heparin lipoprotein lipase slowly hydrolyzes palmitic TG in blood plasma lipoproteins and releases small amounts of FA. If dietary carbohydrate content is low, the biological function of exotrophy does not provide the substrate from which hepatocytes can rapidly produce oleic nonesterified FA de novo. Energy substrate deficiency activates the biological function of adaptation and the biological reaction of compensation. Under the effect of epinephrin NEFA deficiency is compensated via the biological reaction of endotrophy and lipolysis in omental visceral fat cells. In insulin resistance (IR) syndrome, the biological function of feeding is realized nonphysiologically while the biological reaction of adaptation is realized physiologically. An increase in NEFA blood content physiologically blocks glucose uptake in cells. Biological role of insulin consists in conversion of distant ocean-living carnivorous (fish-eating) ancestors of Homo sapiens with palmitic type of FA metabolism into herbivorous dry land-living species with oleic type metabolism of FA. The IR syndrome can be normalized. To this end a) the patient's will to activate the cognitive biological function (intellect) and b) comprehension of the fact that phylogenetically dry land-living Homo sapiens has developed as a herbivorous but not carnivorous species. Concerning death rate, cardiovascular pathologies are dominating in populations of many countries, while feeding function disorders prevail in frequency. These disorders form the pathophysiological basis for all metabolic pandemias: 1) atherosclerosis and atheromatosis, 2) essential arterial hypertension, 3) metabolic syndrome, 4) obesity, 5) insulin resistance syndrome, 6) nonalcoholic fatty liver disease, and 7) endogenous hyperuricemia. Persistent potential deficiency of energy for realization of all biological reactions and functions is the major metabolic disorders in diabetes mellitus. Insulin resistance is a pathology associated primarily with FA and secondarily with glucose.

Physicochemical differences between О3 oxidation parameters for palmitic and oleic fatty acids (FA) during phylogenesis (evolution) are fundamental for а) production of palmitoleic monounsaturated fatty (MFA), b) formation of carnitine palmitoyltransferase as a FA transporter to mitochondria, and c) in vivo production of oleic MFA under humoral regulatory effect of insulin. In the strive for the best kinetic parameters of biological organisms without a possibility of modifying physicochemical and biochemical reactions in the mitochondrial matrix, the mitochondria can be provided with a substrate that increases energy production efficiency and the amount of ATP. Physicochemical parameters of oleic MFA has become the standard of an oxidation substrate for in vivo energy production; this MFA was synthesized in organisms for millions of years. Environmental influences are the second factor which determines kinetic perfection of biological organisms during phylogenesis. Are these influences always beneficial? Mostly, they are not. However, they largely stimulate adaptive functions of the organism, including the biological function of locomotion, cognitive function and the function of positioning in the environment. Biological, energy and kinetic perfection formed in vivo can be easily destroyed if phylogenetically herbivorous Homo sapiens abuses the diet of carnivorous animals (meat) which was not consumed by him and his ancestors during phylogenesis. This abuse is the major cause of metabolic pandemias in human population. They are: insulin resistance, atherosclerosis and atheromatosis, obesity and nonalcoholic liver disease. The most effective measures preventing metabolic pandemias, cardiac heart disease and myocardial infarction are extremely simple. People should remain herbivorous.

Although the biochemistry of the positive effects of medium-chain fatty acids (FA) and triglycerides (TG) of the same name in vivo is not fully understood, food enriched with medium-chain LC and the same TG is effective in patients with type I diabetes, insulin resistance syndrome and in neurodegenerative pathology. Lauric C12 LC is half the FA in coconut oil. Residents of southeast Asia with constant use of coconut oil, have a low level of diseases of the cardiovascular system in the population. With a regulatory intake with food C12:0 laurin FA formed moderate ketosis and neuroprotective effect. Unlike long-chain LC, medium-chain TG cells are not deposited either in visceral fat cells, or in insulin-dependent adipocytes. Medium-chain fatty acids rapidly oxidize mitochondria; the formation of acetyl-CoA cells is used to form ketone bodies, activating thermogenesis in orange and brown adipocytes. Experiments with animals and observations in the clinic showed that taking medium-chain TG with food is more physiological than long-chain oils. This significantly increases the level of cholesterol in high-density lipoproteins. Food enriched with medium chain TG is optimal for increasing the ketone content in blood plasma, cerebrospinal fluid without limiting the carbohydrate content in food. The formation of excess ketone bodies by cells can be achieved by activating the metabolic transformations of medium-chain FAs, without fasting and preserving carbohydrates in food. Coconut oil has a positive effect on the cardiovascular system, preventing the formation of atherosclerosis and atheromatosis. Effective in the prevention of the pathology of the cardiovascular system is a decrease in food amounts of palmitic acid, an increase in oleic acid, polyene FA with a simultaneous increase in the proportion of medium-chain FA.

The abuse of food of carnivores (meat) by phylogeneticallyI herbivorous Homo sapiens (a patient) initiates atherosclerosis. Addressing biogenetic law of E. Haeckel that ontogeny recapitulates phylogeny (a universal anamnesis), we suggest a diagnostic technique that allows evaluation of the meat diet abuse by a herbivorous Homo sapiens. This technique is based on application of phylogenetic theory of general pathology to clinical practice. The degrees of objective evaluation of nonphysiological overeating of meat are: the first, an increase in the fast plasma content of oleic triglycerides palmitoyl-oleyl-palmitate (POP). The second, hyperglyceridemia + an increase in low density lipoprotein cholesterol (LDL-CL) content. The third, increased plasma content of apoС-III. The fourth, an increase in the concentration of apoВ-48. If electrophoregrams are analyzed and hyperlipoproteinemia (HLP) type is determined according to WHO classification, the first degree of meat overeating is not informative, the second, corresponds to type IV HLP; the third, to type IIb HLP, and the forth, to type V HLP, i.e, the patient diet consists practically of the food of carnivores. Hyperlipoproteinemia coincides with insulin resistance syndrome, hyperglycemia and hyperinsulinemia, which is based on blood increase of fatty acids in the form of polar unesterified fatty acids (UFA). According to phylogenetic theory of general pathology, in vivo cells do not internalize glucose if there is a possibility to internalize UFA. Preventive examination allows evaluation of disorders in the biological function of trophology (food consumption). Thus, the use of different methods in the analysis of this function offers evaluation of the effectiveness of diet therapy from the level of disorders when treatment was started.

According to phylogenetic theory of general pathology, formation of multicellular organisms started when each cell (a unicellular organism) reached the first level of relative biological perfection. By that time the stimuli for perfection of the unicellular exhausted, and formation of the multicellular became a biological necessity. All cells, being associated, formed the second level of relative biological perfection within the principle of biological succession. The association included highly organized unicellular organisms with their specific autocrine biological functions and reactions. At the second level of relative biological perfection all humoral mediators in paracrine regulated cell communities (PC) and organs were predominantly hydrophilic and short living. They had a small molecular weight and were probably biologically active peptides (BAP). We believe that functional difference of PC and later of organs is based on differentiation of lysosomal function and production of various enzymes involved in proteolysis of dietary proteins. This allowed various PC and organs to form chemically and functionally different BAP pools from one protein upon proteolysis. Individual peptide pools in PC created the basis for morphologically and functionally different cells and organs. Cell that produces peptides can modify their concentration, chemical parameters and ratios by varying the selectivity of its proteases. In vivo regulation of metabolism by BAP has a common root in bacteria, plants and vertebrates, including Homo sapiens. The third level of relative biological perfection in the organism has formed in close association with cognitive biological function.

According to phylogenetic theory of general pathology, when living in ocean all were carnivorous (piscivorous) fatty acids transferring to cells in form of non-polar triglycerides nitially began apoB-48 chylomicrons, continued lipoproteins of very low and low density and fnalized its apoB-100 endocytosis. The fatty acids are transferred by chylomicrons + lipoproteins of very low density + lipoproteins of low density and non-polar triglycerides are hydrolyzed by hepatic glycerolhydrogenase and co-enzyme apoC-III; according WHO classifcation, hyperlipoproteinemia corresponds to type V. On land, in herbivorous who are not yet synthesized insulin, apoB-48 and chylomicrons left process of non-polar triglycerides transferring. In lipoproteins of very low density and lipoproteins of low density, the carnivorous transfer exogenous palmitic non-polar triglycerides. The herbivorous also transfer palmitic non-polar triglycerides though synthesized by hepatocytes from glucose endogenically. In herbivorous, transferring of palmitic non-polar triglycerides prior to synthesis of insulin is forming apoB-100 in composition of lipoproteins of very low density and lipoproteins of low density. The hydrolysis of palmitic non-polar triglycerides in lipoproteins of very low density is activated by hepatic glycerol hydrogenase and apoC-III; cells absorb lipoproteins of low density by means of apoB-100 endocytosis. The content on lipoproteins in blood plasma under electrophoresis of lipoproteins corresponds to hepatic glycerol hydrogenase type IIb. In frst and second types of fatty acids transferring in form of triglycerides to lipoproteins of very low density + lipoproteins of low density predominate palmitic fatty acid, triglycerides of the same name and palmitic metabolism of fatty acids in vivo. The insulin initiated the third type of transferring of oleic fatty acid by now to insulin-depended cells only in oleic lipoproteins of very low density; hydrolysis of oleic triglycerides is activated by late in phylogenesis post-heparin hepatic glycerol hydrogenase and apoC-II cofactor. The dynamic apoE is actively bound by apoB-100 forming apoE/B-100 ligand. At later stages of phylogenesis insulin formed fatty acids transferring in form of oleic triglycerides in lipoproteins of very low density of the same name without forming of oleic lipoproteins of low density; the electrophoregram of lipoproteins reflects absence of hepatic glycerol hydrogenase. In phylogenesis three types of fatty acids transferring to triglycerides in composition of lipoproteins formed sequentially: 1) chylomicrons + lipoproteins of very low and density + lipoproteins of low density; 2) lipoproteins of very low density + lipoproteins of low density; 3) only in lipoproteins of very low density. The frst one is specifc to piscivorous (carnivorous) while living in ocean. The second one is implemented by herbivorous while they didn't begin to synthesize insulin and hepatocytes not yet transform all endogenous palmitic fatty acid into oleic fatty acid. Insulin initiated: a) transferring of oleic fatty acids to lipoproteins of very low density without forming oleic lipoproteins of low density; b) highly effective oleic metabolism of fatty acids in vivo: c) becoming of biological function of locomotion. The aphysiological induction by substrate, surplus of palmitic fatty acids in food initiate negative alterations in composition of lipoproteins in opposite direction than in case of phylogenesis. When homo sapiens, herbivorous in phylogenesis, begins to misuse carnivorous (meat) food then instead of normolipoproteinemia in blood plasma under electrophoresis of lipoproteins one can initially detect transitory hyperlipoproteinemia type IV and then prolonged hyperlipoproteinemia type IIb. If patient factually passes on to carnivorous diet then hyperlipoproteinemia type V is developing. If content of exogenous palmitic fatty acid in food surpasses physiological capacities of its transferring in oleic triglycerides as palmitoyl-oleyl-palmitate glycerol, palmitic triglycerides as oleyl-palmitoyl-palmitate glycerol begin to form and epigenetically aphysiological non-ligand palmitic lipoproteins of very low density → lipoproteins of low density are formed. Their circulation in blood is a cause of hypertriglyceridemia, higher level of cholesterol-lipoproteins of low density, compensatory increasing of apoC-III. Then occurs induced by substrate formation of hyperlipoproteinemia initially of type IV, then of type IIb and fnally of type V. The pathogenesis of atherosclerosis and atheromotosis is activated when homo sapiens, herbivorous in phylogenesis, begin to misuse carnivorous food affecting biological functions of trophology, reaction of exotrophy (external nutrition), function of homeostasis, endoecology and function of adaptation. The formation of palmitic metabolism if fatty acids instead of oleic one is a cause of chronic defciency of energy and ATP synthesis in vivo. Insulin activates absorption of glucose by cells with purpose to use it for synthesis of oleic fatty acids. In the frst place, insulin regulates in vivo metabolism of fatty acids and only in second place metabolism of glucose.

The comparative assessment was carried out concerning anti-oxidation activity of blood serum of patients with liver pathology using two chemiluminescent techniques with different models of free radical oxidation: «Hb-Н2О2-luminol¼ и «ABAP-luminol¼. The reliable but low correlation of results was established (r=0,798) related mainly to difference in mechanisms of initiation of free radicals and effect of blood serum on initiation process. This effect is stronger manifested in model «Hb-Н2О2-luminol¼. The discrepancy of results of measurement is more expressed in patients with anomalous higher content of bilirubin in blood. Thereupon, oxidation model «ABAP-luminol¼ is to be considered as a more preferable for clinical practice.

RESUMO

The kinetics of ozone interaction with 14 saturated and unsaturated fatty acids with different molecular structures was studied and velocity constants of these reactions were measured. The relationships between the measured constants and some parameters of fatty acid (length of the aliphatic chain, number of double bonds, location of the double bonds, and its configuration) were detected. The highest velocity constant is recorded for monounsaturated oleic fatty acid: 2.58×105 (M×sec)-1. Other studied fatty acids had 1.5-5-lower constants depending on their molecular structures.

In organism, differences in physical chemical properties and physiological role of positional isomeric forms of triglycerides of fatty acids consumed with food are well known. The Raman spectroscopy to analyze there is possible to be applied. Both of these circumstances are to be considered and applied in resolving practical issues of dietetics, i.g., in detection of falsification and replacement of butter and spreads with palm oil, as well as in monitoring ratio of fatty consumed by patients with cardiologic and oncologic pathology. The purpose of study is to determine possibilities of analyzing positional fatty acids isomers specific for butter and palm oil using portable and operational spectrometers of absorption in near infrared range. The technique of projections on latent structures was applied to obtain calibrations of spectrometers to detect content of 7 regulated fatty acids and percentage of milk fat and palm oil in fat-oil mixtures according specters of Raman and near infrared range spectrometers. The possibility of identifying positional isomers of animal and vegetable triglycerides according Raman spectra was confirmed. Besides, it is established that efficiency of determining (reliability, accuracy and selectivity) the proportions of the above oils using both Raman spectra and near-infrared optical density spectra was much higher than when calculating the percentage of the same oils using the content of 7 fatty acids. This fact reflects sensitivity of near infrared range absorption spectra both to length of the carbon chain and degree of unsaturation (number of double bonds C=C) of fatty acids and positional isomeric forms of triglycerides. The obtained data are used in formulating technical requirements and conditions for application of portable near infrared range spectrometer for mass analysis of fat-oil products.

RESUMO

The abuse of food of carnivores (meat) by phylogeneticallyI herbivorous Homo sapiens (a patient) initiates atherosclerosis. Addressing biogenetic law of E. Haeckel that ontogeny recapitulates phylogeny (a universal anamnesis), we suggest a diagnostic technique that allows evaluation of the meat diet abuse by a herbivorous Homo sapiens. This technique is based on application of phylogenetic theory of general pathology to clinical practice. The degrees of objective evaluation of nonphysiological overeating of meat are: the first, an increase in the fast plasma content of oleic triglycerides palmitoyl-oleyl-palmitate (POP). The second, hyperglyceridemia + an increase in low density lipoprotein cholesterol (LDL-CL) content. The third, increased plasma content of apoС-III. The fourth, an increase in the concentration of apoВ-48. If electrophoregrams are analyzed and hyperlipoproteinemia (HLP) type is determined according to WHO classification, the first degree of meat overeating is not informative, the second, corresponds to type IV HLP; the third, to type IIb HLP, and the forth, to type V HLP, i.e, the patient diet consists practically of the food of carnivores. Hyperlipoproteinemia coincides with insulin resistance yndrome, hyperglycemia and hyperinsulinemia, which is based on blood increase of fatty acids in the form of polar unesterified fatty acids (UFA). According to phylogenetic theory of general pathology, in vivo cells do not internalize glucose if there is a possibility to internalize UFA. Preventive examination allows evaluation of disorders in the biological function of trophology (food consumption). Thus, the use of different methods in the analysis of this function offers evaluation of the effectiveness of diet therapy from the level of disorders when treatment was started.

The aim of this work was to assess the relationship of rs2230806 SNP of ABCA1 with lipid profile in patients with severe dyslipidemia. The study included 363 patients (42.8% of males), the average age was 48.7 years, 35.5% of patients received hypolipidemic drugs (mainly statins). Quantitative determination of total cholesterol (ТС) and triglycerides (TG) in fasting serum was carried out by a unified enzymatic method, and high density lipoproteins (HDL) - by a direct homogeneous method. Genotype according to the rs2230806 position in the ABCA1 gene was determined by polymerase chain reaction (PCR) «in real time¼ using adjacent samples and melting reaction products after PCR. The frequencies of alleles and genotypes of variant rs2230806 of ABCA1 gene in patients with dyslipidemia did not differ from those in the control group of healthy individuals (athletes). The levels of plasma lipids - TC, TG and HDL cholesterol, on average, in patients with dyslipidemia were 7.8±3,4, 3,4±6,5 and 1.29±0.4 mmol/l, respectively. Compared to different genotypes, the plasma lipid concentrations did not differ significantly, but the analysis of different inheritance models of the allelic variant studied showed a significant association with the level of TG in the additive model, in which each minor allele (a) further enhanced the effect on the level of plasma TG at 1.02 mmol/l (p=0.044). The results of this study demonstrate the effect of a common variant rs2230806 of the ABCA1 gene on the plasma TG level in patients with severe dyslipidemia.

Kinetic parameters of the interaction between ozone with 13 individual saturated, monoenic, unsaturated and polemic fatty acids (FA) which have 1-4 double bonds (DB) have been determined. Rate constants for FA reaction with ozone were estimated. It was found that the constants depend on a) aliphatic chain length, b) DB number of DB in the chain, c) DB position in the chain and d) cist- or trans- configuration of FA. The highest rate constant was recorded for С18:1 cis-ω-9 oleic FA (2.58∙105, М-1s-1). Oxidation rate constants for other FA were 1.5- to 3-fold lower depending of the FA structure. The greatest role ozonolysis is played in biochemical reactions, in physiologic metabolism of FA and also in aphysiologic reactions of modification of FA as part of phospholipids (phosphatidylcholine and aminophospholipids) in structure of plasmatic membranes of living cells. We believe also that phylogenetically the role of insulin consists in: a) promoting the conversion of carnivorous (fish-eating) animals that left the ocean into herbivorous, b) providing high kinetic parameters for realization of the biological function of locomotion, c) converting in vivo the low-efficiency palmitic pathway of FA metabolism with energy deficiency into a high-efficiency oleic pathway. During phylogenesis, low chemical activity of oleic FA in vivo is the major etiological factor of two nonphysiological processes: atherosclerosis and atheromatosis which are fundamental for the development of ischemic heart disease and myocardial infarction.

According to the phylogenetic theory of general pathology, seven biological functions have been formed over billions of years. 1. biological function of trophology, nutrition; 2. homeostasis function; 3. biological function of endoecology; 4. function of adaptation; 5. function of the continuation of the species; 6. function of locomotion and 7. cognitive biological function, including intelligence. Millions of years in life consistently in the waters of several oceans, all the ancestors of man were carnivorous (Carnivores), fish-eating mammals. When the ocean retreated and the carnivorous (fish-eating) were on land, each individual privatized a "piece" of the ocean. Animals transformed it ito a pool of intercellular medium in vivo. The biological role of the late in the phylogeny of insulin is the formation of new biological functions in vivo. The action of insulin has transformed the carnivorous (fish-eating) ocean into herbivorous (Herbivores) species on land. There was it by synthesis in vivo from exogenous glucose of fatty acids (FA). Regulatory action of insulin was the directed conversion of exogenous glucose into ω-6 C18: 1 cis-oleic FA. Insulin late in phylogeny expressed the synthesis of new, conjugated enzymes: it is palmitoyl-CoA-elongase and stearyl-CoAdesaturase. Two enzymes synthesized FAs along the way: synthesized in situ de novo, from exogenous glucose, C16: 0 palmitic acid → C18: 0 stearic acid → ω-6 C18: 1 cis-oleic acid without accumulation of stearic FA. Insulin is not converted into an oleic FA exogenous palmitic acid from carnivorous food. On land, the action of insulin transformed the species Homo sapiens, into a herbivore, but with carnivorous, fish-eating, past. The idea of a person as omnivorous (Omnivor) - nonsense; such forms of nature did not form. Violation of the function of nutrition, the biological reaction of exotrophy (external nutrition), is the etiological and pathogenetic basis of the seven metabolic pandemics, the diseases of civilization. 1. Atherosclerosis and atheromatosis; 2. metabolic arterial hypertension; 3. metabolic syndrome; 4. obesity; 5. syndrome of insulin resistance; 6. non-alcoholic fatty liver disease and 7. endogenous hyperuricemia. The primary prevention of metabolic pandemics in the biological function of nutrition, in the biological reactions of exo-and endotrophy, will allow us to understand the theoretical bases and implementation of preventive actions that will determine the characteristics of nutrition in the future.

For the biological functions of trophology (nutrition) and homeostasis, we believe that the third place is occupied by the function of endoecology: in a single pool of intercellular environment in vivo it must be "pure". The biological function of endoecology, realizes two biological reactions: the reaction of excretion and the reaction of inflammation. The biological reaction of excretion is the filtration of phlogogens of a small mol. mass through the basement membrane of the glomerulus of the nephron and excretion with the final urine. Flogogens are a great mol. mass (more than 70 kDa), catabolites of biological reactions of apoptosis, autophagy must be disposed of in situ, on site. At the stages of phylogenesis, the cells formed structures for the collection and utilization of phlogogens of large mol. mass and exogenous pathogens in tissues. The sequence of biochemical reactions, biological methods of in vivo collection and in situ utilization of phlogogens of large mol. mass and realizes the biological function of endoecology, the biological reaction of inflammation. It functions constantly, every minute, like the biological reaction of excretion. Inflammation is the reaction of maintaining the "cleanliness" of the "piece" of the ocean that every individual has privatized, before beginning to live on land. The inflammation reaction includes congenital, acquired immunity, humoral mediator systems, cytokines and eicosanoids. In the formation of a closed circulatory system, the pool for collecting and utilizing in situ biological catabolites from the bloodstream is located in the late phylogeny of the intima of the arteries of the elastic (muscular-elastic) type. Abuse of herbivorous species Homo sapiens carnivorous (meat) food; blockade of cellular absorption by polyene fatty acids in low density lipoproteins and the absence of acidic hydrolase expression of polyene fatty acids - cholesterol alcohol ethers in monocytes and macrophages - is the cause of the formation of atheroma of intima of the arteries of the elastic type. Atherosclerosis - the pathology of the biological function of trophology, atheromatosis - pathology of the biological function of endoecology. The appointment of patients statins for primary prevention of atherosclerosis, atheromatosis and coronary heart disease, we finally "passed".
