RESUMO

In this study, we explored the impact of caregiving on quality of life and health perceptions and outlined the profile of grandparent caregivers in Andalusia (Spain) in terms of a range of sociodemographic variables related to the care of their grandchildren. A sample of 171 participants (21.6% men) completed the Health-Related Quality of Life (HRQoL) questionnaire and another ad hoc one providing sociodemographic and caregiving data. We studied the relationships between these variables and HRQoL using ANOVA, chi-square and Multiple Linear Regression. We found a mainly female profile for the care of grandchildren and interesting relationships for the physical and mental components of HRQoL. Some relationships were marked by gender: caregiving for pleasure was more often the motive for men while by imposition was more common among women. We discuss the impact of caregiving on health according to the Self-Determination Theory and suggest practical implications derived from the main findings.

BACKGROUND: Patient safety is a relevant subject in the nursing curriculum. Each university programs patient safety teaching and practical training differently. However, few studies have sought to explore the relationship between patient safety as perceived by nursing students and other important psychosocial competencies in the nursing curriculum, such as self-efficacy, competence, and resilience. OBJECTIVES: To analyze differential patient safety integration into three nursing education programs, and to assess agreement levels regarding patient safety climate, students' knowledge of patient safety and correlations with specific self-efficacy, competence and resilience. METHODS: Participants were 647 undergraduate students from three universities. Patient safety climate and knowledge of patient safety (good praxis) were measured using the Hospital Survey on Patient Safety Culture for nursing students, and other psychosocial variables were also analyzed using other instruments: specific self-efficacy, perceived competence and resilience. Nursing education programs and patient safety climate were analyzed using the Rwg(j) and ICC measures of inter-rater agreement across different academic levels. RESULTS: The ICC and Rwg indexes revealed high inter-rate agreement in all three universities. Differences were observed between Univ-2 and Univ-3 in patient safety climate scores and agreement values between academic levels. Differences in good praxis were found when academic levels were compared in Univ1-and Univ-2. Patient safety climate was found to correlate significantly with the psychosocial variables studied, but only in Univ-1. CONCLUSIONS: Perceived patient safety climate differs between universities and academic levels. This competency is related to self-efficacy, competence and resilience, which endorses the assessment of patient safety integration from a broader perspective.

Objetivo: Explorar las prácticas de promoción de la salud de las enfermeras de atención primaria en la violencia de género con la población adolescente. Metodología: Estudio cualitativo a través de grupos de discusión y entrevistas semi-estructuradas. Participaron 37 informantes, 23 enfermeras y 14 enfermeros, vinculados a atención primaria de Sevilla y provincia. Se llevó a cabo un análisis del contenido con el software QSR NudisNVivo12®. Resultados: Del análisis emergen varias categorías: prácticas para la educación de la población adolescente, prácticas para la detección de situaciones desigualitarias y prácticas para intervenir en la violencia en el noviazgo. Conclusiones: Enfermeras y enfermeros realizan intervenciones educativas promoviendo relaciones igualitarias, con diferentes técnicas. Solo las enfermeras realizan cribado y asesorías para detectar casos. Enfermeras y enfermeros realizan intervenciones para interrumpir relaciones abusivas. Se evidencia sensibilidad a la violencia de género y la necesidad de evaluar la efectividad y la rentabilidad de las intervenciones realizadas.(AU)


Objective: Explore the health promotion practices of primary care nurses in gender violence with the adolescents. Methods: Qualitative study with discussion groups and semi-structured interviews. 37 informants participated, 23 female nurses and 14 male nurses, of primary care in Seville and the province. A content analysis was carried out with the QSR NudisNVivo12® software. Results: Several categories emerge from the analysis: practices for the education of the adolescents, practices for detecting and practices for intervening in dating violence. Conclusions: Female nurses and male nurses carry out educational interventions promoting egalitarian relationships, with different techniques. Only nurses female perform screening and counseling to detect cases. Female nurses and male nurses perform interventions to interrupt abusive relationships. There is sensitivity to gender violence and the need to evaluate the effectiveness and profitability of the interventions carried out is evidenced.(AU)

BACKGROUND: Functional rating scales allow clinicians to document and quantify alterations and progression of recovery processes. There is neither awareness of numerous knee scales nor are they easy to find or compare to select the most suitable. OBJECTIVES: We aimed to compile validated knee functional rating tools and analyze the methodological quality of their validation studies. Also, we aimed to provide an operational document of the outcome measures addressing descriptions of parameters, implementations, instructions, interpretations and languages, to identify the most appropriate for future interventions. METHODS: A systematic review involved a search of PubMed, Web of Science, CINAHL, Scopus, and Dialnet databases from inception through September 2020. The main inclusion criteria were available functional rating scales/questionnaires/indexes for knees and validation studies. Methodological quality was analyzed with the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) and COnsensus-based Standards for the selection of health Measurement Instruments Risk of Bias (COSMIN-RB). RESULTS: We selected 73 studies. The studies investigated 41 knee rating tools (general, 46%, and specific, 54%) and 71 validations, including 29,742 individuals with knee disorders. QUADAS-2 obtained the best results in patient selection and index test (applicability section). COSMIN-RB showed the highest quality in construct validity (most analyzed metric property). The specific tools were mainly designed for prosthesis and patellofemoral and anterior cruciate ligament injuries. More considered issues were specific function (93%), especially gait, pain/sensitivity (81%), and physical activity/sports (56%). CONCLUSIONS AND IMPLICATIONS: We conducted a necessary, useful, unlimited-by-time and feasible compilation of validated tools for assessing knee functional recovery. The methodological quality of the validations was limited. The best validations were for the Copenhagen Knee Range of Motion Scale in osteoarthritis and arthroplasties, Knee Outcome Survey Activities of Daily Living and Lysholm Knee Score for general knee disorders and the Tegner Activity Score for anterior cruciate ligament injuries. The operational document for the scales provides necessary data to identify the most appropriate.

RESUMO

Objetivo principal: explorar las vivencias y sentimientos sobre el cuidado que manifiestan las personas cuidadoras a través de las metáforas, profundizando en las diferencias de género. Metodología: Estudio cualitativo a través de entrevistas semi-estructuradas. Participaron 24 informantes, 7 pacientes y 17 personas cuidadoras vinculadas a una red de apoyo socio familiar. Se llevó a cabo un análisis del contenido con el software Atlas-ti versión 7®. Resultados principales: Las metáforas expresadas a través de los sentimientos muestran sentimientos comunes en hombres y mujeres: el amor y la satisfacción. Se identificaron sentimientos exclusivos de mujeres, que beben de mandatos de género. Se observó que los hombres se están incorporando al cuidado. Conclusión principal: Las metáforas encontradas desvelan los significados que se atribuyen al cuidado. Las mujeres son conscientes del valor de cuidar, pero lo normalizan. Los hombres esperan reconocimiento social.(AU)


Objective: to explore the experiences and feelings about care that caregiver’s manifest through experiential and emotional metaphors, with special focus on gender differences. Methods: Qualitative study through semi-structured interviews. 24 informants, 7 patients and 17 caregivers participated, linked to a social and family support network. A content analysis was carried out with Atlas-ti version 7® software. Results: The metaphors expressed through feelings show common feelings in men and women: love and satisfaction. Feelings exclusive to women were identified, that come from gender mandates. It was observed that men are joining care more and more. Conclusions: The metaphors found reveal the meanings attributed to care. Women are aware of the value of caring, but they normalize it. Men expect social recognition.(AU)

The objective of this review was to compile validated functional shoulder assessment tools and analyse the methodological quality of their validations. Secondarily, we aimed to provide a comparison of the tools, including parameter descriptions, indications/applications, languages and operating instructions, to choose the most suitable for future clinical and research approaches. A systematic review (PRISMA) was conducted using: PubMed, WoS Scopus, CINHAL, Dialnet and reference lists until 2020. The main criteria for inclusion were that papers were original studies of validated tools or validation studies. Pre-established tables showed tools, validations, items/components, etc. The QUADAS-2 and COSMIN-RB were used to assess the methodological quality of validations. Ultimately, 85 studies were selected, 32 tools and 111 validations. Risk of bias scored lower than applicability, and patient selection got the best scores (QUADAS-2). Internal consistency had the highest quality and PROMs development the lowest (COSMIN-RB). Responsiveness was the most analysed metric property. Modified UCLA and SST obtained the highest quality in shoulder instability surgery, and SPADI in pain. The most approached topic was activities of daily living (81%). We compiled 32 validated functional shoulder assessment tools, and conducted an analysis of the methodological quality of 111 validations associated with them. Modified UCLA and SST showed the highest methodological quality in instability surgery and SPADI in pain.

Objetivo: El Hospital Universitario Virgen del Rocío (HUVR) de Sevilla fue elegido centro referente andaluz para atender posibles casos de contagio de Ébola. Tras la alerta sanitaria (OMS, 2014) se conformó un equipo de profesionales voluntarios sanitarios/as y no sanitarios/as, que recibió entrenamiento y atendió un posible caso. Se plantea como objetivo comprender sus motivaciones y experiencias emocionales, e identificar los elementos que facilitan y dificultan su funcionamiento. Método: Estudio cualitativo, interpretativo y fenomenológico. Unidad de observación: equipo de profesionales del HUVR entrenado para atender casos de Ébola. Unidades de análisis: trabajo en equipo, motivaciones, emociones. Se realizan tres entrevistas a informantes clave y tres grupos de discusión; participan 23 profesionales de 60 que integran el equipo (2014-2016). Se analizan con QSRNUDISTVivo10 (análisis de contenido) las categorías: motivaciones, emociones, elementos influyentes en el funcionamiento. Validez: se triangularon fuentes de datos, técnicas, perspectivas disciplinarias y se expusieron los resultados al equipo de profesionales, que mostró su acuerdo. Resultados: Perciben interés formativo, responsabilidad profesional, autoestima profesional, atracción por el riesgo o lealtad al líder como motivaciones para ofrecerse voluntariamente. Expresan su evolución emocional, del miedo y estrés original al autocontrol de la presión, tranquilidad y confianza, a través de la formación y cohesión del equipo, elementos esenciales para su funcionamiento. La familia, el servicio de origen, los recursos, los medios de comunicación y la gestión de las emociones influyen, facilitando o entorpeciendo el éxito del equipo. Conclusión: Conocer los factores que les motivan e influyen puede servir para orientar la gestión de equipos multidisciplinarios eficaces y satisfechos en casos de alertas sanitarias (AU)

Emotional experiences evolved from fear and stress to self-pressure control, while essential elements for the team's operation were found to be calmness and confidence based on training and teamwork. Family, source department, resources, communication media and emotional management were facilitators of or obstacles to the team's success. Conclusion: An understanding of the key motivational and influential factors may be important in the management of effective and successful multidisciplinary teams during health alerts (AU)
