RESUMO

Neonates who are born preterm (PT) are usually characterized by immature physiological development, and preterm birth (PTB) is the leading cause of neonatal morbidity and mortality if intensive medical care is not available to PTB neonates. Early prediction of a PTB enables medical personnel to make preparations in advance, protecting the neonate from the subsequent health risks. Therefore, many studies have worked on identifying invasive or noninvasive PT biomarkers. In this study, we collected amniocentesis-derived (at the second trimester of gestation) amniotic fluid (AF) samples. At delivery, AF samples were classified into PTB or full-term birth (FTB). We first applied protein mass spectrometry technology to globally screen AF proteins, followed by specific protein validation with ELISA. We identified four protein biomarkers of PTB, including lactotransferrin (LTF), glutathione-disulfide reductase (GSR), myeloperoxidase (MPO) and superoxide dismutase 2 (SOD2). Further analyses demonstrated that their abundances were negatively correlated with neonatal weight and gestational age. In addition, by mimicking survival rate analysis widely used in tumor biology, we found that LTF and SOD2 were prognostic factors of gestational age, with higher levels denoting shorter gestational age. Finally, using the abundances of the four protein biomarkers, we developed a prediction model of PTB with an auROC value of 0.935 (sensitivity = 0.94, specificity = 0.89, p value = 0.0001). This study demonstrated that the abundances of specific proteins in amniotic fluid were not only the prognostic factors of gestational age but also the predictive biomarkers of PTB. These four AF proteins enable identification of PTB early in the second trimester of gestation, facilitating medical intervention to be applied in advance.

Placenta accreta spectrum (PAS) accounts for 7% of maternal mortality and is associated with intraoperative and postoperative morbidity caused by massive blood loss, infection, and adjacent organ damage. The aims of this study were to identify the protein biomarkers of PAS and to further explore their pathogenetic roles in PAS. For this purpose, we collected five placentas from pregnant subjects with PAS complications and another five placentas from normal pregnancy (NP) cases. Then, we enriched protein samples by specifically isolating the trophoblast villous, deeply invading into the uterine muscle layer in the PAS patients. Next, fluorescence-based two-dimensional difference gel electrophoresis (2D-DIGE) and MALDI-TOF/MS were used to identify the proteins differentially abundant between PAS and NP placenta tissues. As a result, nineteen spots were determined as differentially abundant proteins, ten and nine of which were more abundant in PAS and NP placenta tissues, respectively. Then, specific validation with western blot assay and immunohisto/cytochemistry (IHC) assay confirmed that heat shock 70 kDa protein 4 (HSPA4) and chorionic somatomammotropin hormone (CSH) were PAS protein biomarkers. Further tube formation assays demonstrated that HSPA4 promoted the in vitro angiogenesis ability of vessel endothelial cells, which is consistent with the in vivo scenario of PAS complications. In this study, we not only identified PAS protein biomarkers but also connected the promoted angiogenesis with placenta invasion, investigating the pathogenetic mechanism of PAS.

OBJECTIVE: Preterm birth severely threatens neonatal health and life. Although the detailed mechanism of preterm birth is not well understood, accurately predicting preterm birth can help people make preparations in advance, greatly reducing the subsequent health risk of neonates. Therefore, in this study, we aimed to identify potential protein biomarkers of preterm birth in amniotic fluid (AF). MATERIALS AND METHODS: We first enrolled pregnant subjects and collected their AF samples when they underwent amniocentesis at the second trimester of gestation. After delivery, the collected AF samples were classified into a full-term birth (sample size n = 21) set or preterm birth (n = 36) set, followed by 2-D DIGE and MS/MS assays. RESULTS: By doing so, we identified seven potential protein biomarkers of preterm birth, three of which were further validated in all samples with ELISA, including Apolipoprotein A-IV (Apoa4), Lumican (Lum) and Kininogen-1 (Kng1). As a result, all three potential biomarkers were significantly differently expressed between preterm and full-term birth AF samples. Furthermore, without prior classification, we found that these three biomarkers were positively correlated with gestation age (correlation coefficient ranging from 0.25 to 0.38) and were able to predict the occurrence of preterm birth. CONCLUSION: In this study, by examining amniotic fluid, we identified three biomarker proteins that may facilitate the identification of preterm birth. There three proteins were never reported to be related to preterm birth. Their pathogenesis roles in preterm birth deserve further investigations by using in vitro cell model or in vivo animal model assays.

Urban streetscapes are outdoor areas in which the general public can appreciate green landscapes and engage in outdoor activities along the street. This study tested the extent to which the degree of awareness of urban street trees impacts attention restoration and perceived restorativeness. We manipulated the degree of awareness of street trees. Participants were placed into four groups and shown different images: (a) streetscapes with absolutely no trees; (b) streetscapes with flashes of trees in which participants had minimal awareness of the content; (c) streetscapes with trees; and (d) streetscapes with trees to which participants were told to pay attention. We compared the performance of 138 individuals on measures of attention and their evaluations of perceived restorativeness. Two main findings emerged. First, streetscapes with trees improved the performance of participants on attentional tests even without their awareness of the trees. Second, participants who had raised awareness of street trees performed best on the attentional test and rated the streetscapes as being more restorative. These findings enhance our knowledge about the role of an individual's awareness of restorative elements and have implications for designers and individuals who are at risk of attentional fatigue.

Interaction between an anionic surfactant, sodium dodecyl sulfate (SDS), and a nonionic polymer, 2-hydroxyethyl cellulose (HEC) hydrophobically modified with benzoyl chloride (bmHEC), is studied by viscometry and two-dimensional nuclear Overhauser effect NMR spectroscopy (2D NOESY) in a semidilute regime of bmHEC. The hydrophobicity of bmHEC was varied with different substitution of benzoyl group to HEC macromolecules. In general, the low-shear viscosity of 1 wt % bmHEC aqueous solution is increased with added SDS surfactant having concentration from 0 to 0.5 wt %, and then decreased significantly with a further addition of surfactant to 3 wt %. The activation energy of transient network formation in 1 wt % bmHEC aqueous solution present with SDS surfactant is found to be dependent with SDS concentration, which varies from 32.7 to 69.80 kJ/mol. The maximum activation energy takes place when 0.5 wt % SDS is added, which coincides with that of the maximal viscosity. The 2D NOESY displays that the surfactants actually interact with bmHEC not only on the hydrophobes, namely benzoyl groups, but also the polymer backbone, i.e., glucose units. In contrast, no interaction is revealed by 2D NOESY in the aqueous system containing SDS surfactant and HEC polymer.

OBJECTIVE: To elucidate the expression of androgen receptor (AR) in human placenta from normal and preeclamptic (PE) pregnancies. MATERIALS AND METHODS: This study was performed at the Division of Obstetrics and Gynecology, Kaohsiung Chang Gung Memorial Hospital, Academic Medical Center. Forty normal pregnant women at 37.0 +/- 0.89 weeks' gestation and 40 women with preeclampsia at 35.9 +/- 1.33 weeks' gestation were included in this study. Serum testosterone was measured before delivery. Immunochemical methodology was employed in formalin-fixed, paraffin-embedded sections from the placentas of all subjects. AR expression was evaluated in placentas collected from 15 normal pregnant women and 12 PE women. RESULTS: Women with preeclampsia had an elevated serum testosterone level (0.52 +/- 0.13 vs. 0.34 +/- 0.11 ng/mL; p < 0.01). The expression of AR in both placentas was confirmed using immunohistochemistry and was primarily present in the nucleus and cytoplasm of syncytiotrophoblasts and stromal cells. There was a significant increase in AR mRNA expression in PE placentas compared with normal placentas. CONCLUSION: These data suggest that the increased AR expression may alter AR-mediated function on syncytiotrophoblasts and stromal cells in PE placentas, and be a possible mechanism for its association with preeclampsia.

OBJECTIVE: To investigate the changes in Doppler indices of the fetal ductus venosus (DV) and umbilical artery (UMA) after amnioinfusion in pregnant women with preterm premature rupture of membranes (pPROM). Pregnancies with pPROM and severe oligohydramnios cause sequelae in newborns and mothers. MATERIALS AND METHODS: This cross-sectional study included a group of 25 patients with pPROM before 26 weeks' gestation. Color Doppler imaging was used to measure the impedance index and quantitative blood flow in the DV and systolic/diastolic ratio (S/D) of the UMA before and 30 minutes after the end of amnioinfusion. The following velocity parameters were measured: (1) DV peak systolic velocity; (2) DV time-averaged velocity; (3) DV maximum forward velocity during atrial contraction; (4) DV S/D; (5) DV pulsatility index (PI); (6) DV Pourcelots resistance index (RI); (7) fetal heart rate; and (8) UMA S/D. RESULTS: Twenty-one of the 25 patients underwent a total of 27 amnioinfusions. The mean PI and RI of the DV, and S/D of the DV and UMA decreased significantly after amnioinfusion (PI, 0.75 +/- 0.24 vs. 0.60 +/- 0.18, p = 0.009; RI, 0.60 +/- 0.15 vs. 0.50 +/- 0.13; DV S/D, 3.07 +/- 1.81 vs. 2.13 +/- 0.66, p = 0.008; UMA S/D, 3.58 +/- 0.87 vs. 2.88 +/- 0.62, p = 0.001). CONCLUSION: Amnioinfusion increases the space for the fetuses and reduces the impedance of the fetoplacental circulation. Improvements in DV and UMA flow may benefit fetuses suffering severe oligohydramnios in mid-pregnancy.

Interaction between the nonionic surfactant Tergitol 15-S-7 and hydrophobically modified 2-hydroxyethyl cellulose (HMHEC) was studied rheologically in a semidilute regime of HMHEC. The low-shear viscosity of HMHEC was increased with addition of surfactant from 25 to 250 ppm, in which the critical micelle concentration of surfactant was near 39 ppm, and then decreased to a value smaller than that of pure HMHEC with further addition of surfactant to 1000 ppm. An interesting shear-induced phenomenon was observed. The steady-state shear measurements show that there exist crossovers between viscosity-shear rate curves of HMHEC solutions with and without surfactant added, whereas it was not observed in the HEC-surfactant systems. Moreover, added Tergitol 15-S-7 reversed the temperature effect on the viscosity of the HMHEC solution. That is, increasing temperature to or near the cloud point raises the viscosity of the HMHEC-surfactant aggregates, in contrast to the viscosity decrease in the pure HMHEC solutions. A possible mechanism based on the necklace model and the clouding phenomenon is conjecturally introduced to explain such phenomena.
