Optimized health-related quality of life (HRQOL) at the time of kidney transplantation (KT) is associated with improved survival. In older KT recipients, we aimed to prospectively evaluate if HRQOL evolution during the first posttransplant year was associated with long-term patient survival. Methods: Recipients older than 65 y at KT who received an organ from a deceased brain-dead donor and survived >12 mo posttransplant were eligible. HRQOL was assessed pre-KT, at 10 wk, 6 mo, and 12 mo post-KT, using the Kidney Disease Quality of Life Short Form version 1.3 survey. A mixed-effect model was used to explore HRQOL evolution during the first posttransplant year in long-term survivors compared with nonsurvivors. Distinct HRQOL clusters were identified using a group-based trajectory modeling and their association with patient survival was investigated with Cox proportional hazard regression models. Results: We included 192 elderly recipients of deceased brain-dead donor kidneys who were transplanted from 2013 to 2020. Eleven died during the first year leaving 181 for evaluation (male, 125; mean age at KT, 72 y [65-84 y]). During a median observation time post-KT of 4.9 y (11.1-8.5 y), 57 recipients died. In survivors, all the generic and kidney-specific HRQOL domains substantially improved during the first year, whereas in nonsurvivors HRQOL deteriorated. Three longitudinal HRQOL trajectories indicating poor, fair, and good HRQOL evolution were identified. Poor physical function trajectory was significantly associated with higher mortality risk independent of covariates, as compared with good physical trajectory (hazard ratio, 2.38; 95% confidence interval, 1.15-5.01). Conclusions: In elderly KT recipients, detection of declining posttransplant physical function may imply impaired survival. Systematic HRQOL monitoring following KT provides added value when evaluating mortality and may guide therapeutic decisions.

Kidney transplantation (KT) is considered the best treatment for end-stage kidney disease (ESKD). In the increasing elderly ESKD population, KT should be reserved for carefully selected candidates who are expected to experience favorable outcomes. We aimed to prospectively evaluate pretransplant recipient factors that may predict patient survival and can eventually guide therapeutic decisions in elderly with ESKD. Methods: Recipient factors were evaluated in KT candidates aged ≥65 y. Comorbidity was assessed at waitlisting according to the Liu comorbidity index (LCI). Health-related quality of life outcomes were measured using the Kidney Disease Quality of Life Short Form, version 1.3. The Cox proportional hazard regression was used to evaluate predictors of patient survival. Results: We included 192 recipients, with a mean age of 72.1 (4.1) y, who were transplanted with kidneys from deceased brain-dead donors. During a median observation period of 4.6 (3.2-6.3) y, 66 recipients died. Elevated LCI consistently predicted poor patient survival. In recipients with LCI ≥4, dialysis >2 y comprised a 2.5-fold increase in mortality risk compared with recipients on dialysis ≤2 y. Self-reported pretransplant physical function was also proven to be a significant positive predictor of survival. Conclusion: The implementation of LCI and a physical function score during the evaluation of older kidney transplant candidates may improve the selection and thereby optimize posttransplant outcomes.

RATIONALE & OBJECTIVE: Assessing the optimal therapy for older patients (aged ≥65 years) with end-stage kidney disease requires knowledge of longevity and health-related quality of life (HRQoL) outcomes. Kidney transplantation prolongs survival but its long-term impact on HRQoL in older recipients is not well defined. We aimed to prospectively evaluate HRQoL changes from enlisting until 3 years posttransplantation and examine pretransplantation predictors of posttransplantation outcomes. STUDY DESIGN: Prospective cohort study. SETTING & PARTICIPANTS: Patients 65 years and older enlisted at the Norwegian National Transplant Center between January 2013 and November 2016. PREDICTORS: Kidney transplantation, dialysis vintage, and pretransplantation comorbidity assessed using the Liu Comorbidity Index. OUTCOMES: HRQoL, assessed using the Kidney Disease Quality of Life Short Form, version 1.3. ANALYTICAL APPROACH: HRQoL scores obtained at 3 years posttransplantation were compared with those obtained pretransplantation and after 1 year using a paired-sample t test. Multivariable linear mixed-effect models were used to identify possible predictors of HRQoL changes over time. RESULTS: Among 289 patients included, 220 (mean age, 71.5 years) had undergone transplantation and 136 had completed the 3-year HRQoL follow-up by October 2020. Posttransplant HRQoL, both generic and kidney specific, substantially improved and the benefit persisted for 3 years. For wait-listed candidates remaining on dialysis, HRQoL gradually deteriorated, and recipients who died within 3 years posttransplantation experienced no improvement during the first year. Moderately elevated pretransplantation comorbidity scores and prolonged dialysis vintage independently predicted poor HRQoL outcomes posttransplantation. Recipients receiving dialysis for 1 year or longer with pretransplantation comorbidity scores ≥ 7 experienced a marked and sustained physical deterioration after transplantation. LIMITATIONS: Homogenous and highly selected population. CONCLUSIONS: Transplantation is associated with a sustained HRQoL improvement and should be the preferred treatment for selected older patients. The value of a pretransplant comorbidity score to predict posttransplantation outcomes warrants further evaluation and may improve the selection process.

In the present study we investigated seasonal and intrinsic variability of neurotoxic and cyto-genotoxic biomarkers in blood plasma and erythrocytes of free-living Eleonoras' falcons, captured during the pre-breeding (May of 2017 and 2018) and breeding period (September of 2017) on the Antikythira Island (Greece). Specifically, blood samples of captured birds were prepared for the determination of cholinesterase (ChEs, i.e. acetylcholinesterase/AChE and butyrylcholinesterase/ BChE) activity, as well as the formation of nuclear (i.e. the formation of micronuclei into the cells/MN, binucleated cells/BN and others), and cellular/cytoplasmic (i.e. echinocytes/EC, acanthocytes/AC and notched cells/NC) abnormalities in blood plasma and erythrocytes, respectively. Our results indicated that birds sampled in late May had higher ChE and BChE activity levels, as well as higher frequency of total nuclear abnormalities. The latter were also higher in second calendar year (2cy) birds. Cellular/cytoplasmic abnormalities were less frequent in falcons having better body condition, sampled in late May, as well as in light-morph falcons. The observed ChEs activities, as well as nuclear and cellular/cytoplasmic abnormalities revealed that Eleonora's falcons are likely to be exposed to chemical agents with neurotoxic and cyto-genotoxic potential year round, while different aspects of their biology and ecology, such as their reproductive and nutritional status, could mediate their levels. Although we encourage more sampling campaigns to verify the identified seasonal and intrinsic sources of variation in biomarkers tested, the current study enriches the existing knowledge about their usefulness in the environmental monitoring and risk assessment of migratory birds, like Eleonoras' falcon.

BACKGROUND: In the elderly, kidney transplantation is associated with increased survival and improved health-related quality of life compared with dialysis treatment. We aimed to study the short-term health economic effects of transplantation in a population of elderly kidney transplant candidates. METHODS: Self-perceived health, quality-adjusted life years (QALYs) and costs were evaluated and compared 1 year before and 1 year after kidney transplantation in patients included in a single-centre prospective study of 289 transplant candidates ≥65 years of age. RESULTS: Self-perceived health and QALYs both significantly improved after transplantation. At 1 year, the costs per QALY were substantially higher for transplantation (€88 100 versus €76 495), but preliminary analyses suggest a favourable long-term health economic effect. CONCLUSIONS: Kidney transplantation in older kidney transplant recipients is associated with improved health but also with increased costs the first year after engraftment when compared with remaining on the waiting list. Any long-term cost-effectiveness needs to be confirmed in studies with longer observation times.

The current study is based on the increasing demand for the assessment of ionic liquid (IL)-mediated aquatic toxicity. Specifically, although a lot of studies have been performed so far, investigating IL-mediated adverse effects on numerous aquatic organisms, little is known about their mode of action. Given that the use of in vitro models is considered as a reliable tool for determining the mediated biological effects, the modulation of specific biochemical pathways and the onset of various forms of damage with great precision and reproducibility, mixed primary cultures of mussel Mytilus galloprovincialis hemocytes were used for investigating whether 1-octyl-3-methylimidazolium tetrafluoroborate ([omim][BF4]) mediated toxicity is related to its interaction with cellular membrane proteins. Specifically, [omim][BF4]-mediated cytotoxic, oxidative and genotoxic effects were investigated in mussel hemocytes before and after pre-treatment of cells with non-toxic concentration of guanidine hydrochloride (1 mM GndHCl). The results showed that [omim][BF4] at concentrations ranging from 0.7 to 1.75 µM can induce cytotoxic (almost <50% reduction of cell viability), oxidative (increased levels of O2•- production and lipid peroxidation by-products) and genotoxic (increased levels of DNA damage) effects, while cells pre-treated with 1 mM GndHCl showed a significant attenuation of IL's toxic potency in all cases. According to the latter, the current study showed that [omim][BF4]-mediated toxicity could be related not only to its well-known interaction with membrane lipid bilayers, but also to its interference with membrane proteins. Using GndHCl, a chaotropic agent that disrupts the hydrogen bonding network and the stability of membrane proteins via its interference with the intramolecular interactions mediated by non-covalent forces on cellular membranes, it was firstly shown that altering the membrane integrity as well as the native state of cellular membrane proteins, by weakening the hydrophobic effect, could attenuate the possible interaction of [omim][BF4] with cellular membranes and the concomitant induction of protein-based intracellular processes, commonly linked with the induction of severe cellular damage.

Control strategies and routine biomonitoring programs are commonly performed worldwide using sentinel marine invertebrates, such as mussels of the genus Mytilus, for assessing the "health status" of the aquatic environment. Those species can accumulate and tolerate xenobiotics at levels higher than those being present into the aquatic environment, thus providing accurate and reliable biological endpoints (e.g. physiological, behavioral, cellular, biochemical and molecular indices) that can be measured in their tissues. Taking under consideration the significance of bivalves for assessing the environmental hazard of xenobiotics being present into the water medium, as well as the key role of digestive gland as a target-tissue for the compounds ingested in the organism, the present study aimed to summarize available data on the effects of different categories of xenobiotic compounds, previously characterized as a potential threat for the marine ecosystems. In this context, different types of pharmaceuticals and personal care products (PPCPs), biocides, microplastics (MPs) and nanoparticles (NPs), currently investigated in mussels' digestive gland, using a battery of experimental approaches and analytical methods, as well as stress indices evaluation, are briefly described and further discussed in order to elucidate not only the presence and the toxic mode of action of xenobiotics, but also the important role of the digestive gland as a reliable target-tissue for investigating the effects of xenobiotics at cellular, biochemical, and molecular levels.

The present study investigated [omim][BF4]-mediated adverse effects on biological models widely used in toxicological studies. Specifically, mussels of the genus Mytilus, human lymphocytes and fruit flies of the species Drosophila melanogaster, were exposed to [omim][BF4] at concentrations ranging from micro- to milligrams per liter, with or without the presence of acetone as a carrier solvent and thereafter [omim][BF4]-mediated adverse effects were analyzed appropriately (stress indices, such as lipid peroxidation byproducts, acetylcholinesterase/AChE activity and micronucleus/MN formation frequency, in mussel gills, Cytokinesis Block Micronucleus/CBMN assay and SMART test in human lymphocytes and fruit flies respectively). LC-MS-TOF analysis was also performed for elucidating [omim][BF4] mode of action in the presence of the carrier solvent. The results showed the toxic potential of [omim][BF4], as well as acetone's ability to attenuate [omim][BF4]-mediated toxicity in almost all cases, probably due to the significant effect of acetone on the hydrophilic-lipophilic character and the viscosity of [omim][BF4], as well as its interaction and permeability on the cell membranes. The slight involvement of acetone in the attenuation of [omim][BF4]-mediated genotoxic effects on D. melanogaster could be due to species feeding experimental conditions, thus favoring the induction of antioxidant defense system against the [omim][BF4]-mediated effects in all cases.

This study investigated the cytotoxic, oxidative and genotoxic effects of two commonly used imidazolium ionic liquids (ILs), [bmim][BF4] (1-butyl-3-methylimidazolium) and [omim][BF4] (1-methyl-3-octylimidazolium tetrafluoroborate), on the marine mussel Mytilus galloprovincialis, as well as whether acetone could mediate their toxic profile. In this context, mussels were firstly exposed to different concentrations of [bmim][BF4] or [omim][BF4], with or without the presence of acetone (at a final concentration of 0.06% v/v), for a period of 96h, in order to determine the concentration that causes 50% mussel mortality (LC50 values) in each case. Thereafter, mussels were exposed to sub- and non-lethal concentrations of ILs for investigating their ability to cause lysosomal membrane impairment (with the use of neutral red retention assay/NRRT), superoxide anion and lipid peroxidation byproduct (malondialdehyde/MDA) formation, as well as DNA damage and formation of nuclear abnormalities in hemocytes. The results showed that [omim][BF4] was more toxic than [bmim][BF4] in all cases, while the presence of acetone resulted in a slight attenuation of its toxicity. The different toxic behavior of ILs was further revealed by the significantly lower levels of NRRT values observed in [omim][BF4]-treated mussels, compared to those occurring in [bmim][BF4] in all cases. Similarly, [bmim][BF4]-mediated oxidative and genotoxic effects were observed only in the highest concentration tested (10mgL(-1)), while [omim][BF4]-mediated effects were enhanced at lower concentrations (0.01-0.05mgL(-1)). Overall, the present study showed that [bmim][BF4] and [omim][BF4] could induce not only lethal but also nonlethal effects on mussel M. galloprovincialis. The extent of [bmim][BF4] and/or [omim][BF4]-mediated effects could be ascribed to the length of each IL alkyl chain, as well as to their lipophilicity. Moreover, the role of acetone on the obtained toxic effects of the specific ILs was reported for the first time, giving evidence for its interaction with the ILs and the modulation of their toxicity.

The main goal of this study was to investigate the toxicity of the imidazolium-based ionic liquids (ILs), [bmim][BF4] (1-butyl-3-methylimidazolium tetrafluoroborate) and [omim][BF4] (1-octyl-3-methylimidazolium tetrafluoroborate), in battery of standard aquatic toxicity test organisms. Specifically, exposure of the algae Scenedesmus rubescens, crustaceans Thamnocephalus platyurus and Artemia franciscana, rotifers Brachionus calyciflorus and Brachionus plicatilis and bivalve Mytilus galloprovincialis to different concentrations of [bmim][BF4], [omim][BF4] and/or a binary mixture of [bmim][BF4]-[omim][BF4] (1:1) with or without acetone (carrier solvent), revealed that solvent can differentially mediate ILs' toxic profile. Acetone's ability to differentially affect ILs' cation's alkyl chain length, as well as the hydrolysis of [BF4(-)] anions was evident. Given that the toxic potency of the tested ILs seemed to be equal or even higher (in some cases) than those of conventional organic solvents, the present study revealed that the characterization of imidazolium-based ILs as "green solvents" should not be generalized, at least in case of their natural occurrence in mixtures with organic solvents, such as acetone.

The present study could serve as a multidisciplinary approach for the assessment of river surface water quality with the use of chemical and biological methods. Specifically, physicochemical parameters, heavy metals, and pesticides were measured in water samples from three different stations (sampling station S1, S2, and S3) along Asopos River (Greece). In parallel, algal species (primary producers)-such as Scenedesmus rubescens and Chlorococcum sp.; consumer invertebrate species, such as the fairy shrimp Thamnocephalus platyurus and the rotifer Brachionus calyciflorus; as well as human lymphocytes-were exposed to those samples for assessing their toxic and genotoxic/mutagenic effects. According to the results, although the values of almost all of the physicochemical parameters tested, heavy metals (zinc, cadmium, lead, and mercury) and pesticides were lower than or within the respective environmental quality standards, thus offering no clear evidence for their natural or anthropogenic origin. Values recorded for nickel, chromium, hexavalent chromium, and malathion represent a typical case of mixed influence from natural and anthropogenic enrichments. In contrast, the algal growth arrest, the acute toxic effects on the freshwater invertebrates, and the increased micronuclei frequencies observed in human lymphocytes showed the presence of human-derived hazardous substances, which were hardly determinable with the use of conventional chemical methods. Given that the presence of priority pollutants in river surface waters, heavily burdened by anthropogenic activities, could give no clear evidence for their biological risk, the results of the present study showed that chemical and biological assays should be applied in parallel, thus serving as a reliable tool for the assessment of river water quality.

The aim of this work was to investigate the effect of zinc oxide nanoparticles (ZnO NPs) on freshwater and marine microalgae cultivated in different media. Freshwater species Chlorococcum sp. and Scenedesmus rubescens were cultivated in modified Blue-Green medium (BG-11) and Bold's Basal Medium (BBM), and marine species Dunaliella tertiolecta, and Tetraselmis suesica, cultured in salt modified BG-11 and f/2 medium. The microalgae species were exposed for 96 h with a daily reading of algal growth rate, to different ZnO NPs concentrations (0.081-810 mg/L). Significant differences were observed on microalgae growth rates, with the marine being more sensitive than the freshwater species, as revealed by their half inhibitory concentration values (IC50). The IC50 values in freshwater species were affected by the culture medium. The lowest IC50 values (<2.57 mg/L) were observed in the marine species. S. rubescens showed the less toxic effect in cultures with modified BG-11, compared to BBM cultures, with IC50 values >810 mg/L and 14.27 mg/L after 96 h exposure time, respectively. ZnO nanoparticles appeared to have toxic effects in all species tested, depended on the species type, the exposure time, the NPs concentration, and mainly the culture medium.

Given that polycyclic aromatic hydrocarbons (PAHs), such as phenanthrene (PH), possess a potent risk for aquatic biota, a great attempt to develop and apply advanced oxidation processes, such as ultrasound (US), is of great concern nowadays. However, because US PAH-derived toxic intermediates are difficult to detect, the present study investigates aqueous PH toxicity before and after high-frequency US degradation, in hemocytes of mussel Mytilus galloprovincialis. Specifically, cell viability (with the use of neutral red uptake/NRU method), and oxidative-stress indices in terms of superoxide anions, (O2(-)), nitric oxides (NO, in terms of nitrites), lipid peroxidation products (in terms of malondialdehyde/MDA content) and DNA damage (with the use of Comet assay method) were investigated in mussel hemocytes exposed to environmentally relevant concentrations of PH (0.01, 0.1, 1 and 10 µg L(-1)), before and after US treatment for 120 min (at a frequency of 582 kHz). According to the results, the NRU method showed a significant attenuation of PH-induced mortality in US PH-treated hemocytes in all cases. Moreover, the increased levels of O2(-) and NO generation, as well as MDA content measured in PH-treated hemocytes, were drastically decreased after US degradation in any case. Similarly, the disturbance of DNA integrity (in terms of % DNA in tail, OM and TM), was negligible in case of US PH-treated hemocytes. Although further in vitro and in vivo studies are needed, the present study showed for the first time that high frequency US could be applied as a highly efficient and "environmentally friendly" process for degrading low molecular weight PAH, such as PH.

This study investigates the pro-oxidant behavior of the antiepileptic drug carbamazepine (CBZ) on the marine algal species Dunaliella tertiolecta and the immune defense-related hemocytes of mussel Mytilus galloprovincialis. A phytotoxicity test, performed in a first step, showed a significant inhibition of the growth rate and the chlorophyll alpha (Chl-α) content in algae after exposure for 24 h to different concentrations of CBZ (1-200 mg L(-1)). On the other hand, the increased levels of lipid peroxidation products, such as MDA, measured in 24 h CBZ-treated cells were attenuated with time (48-96 h), followed by a significant recovery of both the algal growth rate and the Chl-α content in all cases. The latter could be related to the concomitant enhancement of total carotenoids in CBZ-treated algae with time, which in turn could protect algal growth and survival against CBZ-induced oxidative stress. On the other hand, the increased levels of cell death, superoxide anions ((·)O2 (-)), nitric oxides (NO, in terms of nitrites, NO2 (-)) and MDA content observed in mussel hemocytes exposed to environmentally relevant (0.01-1 µg L(-1)) and/or higher (10 and 100 µg L(-1)) concentrations of the drug, clearly indicate the ability of CBZ to induce oxidative effects on cells of non-target species, such as mussels, affecting thus their overall health status. The significant relationships occurred among the tested biological parameters in both bioassays, further reinforce CBZ-mediated pro-oxidant effects on species, widely used in ecotoxicological and toxicological studies and provide a more comprehensive view on its environmental fate and ecotoxicological risk evaluation.

The present study investigates leachate hazardous effects on marine biota and human cells, with the use of a battery of assays, both under in vivo and in vitro conditions. According to the results, mussels exposed for 4 days to 0.01 and 0.1% (v/v) of leachate showed increased levels of DNA damage and micronuclei (MN) frequencies in their hemocytes. Similarly, enhanced levels of DNA damage were also observed in hemocytes treated in vitro with relevant concentrations of leachate, followed by a significant enhancement of both superoxide anions (O2(-)) and lipid peroxidation products (malondialdehyde/MDA). On the other hand, human lymphocyte cultures treated with such a low concentrations of leachate (0.1, 0.2 and 1%, v/v), showed increased frequencies of MN formation and large MN size ratio, as well as decreased cell proliferation, as indicated by the use of the cytokinesis block micronucleus (CBMN) assay and Cytokinesis Block Proliferation Index (CBPI) respectively. These findings showed the clear-cut genotoxic and cytotoxic effects of leachate on both cellular types, as well as its potential aneugenic activity in human lymphocytes.

The present study investigates the harmful impacts of landfill leachate release and/or disposal into the marine environment, as well as its ability to induce lethal and pre-pathological alterations in marine organisms, such as the mussel Mytilus galloprovincialis. In specific, mortality test (96 h), performed first in order to estimate leachate lethal endpoints, showed increased levels of mussel mortality after exposure to leachate higher than 0.5%, v/v (96 h LC(50)=0.526%, v/v), while the exposure to 0.01 and 0.1% (v/v) of leachate showed negligible levels of mortality (96 h LC(10)=0.167%, v/v). Furthermore, the estimation of lysosomal membrane integrity in hemocytes of exposed mussels (Neutral Red Retention Time assay) showed increased levels of lysosomal destabilization in cells of mussels exposed to sub-lethal concentrations of leachate (0.01, 0.1 and 0.5%, v/v) for 4 days. In order to exclude parameters, such as mussel mortality and cell death, which could interfere with the obtained results, leachate at final concentrations of 0.01 and 0.1% (v/v) were finally used for the estimation of a battery of stress indices in target tissues of mussels, such as hemolymph, gills and digestive gland. According to the results, leachate-exposed mussels showed a significant inhibition of acetylcholinesterase activity, increased levels of nuclear abnormalities, as well as increased levels of metallothionein, superoxide anion and lipid peroxides (in terms of malondialdehyde equivalents) in each tissue tested. The results of the present study clearly indicate leachate-induced lethal effects as well as the ability of leachate to induce disturbances on different levels of organism function before mortality occurs.

The present study investigates seasonal variations of leachate composition and its toxic potency on different species, such as the brine shrimp Artemia franciscana (formerly Artemia salina), the fairy shrimp Thamnocephalus platyurus, the estuarine rotifer Brachionus plicatilis and the microalgal flagellate Dunaliella tertiolecta. In specific, leachate regularly collected from the municipal landfill site of Aigeira (Peloponissos, Greece) during the year 2011, showed significant alterations of almost all its physicochemical parameters with time. Further analysis showed that seasonal alterations of leachate composition are related with the amount of rainfall obtained throughout the year. In fact, rainfall-related parameters, such as conductivity (Cond), nitrates (NO(3)(-)), total nitrogen (TN), ammonium (NH(4)-N), total dissolved solids (TDS) and the BOD(5)/NH(4)-N ratio could merely reflect the leachate strength and toxicity, as verified by the significant correlations occurred among each of them with the toxic endpoints, 24 h LC(50) and/or 72 h IC(50), obtained in all species tested. According to the result of the present study, it could be suggested that the aforementioned leachate parameters could be used independently, or in combination as a low-cost effective tools for estimating leachate strength and toxic potency, at least in the case of semi-arid areas such as the most of the Mediterranean countries.
