OBJECTIVE: Determine the sensitivity and specificity of the ESBP for diagnosis in patients with intermediate risk of choledocholithiasis, referred to the specialized surgical Gastroenterology center of Unión de Cirujanos SAS - Oncologists of the West Zentria group - Manizales - Colombia between March 01, 2020 to January 31, 2022. MATERIALS AND METHODS: Retrospective cross-sectional study in patients with intermediate risk for choledocholithiasis. The diagnostic performance of ESBP was calculated and confirmed with ERCP. Negative ESBPs were followed up by telephone. RESULTS: 752 cases with ESBP were analyzed, of which 43.2% (n=325) were positive and 56.8% (n=427) were negative. ERCP was performed in positive cases who accepted the procedure (n=317); 73.5% (n:233) were positive for choledocholithiasis, 25.8% (n=82) tumors and 0.6% (n=2) biliary roundworms. Patients with positive ESBP underwent ERCP. S= 98.3% (95% CI: 95.7-99.5) was obtained; E= 88.1% (95% CI: 79.2-94.1); PPV = 95.8% (95% CI: 92.4-98.0); NPV = 94.9% (95% CI: 87.4-98.7). The AUC of ESBP was 0.9319 (95% CI 0.8961-0.967). CONCLUSION: In patients with intermediate risk for choledocholithiasis, ESBP is a useful diagnostic option in the study of pancreatic pathologies, extrahepatic biliary tree, and the identification of biliary microlithiasis; Therefore, it also allows us to complement it with a therapeutic intervention such as ERCP in a single time.

RESUMO

Patients with hypoplastic left heart syndrome (HLHS) with intact atrial septum have an increased mortality rate. This presentation occurs in 6% to 10% of cases. We present a patient with fetal diagnosis of HLHS with restrictive atrial septum. We performed a cesarean section at 37 weeks of gestation, and under ex utero intrapartum treatment proceeded with a median sternotomy and transatrial stenting for left atrial decompression due to findings of intact atrial septum on the fetal echocardiogram performed during the procedure. Subsequently, the patient underwent hybrid stage I palliation followed by a comprehensive stage II procedure at five months of age, but unfortunately died from postoperative complications.

Objetivo: Analizar el patrón alimentario y el riesgo de deficiencia en la ingesta usual de energía y nutrientes de las mujeres gestantes y lactantes de algunos pueblos indígenas. Metodología: Estudio descriptivo transversal, con información del componente de ingesta dietética tomada del Estudio nacional de la situación alimentaria y nutricional de los pueblos indígenas de Colombia, realizado entre 2013 y 2019, que incluyó 1028 mujeres indígenas gestantes (319) y lactantes (709), de las regiones norte y sur del país. Resultados: Se encontró una proporción superior al 90 % (Desviación estándar = 0,04) de gestantes y lactantes con consumo inferior al recomendado de calorías (p= 0,038). Alrededor del 70 % de las gestantes y lactantes no consumió lácteos el día anterior a la encuesta, y 50 % no consumió frutas y verduras. Con respecto a los micronutrientes, se encontraron, en las gestantes y lactantes, altas prevalencias del riesgo de deficiencia en la ingesta de vitamina C (50,3 y 80,2 %), folatos (80,4 y 95,1 %), zinc (87,6 y 96,3 %), hierro (88,9 y 68,6 %) y calcio (87,5 y 98,5 %). Conclusión: El consumo de alimentos y nutrientes en las mujeres indígenas gestantes y lactantes es deficiente en nutrientes claves para mantener su estado de salud y proporcionar los nutrientes necesarios a su bebé, y su patrón alimentario se clasifica como no saludable.


Objective: To analyze the dietary pattern and deficiency risk in the usual energy and nutrient intake of pregnant and lactating women from some indigenous peoples. Methodology: Cross-sectional descriptive study, with information from the dietary intake component taken from the National Survey of the food and nutritional situation of indigenous peoples in Colombia, conducted between 2013 and 2019, which included 1,028 pregnant (319) and lactating (709) indigenous women from the northern and southern regions of the country. Results: A proportion higher than 90% (SD = 0.04) of pregnant and lactating women with lower than recommended calorie intake was found (p = 0.038). Around 70% of pregnant and lactating women had no any dairy products the day before the survey, and 50% did not eat fruits and vegetables. Regarding micronutrients, high prevalence of deficiency risk in the intake of vitamin C (50.3 and 80.2%), folate (80.4 and 95.1%), zinc (87.6 and 96.3%), iron (88.9 and 68.6%) and calcium (87.5 and 98.5%) was found in pregnant and lactating women. Conclusion: Food and nutrient intake in pregnant and lactating indigenous women is deficient in key nutrients to maintain their general health and provide the necessary nutrients to their babies. Their dietary pattern is considered unhealthy.

Resumen Los pacientes con dermatomiositis presentan un mayor riesgo de asociación con patología maligna subyacente, sin conocerse una causa precisa de esta relación. Son múltiples las posibles localizaciones anatómicas, entre las que se incluyen los ovarios, mamas, zona gástrica, zona colorrectal, sangre, pulmones y próstata. Se presenta el caso de una mujer de 58 años que durante el estudio de pérdida anormal de peso manifestó debilidad muscular grave y alteraciones dermatológicas asociadas con el hallazgo de adenocarcinoma del colon transverso.


Abstract Patients with dermatomyositis are more likely to have an underlying malignancy, although the exact cause of this association is unknown. There are multiple possible anatomical sites, including ovaries, breasts, stomach, colorectum, blood, lungs, and prostate. We present the case of a 58-year-old woman who during abnormal weight loss study showed severe muscle weakness and skin alterations, associated with finding of adenocarcinoma of the transverse colon.

In order to provide a better understanding of the phenomena that define the weld bead penetration and melting rate of consumables in underwater welding, welds were developed with a rutile electrode in air welding conditions and at the simulated depths of 5 and 10 m with the use of a hyperbaric chamber and a gravity feeding system. In this way, voltage and current signals were acquired. Data processing involved the welding voltage, determination of the sum of the anodic and cathodic drops, calculation of the short-circuit factor, and determination of the melting rate. Cross-sectional samples were also taken from the weld bead to assess bead geometry. As a result, the collected data show that the generation of energy in the arc-electrode connection in direct polarity (direct current electrode negative-DCEN) is affected by the hydrostatic pressure, causing a loss of fusion efficiency, a drop of operating voltage, decreased arc length, and increased number of short-circuit events. The combination of these characteristics kept the weld bead geometry unchanged, compared to dry weld conditions. With the positive electrode (direct current electrode positive-DCEP), radial losses were derived from greater arc lengths resulting from increasing hydrostatic pressure, which led to a decrease in weld penetration.

Introducción. La colangiopancreatografía retrógrada endoscópica es la herramienta de elección para el manejo de una gran cantidad de enfermedades pancreáticas y biliares. Al ser un procedimiento invasivo, trae consigo riesgos que aumentan la morbimortalidad en los pacientes en quienes se practica. El objetivo de este estudio fue analizar las complicaciones más prevalentes relacionadas con el procedimiento, de los pacientes de una institución de tercer nivel de Medellín, entre los años 2014 y 2017.Métodos. Se presenta un estudio observacional, retrospectivo y analítico. Se analizaron las historias clínicas de los pacientes sometidos a colangiopancreatografía retrógrada endoscópica con diagnóstico de enfermedades biliares y pancreáticas. La información fue analizada usando estadística descriptiva e inferencial, mediante análisis univariado y multivariado, y se calculó la razón de momios (odds ratio, OR) con un intervalo de confianza del 95 %; se tomó como significativo un valor de p inferior a 0,25 en los análisis bivariados.Resultados. En los 1.546 pacientes sometidos a colangiopancreatografía retrógrada endoscópica, la edad promedio fue de 57,6 ± 19.3 años y el 59,1 % eran mujeres. Las complicaciones identificadas fueron: pancreatitis en 57 pacientes (3,7 %), sangrado en 28 (1,8 %), perforación en 8 (0,5 %) y mortalidad no discriminada de 42 pacientes (2,7 %). Como factores de riesgo con significancia estadística, se encontraron la papilotomía y el contraste del conducto de Wirsung (OR=3 y OR=3,55, respectivamente).Discusión. Los resultados obtenidos se encuentran en concordancia con los de la literatura mundial, con tasas de complicaciones y mortalidad similares. Por otra parte, el perfil sociodemográfico de la población de estudio difiere de las cifras internacionales, con predominancia de la enfermedad estudiada en edades mayores y con mayor cantidad de comorbilidades


Introduction: Endoscopic retrograde cholangiopancreatography is the tool for the management of a large number of pancreatic and biliary diseases. Being an invasive procedure, it brings risks that increase morbidity and mortality in patients in whom it is practiced. The objective of this study was to analyze the most prevalent complications related to the procedure of the patients of a third level institution in Medellin between 2014 and 2017.Methods: An observational, retrospective and analytical study is presented. The information was analyzed using descriptive and inferential statistics, by univariate and multivariate analysis, and the odds ratio (odds ratio, OR) was calculated with a 95% confidence interval; a value of p less than 0.25 was taken as significant in the bivariate analyzes. Results: In the 1,546 patients who underwent endoscopic retrograde cholangiopancreatography, the average age was 57.6 ± 19.3 years and 59.1% were women. The complications identified were: pancreatitis in 57 patients (3.7%), bleeding in 28 (1.8%), perforation in 8 (0.5%) and non-discriminated mortality of 42 patients (2.7%). As risk factors with statistical significance, papillotomy and Wirsung duct contrast were found (OR = 3 and OR = 3.55, respectively).Discussion: The results obtained are consistent with those of the world literature, with similar complication and mortality rates. On the other hand, the sociodemographic profile of the study population differs from international figures, with a predominance of the disease studied in older ages and with a greater number of comorbidities

Introducción Las copas de doble movilidad son implantes en que se introduce una cabeza protésica en un núcleo de polietileno, el cual posteriormente se articula con una copa metálica implantada en el acetábulo. Este tipo de diseño desarrollado en Francia desde la década de 1970ha demostrado disminuir el riesgo de luxación en un remplazo total de cadera. Estas copas han sido utilizadas históricamente de forma no cementada. No obstante, actualmente se pueden utilizar copas cementadas. Los objetivos de este estudio son determinar la sobrevida de las copas de doble movilidad cementadas y medir la calidad de vida de los pacientes. Materiales y métodos Se realiza un estudio multicéntrico retrospectivo, cuya indicación para la cirugía sea fracturas (61,5%), artrosis (34,6%) o tumores (3,8%). El análisis se realizó con los pacientes operados entre los años 2011-2013 y se encontraron 82 caderas operadas con copas de doble movilidad cementadas con un promedio de edad de 76 años. Resultados Se revisó una copa de doble movilidad por infección; por tanto, se obtuvo una sobrevida del 97,6% con un promedio de tiempo de seguimiento de 33,7 (29,7-37,7) meses con un IC 95%, y un promedio en la escala de Oxford de 38,2 (34,8-41,7) con un IC 95%. Discusión Se pudo concluir que la tasa de sobrevida es óptima para el tiempo de seguimiento y el puntaje promedio en la escala de Oxford es bueno si se tienen en cuenta las comorbilidades y la media de edad de los pacientes. Background The dual mobility cups are implants where a prosthetic head is introduced into a polyethylene core, which is subsequently articulated with a metal cup to be implanted in the acetabulum. This type of design developed in France since the seventies has shown to decrease the risk of dislocation in total hip replacement. These cups have been used historically cementless, however now can be use cemented. The objectives are to determine the survival of double cemented mobility cups and to measure the quality of life of patients. Materials and methods A retrospective multicenter study was made. Patients who underwent primary surgery and whose indication for surgery is fractures (61.5%), osteoarthritis (34.6%) or tumors (3.8%) were included. The analysis was performed with patients operated between 2011 and 2013. Results The study was based in 82 hips operated with dual mobility cemented cups with a patient's average age of 76 (13.4) years old. A dual mobility cup was checked for infection therefore obtaining a survival rate of 97.6% and an average follow up of 33.7 months with 95% CI (29,7-37,7) and an average in the Oxford scale of 38.2 with 95% CI (34,8-41,7) Discussion It was concluded that the survival rate is optimal taking into account the follow up time and the average score on Oxford scale were good considering comorbidities and the average age of patients. Chronic stress is a major risk factor for depression. Interestingly, not all individuals develop psychopathology after chronic stress exposure. In contrast to the prevailing view that stress effects are cumulative and increase stress vulnerability throughout life, the match/mismatch hypothesis of psychiatric disorders. The match/mismatch hypothesis proposes that individuals who experience moderate levels of early life psychosocial stress can acquire resilience to renewed stress exposure later in life. Here, we have tested this hypothesis by comparing the developmental effects of 2 opposite early life conditions, when followed by 2 opposite adult environments. Male Balb/c mice were exposed to either adverse early life conditions (limited nesting and bedding material) or a supportive rearing environment (early handling). At adulthood, the animals of each group were either housed with an ovariectomized female (supportive environment) or underwent chronic social defeat stress (socially adverse environment) for 3 weeks. At the end of the adult manipulations, all of the animals were returned to standard housing conditions. Then, we compared the neuroendocrine, behavioral and molecular effects of the interaction between early and adult environment. Our study shows that early life adversity does not necessarily result in increased vulnerability to stress. Specific endophenotypes, like hypothalamic-pituitary-adrenal axis activity, anxiety-related behavior and glucocorticoid receptor expression levels in the hippocampus were not significantly altered when adversity is experienced during early life and in adulthood, and are mainly affected by either early life or adult life adversity alone. Overall our data support the notion that being raised in a stressful environment prepares the offspring to better cope with a challenging adult environment and emphasize the role of early life experiences in shaping adult responsiveness to stress.

RESUMO

Anomalous left coronary artery from the pulmonary artery, or ALCAPA syndrome, is a rare congenital cardiac disease that can cause myocardial infarction, heart failure and even death in paediatric patients. Only few untreated patients survive until adult age. Here we present the case of a 33-year-old female patient with paroxysmal tachycardia, syncope and mild exertional dyspnoea. She was diagnosed with ALCAPA syndrome and underwent surgical correction with an alternative technique of left main coronary artery extension to the aorta.

BACKGROUND: There is growing evidence that maternal obesity and prenatal exposure to a high-fat diet program fetal development to regulate the physiology and behavior of the offspring in adulthood. Yet the extent to which the maternal dietary environment contributes to adult disease vulnerability remains unclear. In the current study we tested whether prenatal exposure to maternal obesity increases the offspring's vulnerability to stress-related psychiatric disorders. METHODS: We used a mouse model of maternal diet-induced obesity to investigate whether maternal obesity affects the response to adult chronic stress exposure in young adult (3-month-old) and aged adult (12-month-old) offspring. RESULTS: Long-lasting, delayed impairments to anxiety-like behaviors and stress coping strategies resulted on account of prenatal exposure to maternal obesity. Although maternal obesity did not change the offspring's behavioral response to chronic stress per se, we demonstrate that the behavioral outcomes induced by prenatal exposure to maternal obesity parallel the deleterious effects of adult chronic stress exposure in aged male mice. We found that the glucocorticoid receptor (GR, Nr3c1) is upregulated in various hypothalamic nuclei on account of maternal obesity. In addition, gene expression of a known regulator of the GR, FKBP51, is increased specifically within the paraventricular nucleus. CONCLUSIONS: These findings indicate that maternal obesity parallels the deleterious effects of adult chronic stress exposure, and furthermore identifies GR/FKBP51 signaling as a novel candidate pathway regulated by maternal obesity.

Antidepressants (ADs) are used as first-line treatment for most stress-related psychiatric disorders. The alterations in brain circuit dynamics that can arise from stress exposure and underlie therapeutic actions of ADs remain, however, poorly understood. Here, enabled by a recently developed voltage-sensitive dye imaging (VSDI) assay in mouse brain slices, we examined the impact of chronic stress and concentration-dependent effects of eight clinically used ADs (belonging to different chemical/functional classes) on evoked neuronal activity propagations through the hippocampal trisynaptic circuitry (HTC: perforant path → dentate gyrus (DG) → area CA3 → area CA1). Exposure of mice to chronic social defeat stress led to markedly weakened activity propagations ("HTC-Waves"). In contrast, at concentrations in the low micromolar range, all ADs, which were bath applied to slices, caused an amplification of HTC-Waves in CA regions (invariably in area CA1). The fast-acting "antidepressant" ketamine, the mood stabilizer lithium, and brain-derived neurotrophic factor (BDNF) exerted comparable enhancing effects, whereas the antipsychotic haloperidol and the anxiolytic diazepam attenuated HTC-Waves. Collectively, we provide direct experimental evidence that chronic stress can depress neuronal signal flow through the HTC and demonstrate shared opposing effects of ADs. Thus, our study points to a circuit-level mechanism of ADs to counteract stress-induced impairment of hippocampal network function. However, the observed effects of ADs are impossible to depend on enhanced neurogenesis.

While it is known that stress promotes obesity, the effects of stress within an obesogenic context are not so clear and molecular targets at the interface remain elusive. The FK506-binding protein 51 (FKBP51, gene: Fkbp5) has been identified as a target gene implicated in the development of stress-related psychiatric disorders and is a possible candidate for involvement in stress and metabolic regulation. The aims of the current study are to investigate the interaction between chronic stress and an obesogenic context and to additionally examine whether FKBP51 is involved in this interaction. For this purpose, male C57BL/6 mice were exposed to a high-fat diet for 8 weeks before being challenged with chronic social defeat stress. Herein, we demonstrate that chronic stress induces hypophagia and weight loss, ultimately improving features arising from an obesogenic context, including glucose tolerance and levels of insulin and leptin. We show that Fkbp5 expression is responsive to diet and stress in the hypothalamus and hippocampus respectively. Furthermore, under basal conditions, higher levels of hypothalamic Fkbp5 expression were related to increased body weight gain. Our data indicate that Fkbp5 may represent a novel target in metabolic regulation.

BACKGROUND: In recent years, computer simulation models have supported development of pandemic influenza preparedness policies. However, U.S. policymakers have raised several concerns about the practical use of these models. In this review paper, we examine the extent to which the current literature already addresses these concerns and identify means of enhancing the current models for higher operational use. METHODS: We surveyed PubMed and other sources for published research literature on simulation models for influenza pandemic preparedness. We identified 23 models published between 1990 and 2010 that consider single-region (e.g., country, province, city) outbreaks and multi-pronged mitigation strategies. We developed a plan for examination of the literature based on the concerns raised by the policymakers. RESULTS: While examining the concerns about the adequacy and validity of data, we found that though the epidemiological data supporting the models appears to be adequate, it should be validated through as many updates as possible during an outbreak. Demographical data must improve its interfaces for access, retrieval, and translation into model parameters. Regarding the concern about credibility and validity of modeling assumptions, we found that the models often simplify reality to reduce computational burden. Such simplifications may be permissible if they do not interfere with the performance assessment of the mitigation strategies. We also agreed with the concern that social behavior is inadequately represented in pandemic influenza models. Our review showed that the models consider only a few social-behavioral aspects including contact rates, withdrawal from work or school due to symptoms appearance or to care for sick relatives, and compliance to social distancing, vaccination, and antiviral prophylaxis. The concern about the degree of accessibility of the models is palpable, since we found three models that are currently accessible by the public while other models are seeking public accessibility. Policymakers would prefer models scalable to any population size that can be downloadable and operable in personal computers. But scaling models to larger populations would often require computational needs that cannot be handled with personal computers and laptops. As a limitation, we state that some existing models could not be included in our review due to their limited available documentation discussing the choice of relevant parameter values. CONCLUSIONS: To adequately address the concerns of the policymakers, we need continuing model enhancements in critical areas including: updating of epidemiological data during a pandemic, smooth handling of large demographical databases, incorporation of a broader spectrum of social-behavioral aspects, updating information for contact patterns, adaptation of recent methodologies for collecting human mobility data, and improvement of computational efficiency and accessibility.

Introducción: la sexualidad está conformada por los elementos biológicos, es decir, el sexo; lo psicológico, como es el sentirse y pensarse como hombre o mujer; y los sociales, que hacen referencia al comportamiento que establece la sociedad para cada sexo. Entonces se puede entender por sexualidad a la forma en la que cada ser humano se manifiesta como hombre o como mujer, de acuerdo a las normas y valores propios de su cultura y de su época. Objetivo: Prácticas, hábitos y conocimientos en los estudiantes de la facultad de salud y medicina en la Universidad de Santander–UDES. Materiales y Métodos: Estudio de corte transversal, en el cual, se identifican aspectos importantes como conocimientos básicos sobre sexualidad humana, nivel de conocimientos sobre métodos anticonceptivos, variables socio demográficas y culturales y conductas relacionadas con la sexualidad en los estudiantes de la facultad de salud y medicina de la Universidad de Santander –Bucaramanga 2010. Resultados: se observa que los estudiantes del área de la salud de la UDES, presentan mayor conocimiento en general sobre sexualidad y menor riesgo tanto en las conductas sexuales como en consumo de sustancias psicoactivas y alcohol. Discusión y Conclusiones: Queda demostrado que con la perpetuación de programas a cerca de la educación y orientación sobre sexualidad humana en las universidades y en las diferentes instituciones educativas, los jóvenes pueden sobrellevar una práctica sexual responsable y poder evitar embarazos no planeados y el contagio de una enfermedad de transmisión sexual con una certeza del 75% por tener previos conocimientos acerca del tema.


Introduction: sexuality is composed of biological elements, ie, sex, psychological, as is the feel and think as a man or woman, and social, that refer to behavior that society sets for each sex. So you can understand sexuality to the way in which every human being is manifested as a man or woman, according to the norms and values of his culture and his time. Objective: Practice habits and knowledge in students of the faculty of health and medicine at the University of Santander-UDES. Materiales and Methods: Cross sectional study in which important issues are identified as basic knowledge about human sexuality, level knowledge about contraceptive methods, socio-demographic and cultural variables and behaviors related to sexuality in the students of the faculty of health and medicine at the University of Santander, Bucaramanga 2010.Resultados: it is observed that students in the area of health UDES, have more general knowledge about sexuality and less risky sexual behavior both as consumption of psychoactive substances and alcohol. Discussion and Conclusions: It is shown that the perpetuation of programs about education and counseling on human sexuality in different universities and educational institutions, young people can cope and responsible sexual practices to avoid unplanned pregnancies and the spread of disease sexual transmission with 75% certainty to have prior knowledge about the subject.

Introducción: en Estados Unidos se realizaron 200 000 reemplazos de cadera en el 2001, de los cuales 36 000 fueron cirugías de revisión. Es determinante detectar la presencia o no de infección en una cirugía de revisión para seleccionar el procedimiento quirúrgico más conveniente para el paciente. El cultivo intraoperatorio es el patrón de oro, pero es inoportuno para tomar decisiones intraoperatorias. La biopsia por congelación es un estudio histopatológico que ayuda a determinar infección durante el procedimiento quirúrgico. El objetivo fue determinar la sensibilidad, la especificidad, el valor predictivo positivo (VPP) y el valor predictivo negativo (VPN) de la biopsia por congelación frente al cultivo, en pacientes sometidos a cirugía de revisión de reemplazo total de cadera. Materiales y métodos: se realizó un estudio transversal de prueba diagnóstica. Se revisaron 61 historias clínicas de pacientes que requirieron cirugía de revisión de reemplazo total de cadera, realizada en la Clínica Reina Sofía entre los años 2004 y 2007. 58 cumplieron los criterios de inclusión. Se verificaron los resultados de la biopsia por congelación y gram y se compararon con el cultivo. Resultados: se encontró una sensibilidad de 64,2% (IC 95% 35-92), una especificidad de 65,9% (IC 95% 50,7-81,1), un VPP de 37,5% y un VPN de 85,2% para el diagnóstico de infección con la biopsia por congelación positiva mayor a 5 polimorfonucleares. Discusión: la biopsia por congelación debe ser tomada como uno de los criterios intraoperatorios en el diagnóstico de infección en la cirugía de revisión de reemplazo total de cadera.

However, not all patients present in this form therefore the diagnosis and treatment can be difficult and a real challenge. This report is of a patient with this disease and other etiologic possibilities are discussed.
